Opis:
A current problem in candidiasis treatment is increasing resistance to azoles, which are often prescribed to patients. The study underlines the high resistance of yeasts to fluconazole, which achieved high MIC (minimal inhibitory concentration) values both alone and in combination with essential oils (EOs). Antifungal activity of Hyssopus officinalis, Thymus vulgaris, Salvia officinalis and Rosmarinus officinalis EOs was determined against 13 clinical isolates of Candida albicans and reference strain Candida albicans ATCC 10231. The synergistic effect was investigated for the combination of itraconazole and fluconazole with Hyssopus officinalis and Thymus vulgaris EOs. Based on the fractional inhibitory concentration index, the synergistic effect was achieved in all of the samples exposed to itraconazole with Hyssopus officinalis (FICI 0.3±0.06). On the other side, the additive effect was proven in use of itraconazole with Thymus vulgaris (FICI 0.75±0.35) and fluconazole with both EOs tested (FICI 0.81±0.19; 0.88±0.57) This study shows the importance of monitoring the synergistic effect of antifungals combined with EOs, because it is a possible solution for reducing the resistance and improving the disease prognosis.

Opis:
The resistance of Candida albicans and other pathogenic yeasts to azole antifungal drugs has increased rapidly in recent years and is a significant problem in clinical therapy. The current state of pharmacological knowledge precludes the withdrawal of azole drugs, as no other active substances have yet been developed that could effectively replace them. Therefore, one of the anti-yeast strategies may be therapies that can rely on the synergistic action of natural compounds and azoles, limiting the use of azole drugs against candidiasis. Synergy assays perperformed in vitro were used to assess drug interactions Fractional Inhibitory Concentration Index. The synergistic effect of fluconazole (1) and three synthetic lactones identical to those naturally occurring in celery plants—3-n-butylphthalide (2), 3-n-butylidenephthalide (3), 3-n-butyl-4,5,6,7-tetrahydrophthalide (4)—against Candida albicans ATCC 10231, C. albicans ATCC 2091, and C. guilliermondii KKP 3390 was compared with the performance of the individual compounds separately. MIC90 (the amount of fungistatic substance (in µg/mL) inhibiting yeast growth by 90%) was determined as 5.96–6.25 µg/mL for fluconazole (1) and 92–150 µg/mL for lactones 2–4. With the simultaneous administration of fluconazole (1) and one of the lactones 2–4, it was found that they act synergistically, and to achieve the same effect it is sufficient to use 0.58–6.73 µg/mL fluconazole (1) and 1.26–20.18 µg/mL of lactones 2–4. Based on biological research, the influence of the structure on the fungistatic activity and the synergistic effect were determined.

In recent years, there has been a rapid increase in the incidence of Candida infections. The different species of the genus Candida vary in their virulence abilities and susceptibility to antifungal agents, depending on several external factors. The result of such modifications may be cross-resistance, which is understood as an acquired resistance to a certain antimicrobial agent after exposure to another drug. The aim of this study was to determine the possibility of cross-resistance between fluconazole, voriconazole, itraconazole, and methotrexate in Candida albicans and Meyerozyma guilliermondii (syn. Candida guilliermondii). Fifteen strains of M. guilliermondii and eight strains of C. albicans, including the standard strains, were tested. For all strains, the minimum inhibitory concentrations (MICs) for fluconazole, voriconazole, and itraconazole were determined before and after stimulation with methotrexate. The median MICs in M. guilliermondii before and after stimulation were 9.333 and 64 mg/L (p = 0.005) for fluconazole; 0.917 and 1.667 mg/L (p = 0.001) for itraconazole, respectively. No significant change in MIC was observed for voriconazole. For C. albicans strains, the median MICs before and after stimulation were 0.917 and 64 mg/L (p = 0.012) for fluconazole; 0.344 and 1.135 mg/L (p = 0.018) for voriconazole, respectively. There was no significant change in MIC values for itraconazole. Thus, this study demonstrates the presence of cross-resistance between voriconazole, itraconazole, fluconazole, and methotrexate for the selected strains. Methotrexate exposure induces different responses when certain drugs are used for various species. Therefore, if a patient was previously exposed to methotrexate, there may be a higher risk of treatment failure with fluconazole than with other azoles such as voriconazole for fungemia caused by M. guilliermondii or itraconazole for C. albicans infection.
