This article presents a comprehensive review of the FMEA method, including its history, principles, and steps. The article also discusses the benefits of the FMEA method for companies, including improved product quality, reduced costs, and enhanced customer satisfaction. The FMEA form is an essential component of the FMEA process, as it helps to ensure that all relevant information is captured and organized in a structured manner. This article will introduce the FMEA method and provide an overview of the FMEA form, including its various sections and how it is used in the FMEA process. The aim of this article is to provide a comprehensive analysis of FMEA and the ways in which it is applied, to ensure the applicability of FMEA to manufacturing processes for manufacturers. In this way, manufacturers will benefit in time, budget, energy and profitability. In addition, with FMEA, problems will be prioritized according to their criticality, and errors will be intervened at the right time and in the fastest way. In the article, an FMEA form will be created for the steel rope production process as a study case, and the benefit and profitability it will provide to the manufacturer will be revealed. In this way, the effect of FMEA in a production process will be embodied.

The purpose of this paper is to propose the methodology for the setup analysis, which can be implemented mainly in small and medium enterprises which are not convinced to implement the setups development. The methodology was developed after the research which determined the problem. Companies still have difficulties with a long setup time. Many of them do nothing to decrease this time. A long setup is not a sufficient reason for companies to undertake any actions towards the setup time reduction. To encourage companies to implement SMED it is essential to make some analyses of changeovers in order to discover problems. The methodology proposed can really encourage the management to take a decision about the SMED implementation, and that was verified in a production company. The setup analysis methodology is made up of seven steps. Four of them concern a setups analysis in a chosen area of a company, such as a work stand which is a bottleneck with many setups. The goal is to convince the management to begin actions concerning the setups improvement. The last three steps are related to a certain setup and, there, the goal is to reduce a setup time and the risk of problems which can appear during the setup. In this paper, the tools such as SMED, Pareto analysis, statistical analysis, FMEA and other were used. Implementing the proposed methodology can change the attitude of the management. The risk analysis helps to prevent problems with a setup. This methodology can be used in production companies which make changeovers, particularly, in the companies where SMED hasn’t been introduced yet.

In the paper the basic design features and essential assumption of calculation models as well as the factors influencing quality improvement and improvement of calculation process of bearing capacity of rolling slewing bearings are discussed. The aim of conducted research is the identification and elimination of sources of errors in determining the characteristics of slewing bearing capacity. The result of the research aims at determining the risk of making mistakes and specifying tips for designers of slewing bearings. It is shown that there is a necessity fora numerical method to be applied and that real conditions of bearing work must necessarily be taken into account e.g. carrying structure deformations as the first ones.

The new maintenance system of semi-trailer combination as a most used road freight transport means is the subject of interest in this article. The real experiences from practice and creation of the FMEA analysis of the system using software support are the basis of wear and damage analysis of air brake system components used on the semi-trailer combination. The desire to improve market position of the company, customer satisfaction, but also the product specifications, government regulatory requirements, agency recommendations, and legal implications are the grounds for continuous improvement of the product quality, reliability and safety. These reasons push the product manufacturers to perform risk analyses, which helps them identify and minimize system failures during the life cycle of product. One of the risk analysis techniques recommended by international standards to identify possible failure causes is the FMEA methodology. It helps to locate the failures and reduce their consequences, so that the intended function of the machine is fulfilled. In this paper was presented the wear and damage analysis of individual components of air brake system and creating of the FMECA analysis for semi-trailer combination. Base on them, new methods to detect failures of components of air systems, preventive measures and set up of a new planned preventive maintenance system for air brakes was designed. The cost of the proposed maintenance system is 2.2 times higher than the cost of the current maintenance system, but it minimizes the failures and their consequences (traffic accidents).

In the article the factors influencing the improvement of the quality and improvement of process of calculation of bearing capacity of rolling slewing bearings. An aim of conducted deliberations is the identification and elimination of sources of errors in determining the characteristics of slewing bearing capacity. W artykule przedstawiono czynniki wpływające na poprawę jakości i udoskonalenia procesu obliczania nośności łożysk tocznych wieńcowych. Celem prowadzonych rozważań jest znalezienie i wyeliminowanie źródeł powstawania błędów przy określaniu charakterystyk nośności łożysk tocznych wieńcowych. Wynikiem prowadzonych badań jest określenie stopnia ryzyka popełniania błędów oraz wskazówki dla projektantów łożysk tocznych wieńcowych.

In the article elements of quality engineering have been presented, such as: Pareto chart and FMEA method for evaluating the scraper conveyor’s failure frequency. A percentage share of particular failures in the scraper conveyor has been estimated by means of the Pareto chart. FMEA was used to identify the causes of failures and their effects. Quality engineering elements can be widely applied in an initial analysis of technical facilities’ failure frequency in various industry branches.
W artykule przedstawiono zastosowanie elementów inżynierii jakości takich jak: diagram Pareto-Lorenza oraz metoda FMEA do oceny awaryjności przenośnika zgrzebłowego. Za pomocą diagramu Pareto-Lorenza oszacowano procentowy udział poszczególnych awarii w przenośniku zgrzebłowym. Metoda FMEA posłużyła do wskazania przyczyn wystąpienia awarii oraz określenia ich skutków. Wykorzystane elementów inżynierii jakości do wstępnej analizy awaryjności obiektów technicznych może znaleźć szerokie zastosowanie w różnych gałęziach przemysłu.

Preventive methods are important group of methods used in the process of design and which are known as quality design methods. The aim of these methods is identification of potential failures and cause-and-effect relationships in consistent and systematic way, and then taking appropriate preventive or corrective actions. The well-known examples of preventive methods also used for modelling and analysis of the criticality (risk) factor are FMEA analysis and FTA analysis. A matrix FMEA analysis method has been presented and discussed in the work. The basic assumptions related to this method were characterized and algorithms for each stages of analysis have been presented. It has been presented practical application of FMEA method on example of selected components of hydraulic system.

Failure Mode and Effects Analysis (FMEA) is a systematic approach for evaluating the potential failure modes in a system, and is mainly employed in three distinct tasks labelled: (1) Functional FMEA – evaluating those failures associated with product functional definition, (2) Design FMEA – analysing those failures associated with design definition and (3) Process FMEA – assessing potential failures in manufacturing and assembly processes. The literature review has shown limited works on the field of synchronising these different tasks into a working model. To address this gap, this research developed a framework for integrating these tasks of FMEAs, and then qualitatively validating the proposed framework. This research adopted a semi-structured questionnaire to collect experts’ feedback and validate the proposed framework. The t-test was then employed to evaluate the collected feedback. The findings highlight that the proposed framework is applicable and could facilitate the synchronisation of the different tasks of FMEA. This research presents a methodological approach for executing and synchronising FMEAs. Therefore, the proposed framework is practically relevant as an aid for the practitioners in catching the cascading failures and reducing the relevant impact.

Opis:
Water scarcity in small Aegean islands is considered a major problem for their development. On the other hand high potential renewable energy sources are available. Therefore a floating desalination unit was developed in order to treat the problem in an ecological manner. The floating unit has been operating for four years at sea close to a small island. Marine environment and autonomous remote operation led to a more complex design with several sensors and safety mechanisms. This paper illustrates and applies Failure Modes and Effects Analysis (FMEA) modeling to Floating Ecological Desalination Unit. FMEA is based on monitoring data and also taking into account failure dependencies between components during the assessment of desalination system reliability. Then two systems, one with safety mechanisms and one without are analyzed by Reliability Block Diagrams (RBD) and the reliability of each system is computed. The comparison between a simple system and the remote autonomous system illustrates the implications of the additional monitoring mechanisms and the impact of these mechanisms to reliability and risk assessment on the floating wind powered offshore desalination unit.

It is state-of-the-art that a Level 1 PSA also includes a fire PSA for all plant operational states. In performing Fire PSA not only the malfunction of the components has to be assessed but also all supply systems and cables have to be traced for a given component. In the past it was assumed in the case of a fire in a compartment that all components and cables in that compartment are not functional anymore. However, this is in many cases a too conservative approach and could lead to overestimated fire induced core damage frequencies. Therefore, a method is necessary to assess in a more realistic manner the effects of cables failures caused by fire. Such a procedure requires a sound data base on equipment, list of cables and their properties as well as cable routing. Two methods are described which are currently developed. One of them is a cable failure mode and effect analysis which is easier to apply in practice.

One of the requirements of the process approach is to identify the methods and evaluation criteria for process measurement. The effectiveness described as the ability to execute scheduled tasks and the objectives may be the measure used to evaluate processes. The article presents a few concepts of efficiency indicators that can be used in assessing the activities carried out within the framework of the implementation of new projects, according to APQP&PPAP guidelines. This paper proposes four concepts of indicators to assess the effectiveness of the above-described process, including index based on the Taguchi loss function.

Everyday, companies are submissive to several risks that may influence the good sequences of their activities. Therefore, risk management to be proved as a good management philosophy that can reduce or limits negative effects of risks. In this paper, we will focus on the Failure Mode and Effects Analysis (FMEA) as an efficient method of risk management in the Tunisian Company of Electricity and Gas (STEG), exactly in the base of electricity transmission district Gremda. After my discussions with the safety manager, the leaders and the executives in this district, we estimate that this method allows to put the risks that may affect the performance of this company and to structure these risks according to their Risk Priority Number. This step is crucial in the risk management process. This research will be useful as an audit for the safety standardization system of this company.
Każdego dnia, firmy narażone są na liczne zagrożenia, które mogą wpływać na kolejność ich działań. Dlatego też, należy udowodnić, że zarządzanie ryzykiem jest dobrą filozofią zarządzania, która może zredukować lub ograniczyć negatywne skutki ryzyka. W tym artykule skupimy się na Failure Mode i Effects Analysis (FMEA) jako skutecznej metodzie zarządzania ryzykiem w Tunezyjskim przedsiębiorstwie energii elektrycznej i gazu (STEG), na bazie przesyłu energii elektrycznej w okręgu Gremda. Po rozmowach z menedżerem bezpieczeństwa, liderami i kadrą kierowniczą w tym okręgu, szacujemy, że metoda ta pozwala przedstawić zagrożenia, które mogą mieć wpływ na działanie tej spółki i uporządkować je według ich Numeru Priorytetu Ryzyka. Krok ten ma kluczowe znaczenie w procesie zarządzania ryzykiem. Badania te będą przydatne jako audyt dla systemu normalizacji bezpieczeństwa tej firmy.

Customers and their perception towards service are considered as a determinant of service failure, and so, service failure and its prevention must be looked into from the perspective of the customers. This paper presents a customer-centric service failure prevention framework, which aims to provide a holistic way of service failure prevention by integrating service delivery assessment and failure analysis from a customer perspective, encompassing failure identification, assessment and prioritization of failures as a basis for corrective actions. Customer journey, service clues, and customer oriented-FMEA are employed to develop the proposed framework. The approach was applied to an enrolment process showing that using customer journey assists in determining customer processes, needs, wants and touch points in the service, and when used together with service clues further facilitates systematic and effective unveiling of potential failures that are important to customers. Assessment of failures and its prioritization with customer perspective leads to better prioritization that is reflective of the voice of customers. The case study shows that higher risk is imposed by actions emanating from the employees, reinforcing further that service failures not only concern functionality of the service but equally important also are the encounter of customers with service employees and the environment.
