Opis:
The aim of this article is to identify and discuss some methodological issues that are of interest among functional safety specialists and experts after publication of the second edition of international standards IEC 61508 and IEC 61511, including the design and implementing the safety-related functions of higher safety integrity levels and protection layers. The basic role of safety-related systems is to reduce effectively and to control in time the individual and/or societal risk with regard to its tolerable levels. These issues include: risk criteria, reliability data, probabilistic models of systems operating in high and/or low mode, dependent failures, human reliability analysis, security of programmable safety-related systems, and reducing uncertainty issues in decision making process applying the cost-benefit analysis. Selected aspects of these issues are discussed and some challenges requiring further research are indicated.

The approach addresses selected technical and organization aspects of risk mitigation in the oil port installations with regard to functional safety and security requirements specified in standards IEC 61508, IEC 61511 and IEC 62443. The procedure for functional safety management includes the hazard identification, risk analysis and assessment, specification of overall safety requirements and definition of safety functions. Based on the risk evaluation results the safety integrity level (SIL) and security assurance level (SAL) will be determined for consecutive safety functions. The proposed approach will be composed of the following items: process and procedure based safety and security management, example of procedure based safety management including insurance, integrated safety and security assessment of industrial control system (ICS) of the oil port pipelines, tanks and critical infrastructure.

The work is devoted important issues of the management in maritime critical infrastructure of functional safety analysis, in particular the safety integrity level (SIL) verification of safety functions to be implemented within the distributed control and protection systems with regard to cyber security aspects. A method based on quantitative and qualitative information is proposed for the SIL (IEC 61508, 61511) verification with regard of the evaluation assurance levels (EAL) (ISO/IEC 15408), the security assurance levels (SAL) (IEC 62443), and the number of protection rings described in the Secure Safety (SeSa-SINTEF) methodology. The proposed approach will be composed of the following items: process and procedure based safety and cyber security management, integrated safety and security assessment of industrial control system (ICS) of the maritime critical infrastructure. Proposed methodology is illustrated on case study that based on the part of installation critical maritime infrastructure.

This article addresses selected technical and organization aspects of risk mitigation in the oil port installations with regard to functional safety requirements specified in standards IEC 61508 and IEC 61511. The procedure for functional safety management includes the hazard identification, risk analysis and assessment, specification of overall safety requirements and definition of safety functions. Based on risk assessment results the safety integrity level (SIL) is determined for consecutive safety functions. These functions are implemented within industrial control system (ICS) that consists of the basic process control system (BPCS) and/or safety instrumented system (SIS). Determination of required SIL related to required risk mitigation is based on semi-quantitative evaluation method. Verification of SIL for considered architectures of BPCS and/or SIS is supported by probabilistic models with appropriate data and model parameters including security-related aspects. The approach proposed is illustrated on example of oil port installations. In final part of the article the insurance aspects are discussed in managing risks, as some risks are to be transferred to an insurance company.

The aim of this article is to identify and discuss some issues of the safety systems’ design for nuclear power plants equipped with the light water reactors using a defence in depth (D-in-D) conception. Because the functional safety solutions play nowadays an important role for the risk control, the basic requirements for the instrumentation and control systems are specified with regard to relevant international standards. For the design purposes the safety functions are categorized into three categories. The I&C systems implementing these functions are assigned to one of three classes that conform to defined design, manufacturing and qualification requirements. These systems are designed to implement functions of relevant categories. Additional design requirements are discussed, including hardware and software aspects, to achieve and maintain the required reliability commensurate with the importance of the safety functions to be performed to reduce risk.

W artykule przedstawiono rozmyte podejście do automatycznej korekcji posuwu roboczego w trakcie zabiegu usuwania naddatku o dużej zmienności, zarówno co do objętości jak i miejscowej skrawalności. Zaprezentowane podejście nie wymaga instalacji w przestrzeni roboczej obrabiarki żadnych dodatkowych czujników pomiarowych. Wykorzystuje ono wyłącznie standardowe możliwości współczesnych układów sterowania numerycznego obrabiarkami. Na podstawie informacji o aktualnym rzeczywistym obciążeniu napędu głównego obrabiarki uaktywniana jest, poprzez akcję synchroniczną, nowa wartość programowego współczynnika korekcji posuwu. Dla jego określenia wykorzystuje się rozmyte funkcje agregacji. Metodę zilustrowano przykładowymi przebiegami regulacji, dotyczącymi zgrubnej obróbki zestawów kołowych.
This paper presents a fuzzy approach to automatic feed rate correction during rough turning with inconstancy both depth of cut and machinability of material. Additional sensors are not required in this approach. It exploit strictly the ability of modern numerical controllers. Based on actual power consumption, through synchronous actions, is a new adaptive value of feed rate activated. An aggregate fuzzy functions has been used to adaptive value of feed rate evaluation. An illustrative example of wheels machining has been used to demonstrate effects of proposed methods.

The aim of this article is to identify and discuss some problems that have been encountered in designing and operating the functional safety solutions of higher safety integrity levels (SIL 3 and SIL 4) in the light of analyses outlined in a new version of the international standard IEC 61508:2010. Examples of such solutions are the electric / electronic / programmable electronic systems (E/E/PESs) and the safety instrumented systems (SISs) being designed and operated respectively according to IEC 61508 and IEC 61511 in the system safety life cycle. The role of functional safety solutions is effective reducing and controlling the individual and/or societal risk with regard to tolerable levels defined. Some aspects of potential influence of danger failures of the E/E/PESs or SISs on the plant safety are considered. The influence of common cause failures (CCFs) and dependent failures in the context of the layer of protection analysis is also discussed.

The aim of this article is to identify and discuss some issues related to functional safety and security management in hazardous industrial plants. The safety functions are to be realised using the electric / electronic / programmable electronic systems (E/E/PESs) or the safety instrumented systems (SISs) that are designed and operated respectively according to IEC 61508 and IEC 61511 requirements in life cycle. Although the role of functional safety solutions in effective reducing and controlling the individual and/or societal risks has been widely recognised, the substantial problems emerge when E/E/PEs or SISs operate in industrial distributed computer networks. Thus, the security-related problems appear that can introduce some additional risks. An integrated systemic functional safety and security concept is proposed, which includes general requirements as well as appropriate using specified methods and international standards.

This article addresses some functional safety assessment procedures with cybersecurity aspects in critical industrial installations with regard to the functional safety requirements specified in standards IEC 61508 and IEC 61511. Verification of the SIL for the considered architectures o fthe BPCS and/or the SIS is supported by probabilistic models with appropriate data and model parameters including cybersecurity related aspects. The proposed approach is illustrated on the example of critical industrial installations.

This article addresses integrated functional safety and cybersecurity analysis with regard to: the generic functional safety standard IEC 61508 and the cyber security standard IEC 62443 concerning an industrial automation and control system (IACS). The objective is to mitigate the vulnerability of information technology (IT) and operational technology (OT) systems, and reduce relevant risks taking into account a set of fundamental requirements (FRs). A method is proposed for determining and verifying the performance level (PL) or the safety integrity level (SIL) of defined safety functions, and then validating these levels depending on the security level (SL) of a particular domain,e.g.a safety related control system (SRCS). The method is general in the sense that it is based on risk graphs prepared for individual risk and/or societal/group risk with regard to the criteria defined.

The paper deals with the problem of choosing an appropriate inspection interval for monitoring of safety related control systems in machinery. According to international standards the safety related systems are categorized according to their Safety Integrity Levels or Performance Levels, depending on their reliability parameters. Extremely simple, approximate models have been proposed in order to provide practitioners without reliability training with useful tools for the determination of inspection policies. The method(s) based on the required availability of the system. The paper presents some practical examples of systems of categories B, 1 and 3, respectively. The frequencies of periodical inspection are calculated for: system monitoring closure of the door, behind which a dangerous element moves slowly, system of monitoring the access door on the automated production line and system, in which a light curtain is employed to monitor the access to the dangerous zone of an automatic assembly machine.

Even the best technology will be ineffective if not used appropriately, therefore education and training about cybersecurity principles and programs are essential components of any cybersecurity strategy. This article presents selected models of the decision support theory from the point of view of cybersecurity education. The analysis of scientific literature and the available research results serve as a base to characterize approaches to raise the awareness of decision-makers about potential cyber threats and the development of appropriate attitudes and the conscious use of information systems and digital resources. The main part of the article is devoted to the issue of the use of teaching methods to increase the involvement of learners. It also describes examples of selected models of the game theory used in IT security education, including examples of simulation games dedicated to decision-making in the domain of IT security.

Opis:
The scope of this study covers events resulting from improper functioning of machine control systems. An accident model providing a basis for formulating a checklist for accident analysis has been developed. Data about 700 accidents were collected. An analysis has proved that in the group of accidents caused by improper functioning of machine control systems, serious accidents happened much more frequently as compared to the group of accidents with no relation to the control system. The reasons for the majority of incidents caused by improper performance of safety functions consist in the errors made by designers. In view of that, incorrect behaviour of a worker should be treated as a normal event instead of a deviation causing an accident.

Opis:
A security level of distributed control and protection system may have a significant impact on the results of functional safety analysis. However, the issue of integrating the safety and security aspects is difficult and usually is neglected during the functional safety analysis. This article presents a method of functional safety analysis which takes into consideration a concept of integrating these two aspects. It is based on proposed classification of communication channels used in the computer system / network and the scope of such system distribution. The functional safety analysis is to be performed at every stage of system lifecycle, but one of the most important parts is defining required safety functions and determining the safety integrity level for them. The integration concept might be taken into account at this stage. The basis of a method proposed is the assumption that the security level is considered as a risk parameter in graphs of functional safety analyses.

Communication-based Train Control (CBTC) system is a widely-used signaling system. There is an increasing demand for innovating the traditional ground-centric architecture. With the application of train-train communication, object control and other advanced techniques, Train-centric CBTC (TcCBTC) system is expected to be the most promising tendency of train control system. The safe tracking interval would be reduced as well as the life-cycle costs. Formal methods play an essential role in the development of safety-critical systems, which provides an early integration of the verifiable design process. In the paper, the architecture design of TcCBTC is first analyzed. The official system specification of TcCBTC has not issued, so it takes efforts to perform the systematic summarization of the functional requirements. Secondly, we propose an integrated framework that combines the Colored Petri Net (CPN) models with the functional safety verification of the underlying systems. Functional safety depends on the logic accuracy and is a part of overall safety. The framework also specifies what kinds of functions, behaviors or properties need to be verified. The train control procedure of TcCBTC is regarded as the link among new functional modules, thus it is chosen as the modelling content. Thirdly, the scenarios and the color sets are prepared. Models are established with the novel design thought from top to bottom. Simulation and testing are implemented during the model establishment to discover the apparent errors. Lastly, the model checking by state space is performed. All possible states are checked in detail. Standard behavioral properties and other user-defined properties are verified by state space report and ASK-CTL (Computation Tree Logic) queries, respectively. Verification results reveal that the models are reasonable to depict the dynamic behaviors of train control procedure. The functional safety properties are satisfied and prepared for further drafting the system functional specification.
