Opis:
This paper describes supraventricular cardiac arrhythmias and sick sinus syndrome in patients with univentricular heart defects, following Fontan procedure. The Fontan operation is a well established surgical procedure in which the right atrium is connected with the pulmonary artery. This technique relieves the functional problems resulting from having a univentricular heart and it is also associated with a lot of complications. Results from 29 patients were included in our study for analysis. All patients with single ventricle were diagnosed and hospitalized in the Department of Paediatric Cardiology in Katowice during 1994-2008. Supraventricular cardiac arrhyhtmias and sick sinus syndrome were common complications in children who went through Fontan procedure. All the stages of treatment with the Fontan procedure are palliative in their character and their main advantage is the improvement in the quality and length of life.

Opis:
The Centre of Excellence STIMCARD operating within the Institute of Medical Equipment and Technology ITAM develops Polish technologies of transesophageal, transcutaneous and transvenous cardiac stimulation as well as technologies supporting the supervision of patients undergoing electrocardiotherapy, such as in the case of an implanted pacemaker or cardioverter defibrillator. The paper reviews non-invasive electro stimulation techniques, as well as the diagnosis and therapy of the coronary artery disease (CAD) and arrhythmias. As far as CAD is concerned, the paper presents solutions applied in the construction of electrodes and pacemakers intended for both transesophageal and transcutaneous cardiac stimulation tests. The clinical value of the developed cardiac stimulation test has been assessed for the diagnosis of stable cardiac symptoms and management of patients with unstable cardiac disease, as well as for the identification of subjects in danger of sudden cardiac death. The system of stimulated ischemic preconditioning has also been discussed. Also, the paper demonstrates dedicated solutions used in ECG recording channels in the area of diagnosis and treatment of arrhythmias. The solutions make it possible to obtain stable and interpretable recordings during stimulation, both on the surface of the body and in the esophagus. Finally, the paper analyses solutions introduced in pacemakers which make it possible to induce (for diagnostic purposes) and stabilize quick paroxysmal heart arrhythmias, as well as the idea of supported hemodynamics, particularly useful during surgery.

The main aim of Self Optimizing Neural Network (SONN), which are presented in this paper, is construction of expert system on the basis of analysis of medical information about group of patients. The expert system is built on the basis of neural network, and the main task of this system is to expect future patient health, based on information about the patient. Such a system can give the doctors a hint about that what can be happen with patient. And what is more important - the SONN construction process is very flexibly and adapts topology and all weights to training data. This is undoubtedly a great advantage of this type of neural network. Moreover the construction process is quite simple. The network topology and all connections between neurons can be easy implemented and kept in such a way, which allows to create very efficient expert system. In this paper we describe the process of construction of neural network which is based on one-shot analysis of learning patterns. On the basis of appropriate computation the SONN topology is built. The construction process can be repeated on the learning group of patients. In this way the expert system (based on SONN) will be better and better.

We developed a music-heart beat feedback system to test the efficacy of the entrainment between music and human heart beat. As a result of regulating the music tempo as 10% faster (namely FAST condition) or slower (SLOW) than heart beat in a real time series, the subjects’ heart rate was successfully modulated as significantly higher in the FAST condition and lower in the SLOW condition. The timeliness regulation of this feedback system may play a key role to bring forth the music-heart beat entrainment.

Opis:
Qualitative and quantitative description of the heart wall motion is a very important field of investigation in modern cardiology. Abnormalities in heart motion are usually symptoms of life threatening cardiac dysfunctions therefore measurements of dynamic heart functions are of great clinical importance. The images of moving spatial heart structures can be efficiently acquired using 4D echocardiography. Unfortunately because of the low quality such images do not allow for precise measurements. To overcome this problem images need to be further processed and moving structures have to be extracted. In this work we present a method for estimating heart motion from 3D echocardiographic image sequence. On the basis of this method we have developed several visualization techniques that enable qualitative assessments of heart motions abnormalities. Together with quantitative measurements they may be become a useful tools in daily clinical practice.

This paper focuses on three-dimensional (3-D) adaptive median filters based on the impulse detection approach designed to effectively remove the impulse noise from cardiographic image sequences. Impulse noise affects the useful information in the form of bit errors and it introduces to the image high frequency changes that prohibit to process and to evaluate the heart dynamics correctly. Therefore biomedical imaging such as vascular imaging and quantification of heart dynamics is closely related to digital filtering. In order to suppress impulse noise effectively, well-known non-linear filters based on the robust order-statistic theory provide interesting results. Although median filters have excellent impulse noise attenuation characteristics, their performance is often accompanied by undesired processing of noise-free samples resulting in edge blurring. The reason is that median filters do not satisfy the superposition property and thus the optimal filtering situation where only noisy samples are affected can never be fully obtained. The presented adaptive impulse detection based median filters, can achieve the excellent balance between the noise suppression and the signal-detail preservation. In this paper, the performance of the proposed approaches is successfully tested for the heart image sequence of 38 frames and the wide range of noise corruption intensity. The results are evaluated in terms of mean absolute error, mean square error and cross correlation.

Research was conducted to analyze the flow of a fluid within mechanical heart valve - Medtronic Hall. Physical experiment and numerical modelling were performed. The aim of the research was to determine the difference between obtained experimental and numerical data. In the experiment a dependency between static flow rate within the valve and static inlet and outlet pressure in the valve duct was examined. Moreover a dependency between static flow rate and angular valve position was also determined. Experimental data was used to perform a numerical flow analysis. The obtained flow rate values and angular positions of the valve were set to a finite-volumes-method model in order to achieve model output pressure values identical or similar to the ones obtained from the experiment. The resulting pressure values from the experiment and numerical analyses proved to be of the same order of magnitude, varying only by up to 10%. However, as far as differential pressure is concerned, numerical results were out of the range of measurement resolution. It can be assumed that numerical flow analyses quite correctly predict the real phenomenon and in view of measurement inaccuracy of used sensors authors would suggest using more accurate ones and repeating measurements for future clarification.

Analysis of variability of the fetal heart rate (FHR) is very important for fetal wellbeing assessment. The beat-to-beat variability is described quantitatively by the indices originated from invasive fetal electrocardiography which provides the FHR signal in a form of time event series. Nowadays, monitoring instrumentation is based on Doppler ultrasound technology. The fetal monitors provide the output signal in a form of evenly spaced measurements. The goal of this work is to present a new method for the FHR signal processing, which enables extraction of time series of consecutive heartbeat intervals from the evenly repeated values. The proposed correction algorithm enables recognition and removal of the duplicated measurements. Reliable evaluation of the algorithm requires the reference event series, thus the FHR signals were obtained from abdominal fetal electrocardiograms to be used in this research study.

Some medical devices such as mechanical circulatory support systems and defibrillators require synchronization with the appropriate phase of the cardiac cycle of the patient. This paper presents a description of the electrocardiographic (ECG) signal amplifier and QRS detector designed for use with such devices. The detector system was built using a programmable analog array AN231E04 and the ECG amplifier with an analog-digital (A/D) converter based on a programmable circuit (front-end) ADS129x. By using the programmable elements and exploiting the potential of dynamic analog matrix reconfiguration, the presented detector achieved the desired features, such as low latency of QRS detection, automatic gain control and the ability to change the time constant by software. High-resolution of the A/D converter in the ADS1298x and the possibility of programming the gain of the amplifier allow the acquisition of the ECG signal acquired from different types of electrodes: disposable standard, epicardial or endocavitary. The tests of the prototype system proved that it is highly effective for the detection of QRS complexes in ECG waveforms recorded from all kinds of electrodes.

The most important features indicating appropriate fetal development are the measures of instantaneous variability of a fetal heart rate (FHR), describing fluctuations of the beat-to-beat heart intervals. The most popular method for the FHR acquisition is the Doppler ultrasound technique. However, it is very sensitive to various motion artifacts distorting the signal being acquired. The aim of our work was to evaluate the influence of signal loss episodes on the parameters quantitatively describing the instantaneous variability of the FHR. For this purpose we artificially inserted signal loss episodes to the recordings, in different patterns and percentage, in accordance with the real characteristics of the signal loss segments. We particularly would like to answer the question if the signals with significant amount of signal loss can be reliably evaluated by means of instantaneous variability measures, and which of these measures (numerical indices) are more robust to the missing values.

In some applications it is important to detect the QRS complex in the ECG waveform with possibly low time delay. Traditional software detectors of the QRS complex implement algorithms usually based on cascades of digital filters that introduce delays up to parts of a second. On the other hand, hardware QRS detectors fulfil the low delay requirements, but have worse adaptive features for the changing ECG shape. In this paper a new approach to QRS detection is presented. The proposed detector is based on a Field Programmable Analog Array (FPAA). This solution makes it possible to modify the parameters of particular blocks of the detector or even the whole structure without any changes in hardware, while the processing path is fully analog and does not introduce an additional delay. The most interesting feature of the FPAA is the dynamic reconfigurability. The parameters can be changed during runtime without any reset of the circuit or any other disturbances of the system functionality. The prototype QRS detector was built using the AN221E04 circuit from the Anadigm company.

The way to model complex shapes has a significant influence depending on the context. Handling an object can be considerably increased if a good underlying model is used. On the contrary, preponderant problems can appear if an unsuited model is associated to the object. The main criterion to discriminate existing models is to determine the balance between: their ability to control global characteristics and the possibility to handle local features of the shape. The fact is very few models are adapted both to structure and to geometrical modelling. In this paper, we first describe an overview of existing approaches. They can be classified principally in two groups: skeleton based models, used to control the global aspect of the shape, and free form models, used to control local specificities of the object. Then, trying to keep the advantages of both techniques in mind, we present an original approach based on a multi-layer model to represent a 3D object. We focus on the ability to take into account both global and local characteristics of a complex shape, on topological and morphological levels, as well as on the geometric level. To do that, the proposed model is composed of three layers. We call the boundary mesh the external layer, including a multi-resolution feature. We enhance this representation by adding an internal structure: the inner skeleton, which is topologically equivalent to the input object. In addition to that, a third layer links the structural entity and the geometrical crust, to induce an intermediary level of representation. This approach is applied to classical and medical data through a specific algorithm.

The stroke volume of pulsatile ventricular assist device (VAD) is one of the key clinical indicators to evaluate the adequacy of the process of mechanical heart assistance and ensuring the patient’s safety. In order to evaluate the volume changes in VAD blood chamber, a bioimpedance method was applied, based on volumetric electric conductivity of the blood. Periodic changes in VAD volume during its operation result in a pulsatile impedance waveform; its amplitude in the subperiods of filling and ejection correlates with blood volume in the chamber. The paper presents the method of analysis of the bioimpedance signal recorded in VAD blood chamber with a reference to the classical method of analysing the cardioimpedance signal from the patient’s thorax. The paper also presents the results of determining stroke volume of blood chamber based on an empirical formula that has been developed.

Opis:
Synchronization of operation of an artificial chamber supporting the sick heart of a patient has a crucial meaning for the effectiveness of support. To make the synchronized operation possible, the information on patient's heart activity should be fed to a controller of the supporting chamber. To this end, the QRS complex detector operating on ECG signal is necessary. The signal must be acquired from a patient's heart. Appropriate signal quality at long-term support may be assured only in a case the signal is acquired with epicardial electrodes. In the paper, the concept of hardware construction of a QRS complex detector has been presented, based on a field programmable analog array FPAA. The results of verification of the developed detector include the correct operation while using the epicardial signals recorded from patients which undergone the cardiosurgical operations.
