Opis:
In the article the ProSIL software for computer aided functional safety management is presented. The software consists of three modules for the determination of the required SIL level (ProSILen) and verification of the SIL level (ProSILver). In the ProSIL the calibrated knowledge-based risk graph method for determining the required safety integrity level (SIL) of the safety functions identified in hazard analysis is implemented. The SILs are then verified for safety-related control and protection systems implementing relevant functions with regard to random failures and potential systematic failures. The assessment methods are compatible with international standards IEC 61508 and IEC 61511. Some current challenges and methodological issues concerning knowledge-based functional safety management in life cycle are also discussed.

The aim of this article is to identify and discuss some problems that have been encountered in designing and operating the functional safety solutions of higher safety integrity levels (SIL 3 and SIL 4) in the light of analyses outlined in a new version of the international standard IEC 61508:2010. Examples of such solutions are the electric / electronic / programmable electronic systems (E/E/PESs) and the safety instrumented systems (SISs) being designed and operated respectively according to IEC 61508 and IEC 61511 in the system safety life cycle. The role of functional safety solutions is effective reducing and controlling the individual and/or societal risk with regard to tolerable levels defined. Some aspects of potential influence of danger failures of the E/E/PESs or SISs on the plant safety are considered. The influence of common cause failures (CCFs) and dependent failures in the context of the layer of protection analysis is also discussed.

The aim of this article is to identify and discuss some issues related to functional safety and security management in hazardous industrial plants. The safety functions are to be realised using the electric / electronic / programmable electronic systems (E/E/PESs) or the safety instrumented systems (SISs) that are designed and operated respectively according to IEC 61508 and IEC 61511 requirements in life cycle. Although the role of functional safety solutions in effective reducing and controlling the individual and/or societal risks has been widely recognised, the substantial problems emerge when E/E/PEs or SISs operate in industrial distributed computer networks. Thus, the security-related problems appear that can introduce some additional risks. An integrated systemic functional safety and security concept is proposed, which includes general requirements as well as appropriate using specified methods and international standards.

The German regulatory body has issued probabilistic safety assessment guidelines, elaborated for a comprehensive integrated safety review of all NPP in operation and containing a newly developed graded approach for the probabilistic assessment of external flooding. Main aspects are explained such as the underlying probabilistic considerations and the mathematical procedures for the calculation of exceedance frequencies. Exemplarily it has been investigated if extreme events such as tsunami waves could be a hazard for NPP at coastal sites in Germany.

This article addresses selected aspects of the alarm system and human factors that should be evaluated during the design and operation of an industrial hazardous installation. In such installations the layer of protection analysis (LOPA) methodology is often applied for simplified risk analysis based on defined accident scenarios. To reduce and control the risks the safety instrumented functions (SIFs) are identified and their safety integrity levels (SILs) determined taking into account defined criteria the risk evaluation results. Given SIF is implemented using the basic process control system (BPCS), the alarm system (AS) and the safety instrumented system (SIS). Nevertheless a crucial role plays the human-operator undertaking safety-related decisions during potential abnormal situations and accidents. Below some issues concerning requirements for the alarm system design in context of human factors are outlined and discussed.

This article addresses an integrated safety and security analysis approach of hazardous industrial plants and systems of critical infrastructure. Nowadays due to new hazards that emerge there are opinions among experts that these issues require an integrated approach in life cycle, from the design concept, through the design and operation of the plant, to its decommissioning. It is proposed to start from an interesting methodology known as the security vulnerability analysis (SVA) developed for hazardous plants of chemical industry. It is based on rings of protection concept to secure widely understood assets. This concept seems to be compatible with layer of protection analysis (LOPA), which is consistent with functional safety concept of the control and protection systems including cyber security aspects. It is outlined how to use these approaches in an integrated way for safety and security analysis of hazardous industrial plants and systems of critical infrastructure.

In this paper selected aspects of human factors are discussed that should be taken into account during the design of safety-related functions for a complex hazardous installation and its protections. In such installations the layer of protection analysis (LOPA) methodology is often used for simplified risk analysis based on defined accident scenarios. To control the risk the safety instrumented functions (SIFs) are identified and their safety integrity levels (SILs) determined with regard to results of risk assessment. Given SIF is to be realised by the electric/ electronic/ programmable electronic system (E/E/PES) or safety instrumented system (SIS) and the human-operator. The SIL is to be verified according to requirements and criteria given in international standards IEC 61508 and IEC 61511. Selected issues related to designing the alarm system (AS) with regard to human factors are outlined. Some aspects of human reliability analysis (HRA) as a part of human-machine interface (HMI) assessing and probabilistic modelling of the system are shortly discussed.

Opis:
A security level of distributed control and protection system may have a significant impact on the results of functional safety analysis. However, the issue of integrating the safety and security aspects is difficult and usually is neglected during the functional safety analysis. This article presents a method of functional safety analysis which takes into consideration a concept of integrating these two aspects. It is based on proposed classification of communication channels used in the computer system / network and the scope of such system distribution. The functional safety analysis is to be performed at every stage of system lifecycle, but one of the most important parts is defining required safety functions and determining the safety integrity level for them. The integration concept might be taken into account at this stage. The basis of a method proposed is the assumption that the security level is considered as a risk parameter in graphs of functional safety analyses.

In this article the Layer of Protection Analysis (LOPA), as a technique for the risk evaluation relating to the hazardous industrial installations performance, is presented. The results of analyses are important in the terms of the safety management process in such installations. Based on obtained estimations the decisions might be undertaken which solutions to apply in order to mitigate the risk of hazardous installations performance to a tolerable level. The risk mitigation is provided by properly designed layers of protection, particularly the alarm system, treated as a part of protection layer. The alarm system should be designed and implemented with comprehensive consideration of the human factors. The role of the operator in hazardous installations is crucial mainly during abnormal and alarm situations in order to provide and/or recover system to normal or safety state. In the article some selected aspects of alarm systems designing process with special treating of human operator are outlined. Moreover, an example of the LOPA analysis for the accident sequence within a reaction container with consideration of human reliability analysis (HRA ) is carried out.

Opis:
In this article some issues concerning the safety management in computerized complex hazardous plant are presented in the context of human factors. It has been shown that the risk of losses can be significantly reduced using appropriate technical solutions in the form of a layer protection system, which includes a basic process control system, human-operator and protection automatics. The significance of appropriate designing of interfaces including functions of the alarm system is emphasized. It will contribute to reducing the humanoperator error probability. The functional safety management, which includes the risk control in a life cycle of complex plant, should be carried out in relation to requirements associated with possible avoiding of software systematic failures in programmable systems and reducing the frequency of hardware random failures.

The article presents the current state of knowledge on explosion protection based on ATEX EC Directives, as well as the processes occurring in Polish industry, which refer to the issue considered. The activities of different subjects operating on the Polish market exert diverse influences on the way of implementing the minimal level of ATEX requirements as well as on optimizing investments in the safety measures. On the basis of experience gained by the ASE company on Polish industrial market in the field of explosion protection equipment the design and delivery as well as experience in carrying out training programs, providing consulting services and expertise, it is possible to propose a comprehensive set of measures aiming at achieving a permanent state of the acceptable level of explosion protection.
