Opis:
The paper presents the concept of using the RCA method to failure analysis on the example of power plant. Nowadays, where the speed plays a big role in business, it’s not easy to get to the root causes of failure. RCA Method (Root Cause Analysis) is an analysis of root causes, it is an independent and comprehensive method of risk analysis used to study, identify and classify the root causes of incidents establishment in the work. The aim of the publication is to examine the benefits of using the analysis of RCA as a reactive method of identifying the causes, discovering problems and solving them in the company.
W publikacji przedstawiono koncepcję wykorzystania metody RCA do analizy awarii na przykładzie elektrowni. W dzisiejszych czasach gdzie pośpiech gra wielką rolę mało kto próbuje dotrzeć do przyczyn źródłowych awarii. Metoda RCA (Root Cause Analysis ) jest analizą przyczyn źródłowych, jest ona niezależną i kompletną metodą analizy ryzyka, służącą do badania, identyfikacji oraz sklasyfikowania przyczyń źródłowych zdarzeń zagrażających zakładowi pracy. Celem publikacji jest zbadanie korzyści płynących z zastosowania analizy RCA jako reaktywnej metody identyfikowania przyczyn, odkrywania problemów i ich rozwiązywania w firmie.

Opis:
Making a complex analysis of the problem in order to identify the root of its occurrence, is the process burdened with the risk of uncertainty. This risk is in case of the quality analysis, in which the choice and making a decision is not confirmed by reliable information (number). But these techniques do not include the risk of uncertainty, so their sequence should be expanded about the appropriate method, to take this into account. It was considered beneficial to demonstrate that this method is the Fuzzy Analytic Hierarchy Process method (FAHP). The aim of the study to implement the FAHP method in the sequence of traditional quality management techniques (Ishikawa diagram and 5Why method) and proposition new, not practiced yet the method to complex analysis of the problem and identify the root of its root cause. The subject of the study was the furniture front, on which the incompatibility was identified (shortened foil). From the categories to which the potential causes of the inadequate foil were assigned, by using the Ishikawa diagram and the FAHP method in an integrated manner, were defined the most important categories (method and machine) in a precise (numerical) way. Next, using the same tools, the relationship between the potential causes was analysed and selected the main causes of the problem. Then, by using the 5Why method the root of the problem was defined (lack of the new machine and failure to comply with labor standards). The obtained results could differ depending on the subjectivity, however, the method itself proved to be effective and can be used to solve other types of the problem.

As part of continuous quality improvement in well-managed enterprises, identifying unconformity should initiate actions to find their causes. Therefore, it was proposed to the enterprise located in Podkarpacie to use in the sequential way the Ishikawa diagram and 5Why method. The aim was to analyse of unconformity (porosity cluster) on the turbine outlet nozzle and identify the root of its creation. In the enterprise, the quality analysis of the products with a fluorescent method was carried out, but after identifying the unconformity, non-analysis of their reason for their occurrence was not practiced. Therefore, it was intentional to propose the use of sequence i.e. Ishikawa diagram and 5Why method to identify the root of unconformity. The subject of study was the turbine outlet nozzle, on which the fluorescent method the porosity cluster was identified. With the use of the Ishikawa diagram, the main cause of the problem was pointed (unconformity during production), and by the 5Why method the root cause of the problem, i.e. unconformity material from the supplier, was identified. The proposed method sequence is a simple and effective way to make analyses of unconformities and it can be used in different products and service enterprises.

Opis:
Effective quality management of the product is an element that allows profit within the enterprise. Identifying unconformity and establishing their source enables quality. However, identifying unconformity is sometimes neglected. Such was evident in the analysis of the service-manufacturing enterprise located in Podkarpacie. In the enterprise, quality was assessed via non-destructive testing (via fluorescence), but decisions on sources of non-compliance were made without utilizing additional methods that allow source identification. Hence, the way of recognizing unconformity was not precise. Therefore, method sequence (Ishikawa diagram and 5Why method) approaches were proposed to enable this. The aim of this paper is to show the effectiveness of such approaches in identifying sources of unconformity. The subject of study was tubing that is used in aircraft engines. Via the applied analysis, the unconformity in production and the source cause was traced to material supplied from outside. The proposed method sequence approach can be used in other enterprises for quality analyses purposes.

The automotive industry is one of the most important branches of the global industry. For this purpose, products are produced by an extensive network of suppliers. They supply components directly to the (OEM - Original Equipment Manufacturer) or to the secondary market of parts. In recent years, the economic situation in these companies has dramatically deteriorated and it is associated with a decrease in new car sales. These companies are constantly making decisions to improve their production processes using various quality tools. These actions may help to increase their competitive advantage and thereby improve their financial situation. The surveyed company produces drive half-shafts for different types of cars. The studies were carried out to analyse the causes of non-compliance - falling band from the housing on the drive shaft. In order to identify and verify the causes for non-compliance and undertake corrective action following methods were used: Ishikawa diagram, 5WHY and brainstorming.
