Introduction and aim. The knee is one of the joints where immobilization is most used, however, it can cause morphological changes in the joint tissues and is a challenge to be overcome during rehabilitation. Whole-body vibration (WBV) is capable of generating repetitive oscillatory movements, which cause mechanical stimuli that interfere with tissue plasticity. The aim of this study was to analyze the knee morphology of Wistar rats submitted to remobilization with WBV. Material and methods. 32 male rats were used, divided into four groups (n=8): Control Group (G1), Immobilization Group (G2), Immobilized Group and Free Remobilization (G3), Remobilized Group with WBV (G4). For immobilization, a plastered apparatus was used for 15 days. G3 and G4 carried out free remobilization or with WBV, respectively, for 2 weeks. The knee joints were processed for light microscopy. Results. The WBV led to a reduction in the inflammatory infiltrate in the articular cavity and greater presence of adipocytes in the subintima of the synovial membrane. Conclusion. Remobilization with WBV induced a better tissue response in the synovial membrane when compared to free remobilization.

Introduction. Osteoarthritis of the knee is the most common presentation of osteoarthritis with prevalence between 12% and 35% of general population and is considered the leading cause of musculoskeletal disability in the elderly population worldwide. Aim. The study compared efficacy of kneading massage and pulsed ultrasound on pain, joint stiffness and difficulty in knee osteoarthritis (KOA). Material and methods. Fifty subjects with radiological evidence of KOA participated in the study. They were randomly allocated into kneading massage group (KMG) (25) and Ultrasound group (USG) (25). KMG received kneading massage for 7 minutes while USG received pulsed mode ultrasound for 15 minutes. Treatment was twice in a week for six weeks. Pain intensity (PI), joint stiffness and difficulty were assessed pre, 3rd and 6th weeks of treatment session with semantic differential scale and WOMAC. The data were analyzed using descriptive and inferential statistics, alpha level was set at 0.05 Results. There was a significant difference in present PI (F=11.45,P=0.001) and stiffness (F= 11.32, P=0.003) in USG. There was a significant reduction in PI (F=7.95, P=0.001) and joint stiffness (F=8.86, P=0.003) in KMG. At the 6th week, there was a significant differences in PI (t=12.23,P=0.000) and stiffness (t=8.08, P=0.000) when USG (3.00+0.4, vs 7.14+ 1.49) was compared with KMG (3.16+0.5 vs 7.50+1.5). Conclusion. Ultrasound (US) and kneading massage (KM) reduced PI and joint stiffness of KOA effectively; however US reduced PI than KM while KM reduces joint stiffness than US.

Introduction and aim. The study assessed and compared the effects of clinic-based and telemonitored home-based interventions on pain intensity, functioning and quality of life in patients with knee osteoarthritis (KOA). Material and methods. Forty-two patients were recruited purposely and randomly allocated into clinic-based (CBG) and telemonitored home-based group (THG) equally. The CBG and THG received isometric exercises to strengthen the quadriceps and hamstring muscles using theraband. THG received the exercise at home they were monitored on phone thrice in a week, while the CBG did the exercise in the hospital. The subjects performed four sets of eight repetitions three days in a week for eight weeks. Pain intensity, functioning and health related quality of life (HRQoL) were assessed at pretreatment, 6th and 8th week of intervention. Data was analyzed with descriptive and inferential statistics. Alpha level was set at 0.05. Results. There was a significant reduction (p<0.001) among pre-treatment, 6th and 8th week intervention in pain intensity, functional pain intensity and quality of life of CBG and THG. CBG showed significant reduction (p<0.001) in pain intensity, function and increase in HRQoL than THG at 8th week. Conclusion. Clinic-based and telemonitored home-based interventions were both effective in the management of KOA but clinic-based intervention was better than telemonitored home-based intervention.

Introduction. A technique used in physiotherapy, but still underinvestigated, is the use of the Russian current as an aid in the improvement of balance. Aim. To verify the influence of the Russian current applied to the rectus femoris on balance in healthy and sedentary individuals. Material and methods. A cross-sectional clinical trial was performed at the Universidade Estadual do Oeste do Paraná – Unioeste, in the city of Cascavel – PR. The sample consisted of 20 healthy female subjects aged between 18 and 25 years, equally divided into two groups where group 1 was placebo and group 2 treatment. Initially, the proprioceptive evaluation was performed by means of a functional test (the Star Excursion Balance Test (SEBT)) and stabilometry using a baropodometer. Russian current was then applied to the femoral rectum of both limbs simultaneously for 2 weeks, 5 days a week. Results. No significant differences were found analyzing the variables, but the elevated effect size points to clinical relevance of Russian Current in functional assessment. Conclusion. The use of the Russian current in the rectus femoris did not present significant alteration on balance.

Wstęp. Coraz większą uwagę w praktyce klinicznej zwraca się na ocenę sprawności sensomotorycznej jako istotnego czynnika determinującego powrót funkcji. Celami niniejszej pracy były: ocena wpływu uszkodzeń związanych z zaawansowaną gonartrozą na zmianę czucia głębokiego i kontroli sensomotorycznej stawu kolanowego, porównanie wyników uzyskanych we własnym teście kontroli sensomotorycznej z obiektywnymi metodami oceny czucia głębokiego, analiza zależności pomię- dzy poziomem czucia głębokiego i sprawnością układu sensomotorycznego a pomiarami stosowanymi w praktyce klinicznej. Materiał i metody. Ocenie poddano grupę osób zdrowych (grupa kontrolna, n=74), grupę pacjentów z chorobą zwyrodnieniową IV st. (grupa 1, n=67) oraz pacjentów po endoprotezoplastyce stawu kolanowego (grupa 2, n=62). Badanych oceniano z wykorzystaniem autorskiego Testu Kontroli Sensomotorycznej (TKS) oraz testu czucia pozycji stawu (JPS). Analizowano również wyniki uzyskane w skalach HSS Knee Score i Stafflstein-Score.Wyniki. W ocenie za pomocą TKS i JPS grupa kontrolna uzyskała wyniki na poziomie odpowiednio 4,9 pkt i 3,9°, natomiast grupa 2 3,7 pkt i 6,7° (p<0,005). W grupie 1 uzyskano 3,2 pkt w TKS, natomiast w JPS 8,2° w kończynie zdrowej i 10,2° w kończynie chorej. Wyniki tej grupy były istotnie gorsze niż grupy kontrolnej oraz grupy 2 (p<0,005). Wnioski. 1. Prezentowany własny test oceniający sprawność sensomotoryczną stawu kolanowego wydaje się obiektywnym i kompleksowym sposobem oceny sprawności kontroli sensomotorycznej stawu kolanowego u pacjentów z gonartrozą i po endoprotezoplastyce stawu kolanowego. 2. Wskazane są dalsze badania z wykorzystaniem autorskiego testu kontroli sensomotorycznej obejmujące próbę jego obiektywizacji z wykorzystaniem specjalistycznych narzędzi pomiarowych.
Introduction. The importance of sensorimotor control assessment as a crucial factor determining functional recovery is being increasingly appreciated in clinical practice. The aim of this study was to assess the inflence of damage associated with severe gonarthrosis on changes in proprioception and sensorimotor control of the knee, compare the results of a new sensorimotor control test developed by the authors with objective methods of proprioception assessment, and analyse the relation between the level of proprioception and sensorimotor system function and the measurements used in clinical practice. Material and methods. We studied a group of healthy persons (control group, n=74), patients with stage 4 knee osteoarthritis (group 1, n=67), and patients after total knee arthroplasty (group 2, n=62). The participants were assessed with a new sensorimotor control test (TKS) and a joint position sense test (JPS). Moreover, we analysed the patients’HSS knee scores and Stafflstein scores.results. In the Control Group, mean TKS and JPS test scores were 4.9 pts and 3.9°, respectively, while the respective results in group 2 were 3.7 pts and 6.7° (p<0.005). In group 1, the TKS score was 3.2 pts and the JPS test result was 8.2° in the unaffcted limb and 10.2° in the affcted limb. The results of this group were signifiantly worse than the scores achieved in the control group and group 2 (p<0.005). conclusions. 1. The new knee sensorimotor control test developed by the present authors appears to provide an objective and comprehensive method to evaluate sensorimotor control of the knee in patients with gonarthrosis and after total knee replacement. 2. Further studies of the new test are advised, including an objective evaluation of the test utilising specialised measuring tools.

Introduction. Patellofemoral pain syndrome (PFPS) is a disorder of the front compartment of the knee joint with incompletely investigated, probably multifactorial pathogenesis. It mostly affects young people and runners. In patients with PFPS conservative management is a therapy of choice with fundamental importance of physiotherapeutic procedures. Therapy should be highly individualized and considering all possible factors that may cause PFPS symptoms. Aim. The aim of this report was presentation of management of a 23 year old female patient with PFPS that developed secondary to a knee sprain. The medical history, diagnostic and therapeutic procedures were thoroughly described, then obtained results were presented and thereafter discussed. Methods. Clinical assessment included functional and provocative tests of the patellofemoral joint as well as thigh and calf muscles tests, range of motion measurement of the knee joint and pain assessment using the VAS scale. Therapeutic management included 5 sessions of post-isometric muscle relaxation (PIR), mobilizations of the patella and applications of elastic tapes. Results. After 5 sessions of therapeutic management PFPS symptoms were significantly reduced. Pain did not occur during normal activity, whereas in heavy joint loading, it occurred later and was of lower intensity. Range of motion as well as subjective sense of joint stability was also improved. Conclusions. Individually adjusted conservative management based on PIR techniques, mobilizations of patella and kinesiotaping seems to be effective form of therapy for PFPS of functional nature

Wstęp. Endoprotezoplastyka jest obecnie najbardziej rozpowszechnioną metodą leczenia zaawansowanych zmian zwyrodnieniowych stawów, w tym stawu kolanowego. Jej głównym celem jest przywrócenie funkcjonalnej niezależności pacjentów w czynnościach dnia codziennego poprzez zmniejszenie natężenia bólu i niepełnosprawności. Cel pracy. Ocena funkcjonalna pacjentów po całkowitej endoprotezoplastyce stawu kolanowego z uwzględnieniem płci, wieku, BMI oraz czasu trwania choroby. Materiał i metoda. W badaniu udział wzięło 70 pacjentów (59 kobiet, 11 mężczyzn), którzy zostali zakwalifikowani do zabiegu całkowitej endoprotezoplastyki stawu kolanowego w wyniku pierwotnych zmian zwyrodnieniowych. Średni wiek pacjentów w badanej grupie wynosił 66,5 roku. Do oceny funkcjonalnej pacjentów posłużono się skalą VAS, skalą Knee Injury and Osteoarthritis Outcome Score, Global Rating Scale, testem Up&Go, testem Five Time Sit to Stand oraz 10-metrowym testem chodu. Dodatkowo wykonano pomiary zakresów ruchu stawu kolanowego w kończynie operowanej i nieoperowanej. Badanie wykonano dwukrotnie; badanie I - tydzień przed zabiegiem operacyjnym i badanie II - 6 miesięcy po nim. Do prezentacji uzyskanych wyników badań posłużono się wartościami średniej, mediany, odchylenia standardowego oraz współczynnikiem korelacji rang Spearmana. W celu analizy statystycznej wykorzystano testy nieparametryczne Wilcoxona i Manna-Whitneya. Poziom istotności statystycznej został określony jako p < 0,05. Wyniki. W okresie 6 miesięcy po zabiegu endoprotezoplastyki stwierdzono poprawę we wszystkich testach i skalach oceny.
Introduction. Endoprosthetics is currently most common method of the treatment of advanced osteoarthritis, including knee joint osteoarthritis. Its main goal is to restore patients functional independence in activities of daily living through reducing pain and disability. Objective. The aim of this study was functional assessment of the patients after total knee replacement (TKR) considering subjects age and gender, BMI and mean duration of the disease. Material and methods. 70 patients (59 females and 11 males), qualified for total knee replacement surgery due to the primary knee osteoarthritis (KO), participated in this study. Mean age of the patients in the study group was 66,5 years. Visual Analogue Scale (VAS), Knee Injury and Osteoarthritis Outcome Score (KOOS), Global Rating Scale, Up&Go Test, Five Time Sit to Stand Test and 10-meters walking test were used for the functional assessment of the patients. Additionally, the measurement of the range of motion of the operated and non-operated knee joint were performed. The study was performed twice; the first study was conducted one week before the surgery and the second study-six months after. For the purpose of the presentation of the results of this study mean value, median, standard deviation and Spearman’s rank correlation coefficient were used. Non-parametric Wilcoxon and Mann-Whitney test were used for the statistical analysis. The level of statistical significance was assumed at α < 0.05. Results. Within six months after total knee replacement surgery (TKR) statistically significant improvement of the results of all functional tests and scales used in this study were observed.
