Introduction. Parents as day-to-day caregivers looking after their children’s health and upbringing are crucial in the process of hearing loss prevention among the young. The aim of the study was to assess the parents’ knowledge and awareness of hazards bringing about hearing loss, possibilities to prevent this, as well as their reaction in the case of problems of their children with hearing. Material and methods. The research group comprised respondents (402 persons), medical or paramedical professionals. None of the surveyed was a doctor. 56% of the surveyed were inhabitants of large cities, 24% of small towns, and 20% of village, mostly located in the area of Greater Poland (Wielkopolska) (97%). To perform the research a survey was devised. GSES scale was applied as an accessory tool. Results. The subjects surveyed posses a great deal of knowledge concerning basic conditions that could adversely affect the hearing of children. Village inhabitants are half as afraid of hearing loss hazards as the inhabitants of large cities. More than 40% of the respondents claimed they do nothing to counteract their children’s overexposure to noise. In 48.5% of cases, GPs had not drawn parents’ attention to possible complications resulting from upper respiratory tract infections in children. Parents know where they should go to in case their children develop hearing problems or sudden hearing loss. Conclusions. Knowledge and awareness of factors that may trigger hearing loss is not synonymous with avoidance of the problem. Main components of hearing loss prevention among children and youths should be administrative actions, extensive education, and proper childcare at home.

Introduction and objective. Ensuring the adequate intake of micronutrients by populations is an important aspect of public health. This study aims to elucidate the process of setting nutrition recommendations and the development of subsequent policies associated with micronutrients in Poland by using the case study of folate. Materials and method. The study was based on 13 interviews conducted between October 2010 – February 2011, and desk research comprising a review of the associated scientific literature and relevant documents, such as scientific opinions and reports of the meetings of scientific committees. The interviews were conducted within the EURRECA (EURopean micronutrient RECommendations Aligned) Network of Excellence in the frame of the European Union’s 6th Framework Programme. Results. Since 1997, the Experts Group of the Ministry of Health recommended that all women of childbearing age should consume folic acid as a supplement to prevent neural tube defects in their offspring. The fortification of foodstuffs with folic acid is not obligatory. This study describes the revision of the folate nutrient recommendation and the process by which strategies to increase the intake of this vitamin by the Polish population were identified. The institution responsible for the choice and implementation of folate policy is the Ministry of Health and the National Food and Nutrition Institute is responsible for setting/updating the nutrient recommendations. Conclusions. The current Polish nutrition recommendations for folate are consistent with the levels set by most other countries. The constant improvement of nutritional knowledge on folate among consumers, especially young women, is necessary.

Introduction and objective. Occupational asbestos exposure is one of the major public health issues. Although asbestos use in Poland has decreased, asbestosis continues to remain an important health concern in the area of occupational medicine. The aim of the study is to perform a descriptive analysis of hospitalization cases in patients suffering from asbestosis in Poland. Materials and method. The authors used hospital discharge records to conduct a retrospective, population-based study. To estimate the asbestosis hospitalization rate, data from the Nationwide General Hospital Morbidity Study conducted by the National Institute of Public Health were used. Events were defined as in-patient hospital discharges during 2006–2016. 1,101 hospitalization records of 764 patients were included in the study. Results. In the study group of first-time hospitalized patients, the mean and median ages were 68.1 and 69 years, respectively; CI: 67.3–68.9; SD: 11.1, min-max:16–99 years. In this group, significant gender differences were observed (560 males vs. 204 female; P<0.001). No statistical differences were observed regarding the place of residence. During the observation period, 61 in-hospital deaths were reported (8% of all patients), 40 of whom (5.2 % of all patients) took place during the first-time hospitalizations. Asbestosis hospitalizations were mainly clustered in the south-western region, whereas the highest hospitalization rates were clustered in several areas of Poland. Conclusions. The authors believe this study to be the first evaluation of asbestosis in Poland made on the basis of the hospital morbidity database. The data presented may be helpful in comparative studies on the epidemiology of asbestosis across European countries.

Cannabis is the most popular illicit drug on the European market. Over 16 million young Europeans have used it at least once in the last few years. The recent trends in the consumption of marihuana differ between countries. Some countries face an increase in the prevalence of cannabis use, including Poland, where the level cannabis use has been systematically increasing since the 1990’s. According to a recent ESPAD study, 19% of Polish adolescents aged 15–16 have used cannabis in the last year. Marihuana is also a leading substance when analyzing the data of seizures and crimes. The recent EMCDDA Annual report on the drug situation in Europe notes the increasing potency in cannabis available on the market. Some countries face an increasing number of emergencies caused by marihuana, which was unlikely to have happened previously. In almost all European countries there is an ongoing discussion about loosening marijuana laws or its complete legalization. There is also ongoing discussion on the use of marihuana in therapy as a medicine. Many scientific studies are being conducted in this field. Some of the results are promising; however, there is no well-designed human trial which would unequivocally confirm that medical cannabis is effective as a medicine, or more effective than other medicines on the market. The problem is that the debate on the medical use of marihuana becomes more ideological and less professional. The medical use of marihuana is strongly supported by organizations lobbying for the legalization of cannabis use. Research on the medical use of cannabis should be continued, as there are some promising results supporting therapy in different medical conditions. However, the use of cannabis as a medicine should be discussed only among professionals. If marihuana is to be used for medical purposes, the fact that it is the most popular illicit drug in Europe is irrelevant.

Objectives. The aim of the study was to identify socio-economic factors that may influence the emotional changes which occur among new mothers in the first days postpartum. Materials and method. A group of 541 women completed a questionnaire consisting of 30 multiple-choice questions, Edinburgh Postnatal Depression Scale (EPDS), and Hospital Anxiety and Depression Scale (HADS). Statistical calculations were performed with the use of Statistica v.10 and Cytel Studio v. 9.0.0. Results. The findings revealed the presence of factors which might increase the risk of mood disorders during the postpartum period. Conclusions. Women who demonstrate warning symptoms should be screened for postnatal emotional changes and mood swings during their hospitalization after delivery. EPDS seems to be a suitable tool for early detection of emotional disturbances.

Introduction. Bovine tuberculosis (bTB) is an infectious disease that occurs in many species of both domestic and freeranging animals, as well as animals kept in zoos. According to the Polish regulations, cattle tuberculosis are slaughtered and microbiological examinations are performed, the rest of animal species can be treated and laboratory diagnostics are not obligatory. Case report. The presented case concerns two male giraffes which were purchased by the zoo and united with a third male. After a year, the oldest male died. Post mortem examinations confirmed generalized tuberculosis. After a further six months, the second male was euthanized after suffering great pain. The material for the study of drug resistance was a swab from the nose, obtained ante mortem from the third male. Attempted treatments did not produce the expected results. Genotyping allowed the exclusion of a common source of transmission. Conclusions. The final effect of the anti-tuberculosis therapy in the male giraffe raises the question whether the research team should have undertaken the treatment of the animal with active tuberculosis.

Introduction and objectives. Chlamydia (C.) felis can cause infection which may be associated with conjunctivitis and/or respiratory tract disease, particularly in kittens, but could also be the cause of the disease in adult cats. Infection is more common in multi-cat environments. The zoonotic potential of C. felis appears low, but exposure to this microorganism is possible by handling the affected cats, by contact with their aerosol, and also via fomites. Materials and method. In the study, 140 cats of various breeds from Košice region in Slovakia were studied. Conjunctival samples were obtained from 71 clinically healthy cats (50.7%) and 69 cats with clinical signs of conjunctivitis and upper respiratory tract impairment (49.3%). Cats were divided into 4 groups according to breed and type of environment in which they lived. In the 1st group were cats kept inside only (n=33), in the 2nd group, free-roaming cats (n=50), the 3rd group comprised stray cats, taken from the streets (n=28), and the 4th group included cats kept in shelters or deposit devices (n=29). Molecular method PCR and DNA sequencing was used as the diagnostic method. Results. Overall positivity was 17.1%. Of the 24 positive cats, the highest positivity was detected in the population of stray cats (35.7%) and shelter cats (31%). In the group of free-roaming cats, 10% had positivity. No positive animals were detected in the group of cats kept inside only. It was also found that the risk of C. felis in cats with clinical signs of disease was more than 7-fold higher than in cats without clinical signs of conjunctivitis and respiratory tract. Conclusion. The obtained results show that cats, especially stray and shelter cats, can be important sources of feline chlamydiosis, and due to their close contact with people they can present a risk for transmission.

Viral gastroenteritis and other water-borne diseases are the most neglected areas of research in Pakistan. To determine the quality of water, 4 enteric viruses were studied from different localities of Peshawar, Pakistan. The study validates the viral detection method for Rotavirus (RV), Human adenovirus (HAdV), Enterovirus (EV) and Hepatitis A virus (HAV), directly from water sources of rural areas of Peshawar, KPK, Pakistan. Overall, 95 five water samples were tested; among them, 9.47% were positive for RV, 38.94% for HAdV, 48.42% for EV and 12.63% for HAV. The presence of these viruses in water was directly correlated with meteorological data. High prevalence of EV and HAdV was detected frequently in the wet season from May – September, which can be the potential cause of spreading of gastroenteritis in the population. Environmental surveillance is an additional tool to evaluate the epidemiology of enteric viruses circulating in a given community.

Dogs serve as the vectors of serious zoonotic parasitic diseases. In the month of May 2012 – 2014, 339 dog faeces samples from seven public sites in Chełmno, a town in northern Poland, were collected and examined to determine the gastrointestinal parasite fauna of dogs. Each faecal sample was dissected with a needle, checked for tapeworm segments and examined for parasite eggs and oocysts using the flotation and decantation method and a modified Baermann technique. Differences were observed in the degree of parasite species occurrence. The most dominant were Toxocara canis and Ancylostomatidae. The detected species included: T. canis and Toxascaris leonina eggs (23.4% and 10.2%, respectively), as well as eggs from the Ancylostomatidae family (16.2%), Trichuris vulpis eggs (6.6%), Taenia type eggs (4.6%), Dipylidium caninum (5.2%) and Cystoisospora (Isospora) spp. oocysts (10.9%).

Introduction. The epidemic of tobacco smoking remains one of the most important challenges in public health. It is associated with premature death. For this reason, the Act of 9 November 1995 on the Protection of Public Health against the Effects of Use of Tobacco and Tobacco Products, as amended on 15 November 2010, implemented significant additional restrictions on smoking in public places. Objective. The objective of this study is evaluation of the impact of smoking regulations on social behaviour and attitude to the legal forms of tobacco control, due to the new smoke-free policy. Materials and method. A secondary statistical analysis of third party data was used in the study. The material was obtained from the Public Opinion Research Centre. Polls are conducted among representative samples of about 1,000 adult inhabitants of Poland. The studies on cigarette smoking were carried out in 3 editions (2010, 2011, 2012). Results. The collected data on tobacco smoking before and after implementation of the Act show that the number of smokers remained at a comparable level – about 30% (p=0.891). Between 2010–2012, an increase was observed in the percentage of people supporting the smoking ban in public places. The total number of people who definitely or rather supported the smoking ban increased from 75.6% in 2010 to 85.0% in 2012 (p<0.001). Conclusions. The restrictive smoking ban in public places implemented in 2010 did not have an impact on the number of smokers in the studied time interval, although an increase in the number of people supporting the smoking ban in public places was observed.

Introduction. The issue of inequalities in the use of health services and health inequalities between urban and rural dwellers is a subject of numerous scientific reports, but similar publications on dental health are scarce. Objective. The aim of this study is to assess the correlations between the use or non-use of dental services and the classical socio-economic factors (SES) in urban and rural groups in a representative population of Poland. Materials and method. The analysis consisted in desk research based on a questionnaire survey conducted by the Central Statistical Office of Poland on 12,532 individuals (urban areas – 6411, rural areas – 6121) in 2013. The chi-squared test, multivariate and univariate logistic regression models were applied. Results. There were major differences in the use of dental services depending on income level, education and source of income among rural and urban populations, as well as differences between these populations. Urban dwellers had 1.34 times greater odds of using dental services than rural ones (95% CI: 1.20–1.51). In the case of the highest income group, the odds were 3.26 (95% CI 2.21–4.83) times greater in cities and 2.07 (95% CI 1.51–2.85) times greater in villages than the odds in the lowest income group. In the highest education group, the odds were 1.58 (95% CI 1.17–2.13) times greater in urban areas and 2.08 (95% CI 1.48–2.91) times greater in rural areas than the odds in the lowest education group. Conclusions. There are considerable inequalities in the use of dental services, yet the differences in non-use, despite such a need, are less evident. This could imply that health disparities arise not only from economic constraints and unavailability of health care, but also from health attitudes and behaviours.

A network of pediatric environmental health specialty units (PEHSUs) in the United States was formed in 1998 out of a recognized need for clinical expertise in children’s environmental health. Documented trends in a rise of pediatric diseases caused or exacerbated by environmental conditions, coupled with the failure of medical schools and residency programs to cover these issues in a significant way, leaves health care providers, parents, communities, and governments at a loss for this specialized knowledge. The PEHSUs fill this gap by providing: 1) medical education, 2) general outreach and communications, and 3) consultative services to communities and health care professionals. This paper presents examples of key situations where PEHSU involvement was instrumental in improved patient outcomes or advancing clinical expertise in children’s environmental health. Challenges and opportunities for future directions for the program are also discussed.
Sieć Pediatrycznych Specjalistycznych Poradni Medycyny Środowiskowej (PEHSU) w Stanach Zjednoczonych została utworzona w 1998 roku w wyniku uznania konieczności przeprowadzania ekspertyz w zakresie zdrowia środowiskowego dzieci. Udokumentowane trendy wzrostu chorób dzieci wywołanych lub zaostrzonych przez czynniki środowiskowe i nakładający się równocześnie niedostatek programów nauczania z tego zakresu w szkołach medycznych lub w czasie rezydentury spowodowały brak tej specjalistycznej wiedzy wśród pracowników opieki zdrowotnej, rodziców, społeczności i władz. PEHSU wypełniają ten brak wykonując: 1) edukację medyczną, 2) promocję zdrowia środowiskowego 3) porady konsultacyjne dla społeczności i dla pracowników ochrony zdrowia. W niniejszej pracy przedstawiono najważniejsze przykłady, kiedy udział PEHSU był instrumentem dla polepszenia stanu zdrowia albo postępu ekspertyzy klinicznej w zakresie zdrowia środowiskowego.

Introduction and objective. Bovine tuberculosis (bTB, bovine TB) is caused by mycobacteria which are grouped within the MTBC. TB in animals is a highly infectious and progressive disease which can be transmitted to humans. Since 2009, Poland has gained official bTB-free status. Despite the official fact of bTB-free status, a dozen bTB outbreaks are still noted each year. Since 2000 in Poland, every year 1/5 of the national herd is subject to intradermal skin TB testing to control the bTB outbreaks in the cattle population. Application, with 5-year intervals between each government-funded skin test, undoubtedly resulted in financial savings. However it also seems to have caused several adverse and worrying events, e.g. an increase in the number of reactors detected and removed from a single tested herd. The objective of this study was the examination of 898 cattle imputed with bTB infection in Poland between 2008–2012. Materials and method. The study concerned a potential epidemic outbreak with suspected bTB transmission. 20 cows came from 3 herds in the same county located in the same province in southern Poland. Results. 134 MTBC strains were identified. In MIRU-VNTR, all isolates showed the same genetic pattern 322532243421232. Based on molecular investigation, the characteristics of M. bovis strains isolated from cattle from 3 different herds confirmed the common source of this zoonotic disease. Conclusions. Although not bacteriologically proven, everything points to the fact that humans were the vector of bovine tuberculosis transmission between herds. This finding confirms transmission between 3 cattle herds in the Małopolskie Province in southern Poland (Podhale). The outbreak of tuberculosis in animals finally compromised public health.
