The shift work system may affect the temporal distribution of eating and diet quality. The paper aimed at reviewing a body of research examining the associations between night shift work and dietary habits among nurses. Data from the PubMed and Google Schoolar databases, as well as references lists in selected papers were searched. The authors used the following keywords: nurses, shift work, diet, nutrition. Papers published in English or Polish were selected for the review, and as many as 19 papers published in 2000−2017 were eventually identified. The studies varied greatly with respect to the study size, subjects’ age and the duration of night shift work. The major problem was the heterogeneity of the tools used for dietary assessment. Self-administered questionnaires were used and analyses were rarely adjusted for confounders. Alcohol consumption was the most frequently analyzed aspect (N = 8 studies), followed by the total energy (N = 7), protein, fat (N = 6), and carbohydrate intake, coffee and fruit consumption (N = 5). The results showed quite a consistent association of night work with higher coffee (caffeine) consumption, as well as lower alcohol, and fruit and vegetables consumption. Few studies also reported more frequent snacks consumption, later time of the last meal, eating at night, meals irregularity, and a poorer diet quality among night shift nurses when compared to the reference. The review showed some poor nutritional habits among nurses working night shifts. However, the topic warrants further attention, owing to the relatively small number of the studies performed so far, and their numerous methodological limitations. Med Pr. 2019;70(3):363–76

Objectives. The aim of this study was to investigate the circadian adaptation of tcr (core body temperature), BP (blood pressure), HR (heart rate) and subjective sleep quality after 7 days of working 12-h night shifts in offshore fleet workers. Methods. Night workers (N = 7) (18:00–6:00) and day workers (N = 7) (6:00–18:00) were recruited from a Norwegian offshore company operating in the North Sea. We measured tcr, BP and HR on days 1 and 7. Results. An increase of 0.6 oC (p = .03) was observed within the group of night workers from day 1 to day 7. Between the night and day workers there was a significant difference of 0.6 oC from day 1 to day 7 (p = .01). Sleep latency and sleep length also showed significant differences between the groups (p = .01 and p = .04). There was an interaction effect in tiredness during the shift (p = .02). Conclusion. The significant increase in tcr indicates an adaptation in the night workers to the new working schedule, and the extended working hours and sleep deprivation are hypothesized to be the main cause of the increased tcr. Light exposure, altered pattern of food availability and physical activity are likely to have contributed as well. Subjective sleep quality showed inconclusive results.

In this study, the effects of bright light (BL) on the rhythms in body temperature, plasma melatonin, plasma cortisol and subjective alertness, in 34 shift work nurses at a university hospital were assessed. They were exposed to very BL (4 500 lx) during 2 breaks (21:15–22:00 and 3:15–4:00) or dim light (300 lx). The subjects were studied under 24 h of realistic conditions during which their plasma cortisol and melatonin were measured at 3-h intervals; their body temperature was also measured during and after night shift work. Subjective alertness was evaluated with the Karolinska sleepiness scale. Administration of BL significantly suppressed night-time melatonin levels. A one-way ANOVA revealed that BL tended to increase cortisol levels and body temperature and significantly improved alertness. These results demonstrate that photic stimulation in a hospital setting can have a powerful influence on the adjustment of the circadian system.

Najlepiej poznane rytmy dobowe człowieka to: rytm sen–czuwanie, okołodobowa zmienność temperatury głębokiej ciała, dobowe zmiany ciśnienia tętniczego krwi oraz częstości skurczów serca, dobowe zmiany wydzielania hormonów: m.in. melatoniny, kortyzolu, hormonu wzrostu, prolaktyny. Temperatura głęboka ciała u ludzi ma swój charakterystyczny, sinusoidalny przebieg, z wartościami maksymalną około godz. 15:00-17:00 i minimalną, około godz. 03:00-05:00 rano. Analiza piśmiennictwa wskazuje, że na uzyskiwane wyniki dotyczące temperatury głębokiej ciała w dużej mierze ma rodzaj zastosowanej metody pomiaru. W literaturze można spotkać różne sposoby pomiaru temperatury głębokiej ciała, stosowane w zależności od protokołu badania. Jedną z nowszych metod pomiaru wewnętrznej i zewnętrznej temperatury ciała jest zastosowanie zdalnych czujników temperatury, transmitujących zmierzone wartości drogą radiową. Zaletami tej metody są: możliwość uzyskania ciągłego pomiaru temperatury głębokiej, obserwacja dynamicznych zmian temperatury głębokiej ciała zachodzących w cyklu okołodobowym oraz dokładność pomiaru, powtarzalność i wiarygodność uzyskanych wyników, co prezentowane jest w wielu doniesieniach naukowych.
The best known daily rhythms in humans include: the sleep-wake rhythm, the circadian core body temperature variability, daily fluctuations in arterial blood pressure and heartbeat frequency, and daily changes in hormone secretion: e.g. melatonin, cortisol, growth hormone, prolactin. The core body temperature in humans has a characteristic sinusoidal course, with the maximum value occurring between 3:00-5:00 pm and the minimum between 3:00-5:00 am. Analysis of literature indicates that the obtained results concerning core body temperature are to a large extent influenced by the type of method applied in the measurement. Depending on test protocols, we may apply various methodologies to measuring core body temperature. One of the newest methods of measuring internal and external body temperature consists in the utilisation of remote temperature sensors transmitting the obtained value via a radio signal. The advantages of this method includes the ability to perform: continuous core temperature measurement, observe dynamic changes in core body temperature occurring in circadian rhythm and the repeatability and credibility of the obtained results, which is presented in numerous scientific reports.

ObjectivesExposure to light at night (LAN) can disturb circadian endocrine and metabolic rhythms. Hyperuricemia (HUA) is an early-onset metabolic disorder. However, it is still not clear whether LAN exposure increases the prevalence of HUA.Material and MethodsThe authors used crosssectional data on the Beijing-Tianjin-Hebei occupational populations cohort from March–July 2017. A total of 7664 steel workers were finally selected to investigate the relationship between LAN exposure and the prevalence of HUA among steel workers. The authors collected demographic and socio-economic data, as well as information on lifestyle factors, anthropometric measures, and laboratory tests. The restricted cubic spline method was used to analyze the dose-response relationship between cumulative LAN exposure and the prevalence of HUA. Logistic regression analyses were used to fit the relationship between them.ResultsThe average age of the participants was 43.5±8.6 years; 7051 (91.7%) of them were males, 2749 (35.9%) reported to suffer from HUA, and 1241 (16.2%) were not exposed to LAN. There was a significant non-linear dose-response relationship between them. After adjustment for the confounding factors, including demographic data, lifestyle factors, etc., the lower LAN exposure was significantly associated with HUA (0–1931.7 days, OR = 1.180, and the 95% CI: 1.000–1.394; 1931.7–4343 days, OR = 1.215, 95% CI: 1.035–1.426).ConclusionsThis study revealed that a certain amount of exposure to LAN is independently related to the prevalence of HUA in steel workers in China.

Since 1901, the Nobel Prize has been awarded to scientists who have made the most important discoveries for the benefit of humanity. The 2017 Nobel Prize in Physiology or Medicine was awarded jointly to Jeffrey C. Hall, Michael Rosbash and Michael W. Young “for their discoveries of molecular mechanisms controlling the circadian rhythm.” It may be surprising to learn that those three scientists dedicated their entire careers to research on the fruit fly, Drosophila melanogaster. However, as their studies progressed, it became increasingly clear that the mechanism of the biological clock that they discovered in Drosophila is very similar to a timekeeping mechanism present in mammals, including humans. Through interdisciplinary work between scientists performing basic research on model organisms and medical doctors, we have learned over time that daily rhythms support human health while disruption of these rhythms is associated with a range of pathological disorders such as cardiovascular problems, metabolic, neurological, and many other diseases. This short review highlights critical milestones on the way to understanding biological clocks, focusing on the roles played by the three Nobel Prize winners.
Od roku 1901 Nagroda Nobla jest przyznawana naukowcom za najważniejsze odkrycia służące dobru ludzkosci. Nagrodę Nobla w dziedzinie fizjologii lub medycyny w 2017 roku otrzymali trzej amerykańscy uczeni Jeffrey C. Hall, Michael Rosbash i Michael W. Young "za odkrycie mechanizmu molekularnego, który kontroluje rytmy okołodobowe". Może się to wydać zaskakujące, ale ci trzej nobliści poświęcili swoje kariery naukowe badaniom nad muszką owocową, Drosophila melanogaster. Jednak w miarę postępu ich badań stawało się coraz bardziej oczywiste, że mechanizm zegara biologicznego, odkryty u muszki Drosophila, jest bardzo podobny do zegara, który posiadają ssaki, łącznie z człowiekiem. Interdyscyplinarna współpraca między naukowcami prowadzącymi badania podstawowe na organizmach modelowych i lekarzami prowadzącymi badania kliniczne ujawniła istotną rolę rytmów dobowych w utrzymaniu zdrowia człowieka. Dlugotrwałe zakłócenie tych rytmów stanowi czynnik ryzyka wielu patologii, takich jak choroby serca, cukrzyca, otyłość czy choroby układu nerwowego. Artykuł krótko podsumowuje odkrycia, stanowiące kamienie milowe na drodze poznania mechanizmu zegara biologicznego, ze szczególnym uwzględnieniem roli trzech noblistów 2017 w tym procesie.
