The presented study aims to compare occupational stress of 50 teachers and 56 firefighters. Both these groups are laden with high exposure to work-related stress. For assessing the occupational load the Meister questionnaire was used. We found that teachers percieved their occupational stress worse than firefighters and reached the 2nd degree of load. They complained mostly about the factor of overload and factors of the stress reactions – fatigue, mental overload and unacceptability to perform pedagogical work in the same intensity for a long time. Firefighters achieved better results (1st degree of load), even better than the population norm. An explanation may be found in the fact that firefighters represent a selected population of emotionally, psychically and physically resistant individuals.

Purpose. To verify the relationship between stress indicators and the age and years of service of military firefighters from the fire rescue corps of the metropolitan area of the state of Sao Paulo. Methods. Forty military firefighters with the mean age of 37.9 ± 5.4 years, from the 1st, 2nd and 8th fire brigades of the metropolitan area of Sao Paulo participated in this study. All underwent clinical, physical and psychological evaluations to verify the relationship between the age and years of service with the variables of levels of stress (tension, depression, anger, vigor, fatigue, confusion and total stress) on workdays and on days off. Results. On workdays, the mean scores for tension, depression, fatigue and stress were overall higher compared to those on days off. The older the firefighter, the higher the levels of stress, depression, anger and total stress. The more years of service, the greater the depression and anger. Conclusions. The levels of stress were high on workdays. Chronological age and years of service influenced the increase in the scores of stress.

Introduction and objective. Firefighters are considered a healthy and fit group of individuals, well-prepared for taking action in disaster situations. While working, they suffer from exposure to certain toxic agents, especially combustion products generated when a fire takes place. Among them, the most frequent and the most toxic are: carbon monoxide, hydrogen cyanide, ammonia, and those resulting from PVC combustion – hydrochloride, phosgene and chloride. Additionally, fireextinguisher powder can be inhaled and lead to certain lesion in the airways. The aim of study was to ascertain the influence of toxic agents present at the scene of fire on the lung tissue of firefighters, and also to study this on an animal model. Materials and methods. The study group consisted of firefighters who had a minimum of 10 years service. After completing a questionnaire, their clinical status was ascertained based on a general examination, laboratory tests and lung function tests. Results. Questionnaire analysis showed a high percentage of pathological symptoms in the studied group. The incidence of the symptoms correlated with the duration of occupational exposure to toxic agents. Among other results, obstruction of flow in medium airways in about 30% of the studied individuals represented the most important finding. Experimental tests were next performed on male Wistar rats, aged 3 months. They were insufflated with the solution of powdered fire-extinguisher, after which morphology specimens of lung tissue were studied. Evidence for disseminated fibrosis was obtained, which supported the previous clinical findings in the firefighters. Conclusion. The above shows correlation between occupational exposure and respiratory system involvement in firefighters. This justifies covering the group of firefighters with special medical care focused on prophilaxis, early detection and therapy of pulmonary diseases.

The aim of this study was to analyze the quality of life (QOL) of 142 primary school teachers and 145 firemen-rescuers. To determine QOL we used a WHOQOL-BREFF questionnaire. Teachers’ QOL was significantly lower in all four domains when compared with firefighters’ QOL; however, it did not differ statistically from the Czech population norms. Significantly lower compared to the norm was only teachers’ satisfaction with their health. Our results showed a significant influence of profession, age and gender on QOL. The influence of subjectively perceived mental stress as a factor reducing the QOL was manifested only in teachers. The explanation for this difference in QOL could be predicting better physical and mental health of firefighters.

A rare type of risk associated with firefighting activities and affecting the central nervous system (CNS) is acoustic trauma. The main causes of acoustic trauma in professional firefighters include long-term exposure to noise from fire engine sirens, emergency call alarms in the fire stations (fire halls), and the operation of equipment used during rescue and firefighting operations. The study was based on an analysis of data obtained from the HR Department of the National Headquarters of the State Fire Service (NHSFS). The analysis included cases of acoustic trauma suffered by firefighters while on duty, which were reported to the control room of their firefighting units as rescuer accidents, and followed up with medical interventions. The average number of accidents among SFS firefighters per year during the study period was n=1.617. During the study period (2015-2022), there were seven incidents in which a total of 11 firefighters suffered acoustic trauma affecting the CNS and the organ of hearing. To prevent on-duty accidents, both practical and theoretical training sessions are provided, covering measures aimed at reducing accident rates to minimum levels. Other important activities include hearing prophylaxis; medical examinations; breaks during work, appropriate organisation of work and the working environment; compliance with the applicable regulations, internal procedures and safety instructions; the use of personal protective equipment, and the good technical condition of vehicles and equipment.

Wstęp. Celem pracy było określenie źródeł stresu zawodowego w opinii strażaków, analiza sposobów radzenia sobie z nim oraz analiza rodzaju deklarowanych symptomów zdrowotnych występujących w związku ze stresem zawodowym. Materiał i metody. Grupę badaną stanowiło 110 strażaków Podziału Bojowego Komendy Miejskiej Państwowej Straży Pożarnej w Białej Podlaskiej. Badania przeprowadzono na przełomie roku 2014/2015, zastosowano metodę sondażu diagnostycznego i autorski kwestionariusz ankiety. Wyniki. 22% ankietowanych strażaków deklaruje, że nie odczuwa stresu podczas wykonywania czynności związanych z wykonywanym zawodem. Największym źródłem stresu dla ankietowanych była odpowiedzialność za życie osób ratowanych (38%) i presja czasu w podejmowaniu decyzji (25%). Deklarowano zdrowotne symptomy stresu: przyspieszony oddech, tętno, podwyższone ciśnienie (25,5%), rozdrażnienie (22,6%), zmęczenie (22,6%); 18,2% ankietowanych korzystało z pomocy psychologa, z zasadami systemu pomocy psychologicznej w Państwowej Straży Pożarnej zapoznało się 69,1% ankietowanych. Wnioski. Konieczne jest podniesienie świadomości strażaków w zakresie istotności pomocy psychologicznej w kwestii radzenia sobie ze stresem towarzyszącym pracy.
Introduction. The aim of the study was to identify sources of occupational stress as seen by fire-fighters, analyse the ways of dealing with it, as well as consider types of the declared health symptoms which occur due to occupational stress. Material and methods. The surveyed group consisted of 110 firefighters from the Combat Division of the Municipal Command of the State Fire Service in Biała Podlaska. The study was conducted at the turn of 2014/2015, using the diagnostic survey method and the author’s own questionnaire survey. Results. 22% of the surveyed firefighters declare that they do not feel stressed during their occupational activities. The biggest source of stress for the respondents was the responsibility for the lives of rescued people (38%) and the pressure of time in the decision-making process (25%). The declared health symptoms of stress included: accelerated breath and heart rate, increased blood pressure (25.5%), irritability (22.6%), fatigue (22.6%). 18.2% of the respondents sought help from a psychologist and 69.1% were familiar with the principles of the psychological help system in the State Fire Service. Conclusions. It is necessary to raise fire-fighters’ awareness in terms of the relevance of psychological support in coping with occupational stress.

INTRODUCTION: National Firefighting Rescue System (NFRS) often supports the Emergency Medical Service (EMS) units. If EMS is not yet at the scene, firefighters are able to perform necessary medical procedures while waiting for an ambulance. The aim of the study was to define the profile of patients based on experience of Polish Professional Fire Services (PFS). MATERIAL AND METHODS: 123 interventions were studied when firefighters arrived on the scene before ems, carried out in 2017 by PFS unit located in centre of Poland. For this retrospective study were analyzed a total of 100 complete and readable "rescue procedure charts" filled out by PFS firefighters. They were studied time of arrival, patients' demographic (age and sex), type of injury/ illness, type of medical procedures carried out by firefighters and duration of taking care of the patient before EMS arrival as well as types of equipment used during interventions. RESULTS: A total of 100 patients were included in the analysis (34 woman and 66 men). The mean age was 40.3 years (SD ± 16.56). The average time from arrival at the scene to initiation of medical procedures was 1.24 (SD ± 0.47) minutes. Before EMS arrival, firefighters were taking care of the patient for an average of 5.88 (SD ± 4.45) minutes. The most common type of injury was cervical spine trauma (41.34%), and the rarest condition was a hypothermia (19.23%). Cardiopulmonary resuscitation were performed in four cases. The most commonly used equipment were gloves (67.60%). The most common activity during taking care of patient were: psychological support (34.33%). CONCLUSIONS: During the interventions, firefighters perform rescue procedures mainly in case of injuries. The most common types of injuries include cervical spine and head trauma. Despite the fact that medical equipment is standardized in PFS units, some of the equipment are not used as often as the situations requires, which may result in a decrease in the effectiveness of rescue procedures.
WSTĘP: Strażacy posiadają sprzęt i przeszkolenie w zakresie procedur ratunkowych. Niejednokrotnie udzielają pierwszej pomocy poszkodowanym jeszcze przed przybyciem personelu medycznego EMS. Celem badania była próba określenia profilu poszkodowanego, któremu strażacy pracujący w Professional Fire Services (PFS) udzielają pomocy samodzielnie. MATERIAŁ I METODY: Badanie przeprowadzono w 2017 roku w jednostce PFS zlokalizowanej w centralnej Polsce. Wyselekcjonowano 100 spośród 123 interwencji, które posiadały czytelne adnotacje w dokumentacji medycznej. Analizie poddano dane dotyczące wieku i płci poszkodowanych, obrażeń ciała jakich doznali, rodzaju udzielonej pomocy, a także sprzętu ratowniczego użytego podczas akcji. Oceniono także ramy czasowe wykonywanych przez strażaków czynności aż do chwili przekazania poszkodowanego zespołowi EMS. WYNIKI: Grupę poszkodowanych stanowiły 34 kobiety oraz 66 mężczyzn. Średnia wieku wyniosła 40,3 lat (SD±16,56). Średni czas od momentu przybycia na miejsce zdarzenia do podjęcia czynności ratunkowych wyniósł 1,24 (SD±0,47) minuty. Pomocy poszkodowanym udzielano średnio przez 5,88 (SD±4,45) minut do czasu przekazania EMS. Najczęstszymi obrażeniami było uszkodzenie odcinka szyjnego kręgosłupa (41,34%) i urazy głowy (19,23%). W czterech przypadkach prowadzono CPR. Najczęściej wykorzystywane wyposażenie ratunkowe stanowią rękawiczki i opatrunki. Strażacy w 34,33% wszystkich czynności udzielają wsparcia psychicznego. WNIOSKI: Profil poszkodowanego, któremu udziela pomocy PFS to mężczyzna w wieku 40 lat z obrażeniami szyi i głowy. Strażacy zużywają przede wszystkim środki ochrony osobistej i opatrunki. Przed przybyciem EMS strażacy wdrażają najczęściej pomoc psychologiczną, stabilizację głowy oraz zakładają opatrunki osłonowe. Należy rozważyć modyfikację procesu dydaktycznego w PFS, w celu efektywniejszego wykorzystania zestawów ratunkowych.

ObjectivesThe objective of the work was to analyze the impact of selected factors concerning the use and maintenance of firefighters’ protective clothing worn during rescue operations on the hazard of contamination by chemical substances.Material and MethodsThe participants were firefighters (N = 688) from rescue and firefighting units of the State Fire Service in Poland, aged <30, 31–40, 41–50 or >60 years, with different seniority: up to >21 years of service. The survey questionnaire developed by the authors was used. The questionnaire was available online. The Statistica 10.0 statistical package using the χ2 test was applied in the analysis of the significance of the results.ResultsAs reported by the vast majority (>60%) of the firefighters, the maintenance was carried out after recording an average or a high level of contamination. It was pointed out that removal of the contaminants from protective clothing was difficult (83%). The surfaces of the legs and sleeves of protective clothing were the most contaminated areas. A feeling of discomfort was observed (90%) after returning from firefighting operations due to fire, smoke, or combustion residues.ConclusionsIt is necessary to conduct training and information actions concerning the use and maintenance of protective clothing and the harmfulness of chemicals contaminating the garments used by firefighters.

INTRODUCTION: At the moment, the role of the Fire Service in the vast majority of cases differs from actions related to fires or natural disasters. The type of intervention depends on the specificity of the region and the equipment of the unit. The aim of the study was to analyze the activities of the Volunteer Fire Services (VFS) in a rural agglomeration. MATERIAL AND METHODS: 164 interventions from 2018 were analyzed. The VFS unit was located in the Zbuczyn area in Poland. Tables showing the activity of the unit in numerical terms were used to present the research results. Statistical analysis was performed by assessing the normal distribution with the Shapiro-Wilk test. Spearman's Rho test was used to calculate the significance of variable correlations. The significance level was assumed at p<0.05. RESULTS: More than 1/3 of all interventions (n=55; 33.60%) concerned traffic events. Calls to fire constituted 21.90% (n=36). A total of 586 activities were undertaken, of which 78 (13.31%) were related to securing the incident site, 50 (8.53%) to stopping hemorrhage and dressing wounds, and 46 (7.85%) to cutting various structures in order to free the casualties. Rescue equipment was used 698 times. Firefighters usually used dressing sets (n=74; 10.60%), as well as stretchers and immobilizing equipment (n=62; 8.88%). Rescue operations were usually conducted in the open air (n=84; 51.22%) in the afternoon (4:00 p.m -5:00 p.m). There was a significant correlation (p=0.004) shown between the hours per day cycle and the number of calls. CONCLUSIONS:In case of incidents in rural areas, VFS firefighters are most often called to road accidents. The majority of on-site interventions are related to providing safety and dressing the wounds of the injured persons. The number of interventions closely correlates with the time of day and is the highest in the afternoon.
WSTĘP: Obecnie rola Straży Pożarnej w zdecydowanej większości przypadków odbiega od działań związanych z pożarami, czy klęskami żywiołowymi. Rodzaj interwencji jest zależny od specyfiki regionu oraz wyposażenia danej jednostki. Celem pracy była analiza działań Ochotniczej Straży Pożarnej (VFS) w aglomeracji wiejskiej. MATERIAŁ I METODY: Analizie poddano 164 interwencje w roku 2018. Jednostka VFS znajdowała się w rejonie Zbuczyn, Polska. Do przedstawienia wyników badań wykorzystano tabele ukazujące działalność jednostki w ujęciu liczbowym. Analizę statystyczną przeprowadzono oceniając normalność rozkładu testem Shapiro-Wilka. Dla wyliczenia istotności korelacji zmiennych wykorzystano test rho-Spearman’a. Poziom istotności przyjęto dla p<0.05. WYNIKI: Ponad 1/3 wszystkich interwencji (n=55; 33.60%) dotyczyła zdarzeń drogowych. Wezwania do pożarów stanowiły 21.90% (n=36). Podjęto łącznie 586 czynności, z czego 78 (13.31%) dotyczyło zabezpieczenia miejsca zdarzenia, 50 (8.53%) – tamowania krwotoków i opatrywania ran, a 46 (7.85%) – rozcinania konstrukcji. Sprzęt ratunkowy wykorzystano 698 razy. Strażacy stosowali najczęściej zestawy opatrunkowe (n=74; 10.60%) oraz nosze i sprzęt unieruchamiający (n=62; 8.88%). Działania ratunkowe prowadzono najczęściej na otwartej przestrzeni (n=84; 51.22%) w godzinach popołudniowych (16:00-17:00). Wykazano istotną korelację (p=0.004) godzin w cyklu dobowym do liczby wezwań. WNIOSKI: Zdarzenia w rejonie wiejskim, do których najczęściej dysponowani są strażacy VFS to wypadki drogowe. Podejmowane na miejscu zdarzenia interwencje w przeważającej ilości dotyczą zapewnienia bezpieczeństwa oraz opatrywania obrażeń poszkodowanych. Liczba interwencji ściśle koreluje z porą dnia i jest największa w godzinach popołudniowych.

Background Musculoskeletal disorders mainly affecting the spine constitute one of the major social problems. Frequently, they constitute the cause of prolonged absence at work. Working in a fire brigade is specific because of the physical strain, working time under adverse conditions or stress related to rescue operations. Material and Methods The study group comprised 61 firefighters, 60 men (98.36%) and 1 woman (1.64%). The age of examined firefighters ranged 20–56 years old. The average age of the examined persons was 33.77 years old and the average working service in a fire brigade was 9.59 years. The following research instruments were applied as the study methods: own survey, the Numerical Rating Scale, the International Physical Activity Questionnaire (IPAQ) – long version and the Oswestry Disability Index (ODI) questionnaire. The Pearson correlation coefficient at significance level of p < 0.05 was used for determining the dependencies among the variables. Results Knowledge of ergonomic lifting and handling principles concerning heavy loads was reported by 84% of the group, the remaining 16% declared ignorance of such rules. The dependence of pain on the length of service of the examined was insignificant and amounted to 0.281 Pearson’s r. The results of the ODI questionnaire did not significantly change statistically with age. Conclusions The IPAQ confirmed the high level of physical activity of the study population. Even though the study group was small, the results of ODI showed a minor degree of disability. Med Pr 2018;69(4):365–373

Dbałość o dobry stan zdrowia i wysoką sprawność fizyczną strażaków wpływa na zmniejszenie prawdopodobieństwa doznania przez nich urazu podczas prowadzenia akcji ratowniczo-gaśniczych. W artykule przedstawiono zagadnienia związane z utrzymywaniem sprawności fizycznej przez strażaków, dotyczące wymagań i ich realizacji, zarówno w kraju, jak i za granicą. Zaprezentowano zalecane formy treningów prowadzące do lepszego przygotowania zawodowego strażaków. Zwrócono uwagę na fakt, że w Polsce nie ma regulacji prawnych dotyczących sposobu przeprowadzania treningu przez funkcjonariuszy Straży Pożarnej, jaki powinni wykonywać w celu utrzymania wysokiej sprawności fizycznej. Obowiązuje za to zasada corocznego sprawdzania sprawności strażaków i to od nich samych zależy, czy i jak dbają o swoja formę fizyczną. Care for good health and high physical fitness of firefighters reduces the likelihood of them being injured during rescue and firefighting operations.The article presents issues related to the maintenance of physical fitness by firefighters, regarding the requirements and possibilities of their implementation, both at home and abroad. Recommended forms of training leading to better professional preparation of firefighters were presented. Attention was drawn to the fact that there are no legal regulations in Poland regarding the method of conducting training by Fire Brigade officers that they should perform in order to maintain high physical fitness. Instead, the principle of annual checking of firefighters' fitness applies, and it depends on them whether and how they care about their physical form.

Objective: There are two theories (by Horowitz and by Foa) which attempt at explaining the process of posttraumatic stress disorder (PTSD) development by information dissonance. The purpose of the present study was to verify these theories via cross-sectional and longitudinal studies. Materials and Methods: The study based on a cross-sectional design was performed on a representative group of Polish firefighters. The study using a longitudinal protocol was conducted among police officers. The level of PTSD was assessed using the Questionnaire for PTSD Measurement (K-PTSD). The sense of coherence (SOC) was measured with the Polish adaptation of the Orientation to Life Questionnaire by A. Antonovsky. Results: In the cross-sectional study, the correlation coefficients between K-PTSD and SOC were –0.35 (p < 0.001) in the group of firefighters and –0.47 (p < 0.001) in that of police officers. In the longitudinal study, the relationship between SOC and PTSD symptoms appeared to be curvilinear and had the U-letter shape. Therefore, we used ANOVA for statistical analysis. Unfortunately, the F(2.31) = 1.900 and p > 0.05 indicated that the tendency we observed was not statistically significant. Conclusions: We would like to propose two possible explanations for the difference in the relationships between SOC and PTSD symptoms obtained in the studies employing the cross-sectional and the longitudinal protocols.

Objectives Rescuers work in 24-h shifts and the demanding nature of the occupation requires adequate recovery between work shifts. The purpose of this study has been to find out what kind of changes in autonomic control may be seen during work shift and its recovery period in the case of rescuers. An additional interest has been to see if aerobic fitness is associated with recovery from work shifts. Material and Methods Fourteen male rescuers (aged 34±9 years old) volunteered to participate in the study. Heart rate variability (HRV) was recorded for 96 h to study stress and recovery, from the beginning of a 24-h work shift to the beginning of the next shift. Aerobic fitness assessment included maximal oxygen uptake (VO2max) estimation with a submaximal bicycle ergometer test. Salivary cortisol samples were collected 0 min, 15 min, and 30 min after awakening on the 3 resting days. Results Some HRV parameters showed enhanced autonomic control after the work shift. Stress percentage decreased from the working day to the 2nd rest day (p < 0.05). However, maximal oxygen uptake was not associated with enhanced parasympathetic cardiac control (p > 0.05). Cortisol awakening response was attenuated right after the work shift. Conclusions The HRV findings show that recovery after a long work shift takes several days. Thus, rescuers should pay attention to sufficient recovery before the next work shift, and an integrated model of perceived and physiological measurements could be beneficial to assess cardiovascular strain among rescuers with long work shifts. Opis:
ObjectivesThe aim of the research was to analyze the impact of selected factors: years of service, the number of interventions, self-efficacy and stress, on occupational burnout. It was hypothesized that firefighters with more years of service and a bigger number of interventions would be characterized by higher perceived stress and burnout, and that self-efficacy would have an impact on reducing the level of perceived stress and burnout.Material and MethodsThe participants were firefighters (N = 576) from 12 Polish voivodeships, aged 20–58 years, with different seniority: up to 3, 4–8, 9–15 or >15 years of service. The following research tools were used: the Link Burnout Questionnaire, the 10-item Perceived Stress Scale, the Generalized Self-Efficacy Scale, and an independent questionnaire to gather additional information. A correlation between particular variables was carried out; the Kruskal-Wallis test was performed together with a post-hoc analysis to examine differences in the severity of individual variables depending on seniority, followed by a path analysis studied together with the identification of direct and indirect impacts.ResultsThe number of interventions did not affect the severity of experienced stress or any of the aspects of burnout. Work experience directly influenced the level of perceived stress (β = 0.219), disillusion (β = 0.076), and relationship deterioration (β = –0.156). The generalized sense of self-efficacy was found to impact both on reducing the sense of stress (β = –0.418) and on all 4 aspects of professional burnout: psychophysical exhaustion (β = –0.181), relationship deterioration (β = –0.16), the sense of professional inefficacy (β = –0.275) and disillusion (β = –0.143).ConclusionsThe results have shown that: 1) the number of interventions does not affect the severity of experienced stress or particular aspects of burnout; 2) years of service increase the severity of experienced stress and occupational burnout; 3) self-efficacy has an impact both on reducing the sense of stress and on all aspects of burnout.

Objectives The aim of this study was to evaluate the potential association between occupational stress and musculoskeletal symptoms in firefighters. Material and Methods Data were collected among Cypriot firefighters through a battery of adapted questionnaires completed anonymously. Results A total of 430 firefighters (a response rate of 68%) completed the survey (the age range: 21–60 years). A total of 11% of firefighters reported moderate to extremely severe stress through the Copenhagen Psychosocial Questionnaire and Depression, Anxiety and Stress Scale. A total of 40% of firefighters reported musculoskeletal symptoms, the most frequent being back pain. Multivariable-adjusted logistic regression models showed that occupational stress was associated with a 50% higher risk of musculoskeletal symptoms in firefighters after adjusting for age, smoking and obesity (OR = 1.52, p = 0.04). In addition, a positive dose-response relationship was found between occupational stress and musculoskeletal symptoms. Conclusions Occupational stress constitutes a significant risk for firefighters and is associated with higher prevalence of musculoskeletal symptoms at work.
