Based on NFZ published aggregate data, the public spending on cancer care in Poland was 6.3 billion PLN in 2011 (or approximately 10% of total public health spending). Poland is one of the few large countries in the world that has two centralised and public data sources for cancer, namely the National Cancer Registry (NCR, pol. Krajowy Rejestr Nowotworów – KRN) and activity expenditure database run by the National Health Fund (NFZ, pol. Narodowy Fundusz Zdrowia). We show in our article that having a population-based registry and a complete treatment/clinical care dataset is a necessary condition to have a useful cancer strategy data set that can in turn lead to evidence based health policies in the area of cancer. Lack of audited and publicly available cancer data means that a coherent cancer strategy, assessing service provision and cost effectiveness of treatments and monitoring outcomes is, in our opinion, currently not possible in Poland. We postulate that Poland should create a task force to create cancer data strategy based on NCIN (National Cancer Intelligence Network) in the UK or Cancer Australia.

Breast cancer is the most common cancer among females worldwide. The mean age of breast cancer patients is > 60 yrs old, and it is seldom found in women < 40 yrs old (6.5%) and in very young women < 35 yrs old (0.6%). In young females, fertility and all its aspects are an additional therapeutic challenge. Before initiating treatment, the oncologist should offer effective contraception to be applied throughout the therapy, bearing in mind that fertility preservation is of utmost importance. When it comes to breast cancer in pregnancy, the attending physician should use a therapy which is safe for both the mother and the foetus. Chemotherapy, radiotherapy, hormonal therapy and immunotherapy can, to a lesser or greater degree, damage the ovarian function resulting in amenorrhea in women < 50 yrs (33–76%). However, owing to fertility preservation strategies, more and more pregnancies are successful, even in breast cancer survivors.

On 2–4 May 2013, in Brussels took place the 5th IMPAKT conference on translational research in breast cancer. The most important issue raised during the conference was the phenomenon of breast cancer heterogeneity, both inter- and intratumour. Heterogeneity of breast cancer was confirmed at the molecular level, showing the differences in the frequency of genetic aberrations (typically PIK3CA, TP53 and ARID1A) in patients with hormone-dependent advanced breast cancer (BOLERO-2 trial). The results of genetic tests were also presented, which showed that in order to reduce the impact of heterogeneity within the tumor, 3 biopsies should be performed from each breast cancer. The most common disorders of PI3K/mTOR pathway were analyzed depending on the molecular type of breast cancer. However, an attempt to determine by immunohistochemistry biomarkers predicting response to combination therapy with everolimus and trastuzumab failed. The results of two studies, which analyzed the value of molecular tests in predicting late recurrence of breast cancer (in 5–10 years after diagnosis), were presented – the best results were achieved with PAM50 and Breast Cancer Index. The latest discoveries in molecular imaging were also presented – it allows for early assessment of treatment efficacy.
W dniach 2–4 maja 2013 r. w Brukseli odbyła się 5. konferencja IMPAKT poświęcona tematyce badań podstawowych nad rakiem piersi. Najważniejszym zagadnieniem poruszanym podczas konferencji było zjawisko heterogenności raka piersi, występujące zarówno między guzami, jak i w obrębie jednego guza. Potwierdzono heterogenność raka piersi na poziomie molekularnym, wykazując różnice w częstości aberracji genetycznych (najczęściej w obrębie PIK3CA, TP53 i ARID1A) w guzach u chorych na hormonozależnego, zaawansowanego raka piersi (badanie BOLERO-2). Przedstawiono również wyniki badań genetycznych, które wykazały, że aby ograniczyć wpływ heterogenności w obrębie guza, należy wykonywać 3 biopsje z każdego raka piersi. Zbadano najczęstsze zaburzenia szlaku PI3K/mTOR w zależności od typu molekularnego raka piersi. Natomiast próba określenia za pomocą immunohistochemii biomarkerów odpowiedzi na terapię skojarzoną ewerolimusem z trastuzumabem nie powiodła się. Zaprezentowano wyniki dwóch prac naukowych, w których analizowano wartość testów molekularnych w predykcji późnego nawrotu raka piersi (5–10 lat od rozpoznania) – najlepsze wyniki osiągnięto w przypadku PAM50 i Breast Cancer Index. Poza tym przedstawiono najnowsze doniesienia w zakresie obrazowania molekularnego pozwalającego na wczesną ocenę skuteczności stosowanej terapii.

Palliative chemotherapy in patients with thrombophilia is a challenge for clinical oncologists. On one hand the optimal treatment should be very safe for patient, on the other hand – as effective as possible. That therapy is also connected with proper thromboprophylaxis. The incidence of multiple cancers is a serious problem of modern oncology. In patients whose medical history includes two or more cancers, it is essential to obtain histopathological diagnosis before the administration of treatment of disseminated disease. It allows to avoid improper therapy which is often toxic. The article presents the case of a patient with antithrombin deficiency and two cancers in medical history: colon cancer and endometrial cancer. The treatment of the patient because of the metastatic cancer was started after the histopathological diagnosis. In metastatic lymph node the endometrial cancer was recognized. The carboplatin and paclitaxel were used in the therapy. The treatment was conducted in four course chemotherapy. It revealed considerable polyneuropathy and hematologic toxicity limiting from further therapy. Treatment allowed to obtain biochemical response, decrease of metastases and the condition of a patient improved. During the systemic therapy thromboprophylaxis with acenocumarol was used. The incidence of venous thromboembolism or bleeding complications were not noticed.

Breast cancer is the most common malignancy in women in Poland and the entire world, but thanks to early diagnostics and efficacious therapeutic methods, its prognosis is good. In some patients with advanced breast cancer, long-term survival is observed, which is why it is essential to select the most appropriate therapy, and to follow up on the course of disease and possible adverse events related to the treatment applied. The slow development of hormone-receptor-positive cancer, and a multitude of therapeutic options constitute a real challenge in palliative treatment. The use of fulvestrant resulted in a good response in both of the presented cases, and did not in any way comprise the daily lives of the patients over a long period of time.

Introduction: Cancer treatment is multidisciplinary in its character and places a heavy burden on the patient. Quality of life is a concept that is closely related to the satisfaction with life. Cancer patients’ satisfaction with life, especially after recovery, is an increasingly examined indicator of treatment quality. Aim of the study: Evaluation of satisfaction with life in patients after radical cancer treatment. Material and methods: The study included 100 people, 81 women and 19 men of all ages, following radical treatment of various cancers. The study was conducted in 2016 at the Centre of Oncology in Bydgoszcz. The study used the Satisfaction with Life Scale (SWLS), adapted by Z. Juczyński, as well as a sociodemographic data questionnaire. Results: In general, cancer patients have an average level of satisfaction with life – the average score was 22.37 and 6.19 sten. The greatest satisfaction with life (average points) was reported for professionally active people – 22.67, patients between 41–50 years old – 23.5, patients with secondary education – 23.4, and patients with at least one child – 22.73, though the differences were not statistically significant. Additionally, patients 3–5 years after treatment declared a higher satisfaction with life – 23.05, as did patients without concomitant diseases – 22.77, though again the results were statistically non-significant. Conclusions: In general, patients after radical anti-cancer treatment have an average satisfaction with life, and in half of them satisfaction with life is high, regardless of the duration and type of cancer. Sociodemographic factors have no impact on the satisfaction with life scores.

Triptorelin as the luteinizing hormone releasing hormone (LHRH) analogue has its own place among other available forms of androgen-depravation therapy (ADT) of locally advanced and metastatic prostate cancer (PCa). Nowadays, in times of development of new therapies in castration-resistant PCa, ADT remains the back bone therapy, which may be supplemented with one of novel drugs. The results of basic research indicate, that apart from the main mechanism of action based on lowering a testosterone concentration to the castration level, triptorelin may have a direct inhibitory effect on tumor cells. Formulations of tryptorelin are available to administer as 1-month, 3-months and 6-months sustained-release forms that may be given intramuscularly or subcutaneously. Constant concentration of triptoreline is maintained by using special microspheres in drug production. Pharmacokinetic and pharmacodynamic properties of particular forms were extensively tested, which allows for safe usage and retain of predictable and high efficacy. Indicators of effective ADT are fast reduction and maintenance of the state of castration. This phenomenon translates into a decrease of the prostate-specific antigen and longer survival. Triptorelin successfully meets these objectives based on a number of phase I–III studies. There is a noticeable lack of comparative studies on effectiveness of particular ADT forms. This may stem from an assumption, that all LHRH analogues demonstrate similar effectiveness because of the class effect. However, some evidence highlight significant differences in efficacy among these drugs. Triptorelin compares especially favourably, particularly as a drug reducing the testosterone level to the lower recommended values (< 20 ng/dl). Side effect profile during the therapy with triptorelin is largely the result of inhibition of the hypothalamic pituitary-testicular axis. Hormonal disturbances linked to hipoandrogenism cause changes in lipid metabolism and glucose tolerance, which may influence the cardiovascular risk. This article is a review of key reports regarding triptorelin and a summary of the role that triptorelin plays in contemporary ADT in advanced PCa.

The relationship between cancer and an increased incidence of venous thromboembolism (VTE) is well documented. VTE is a common complication of cancer, which usually occurs in the advanced stages of the disease. It often manifests itself before the cancer has been diagnosed and frequently is the first symptom of malignancy. In particular, pulmonary embolism is a major epidemiological problem because of its prevalence and high mortality. This article contains a review of the literature on VTE and occult cancer. It indicates the problem of a lack of an effective diagnostic strategy for malignancy in patients with idiopathic venous thromboembolism.

Cancer-related microangiopathic hemolytic anemia is rarely recognized as a paraneoplastic syndrome with a very poor prognosis in cancer patients. The treatment and prognosis are significantly different from that in other thrombotic microangiopathies, such as thrombotic thrombocytopenic purpura and hemolytic uremic syndrome. The case of described patient demonstrates the diagnostic difficulties in diagnosing the causes of hemolytic anemia in patient with breast cancer and appropriate treatment.

Apalutamide belongs to the ARTA group. This drug is a new generation selective androgen receptor inhibitor. Patients with prostate cancer develop resistance to castration testosterone levels over time during hormone therapy. Therefore, it is necessary to consider additional therapeutic options based on drugs from the ARTA group. The article presents the role of apalutamide in the treatment of prostate cancer and its effectiveness. Moreover, attention was drawn to the need for close cooperation between the urologist and the oncologist in order to optimize the treatment of a patient with progressive prostate cancer.

Breast cancer is one of the most common malignancies across the world, including Poland. Chemotherapy plays an important part in the treatment of the disease. Most of the available chemotherapy drugs and regimens have undergone randomized clinical studies and have been registered for that specific indication. However, a number of drugs are used in an off-label manner, i.e. outside the officially approved product specifications. The paper discusses the use of several off-label therapies in breast cancer in order to demonstrate that such treatment may be well-grounded and indeed turns out beneficial in many cases. It describes the use of liposomal doxorubicin in pre- and post-operative treatment, capecitabine for incomplete efficacy of preoperative treatment, and the administration of metronomic vinorelbine. Moreover, the paper is aimed at demonstrating the legal basis and the principles of marketing authorization of off-label drug use.

Hormone therapy is one of the basic methods of prostate cancer treatment, both in adjuvant and palliative treatment. Castration-resistant prostate cancer is an incurable disease. However, the introduction of new hormonal drugs to therapy significantly extended the survival time of patients with relatively low harmfulness of therapy.
