Opis:
The last decades, when robots have appeared in the operating room, showed the possibility of surgery enhancement by improving precision, repeatability, stability and dexterity. However, taking into consideration still existing limitations of robotics in surgery, and treating the robots as medical devices with the highest degree of safety level requirements, one must take a number of complex actions when preparing the experimental clinical application of a new modern robot Robin Heart. Presented paper describes the current state of procedures carried out in the Robin Heart project of surgery robots prepared for clinical application. Based on experiences with the devices existing on the market and, first of all, thanks to knowledge and expertise gained by our team during last 12 years, intensive work are currently done in order to introduce both mechanical and electronic modifications as well as to improve the safety system. As far as human resources are concerned, a professional team able to carry out the robot-supported surgery is prepared based on the created system of technical and functional trainings on simulation stands, which also includes the developed operation planning procedures. The first telemanipulator designed for clinical practice is the Robin Heart Vision - endoscopic system manipulator.
Pojawienie sie w ostatnich dekadach na salach operacyjnych klinik zautomatyzowanych telemanipulatorów wprowadziło nowy standard w chirurgii małoinwazyjnej dzięki poprawie precyzji działania, powtarzalnosci ruchów i procedur, zwiększenia komfortu pracy chirurga-zdalnego operatora - bardzo istotnego przy czesto długotrwałych zabiegach. Aby osiągnac ten etap wdrożenia klinicznego, biorac pod uwage z jednej strony wciaż istniejące ograniczenia tego typu zabiegów oraz konieczność spełnienia bardzo rygorystycznych wymagan certyfikacji samego produktu medycznego o najwyzszym w tym przypadku stopniu inwazyjności konieczne jest nie tylko opracowanie samego produktu finalnego spełniajacego wymagania norm lecz równiez certyfikacja całego procesu technologicznego jego wytwarzania. Celem pracy jest przedstawienie przygotowan do badan klinicznych, po fazie testów laboratoryjnych i na zwierzetach samego systemu robota, stanowisk testowych oraz przygotowania kadry dla jego obsługi dla projektu polskiego telemanipulatora chirurgicznego RobinHeart. Przedstawiony został projekt mechaniczny, systemu sterowania oraz stanowisk trenazerów-symulatorów i wybrane elementy modyfikacji podsystemów, powstałe na bazie 12 letnich doswiadczen zespołu i analizy rozwiazań swiatowych, dla najblizszego wdrozeniu robota RobinHeart Vision, przeznaczonego do zdalnej manipulacji torem wizyjnym podczas operacji małoinwazyjnych.

Opis:
Computed tomography and rapid prototyping techniques can be used to construct and fabricate large size bone models for orthopaedics. Rapid Prototyping technology enables the fabrication of a true-to-scale bone joint model based on 3D virtual models, generated by segmenting patients’ CT images. The model can be used to plan, to simulate, to assist prosthesis implantation for difficult cases of THR (Total Hip Replacements). The main restriction of implementing such models into medical practice is high cost of their production. Physical models of pelvic bones, were constructed on the basis of data collected during standard CT examinations. A method of development of a large-size model while fulfilling the requirement for reducing the price of model fabrication in the article was presented. The method can be used for fabrication the models with 3DP technique. This paper also discusses the issue of production costs when utilizing other RP techniques as well as their usefulness in practice.

Whatever the type of surgery related to inner organs, traditional or robotic, the contact with them during surgery is a key moment for pursuing the intervention. Contacts by means of surgery instruments namely scalpels, staples, clamps, graspers, etc. are decisive moments. False, and erroneous touching and manoeuvring of organs operated on can cause irreversible damage as regard morphological aspects (outer impact) and physiological aspects (inner impact). The topic is a great challenge in the effort to measure and characterize damages. In general, electrical instruments for surgery employ the following technologies: ultrasound, radiofrequency (monopolar, and bipolar), and laser. They all result in thermal damages difficult to evaluate. The article proposes a method for a pre-screening of organ features during robotic surgery sessions by pointing out mechanical and thermal stresses. A dedicated modelling has been developed based on experimental activities during surgery session. The idea is to model tissue behaviour from real images to help surgeons to be aware of handling during surgery. This is the first step for generalization by considering the type of organ. The measurement acquisitions have been performed by means of an advanced external camera located over the surgery quadrant. The modelling and testing have been carried out on kidneys. The modelling, carried out through Comsol Multiphysics, is based on the bioheat approach. A further comparative technique has been implemented. It is based on computer vision for robotics. The findings of human tissue behavior exhibit reliable results.

The rabbit is a good experimental model for brachial plexus surgery. The risks of death during anesthesia were significantly greater in rabbits than cats or dogs. This article presents the protocol of injectable anesthesia for a short surgical procedure, safe for the rabbit patient and convenient for the surgeon.

In this article a three-dimensional mathematical model of radiofrequency ablation during open-heart surgery is presented. It was developed to study temperature field distribution into myocardial tissue. This model uses an anatomically correct 3D model for the left atrium, obtained by magnetic resonance imaging (MRI) processing of a patient; takes into account thermoelectric characteristic differences depending on the area of electric current application; considers cooling by the air flow. An ex-vivo experiment on the pig’s heart was performed where the depth of myocardium tissue damage was measured for the model validation. It was shown that the deviation of the model data from the experiment is within the limits of instrumental measurement error. The developed model is proposed to be used for heart ablation procedures planning, or new equipment development.

This study presents and evaluates side-to-side nerve repair techniques for their ability to induce collateral nerve sprouting. The coaptation of the ventral branches of spinal nerves C5 and C6 to C7 through an incision epineurium was used to repair the nerve. The number of myelinated fiber axons and G-ratio was evaluated. Preliminary results indicate the possibility of using side to side coaptation in brachial plexus nerve surgery.

Grippers are routinely used to hold, lift and move organs in laparoscopic operations. They are generally toothed to prevent organs from slipping during retention. Organs held by grippers are always at risk of being damaged by the clamping force. In this study, noncontact grippers working with the Bernoulli principle and using air pressure were developed, and vacuum performance was compared in terms of maximum tissue weight holding capacity. For this purpose, Taguchi method was employed for experimental design and optimization, and Taguchi L16 orthogonal array was selected for experimental design. The experimental parameters were 4 gripper types, 4 air-pressure levels (3.5, 4.5, 5, and 5.5 bar), 4 flow rates (2.2, 2.6, 2.8 and 3 m3/h) and two animal tissue types (ventriculus/gizzard and skin). Values from the experimental procedures were evaluated using signal-to-noise ratio, analysis of variance and three-dimension graphs. An equation was obtained by using 3rd-order polynomial regression model for weight values. Optimization reliability was tested by validation tests and the revealed test results were within the estimated confidence interval. The results obtained from this study are important for future studies in terms of organ injury prevention due to traditional grippers in laparoscopic surgery.

The assumption was verifi ed that for patients suffering from cancer levels of anxiety and self-esteem differ compared to other patients before surgery. 120 patients of urology were assigned to subgroups according to diagnosis (cancer vs. non-cancer) and the duration of hospitalization (5 days vs. 1 day). Patients suffering from cancer declared higher anxiety than other patients. Longer hospitalization was connected to higher anxiety. A threat-congruent difference in explicit self-esteem was revealed only between two groups: 1. cancer and long hospitalization and 2. non-cancer and short hospitalization. For implicit self-esteem the phenomenon of implicit compensation of self-esteem was predicted and confi rmed: among cancer-sufferers the Name Letter Effect was greater than among other patients. Also, in the cancerpatients group, the result of Rudman et al. (2007) was replicated: increasing anxiety was connected with increasing implicit self-esteem.
