Opis:
The aim of the study was to determine balance parameters in a group of young patients with vertigo symptoms and to verify posturography helpfulness in clinical evaluation of vestibular system pathology. Methods: 77 children and adolescents of age 3–18 suffering from vertigo episodes participated in the study (46 girls, 31 boys). They underwent audiology objective tests and balance test on stable surface. Calculated balance parameters were analyzed in reference to: eyes opened and closed, age influence, sway comparison in anterior-posterior and medial-lateral, differences between subgroups with and without vestibular deficits. Discriminant analysis was performed to assess classification ability to impaired group in two cases: only balance parameters and both audiology and balance parameters. Results: Patients with vertigo symptoms generally keep their balance properly on stable surface. Balance parameters do not depend on presence of vestibular system pathology. Values increased in eyes closed conditions. Left/Right and Anterior/Posterior differences were not statistically significant. The negative correlation between age and some balance parameters is present, stronger in the case of eyes opened and weaker or absent in vestibular impaired group. Also, correlations between axes were found, higher in impaired group in comparison with not impaired one. Conclusions: Discrimination based on balance parameters is poor not comparable to one built on combined: audiology and balance parameters, so typical balance parameters’ analysis is not so useful in clinical practice when the reason of vertigo episodes should be assessed, but verify compensation process and measure with objective numbers the progress of recovering, the actual functional patient’s status.

The aim of the study was to find out whether among young active people – the future healthcare professionals – there is a risk of developing lumbar spine overload disease depending on the occurrence of the disbalance of paravertebral muscles and to assess the fatigue of the examined muscles depending on the body position during the work. Methods: The study involved 50 randomly selected students of physiotherapy and emergency medical services of a mean age of 21 years. The surface electromyography (sEMG) was used to measure the activity level of spinal extensors of the cervical and lumbar spine. The statistical analysis of the results was made using the RStudio software. Results: Study shows that the majority of the examined students had a slight asymmetry between the bioelectric activity of the right- and left-side dorsal extensor bands, both in the cervical and lumbar sections (right side 118.6 Hz / left side 115.7 Hz / extension and 98.6 Hz / 95.5 Hz /flexion). Depending on the position, significant fatigue in the examined muscles was found ( p < 0.05). Conclusions: The experiment showed a slight disbalance in the average bioelectromyographic activity in the area of spinal extensors during their symmetrical work. This can be the basis for the development of the overload disease in the weaker muscle parts, which generate more fatigue during the activity. It is worth to carry out similar tests in a much greater group, taking the longer-lasting muscular effort into account.

The number of primary hip arthroplasties is growing every year. One of the most common long complications is the aseptic loosening of the prosthesis leading to serious complications in patients life, pain and following surgeries. In the literature, there is a lack of studies regarding balance impairment in this group of patients, which is why there was a need to conduct the study in order to find possible falling risk factors and simple functional, diagnostic test. The aim of the study was to assess functional state and static balance disorders in patients admitted to orthopaedic department due to loosened hip prosthesis prior to its replacement or removal. Methods: The study population were 30 patients (15 women and 15 men) admitted to Orthopaedic Department for diagnosis and treatment of loosened hip prosthesis. Patients’ mean age was 71.67 ± 9.21, body height 169.20 ± 8.75 cm, body weight 77.83 ± 12.96 kg. The clinical control group consisted of 30 healthy subjects age-, body height- and weight-matched, without degenerative disorders of lower extremities. Function was assessed with Harris Hip score. The static balance test was conducted in the mornings, in quiet separate room on bi-modular stabilometric platform CQStab2P, registering the movement of centre of foot pressure (COP). Results: Measurements with eyes open and closed showed significant differences in limb loads – the load to the healthy limb was several times greater. Results of tests on the platform with eyes closed showed significantly worse balance in patients group. Their total statokinesiogram path was longer, and their COP sways number was greater. Similarly to the test with eyes open, the load of the healthy limb was much greater. Conclusion: Hip replacement loosening results in significant worsening of balance parameters. Balance assessment could be an additional useful diagnostic tool for hip replacement loosening available for physiotherapists.

Opis:
Chronic ankle instability (CAI) is a common sequence of ankle sprain. Conservative treatment of CAI is currently accepted as a primary mean of managing the condition and preventing of recurrence. Objectives: The purpose of the current study was to evaluate the effect of Weight-bearing Exercise for Better Balance (WEBB) program and unilateral balance training exercises on postural control in females with CAI. Methods: Thirty-five females with CAI participated in this study. The participants’ mean values of age, body mass, height, and body mass index were 21.76 ± 1.96 y, 64.82 ± 9.37 kg, 161.85 ± 7.24 cm, and 24.76 ± 3.76 kg/m², respectively. They were randomly assigned into three groups: experimental group A (n = 13) who received WEBB program, experimental group B (n = 12) who received unilateral balance training, and control group C (n = 10) using statistical random tables. Balance parameters were measured for all participants via Biodex Balance System (BBS) before and after four weeks of balance training. They included overall stability index (OASI), antero-posterior stability index (APSI), and medio-lateral stability index (MLSI). Results: There was a significant improvement in the post testing mean values of OASI and APSI, compared with the pre testing values in both experimental groups (A and B). There was no significant difference between the pre and post testing mean values of MLSI in the experimental group (A). Conclusions: Both WEBB program and unilateral balance training have positive effects on postural control in females with CAI.

It has recently been shown (Arroyo et al. [1]) that 3D DEM models are able to reproduce with reasonable accuracy the macroscopic response of CPT performed in calibration chambers filled with sand. However, the cost of each simulation is an important factor. Hence, to achieve manageable simulation times the discrete material representing the sand was scaled up to sizes that were more typical of gravel than sand. A side effect of the scaled-up discrete material size employed in the model was an increased fluctuation of the macro-response that can be filtered away to observe a macroscopic steady-state cone resistance. That observation is the starting point of this communication, where a series of simulations in which the size ratio between penetrometer and particles is varied are systematically analyzed. A micromechanical analysis of the penetrometer–particle interaction is performed. These curves reveal that a steady state is arrived also at the particle–cone contact level. The properties of this dynamic interface are independent of the initial density of the granular material.
