Opis:: This paper shows the current state of research and development over the telemedical services system for family doctor's practices. The introduction of this telemedical system should result in improvement in accessibility to medical services for the patient and to actual medical information, publications and up-to-date consultations for the physician, regardless of the distance from physician's clinic to the consultation center. This system should reduce the total costs of basic medical services. The system consists of two autonomous subsystems, that work in two arrangements: patient ↔ family doctor's practice and family doctor's practice ↔ information & consultation center. The remote medical care system provides vital signs monitoring through telephone line - physician can observe electrocardiogram and oxidation of blood, while the patient is many kilometres away. In case of unstable state of health, this system is able to alert the physician. The teleconsultation system provides a videoconference service with shared multimedia document (audio-video streams, graphic, text, etc.).

Opis:: The scenarios of cooperation between personal healthcare systems are discussed in this paper. Current telemedical solutions for home care provide the patient with a seamless diagnosis based on selected vital parameters. The application of two or more specialized systems monitoring the same subject rises the question of possible interference and cooperation. The cooperation between home care and wearable systems is of particular interest, because of their complementary features. Considerations of various cooperation scenarios led to the specification of three cooperation levels with regard to the required software integration. The paper presents also the design of a cooperation protocol for two prototype surveillance systems, enabling the use of communication resources of the stationary system to carry the messages of the wearable system over the wired transmission channel.

Opis:: This paper presents an effective solution of packetizing of non-uniform data for the purpose of reporting in telemedical surveillance systems with adaptive interpretation. Unlike the regular systems, where the data continuity is guaranteed by the common reporting interval and unified report content, the adaptive systems must implement a reservation procedure in order to proper data delivery, accordingly to sampling rates set individually for each of the diagnostic parameters. This procedure combines the content of every packet with respect to changes in data flow from particular diagnostic data, caused by time-variable requirements for the update rate of their time series. Our approach postulates appending to the information structure of two auxiliary data attributes, the validity period and the priority. The proposed solution was implemented and tested in a prototype cardiology-oriented monitor using two alternative reporting modes following the monitoring requisites. In immediate mode diagnostic packets are transmitted immediately accordingly to the time requirements, what allows the telediagnostic system to respond in short time in case of emergency. In delayed mode the transmission is deferred until packets are entirely filled with valid data, what limits the usage of data carrier for long-time regular reporting.

Opis:: The electrocardiogram is a digital record of effective action potentials of heart muscle fibres, thus it contains an important information about the heart function. Unfortunately, the signal theory-based description of the ECG contents is not efficient in extracting these data, and the practical interpretation software is mostly based on heuristic statements. This conclusion was a motivation for our research on the expert-like machine reasoning based on ECG perception analysis, physiology-driven feature extraction and fuzzy logic. This paper highlights the aspect of optimal description of an electrocardiogram contents in a digital record.

Opis:: Main role of this article is presentation, in a very short way, most important results of over two years of work board of experts from two universities, two clinical hospitals, city hall and also medical software and hardware suppliers. As a main purpose of strategy improvement of health care level in region was set. This aim will be achieved by telemedical infrastructure development, creation and implementation of procedures and instant training of staff. Strategy is focused on three main aspects. First one groups medical projects, second infrastructure project and third one is regional centre of coordination. In medical area proposed changes are mainly focused on telemedical support for emergency services, remote medical consultations and complex home monitoring of patients. Main solution for infrastructure assumes creation of integrated information system for hospitals and emergency units. I this part of the strategy selection of the proper standards for data storing and exchange. Probably one of the best choices is OpenEHR. Last part is centre of cooperation which consists of coordination centre of telemedical services, telemedical projects management unit, and training facility.

Opis:: This paper presents some aspects of the on-line remote fetal monitoring system based on the GPRS data transmission service and WAN network. The remote signal recording instrumentation consists of bioelectrical signal recorder and tablet PC computer. The central surveillance system located in hospital has a capability of simultaneous monitoring of up to 24 patients, both remotely and within hospital. The system enables analysis, dynamic presentation and archiving of signals and medical data. The clinical interpretation is immediately available in hospital as well as at patient's home or attending doctor office.

Opis:: The paper presents some aspects of telemedical fetal monitoring where the biomedical signals are acquired remotely at patient’s home and wirelessly transmitted to the central computer through the GSM network and Internet. Signals are acquired during the monitoring sessions, and along with analysis results, they create uniform data structure describing the medical history of patient since the first monitoring to the labour. The proposed structure of database was based on the currently used in centralized monitoring systems, and extended by information essential for remote monitoring purposes. Internet-based telemedical systems allow for remote access to collected data, however, it is necessary to secure the database against unauthorized access to patient’s personal data and medical history. The proposed structure of database records allows for easy searching and viewing of patient data forms and monitoring traces. These features cause that the database constitutes valuable research material, which enables to relate parameters of particular monitoring records, acquired during pregnancy, to the real fetal outcome.

Opis:: Telemedical system for fetal home monitoring with smart selection of signal analysis algorithms is presented in this paper. Fetal monitoring signals are provided by a mobile instrumentation consisting of bioelectrical signal recorder and tablet PC which retrieves and processes the data as well as provides wireless data transmission based on Internet. The fetal surveillance system enables analysis, dynamic presentation and archiving of acquired signals and medical data. Novelty of the proposed approach relies on smart fitting of the algorithms for analysis of the abdominal signals in mobile instrumentation, as well as on controlling of the fetal monitoring session from the surveillance center. These actions are performed automatically through continuous analyzing of the signal quality and the reliability of the quantitative parameters determined for the acquired signals. Using that approach the amount and content of data transmitted through remote channels to the surveillance center can be controlled to ensure the most reliable assessment of the fetal well-being.

Opis:: The paper presents the preliminary study of the utilization of a low resolution ECG signal analysis. The analysis was performed on the signals obtained from a hand-held ECG monitor usually used in primary health care. The aim. The main aim of the study was a registration of series of data by volunteers within couple of months and determination of signal quality and main ECG parameters as follows: Q, R, S waves, QRS duration as well as the end of PQ and the beginning of ST segment. Additionally, the heart rate variability was determined. Materials and methods. The data was registered by 12 volunteers aged from 35 to 55. The ECG tests were carried out for 7 months. The sample rate of the signal was 100 Hz. To determine the ECG parameters the signal processing and statistical methods was used. Results. The sensitivity of the following ECG parameters were: R wave detection - 99,2 %, Q wave detection - 99,1 %, S wave detection - 99,0 %, QRS duration - 99,2 % respectively.

Opis:: The expectation that computer networks combined with the Internet and associated technologies will be common in all businesses of the future has resulted in a flurry of investment in ICT around the globe, and in wide interest in the Internet, e-commerce and e-business. For example the Malaysian government has created a high-speed data highway, a federal capital that is a 'wired city', and a 'paperless hospital' [1]. The UK Government too has recently approved plans for major ICT projects [5, 10]. These projects are in healthcare, and involve large financial sums. It is important that the use of these public funds leads to successful outcomes. This paper analyses a large successful ICT project that uses e-business technologies. The aim of the paper is to identify the success factors in this completed project to see if these pointers indicate favourable prospects for the outcome of these new healthcare projects.
