This paper aims at analysing the evolution of palliative care in the international context and their role in the path of care for the patient and the family. Method: born in 1967 by Cicely Saunder, palliative care were aimed at assisting the terminally ill, accommodating both the needs of the patient and the family. Not only to be cured or healed, but to be taken care of. The paper examines the definitions of palliative care provided by the World Helth Organization. We observe that palliative care is not only an effective and timely response to the clinical, psychological, social and spiritual needs of the sick person and their family in an advanced and terminal stage, but an integrated care to support specialist treatment in the presence of an advanced disease picture; a space for in-depth study for the sick person and the family so that the sick person can consciously and freely choose the available treatment proposals, their limits and their consequences. A treatment path in which the transparency of the proposals is a condition for building a shared consensus with the patient and adequate communication with the family. Palliative care has acquired its own identity, its own role in the path of care for the patient and the family, pursuing the proportionality of therapeutic options and the support of the patient and the family without discrimination, with equity and equality.

The article covers the issue of the initiative of organising care for small children at care homes in Krakow in the first half of the 19th century. Krakow’s civic-minded townspeople, referring to examples from all over Europe and facilities organised in Warsaw, implemented a project setting up a care shelter in Krakow. The analysis of archive material indicates that towards the end of the 1830s and the beginning of the 1840s, in Krakow, one could speak of the beginnings of a movement aimed at extending care over children of working families, including helping to feed them, morally educate them and provide for them with a basic education. Members of Krakow’s Charity Society actively participated in this work, recruiting people interested in creating care shelters, and drawing up projects for such institutions. In the end, care shelters were established and operated in the second half of the 19th century on the initiative of the private individuals who founded the Committee for Shelters for small children in Krakow. It must be noted that these included members of the Charity Society.

Pursuant to Article 68(3) of the Constitution of the Republic of Poland, a pregnant woman is entitled to special care from the State. Therefore, it seems extremely important to indicate uniform, universally binding standards in order to improve the quality of services provided and to respect the fundamental rights due to women and their children in maternity care. Unfortunately, as the analysis and reports concerning the observance of patients’ rights during the perinatal period show, the current standards are not always properly implemented by medical institutions, and the legal guarantees of observance of perinatal care standards are not sufficient. The aim of the article is to discuss the legal regulations in force in the field of perinatal care, to indicate their subject and subject scope and to conduct a legal analysis of the proposal of new organisational standards of perinatal care, which are to enter into force as of 1 January 2019. The basis for the discussion is the presentation of the legal form of the existing standards of perinatal care. It is extremely important to answer the question whether the principles of medical knowledge should be enacted in the form of standards of medical procedure in the form of universally binding normative act, or whether they should constitute guidelines and recommendations that are legally non-binding. Perinatal care standards in the context of the duty of doctors and medical staff to apply current medical knowledge and to act with due diligence deserves additional attention. The article contains a detailed analysis of the proposal of new standards and presents de lege ferenda postulates, especially in the field of legal guarantees of observance of perinatal care standards in Poland.

The Second Polish Republic developed an advanced and, in many ways, modern system of social care; however, the services which the citizens were entitled to seemed to be privileges available only to a small part of the population. The origins of this situation are to be found in the specific social and occupational structure of the population, low industrialization rate and the modest financial capabilities of the state and local governments. These resulted in a limited number of people with access to social insurances, a limited scope of public health care, a selective nature of access to unemployment insurance or radical differences in access to social care. And it is this unavailability of the social offer which determines the consideration of those benefits in Poland as privileges rather than commonly available rights of the Polish citizens.

 In this essay I want to concentrate on observers’ baseline assumptions on how we should be, or should have become in order to be accounted as morally ‘good.’ I will point out the significance for adult children who decided to not care for their elder parents. In three selected studies I show that observers, in trying to explain the decisions of others, or their moral development, respectively moral standing, misjudge or ignore their own implicit baseline assumptions. These assumptions are symptomatic of an implicit belief in all of us that wishes to see that ’good begets good’ for most of us, and infers, thereafter, that ‘bad begets bad’ for some who would show ‘no good.’ It is this implicit belief that guides the observers to make assumptions about the morally doubtful upbringing of a person, or their negative behavior that they wish to explain by flaws in the person’s personality. This biased belief says “it is this way, and only this way”, but, in fact, one cannot be certain about it. The baseline assumptions that observers bring along are basically the biased observer’s points of view which can be explained with the ultimate attribution error.

The article presents a discussion on the importance of early therapeutic interactions for the development of a child at risk of disability or with a disability and his/her family. It indicates the importance of the time of taking action in relation to a child with developmental disorder for the period from birth to the age of three years. The article pays special attention to the place of early intervention and early support in the development process, its preventive, compensatory and corrective role. It describes barriers to the development of this system in Poland and points to potential solutions to these difficulties.

Many conceptual analyses of the structure and mode of operation of the contemporary society outline a worrying and discouraging framework. They describe the decadence of values and they depict a world populated with individuals who are narcissistically self-oriented, looking for the satisfaction of their own desires, unable to desire, build and keep stable, continual and deep relations and affects. Nevertheless, recent psychological research, supported by the evidence of neuroscientific research, has evidenced that the origins of the psychic development of the individual are connected to the domain of the relational experience. New-borns become persons in and thanks to the context of relations where they live and grown up. Thus, relations are an essential feature and a fundamental and peculiar experience for the human being. This is evident if we try to analyse aspects and dimensions, such as attachment, trust, desire and care. Attachment allows not only the survival but also the internalization of relational patterns which are necessary for life. The trust experience in the primal relations allows the vital energy to be addressed towards the world and to build ties thanks to the experience of faith in the other. Experimenting satisfaction and frustration, possibility and limits in the interpersonal relations fosters the development of desire and of the capability of wait, renounce, choice, care, as well as the necessity/opportunity of recognizing/knowing the other. Feeling himself or herself understood by a caring other prepares the ground for the experience of personal safety and thanks to some special pedagogical reinforcements, it opens to the possibility of taking care of someone else, in a structure of mutual exchanges and material, affective and spiritual support which is necessary for any human being. Significant socio-cultural changes occurred in the last decades have influenced those fundamental experiences and contributed to produce new relational styles and models which are characterised by precariousness and discontinuity. It is thus of primary importance to promote and sustain a reflection and education on these themes with the aim of fostering our needs and relations, whose expression is essential for the wellness and selfrealization of individuals and communities. And this is not only in order to achieve a harmonic development of growing-up subjects but even for adults who want to fully accomplish their life and the related needs of sharing, intimacy and generativity. These results have to be achieved by taking into consideration the peculiarity of modern life and by searching for creative solutions thanks to which, at least partially, we can try to combine the new claims with the essential experience of “being” in a relation.

In 2019, twenty-five years have passed since the implementation of the first legisla-tive act introducing for the first time family doctors (primary health care physicians) into the Polish model of basic health care. The beginning of changes in this area, falling in the nineties of the last century, was not easy and has not yet been complete but will continue until the end of 2024. Over the years, not only the conditions for the education and professional development of primary health care physicians, in-cluding family doctors, have undergone changes. Also the organisational and legal forms of providing basic health services by this group of doctors were changing, as well as the conditions allowing patients, the recipients of medical services, to choose their doctor from among those with whom health insurance institutions (health insurance funds, or the National Health Fund) concluded agreements on the provision of basic health care services, or those employed by a medical entity with whom the National Health Fund signed relevant agreements.The scope of changes introduced at that time was wide, and when it started at the beginning of the 1990s it was almost impossible to achieve the goals without the support of the World Bank and the PHARE programme, alongside the efforts of the Ministry of Health and Social Welfare and three regional health care consortia (unions). Their work and contribution of each of them and their activities were sig-nificant and together constituted a solid basis for further work on the improvement of the model of a universal primary health care after 1998.The Act on primary health care of 2017 has completed the process of changes in this respect. For the first time, the objectives of the basic, or primary health care have been clearly defined. Although the implementation of each of these objectives is to serve the patient who is the recipient of the services, the value of the services pro-vided by primary care physicians varies. Family doctors (general practitioners) are to provide the patient and the patient’s family with health care services, coordinating at the same time all the stages of the process of their provision. However, the social mission of their profession is to ensure the implementation of broader tasks, such as health promotion adapted to the needs of various groups of society, education of the medical services recipients in the field of responsibility for their own health, as well as shaping pro-health awareness, setting health priorities of the population covered by care and implementation of preventive actions.

The issue of institutional care of the youngest children has been rarely discussed in scientific studies. In the context of the contemporary debates about improving the quality of toddlers’ care and education, it is a good idea to remind about the origin of nurseries and the development thereof. The first nurseries were established in Poland over 100 years ago. From the beginning, before Poland even regained independence until present day, they have changed their functions and organization. The aim of the article is to show the changes to the function of crèches against a political, social and cultural background since the very beginning of the existence of nurseries until the outbreak of World War II. The method used in this article is historical explanation, and the technique is an analysis of documents: literature, magazines, legal acts. The responsibilities of crèches discussed in this article include care, health -related, educational, upbringing and family-related activities This is the first of two articles related to changes in the function of crèches in a historical approach.

Writer raise an issue on inclusion and exclusion in three different levels. The firs level represents fulfiling child’s individual needs, the second level is about protection of other children’s interests and needs. Third level stends for actual social context with its own possibilities and limits. Writer points out the fact that ideology and its proclamations often don’t mach the applied proffesional solution. Experts face the pressure of political ambitions. Author gives several particular examples of “segregation” of – Romany (gypsy) ethnic minority, mentally disadvantaged children, disadvantaged children of intact popolation, international adoption. The writer is conviced that child in foster care is a part of whole children assembly and it is not appropriate to narrow the search for the optimal solution only for the child in foster care.

The following paper is about care1. It proposes a political vision to move towards a care-centred society that will allow the flourishing of everyone while keeping the planet inhabitable and thriving. For this utopian horizon that we name an ecology of care, we propose the creation of a Basic Income system that is constituted outside of and beyond the realm of the nation-state, as a means of changing humanity’s relationship to itself and transitioning from capitalism to a commons-based society. It argues for a dis-embedding of work—and the time allocated to it—from money, through a reformulation of the production of money in the form of an income distributed as an equal share to all those who are part of the planetary commons. Finally, it connects this Basic Income proposal with degrowth as a radical and necessary reformulation of society that considers its ecological roots and replaces the obsession with endless economic growth with the principle of taking care of people.

The development of increasingly intelligent and autonomous technologies will eventually lead to these systems having to face morally problematic situations. This is particularly true of artificial systems that are used in geriatric care environments. The goal of this article is to describe how one can approach the design of an elder care robot which is capable of moral decision-making and moral learning. A conceptual design for the development of such a system is provided and the steps that are necessary to implement it are described.
