Opis:
Change management is considered to be an important element of safety management systems, lack of which can lead to industrial accidents and disasters. Therefore, risk management related to changes introduced to the railway system has become one of the processes covered by regulations aimed at harmonising the rules of railway transport in the whole European Union (EU). This article presents the results of a comparative analysis of available source materials in terms of determining the manner and level of implementation of EU-wide rules for risk management related to introduced changes. The analysis was based on data from five selected EU Member States. A common issue raised in practically all of the reports analysed is the lack of detailed definitions of terms such as significant change, insignificant change, the impact of change on the safety of the railway system. Attempts to solve this situation have been specified by us and evaluated from the viewpoint of safety engineering.

Opis:
The issue of managing the risk associated with introducing changes to the railway system is very important from the point of view of safety management, as any change can significantly reduce the level of safety of a railway system. For this reason, such changes are regulated at the European Union (EU) level through a dedicated legal act known as the Common Safety Method for risk assessment (CSM). The example presented in this paper is a portion of the analysis carried out for an Estonian freight carrier with a bowtie method, which we improved so that it fully complies with the CSM requirements. This analysis concerns a change consisting of allowing the possibility of a freight train being driven by one person (with no assistant driver). The case study presented in the paper, although limited due to confidentially issues, allows a full description of how to use the proposed method in real-world applications.

Opis:
The Tactical Aircraft Operating System (a system for tactical aircraft operational usage) constitutes the main research area of this paper. The authors present this system via an innovative model for an anthropotechnical system in the format of an aggregated analysis domain. This model constitutes a detailed explication of a fragment of the existing Tactical Air Force System metamodel at the level that concerns tactical aircraft operation systems. The aggregated analysis domain for tactical aircraft operations is built on seven single-analysis domain, each of which includes a series of processes, operations and events, related to the airman’s aviation activities cycle, what happens during an air mission and when an aircraft operating cycle is carried out. The subsequent individual analysis domains are connected to each other by an airman-aircraft anthropotechnical pair. The introduced method of modelling the Tactical Aircraft Operating System ensures the desired generality, comprehensiveness, coherence and transparency of such a model, which allows for its implementation in various types of tactical aircraft and tactical air force bases. In principle, within the presented considerations, the model is described in relation to a single aircraft operating cycle, which is carried out by one airman during a single air mission. However, the authors have taken care when creating this model structure, so that its implementation in more complex aircraft operating cycles (more than one aircraft and one airman) is possible. Apart from the analysis domains (“before take-off”, “departure”, “air task”, “arrival”, “after landing”), which are closely related to the essential aircraft operations, the model also includes the analysis domains associated with air mission preparations and summaries. The model for the Tactical Aircraft Operating System is described by the authors in textual form, as well as in tabular form. The presented concept constitutes an attempt to formally notate the Tactical Aircraft Operating System model, in which each of the individual analysis domains can be treated as an area of interest for hazard risk management processes within the Tactical Air Force System.

The process of describing human activities in procedures has been used since the beginning of the 20th century. However, it is increasingly evident that procedures understood as sets of orders and prohibitions can be counter-productive because they do not allow the proper use of employees’ knowledge and experience. Therefore, it is postulated that guidelines for practice should be co-developed by employees from the ‘sharp end’ of the organization, but there are no simple methods that can achieve this aim. In the present work, we propose a procedure based on sticky notes, inspired by how information technology teams function. We present a description of the original sticky notes method (SNM) and demonstrate its application in the railway sector. As a result of the workshops conducted with the participation of experts, we gained knowledge about practices that were not included in the documentation, but that could significantly improve the process under research. The primary purpose of the SNM is to ensure the involvement of employees in the process of creating guidelines for practice. This method is particularly useful for describing linear processes in which activities can be arranged chronologically.
