The global spread of cancer makes every preventive action aiming at stopping the increase in number of cases and deaths related to cancer extremely important. Preventive actions may be especially effective in diseases etiologically related to lifestyles, including above all eating habits and level of physical activity. It is worth implementing them in compliance with the participatory approach, which constitutes a key component of health promotion and health education. The aim of this article is to present the reasons for and the need to implement prevention based on the idea of participation. This is justified by the results of the research conducted as part of a grant sponsored by Polska Liga Walki z Rakiem [Polish Cancer League] (“Onkogranty II” contest), entitled Promotion of healthy eating habits and physical activity in primary schools throughout the city of Bytom – a diagnosis of educational needs in the context of planning effective actions in primary cancer prevention. It seems that involving students in the process of identifying needs, determining priorities, planning and evaluating these actions is currently one of the most effective ways of developing awareness of cancer prevention.

Governments all over the world have taken unparalleled measures to combat the spread of the co[1]ronavirus. Severe restrictions have been imposed, reducing face-to-face contact where possible. This situation seems to be particularly acute for children and youth, who may not understand the orders and bans. Many studies analyzed in this paper have provided evidence that children’s and adole[1]scents’ health has been negatively affected within the last months due to school closures and the need for social and physical distancing because of the coronavirus. Adults should be aware of children’s and adolescents’ difficulties and support them during this unique time. In the face of serious threats currently emerging due to COVID-19, health promotion is essential, as people regain control of their health, protect themselves and manage the potential consequences of the disease. The schools are faced with a huge task of promoting health, preparing students to cope with difficult and uncertain situations, including strengthening their health literacy. Health literacy, which is a set of skills, varies from individual to individual, which means that interventions in this area should focus on improving people’s skills and capacities through educational activities, discussed in this article.

The creation of conditions aimed at ensuring the health of the population is a supreme value. Polish legal solutions make it a constitutional obligation of the State and local self-government. The current Constitution of the Republic of Poland of2 April 1997 does not define the limits of this obligation; it only states that the conditions for ensuring everyone’s right to health protection and the scope of health services that are financed from public funds are to be specified by statute. In fact, the system leaves almost complete freedom in this area to the legislator. In the article the solutions adopted by local self-government units in 1991–2018 are evaluated. In order to fulfil certain competences and tasks imposed by law on the commune, poviat and self-governing regional bodies, the legislator has determined two spheres of their activity: (i) implementation of their own tasks in the field of health protection and health promotion, and (ii) establishment and running of entities which before 2011 were termed public health care facilities and after 2011 changed their name into medical entities which are not entrepreneurs. In neither of these spheres of activity is the participation of local government units identical. In the first sphere, the differences result from both the constitutional acts shaping their own tasks in the field of health protection, as well as health programmes which they adopt for their implementation. In the second sphere, however, the differences are a result of statutory solutions that base the model of health care on values that are not uniformly understood. Before 2011, they were conducted and maintained fully, or to a limited extent, from public funds, whereas after 2011 they were maintained from public funds only, when a budgetary unit was selected as an organisational formula of a self-government medical entity. The solutions of the Act of 2011 in its original wording pointed tended to favour commercialisation of the public health sector. The negative side of the solutions adopted is the extraordinary lack of diligence in exercising legislative diligence, especially in the scope of defining the competences and tasks of local self-government units in relation to the medical entities created/ run by these units.

The societies of developed countries, including Poland, are characterized by the dissemination of manifestations of the consumer lifestyle, which leads to a systematic deterioration of health and quality of life. The health and health safety should be systematically monitored and given in-depth scientific analyses, also from the perspective of pedagogy and sociology. In the scientific discourse there are various issues concerning both health hazards (inter alia related to globalization changes and lifestyle) and health promotion (for example, shaping pro-health attitudes and dissemination of participation in physical culture). Promoting a healthy lifestyle must be accompanied by investments in the material cultural-educational and sports-recreational base as well as by educational influences shaping the necessary level of knowledge and skills in the field of health.
