Introduction and Objective. Interferential current IFC is one of the common electrotherapeutic modalities used in the treatment of painful conditions. Patients with musculoskeletal pain seek medical help in order to reduce their pain that could be achieved using IFC. The current review aims to analyze the recently available information regarding the efficacy of the IFC in alleviating the pain of musculoskeletal origin. State of knowledge. IFC, as one of the medium frequency currents, has the advantage of being more comfortable and deeply penetrating so that it can reach deeper painful tissues. It has been proposed that IFC can relieve pain through stimulating different body mechanisms, such as the gate mechanism and the release of body opioids. However, the evidence behind the effectiveness of IFC as a pain-relieving modality for musculoskeletal pain has been poorly studied and still not conclusive. Conclusions. This systematic review will summarize the effects of IFC on relieving musculoskeletal pain as reported through improvement in visual analog scale, numeric pain rating scale, or the McGill pain questionnaire. Through searching multiple databases and including randomized controlled trials published during the last ten years, the findings of the current systematic review and meta-analysis will establish the quality of the recently available evidence and demonstrate if there will be a need for further studies.

Introduction. Pain is one of the most prevalent unpleasant sensation in people that may significantly lower the quality of life. More than a half of cancer patients suffer from various forms of pain, which becomes more frequent and intense as disease progresses. Objective. The objective of the study was to assess the degree of pain control in patients diagnosed with breast, lung, colorectal and prostate cancer. The analysis also covered the effect of socio-economic factors on pain management in patients with the above types of cancer. Materials and method. The study included 902 patients treated at the Outpatient’s Department of the Maria Sklodowska-Curie Memorial Cancer Center – Institute of Oncology in Warsaw in 2013. The patients consisted of those diagnosed with breast, lung, colorectal or prostate carcinoma. The Paper and Pencil Interview (PAPI) technique was applied. A questionnaire interview included demographic-type questions (socio-economic variables) and the Beliefs about Pain Control Questionnaire (BPCQ) test which measures the power of factors influencing pain control in patients. Results. It was demonstrated that regarding beliefs in the source of pain control, patients attributed the highest importance to the power of doctors (mean value = 16.60) and the lowest to chance events (mean = 15.82). The internal factors are regarded as having the strongest influence by respondents diagnosed with colorectal or breast cancer. With regards to the locus of pain control, only the internal control of pain is diversified by the primary site. Conclusions. With regards to the source of pain management, only the internal control of pain is diversified by the primary site. The external factors were regarded as having the strongest influence by respondents diagnosed with colorectal or breast cancer. The major socio-economic variables differentiating the way in which pain control is perceived are education and net income-per-household-member. The results of analyses of individual groups of patients revealled strong correlations between the beliefs in the doctors’ influence, and the beliefs in chance events and socio-economic factors.

Introduction. Lumbar disc herniation (LDH) is a common cause of back pain and leg pain. For more than 60 years, standard discectomy by fenestration has been used to treat LDH. In his study, three commonly used scales were used to assess the outcome after microdiscectomy, such as the Low Back Pain Rating Scale (LBPRS), Oswestry Disability Index (ODI) and the Visual Analogue Scale (VAS). To the best of the authors’ knowledge, this is the first study analyzing treatment results at 3 months after microdiscectomy. Materials and method. During almost 5 years, a total of 327 patients underwent microdiscectomy operations for single level disc disease. Of these, 286 patients were available for 3-mounth follow-up. All patients were operated on at the Neurosurgery Department in the Specialist Hospital in Sandomierz, Poland. Before surgery and 3 months after burgery, all patients were asked to complete LBPRS, ODI and VAS scales. The statistical analysis used was p2, the effect size indicator for ANOVA. Results. Significant improvement was observed in all scales used 3 months after surgery. Conclusions. There are new, minimally invasive percutaneous techniques available, such as percutaneous laser disc decompression and intradiscal injection of radiopaque gelified ethanol. There are also trials with platelet-rich plasma or mesenchymal stem cells injected into the disc, aimed at the restoration of healthy disc properties. The hypothesis that metabolic products of bacteria propionibacterium acnes can contribute to disc degeneration inspired attempts at antibiotic therapy This study was conducted on a large group of patients and confirmed that discectomy is the recommended method of surgery which produces good treatment results in 3-mounths follow-up.

Introduction and objective. Postmenopausal women working in agriculture suffer from spinal pain for two overlapping reasons, the first is related to the menopause and the second to the specificity of rural work, which includes lifting heavy objects and changing weather conditions. Spinal pain affects the daily life of women as well as their ability to work. The objective of the study was to analyse the impact of spinal pain on activities of daily life in Polish postmenopausal women performing agricultural work. Materials and method. The study was conducted in 2016 in Poland and included 1,119 post-menopausal women living in rural areas and working in agriculture. The women assessed the severity of spinal pain in 3 sections: neck, thorax and lumbar. Neck Disability Index (NDI) and Oswestry Low Back Disability Index (ODI) questionnaires were used to assess the impact of spinal pain on daily life activities. Generalized linear models were estimated in statistical analyses. Results. Postmenopausal women working in agriculture suffered most often from pain in the lumbar spine, less frequently in the neck, and the least in the thoracic. The most common was an isolated pain in only one section of the spine. Spinal pain disturbed the most the women’s rest, standing, lifting objects, while sleep, concentration, and walking the least. The impact of spinal pain on the activities of daily life, on average, was moderate, and increased with greater pain severity, the earlier the age the pain started, the higher the body weight, the lower education level and if there was a co-existing pain in any of the other spine sections. The impact of spinal pain on daily life activities did not depend on age between 45–65, WHR, age at last menstruation, parity, and number and types of births. Conclusions. The impact of spinal pain on daily life activities in postmenopausal women working in agriculture was assessed as moderate, on average, and depended mainly on spinal pain-related characteristics, such as severity, age at onset and co-existence of pain in any other spinal sections.

Introduction. Pelvic pain is typically associated with pelvic inflammatory disease (PID). The most common cause of PID is Chlamydia trachomatis. The aim of this study was to verify the role of serological testing for Chlamydia trachomatis in patients with suspected PID. Materials and method. The retrospective study included 185 patients with pelvic pain hospitalized at the Department of Obstetrics and Gynecology in 2003 and 2004. Titers of anti-Chlamydia trachomatisIgG and IgA were measured by means ELISA immunoassays. Erythrocyte sedimentation rate (ESR), serum concentration of C-reactive protein (CRP) and leukocyte count (WBC) were also determined. Final diagnosis was established on the basis of laparoscopic examination. Results. The presence of anti-Chlamydia trachomatis antibodies correlated significantly with abnormal values of ESR, WBC and CRP. The most common laparoscopic pathology were pelvic adhesions, typically found in women with elevated titers of anti-Chlamydia trachomatis IgG. Conclusion. Serological examination for Chlamydia trachomatis is helpful in evaluation of patients with suspected PID. Elevated titers of anti-Chlamydia trachomatis antibodies are frequently associated with laparoscopic evidence of pelvic adhesions and inflammation.

Introduction. Year after year, we spend an increasing amount of time in a sitting position. Often, we sit with poor posture, as indicated by numerous pain syndromes within the musculoskeletal system. Several reports confirm that body posture and the amount of time spent in a seated position have extensive implications for our health. Previous studies and a literature review suggest there is limited knowledge regarding an ergonomic sitting position. Objective. The aim of the study was to analyze the research relating to a proper sitting position and the consequences of incorrect sitting posture. A database search was conducted in Science Direct, Scopus, PubMed, Medline, and Google Scholar. Selection was made on the basis of titles, then abstracts and full texts of the studies. No limits were applied to the date of publication. Conclusions. Incorrect sitting posture contributes to many disorders, especially in the cervical and lumbar spine. It also determines the work of the respiratory system. Most authors suggest that maintenance of the physiological curvature of the spine is crucial for the biomechanics of the sitting position, as well as the location of the head and position of the pelvis. It raises awareness of work-related hazards and the introduction of education on the principles of proper seating. It is necessary to draw attention to the risks associated with work performed in a sitting posture, and education on the principles of ergonomical sitting.

Introduction and objective. Since the role of vitamin D is essential in numerous biological processes its deficiency was suggested to be a risk factor for e.g. osteoporosis, musculoskeletal pain and spine pain. The purpose of the study was to analyse whether serum vitamin D concentration is related to pain involving the motor system in Polish postmenopausal women working in agriculture. Material and methods. The study group consisted of 1,751 post-menopausal women, aged 45–65, at least 12 months from the last menstrual period, living in rural areas and working in agriculture. The research method was self-assessment of pain involving the motor system using VAS, laboratory test of serum vitamin D concentration and a medical interview. Statistical methods included generalized linear models, analysis of variance, t test for two means in two independents, χ2 test of stochastic independence. Results. Postmenopausal women working in agriculture and suffering from pain in at least one part of the motor system were younger and lower educated, they also had higher abdominal obesity and lower serum vitamin D, compared to those without pain in any part of the motor system. Decreased serum vitamin D concentration in postmenopausal women working in agriculture is important from the aspect of a higher prevalence of pain in the thoracic spine and more severe pain in the neck spine, but not for severity of pain in the lumbar spine; higher occurrence of pain in both hands or wrists; higher prevalence and more severe pain in at least one knee; and no prevalence or severity of pain in the shoulders and elbows. Conclusion. Serum vitamin D concentration is important for the prevalence and severity of pain in the neck and thoracic spine, knees and hands or wrists, but not for the lumbar spine, shoulders and elbows.

When faced with a chronic disease such as rheumatoid arthritis, the patient attempts to cope with the stressful situation by applying coping strategies. The main aim of rheumatoid treatment is not only improving health but also increasing the quality of life. The research objective was to determine the relationship among socio-demographic factors, duration of the disease and its associated ailments, attitude to the disease, self-assessment of one’s knowledge of RA, and the application of coping strategies in stressful situations by women with rheumatoid arthritis. The study involved 193 patients of the Clinic of Rheumatology and Systemic Connective Tissue Diseases, and the Rheumatology Unit of the Specialist Outpatient Clinic of the Independent Public Teaching Hospital No. 4 in Lublin, from November 2016 – June 2017. The Coping Orientations to Problems Experienced Inventory (COPE) Questionnaire and an author’s Original Questionnaire were used in the study. Analysis of variance (ANOVA) and Tukey’s range test were applied for statistical analysis. A p-value<0.05 defined the statistical differences. Analysis was performed using the commercial SPSS Statistics 19 software (IBM Corp., Armonk, NY, USA). The respondents usually use instrumental social support (11.5±1.7), focus on and vent emotions (11.4±1.9), use emotional social support (11.4±1.8), employ active coping (11.4±2.1) and positive reinterpretation and growth (11.2±1.8), and least often rely on their sense of humour (5.5±1.4) or use alcohol or drugs (5±1.1). The factors which determine the types of strategy used are age, education, the duration of the disease, ailments experienced, and attitude towards the disease. Those respondents who declared a greater knowledge of RA more often applied positive reinterpretation and growth, and more rarely used alcohol or drugs. In the treatment and rehabilitation processes it is important to reinforce in the patient positive expectations for the treatment, seek advantages and benefits in one’s present health status, and educate patients about the disease, its therapy and appropriate coping strategies.

Introduction. According to the European Occupational Diseases Statistics (EODS-2005) diseases related with musculoskeletal disorders occupy the first position on the obligatory list of occupational diseases. These disorders most frequently occur in the agriculture-hunting-forestry sector in such countries as: Finland, Holland, Germany, France and Spain (data: EU-OSHA). Materials and method. The study included a selected group of males – 414 forestry workers, employees of the State Forests, aged 25–65, mean age 48. The control group were 119 office workers aged 23–64, mean age 45.8. The basic research instrument was a questionnaire designed by specialists concerning pain complaints occurring in 7 areas of the motor system: neck, arms, upper and lower back, hips, knees and feet. Results. The results of the survey showed that in the selected group of forestry workers, among 7 investigated areas of the motor system, the most frequently reported pain complaints involved the lower part of the spine (272 persons, 65.7% of the total number of respondents). In the control group such pain complaints occurred in 55.5% (66) of office workers (p = 0.04). Forestry workers most often described this pain as permanent, radiating to the leg, or as an acute pain. According to the frequency of reporting pain, the area of the knees was placed on the second position (214 foresters; 51.7%). Conclusions. The work of a forester which consist, among other things, in relocation on foot or by vehicle over long distances along an uneven terrain and in various weather conditions, is a risk factor of the occurrence of musculosceletal disorders concerning the low back and knees.

Introduction. Many known substances affecting the serotoninergic system induce definite physiological effects, including those which are therapeutic. For instance, the enhanced serotoninergic transmission due to decreased functions of autoreceptors and increased inhibitory functions of postsynaptic 5-HT1A is associated with antidepressant effect. The central serotoninergic system takes part in the regulation of many bodily functions, such as sleep, wakefulness, blood pressure, pain perception or sexual behaviours. Moreover, it is involved in the pathogenesis of depression, anxiety, addictions, migraine and other headaches. In pain therapy, not only typical analgesics are used, but also substances without obvious analgesic effect, thus allowing potential pharmacological modulation of analgesic activity in the treatment of pain. Objective. The aim of the study was to determine whether a chemical lesion to the central noradrenergic system at an early stage of individual development alters reactivity of 5-HT3 receptors in adult rats. Materials and method. The study used newborn and adult Wistar rats aged 8–10 weeks. Behavioural tests (writhing test, formalin assay) were used to assess the analgesic action of ondansetron as a 5-HT3 receptor antagonist. Results. The analgesic effect of ondansetron (1.0 mg/kg b.w., i.p.) in the writhing test was weak and short. Pain intensity score after ondansetron injection (1.0 mg/kg b.w., i.p) was 2–3 points and did not differ significantly between the study groups. Conclusions. Damage to the central noradrenergic system at an early stage of individual development has no effect on the antinociceptive effects of the serotonin (5-HT3) receptor antagonist, ondansetron, in the persistent pain model.

Introduction. Fibromyalgia syndrome (FMS) and myofascial pain syndrome (MFPS) can be ranked among disease entities being difficult to diagnose clinically, manifesting themselves mainly through pain in specific hypersensitivity points. Aim. To present the current state of medical knowledge about pain spots appearing on hypersensitive points of soft tissue in the context of selected disease entities. Summary of the knowledge. MFPS is defined as sensory, motor and autonomic complaints, caused by the occurrence of trigger points (TrP). Yet the FMS is stated during the anamnesis on the basis of generalized pain, and pressure achiness of at least 11 out of 18 tender points (TP) of precisely determined location. Patients with FMS report numerous additional complaints – apart from the above mentioned ones; these are however highly non-specific and are not confirmed during routine medical check-ups. There are also no laboratory tests that can confirm presence of TrP being characteristic to MFPS and differentiating it from other muscles’ disease entities. Such points are identified only with the use of palpation. Unfortunately while examining a patient this way TrP – being symptoms of MFPS – can be quite easily confused with TP – being symptoms of FMS. Patients with MFPS which is developing in consequence of long-lasting global disorder of muscle tension balance and sensitivity of nociceptors as a result of chronically remaining pain, frequently suffer from achiness fulfilling the criteria of generalized pain. Moreover – in effect of static overload of soft tissues (especially of tonic muscles) – there occur hypersensitive palpable areas (points). Stimulating them cause lively reaction of the patient. Described symptoms can suggest a suspicion of FMS – the more so that making a diagnosis of MFPS does not exclude its coexistence. Having this in mind, there is a pretty large group of authors who raise a supposition that the differential diagnosis between TrP and TP should be observed in the quantitative rather than in the qualitative categories, despite the still binding definition and nomenclature. Recapitulation. Looking at the MFPS and at the FMS from the perspective of evolution of knowledge about them and from the point of view of period when scientific researches were conducted and their results published, it must be stated that during last years a considerable progress has been obtained in scope of better understanding of pathogenesis and pathophysiology of pain in specified points of soft tissue hypersensitivity, and the parallel clinical studies – confirming the hypotheses that were made – clearly increased the diagnostic and therapeutic capabilities of clinical practice.

Introduction and objective. Patient satisfaction with nursing care is one of important indicators of health care quality. As for pain, it is an indicator of patient health. The goal of the study is to identify socio-demographic and medical factors that differentiate the assessment of satisfaction with nursing care of hospitalized patients. Materials and method. The study involved performing a diagnostic survey using standardized instruments, i.e. the Newcastle Satisfaction with Nursing Scale (NSNS) and the Visual Analog Scale by Barbara J. Headley for pain scoring. The study included 205 patients hospitalized for lumbar pain syndrome on two neurosurgery wards in Poland. The mean patient age was 51 years. Most respondents declared having high school or vocational education. The mean time of hospitalization before the study was 2 days. Results. The results demonstrated that patient satisfaction with nursing care is independent of patient age, education, duration of hospitalization, or number of hospitalizations. In contrast, the experience of nursing care is dependent on education, and less educated patients (primary or vocational education) had higher scores in this category. With regards to pain, it was perceived as more intense by women and by patients hospitalized for a shorter period of time. Conclusions. Satisfaction with nursing care among patients with lumbar pain syndrome does not depend on socio- demographic or medical variables. Experience of nursing care depends on patient’s education.

Introduction and objective. Pain is an integral element of the pathogenic process and sometimes determines its course. Disorders in pain sensation, as well as its lack, the pain threshold, and variability in sensation of the same pain stimuli as more or less intensive by different persons, may be genetically conditioned. The aim of the study is to examine genes in pathogenesis of chronic pain. Materials and method. The study was conducted in a specially selected group of 31 persons: study group – 20 patients with chronic pain, and control group – 11 healthy individuals who did not experience pain. The control group of 11 healthy persons, compared with the study group, was the catalyst for determining the relative quantification (RQ) of gene expression. Biological material in the form of venous blood was collected from the study participants into the tubes containing anticoagulant EDTA KE/2.7 ml (ethylenediaminetetraacetic acid), preventing extracorporeal blood clotting. Results. Analysis of expression of the examined genes showed over-expression of the DRD1 gene in patients experiencing chronic pain, which means that in these patients an increased number of dopamine D1 receptors encoded by this gene should be expected. The dopamine D1 receptor is a G-protein-coupled receptor which regulates (stimulates or inhibits) adenyl cyclase – the enzyme responsible for synthesis of cyclic AMP (cAMP). An increase in the concentration of cAMP in neurons enhances the sensation of pain. Conclusions. The genes (DRD1, COMT, OPRK1, HCN2) have a significant role in the pathogenesis of chronic pain in various diseases; they can also influence the perception of pain. Knowledge of these genes can contribute to the development of effective methods of combating pain.
