Dry eye syndrome is a group of medical conditions that can be caused by many factors, as a result of which there is an abnormality in moisturizing of the eye surface by a tear film. One of the causes of this disorder is Sjögren’s syndrome, which is an autoimmune disease in which the body produces antibodies against its own proteins. The glands of external secretion, mainly lacrimal and salivary glands are damaged as a result of inflammation. Associated with this side effects, affect negatively and reduce the quality of life, which is why in the following article we present the available therapeutic options for patients suffering from ophthalmological manifestations of Sjögren’s syndrome.

Acute coronary syndrome (ACS) and oncological disease are more frequently observed in the general population as discussed in the part I of this article. Treatment of myocardial infarction in oncological patients becomes a real struggle for clinicians, especially that the data from clinical trials including cancer patients with ACS are very limited. The choice of treatment modality should consider many existing factors considering the type of ACS – non-ST-segment elevation myocardial infarction vs ST-segment elevation myocardial infarction, patient’s condition, type of cancer and oncological treatment applied. Taking into consideration above mentioned factors, clinicians have to face three therapeutic options: invasive, conservative or combination of both in order to choose the best and most beneficial treatment. This article summarizes the current therapeutic approach to the management of ACS in cancer patients.

The paper presents the results of research on the occurrence, severity and persistence of the dry eye syndrome in patients after laser vision correction. The patients were treated with preservative-free lubricating eye drops, with a specific percentage of sodium hyaluronate. The research was carried out on a group of 70 patients who underwent laser vision correction.
W pracy przedstawiono wyniki badań dotyczących wystąpienia, nasilenia i długości trwania zespołu suchego oka po wykonaniu laserowej korekcji wzroku i zastosowaniu kropli nawilżających bez konserwantów o określonym stężeniu procentowym hialuronianu sodu. Badania przeprowadzono na grupie 70 osób, u których wykonano zabieg laserowej korekcji wzroku.

We present 2 cases of acute coronary syndrome in patients with concomitant neoplastic disease during chemotherapy. In both patients, we found unusual angiographic image which showed that despite the presence of very large thrombi, there were no or hardly any atherosclerotic lesions in the vessel occlusion site. Aspiration thrombectomy was successfully performed in both cases to restore coronary flow and in the second patient, two bare metal stents were also implanted. After the patients were discharged from hospital, we decided to add enoxaparin to the routine dual antiplatelet therapy of both of them. So far, no guidelines have been developed on how to manage patients with acute coronary syndrome and concomitant neoplastic diseases while on chemotherapy.

Chronic lymphocytic leukemia is the most commonly recognized type of leukemia in adults. The appearance of systemic symptoms such as weight loss, fever, or local symptoms in the form of rapidly growing organomegaly, lymphadenopathy in a patient with CLL raises the suspicion of transformation into a high-grade lymphoma – defined as Richter syndrome which is usually associated with very poor prognosis. The described case concerns a 71-year-old patient with this diagnosis, in whom due to the confirmed resistance to subsequent lines of immuno- and chemotherapy, an „emergency” treatment with a modern chemotherapy drug from the aza-anthracendion group – pixantrone was used. Treatment with pixantrone was associated with a relatively good response, translating into partial remission (also in the area of infiltrative changes in the head and neck structures), stabilization of the course of the disease and, consequently, allowed to extend the patient’s life.

Paraneoplastic neurological syndromes (PNS) are a group of rarely observed disorders, accompanying about 1% of cancer diseases. They have a typically aggressive course, leading to profound and essentially irreversible disability. The pathogenic cause behind PNS is damage of the nervous system structures as a consequence of the body’s immunological reaction induced by cancer. Onconeural antibodies generated as part of the reaction target the tumour tissues, but also the regular nervous tissue recognized as antigens. Presence of onconeural antibodies reveals a 50–60% sensitivity and 100% specificity in diagnosing PNS. Imaging and laboratory tests have a limited impact on diagnosis. What is crucial is the analysis of clinical systems following Graus’s criteria (elaborated in 2004). In the case presented here, the patient underwent treatment in many neurological departments due to the symptoms of progressive damage of the nervous system. She was eventually diagnosed with the stiff person syndrome related to breast cancer. In spite of the treatment, including an efficacious oncological therapy, the severe neurological deficit resulted in serious motor disability.

Exfoliation syndrome is an age-related disorder of the extracellular matrix, characterized by progressive accumulation of abnormal fibrillar material in several ocular and extraocular tissues. Although the exact etiopathogenesis is still unknown, several genetic and environmental factors appear to be involved in disease pathogenesis. Recently, single nucleotide polymorphisms in lysyl oxidase-like 1 have been found to be strongly associated with exfoliation syndrome. Dysregulation of lysyl oxidase-like 1 specifity and activity, an enzyme with a role in tropoelastin cross-linking and elastin homeostasis is thought to be involved in the development of exfoliation syndrome. This review aims to examine the recent genetic findings in the disease process.
Zespół pseudoeksfoliacji to związane z wiekiem zaburzenie macierzy zewnątrzkomórkowej, charakteryzujące się postępującym odkładaniem się nieprawidłowego materiału fibrylarnego w różnych tkankach oka i poza nimi. Dokładna etiopatogeneza choroby pozostaje nieznana, jednak wydaje się, że istnieje kilka czynników genetycznych i środowiskowych w nią zaangażowanych. Niedawno wykazano, że polimorfizmy pojedynczego nukleotydu w obrębie oksydazy lizynowej 1 są silnie powiązane z zespołem pseudoeksfoliacji. Dysregulacja swoistości i aktywności oksydazy lizynowej 1, czyli enzymu odgrywającego rolę w sieciowaniu tropoelastyny i homeostazie elastyny, jest uważana za jeden z czynników rozwoju zespołu pseudoeksfoliacji. Celem niniejszej pracy jest przeanalizowanie najnowszych doniesień dotyczących genetyki wspomnianego procesu chorobowego.

Nelson’s syndrome is a rare condition of enlarging pituitary mass lesion producing ACTH that can develop after total bilateral adrenalectomy (TBA) in patients with Cushing’s disease. It is characterized by hyperpigmentation of the skin and mucous membranes and elevated plasma levels of ACTH. In this clinical review, the diagnosis and treatment strategies of patients with Nelson’s syndrome including surgery, radiation, and pharmacotherapy are presented.

Follicular mucinosis is a rare dermatosis characterized by accumulation of mucin in the follicular epithelium and sebaceous glands. Clinically, it is characterized by the presence of papules or well-circumscribed and infiltrative plaques. In this paper, we report a case of a 15-year-old girl, in which an asymptomatic, erythematous and infiltrative plaque evolved within a few weeks in the forehead region. There was no regression of the lesion with the use of topical glucocorticosteroids and isotretinoinum for 30 days.

We present the case of a 70-year-old male patient with metastatic and castration-resistant prostate cancer as well as advanced bladder cancer, who suffered a non-ST-elevation myocardial infarction (NSTEMI). The cause of ischemia was critical left main stem stenosis, and right coronary artery stenosis. Initially, the patient was qualified for conservative treatment, but due to the intensifying symptoms of myocardial ischemia, it was decided that a hemodynamic intervention was necessary. Two drug-eluting stents were implanted, leading to clinical improvement. Following the treatment, the patient was considered as a candidate for further anti-cancer therapy. Unfortunately, due to the bladder cancer progression, anaemia and haemorrhage, his clinical condition exacerbated, and the patient died about 1 month from the cardiac intervention. Apart from the case description, the paper includes a review of literature on the treatment of acute coronary syndromes without ST-segment elevation in cancer patients.

Angioedema is a localized and self-limiting oedema of the subcutaneous and submucosal tissue. Case presentation: 73-year-old man was admitted to the Division of Allergology with the diagnosis of angioedema. Detailed physical examination led to the suspicion of superior vena cava syndrome with secondary face swelling. Biopsy revealed metastasis of small cell carcinoma in superclavicular lymph nodes. Conclusions: Facial swelling, especially recurrent and non-inflammatory, is often, but not always caused by angioedema. Insightful differential diagnosis is crucial before making the final one.
