Introduction. Clear cell carcinoma, not otherwise specified/hyalinising clear cell carcinoma of the salivary gland (HCCC) is a malignancy that arises in minor salivary glands. It rarely leads to distant metastases or cancer-related death but has the potential for recurrence and focal metastases. Aim. A case is reported. Description of the case. A 72 years old female patient has reported to the Clinic of Otolaryngology with a tongue lesion. The patient had no history of malignancy. The lymph node has been surgically removed for further examination. Cords and nests of clear cells and cells with eosinophilic cytoplasm in a hyalinized stroma were identified within the lymph node. After the diagnosis the patient has been transferred to another Oncology Hospital for further treatment. Conclusion. The diagnosis of clear cell carcinoma may be challenging because many of it‘s features frequently overlap with other salivary gland lesions.

Introduction. The most common malignancy of the biliary tract is gall bladder carcinoma and the main subtype according to the histological classification is Adenocarcinoma. Pure squamous cell carcinoma of the gall bladder is very rare entity accounting for only 1.1-3.7% of the gall bladder carcinomas.It is highly malignant with poor prognosis due to high proliferative rate and local invasiveness to the adjacent organs. The patients are usually diagnosed at an advanced stage with a bulky tumor owing to its aggressive behavior. Aim. In this paper, we describe a female patient with primary pure squamous cell carcinoma of the gall bladder. Description of the case. A 42-year old female patient presented with chief complaints of pain in abdomen associated with nausea and vomiting and gradually progressive jaundice since 02 months. Contrast Enhancing Computed Tomography (CECT) abdomen showed an enhancing mass lesion in gall bladder involving adjacent organs for which she underwent extended cholecystectomy with pancreaticoduodenectomy. Conclusion. Diagnosis as well as the management of this exceptionally rare type of tumour is undoubtedly challenging because of non-specific clinical as well as imaging findings. This case report is an attempt to add to the literary evidence for better pathological as well as clinical understanding of this rare and aggressive entity thereby providing additional material for the early diagnosis as well as the development of effective targeted therapies which will certainly help in increasing the lifespan of these patients.

Introduction. Renal cell carcinoma (RCC) is the most common malignant renal tumor in adults accounting for 80-90% of primary malignant adult renal neoplasms. RCC represents a very heterogenous groups of tumors with a number of distinct histological varieties, of which the major 3 subtypes are clear cell RCC (70-80%), papillary RCC (13-20%), and chromophobe RCC (5%). Imaging features are varied from solid and relatively homogenous appearance to markedly heterogenous appearance with cystic changes, hemorrhage and necrosis. The use of diffusion weighted imaging (DWI) for RCC subtyping and also for differentiation of high grade and low grade tumors has been showed to be useful in many studies in the literature. Aim. In this study, we aimed to determine the comparative contribution of DWI in differentiation of the clear cell RCC from the major non-clear cell RCC subtypes at standard high b-value (1000 s/mm2) versus low b-value (500 s/mm2). In addition, we also aimed to assess the diagnostic performance of DWI for differentiating high grade clear cell RCC from low grade clear cell RCC based on Fuhrman grades in our patients. Material and methods. 62 cases with a prediagnosis of RCC according to MRI findings including DWI sequence with histological verification and subtyping of renal cortical tumor following a total or partial nephrectomy were included in the study. Results. Among 62 cases, 46 were male and 16 were female, with mean age of 59.5±15.7. Pathological diagnoses of 62 cases were as follows, clear cell RCC, (44) papillary cell RCC (14) and chromophobe cell RCC (4). They were divided into two groups as clear cell RCC group (44 cases) and non-clear cell RCC group (18 cases). There was no statistically significant difference between the mean ADC values of clear cell and non-clear cell groups at b-value of 1000 s/mm2 (p>0.05). However, the mean ADC level for clear cell RCC group at b-value of 500 s/mm2 were significantly higher than for non-clear cell RCC group (p<0.05). When a value of 0.99x10-3 mm2/s was set as cut-off for ADC at b-factor of 500 s/mm2, differentiation was achieved with a high sensitivity (91%) and specificity (56%). Regarding the diagnostic performance of DWI for differentiating high from low Fuhrman grades clear cell RCCs, there was no statistically significant difference between the ADC values of Grade I-II clear cell RCC cases and Grade III-IV clear cell RCC cases at b-factor of 1000 s/mm2 (p>0.05). However, ADC values for grade III-IV group was statistically significantly lower than ADC values for Grade I-II group at b-factor of 500 s/mm2 level. Conclusion. ADC measurements at moderate b-value of 500 s/mm2 were more sensitive in subtyping and grading of RCC cases. This technique can be used in clinical practice as a fast and additional sequence in abdominal MRI.

Introduction and aim. Tumour necrosis factor-alpha (TNF-α) belongs to the cytokine family TNF/TNFR. As a multifunctional cytokine, TNF-α plays a significant role in diverse and a variety of cellular events such as cell survival, proliferation, differentiation, and death. As a pro-inflammatory cytokine, TNF-α acts as a bridge between inflammation and carcinogenesis. Receptor for advanced glycation end products (RAGE) are cellular receptors belonging to the immunoglobulin superfamily. As one of the primary mediators of innate immunity, acute and chronic inflammatory disorders, and certain cancers, RAGE signaling plays an important role. The aim of the present study is to analyze the prognostic significance of salivary TNF-α and RAGE in oral squamous cell carcinoma. Material and methods. A study was conducted testing saliva samples collected from ten patients with well-differentiated and moderately differentiated oral squamous cell carcinomas. To determine the levels of TNF-α and RAGE in unstimulated saliva from patients, an ELISA kit from RAY BIOTECH was used for the study, and the readings were read at 450 nm. Statistical analysis was performed using SPSS software. Version 23 of SPSS was used to plot the standard curve. Statistical comparisons were done using Mann-Whitney U test and ROC analysis. Results. Salivary TNF-α and RAGE in patients were considered to be induced by radiotherapy at a higher level in moderately differentiated squamous cell carcinoma when compared to well differentiated squamous cell carcinoma. Thus, there is an increase in the induced Salivary TNF-α and RAGE levels by radiotherapy with increase in the histological stages of oral squamous cell carcinoma. The statistical analysis also proved the same. Conclusion. Hence salivary TNF-α and RAGE may be used as a biomarker for oral cancer to predict the prognosis.

Introduction and aim. Angiogenesis, which is accomplished by capillary sprouting, is the process by which new vessels are created from pre-existing ones. In tumor, once their initial blood supply is depleted, a tumour is unable to grow without additional blood flow. Additionally, a tumor’s microvasculature, or microvessel density (MVD), increases along with its capacity to produce angiogenesis. We aimed to observe the relationship between the expression of vascular endothelial growth factor (VEGF) and MVD (using CD34) in oral squamous cell carcinoma (OSCC). Material and methods. The expression of VEGF and CD34 antibodies was analysed using immunohistochemistry method on 50 cases of histopathologically proved OSCC. The expression was correlated with clinicopathological parameters. Results. A significant correlation was observed between VEGF expression and gender, LVSI. No correlation between any other factors and the difference in VEGF expression was statistically significant. Similarly, the MVD expression was not found to be statistically significant in any of the pathological parameters. Conclusion. VEGF positivity as well as MVD were found to be independent of the tumor pathology. Tumor MVD was found to be independent of the expression of VEGF. Further studies in a larger study group may establish a significant association so that antiangiogenic targeted therapy may be initiated.
