Introduction and objective. A decreased concentration of iron, and consecutively haemoglobin, ferritin and decreased level of saturated transferrin, were observed in obese individuals more often than in healthy subjects. The purpose of this study was to determine whether iron, ferritin, transferrin saturation are significantly diminished in obese female patients compared to non-obese counterparts, and whether excess adiposity and inflammation were associated with depleted iron. Materials and methods. Female patients (n=48) diagnosed with obesity (BMI > 30 kg/m2), aged 18–40 were accepted for the study. A control group (n=30) encompassed normal weight women, aged 18–30. All obese women obtained an individually adjusted dietary plan with an energy content of 1,500 kcal. Blood glucose, insulin, lipids, ferritin, TIBC and iron concentrations were assayed in serum twice, initially and after 8 weeks of dieting. Results. The obese women at the initial evaluation, in comparison to non-obese control women, were characterized by a significantly lower mean red blood cell volume (MCV; 84.2±12.4 vs. 91.3±9.3 fL; p<0,0001), serum iron level (92.6±42.4 vs. 119.8±44.0 μg/dL; p<0.01), and transferrin saturation (TSAT; 25.9±12.7 vs. 38.8±15.7%; p<0.01), but by higher plasma level of the C-reactive protein (CRP; 7.0±6.7 vs. 1.2±1.3 mg/L; p<0.01). The obese women after 8 weeks of diet decreased their mean total body weight from 104.1±21.3 to 99.2±20.7 kg (p<0,0001). CRP level decreased slightly but significantly from 6.9±7.1 to 6.2±7.5 (p<0.05). Conclusions. Obese women exhibit an increased level of CRP which may affect iron homeostasis. Weight loss leads to decrease in the CRP level, but it does not change haematologic parameters in the period of 8 weeks.

The built environment, which refers to human-made or modified features of the physical neighbourhood, such as homes, schools, workplaces, or parks and recreational areas, undoubtedly has a great impact on our lifestyles and health behaviours. The objective of this paper is a review of the literature on the correlations between neighbourhood environment and weight status of local residents. This paper presents factors in the built environment which may be related to overweight and obesity prevalence among local residents, resulting in changes in energy balance and affecting BMI. The focus is especially on the relationship between the distance from home to parks and green spaces and physical activity levels, and between the access to fast food restaurants and BMI status or obesity prevalence among residents. The results of recent studies are inconsistent, primarily because of several limitations and methodological problems, and due to multifactorial and complex relationships between the built environment and lifestyle. However, with the still unsolved problem of the increasing prevalence of overweight and obesity, even small changes in individual health behaviour may be important and have a positive impact on the reduction in BMI over time. Therefore, all activities aimed at shaping neighbourhood environments in ways which facilitate healthy food choices and create opportunities for as well as encourage physical activity should be supported.
Zurbanizowane środowisko, obejmujące stworzone lub przekształcone przez człowieka elementy środowiska fizycznego, takie jak dom, szkoła, miejsce pracy, czy parki i tereny rekreacyjne, bez wątpienia ma duży wpływ na styl życia i zachowania zdrowotne. Celem pracy jest przegląd piśmiennictwa na temat zależności między środowiskiem miejsca zamieszkania a masą ciała okolicznych mieszkańców. Opisano potencjalny związek między występowaniem nadwagi i otyłości a elementami zabudowy lokalnego środowiska, które poprzez zmiany w bilansie energetycznym mogą wpływać na BMI mieszkańców. Uwzględniono w szczególności zależność między odległością parków i terenów zielonych od domu a poziomem aktywności fizycznej oraz dostępnością restauracji typu fast-food a masą ciała lub występowaniem otyłości wśród mieszkańców. Wyniki ostatnich badań nie są jednoznaczne, przede wszystkim z powodu szeregu problemów metodologicznych oraz wieloczynnikowych i złożonych zależności pomiędzy zabudową środowiska a stylem życia. Jednakże, ze względu na coraz częstsze występowanie nadwagi i otyłości, nawet niewielkie zmiany indywidualnych zachowań zdrowotnych mogą być ważne i pozytywnie wpływać na redukcję BMI z upływem czasu. W związku z powyższym, należy wspierać wszystkie działania na rzecz kształtowania lokalnego środowiska, które ułatwiają dokonywanie zdrowych wyborów żywieniowych oraz stwarzają możliwości i zachęcają do podejmowania aktywności fizycznej.

In recent years, attention has been focused on pleiotropic directions of effects exerted by vitamin D. Epidemiological data indicate that deficiency of vitamin D in various population groups represents an increasingly widespread phenomenon, while a decreased serum concentration of calcitriol correlates with manifestation of civilization-linked diseases, including visceral obesity. This study aims at a review and synthesis of data linked to relationships between lowered vitamin D concentrations in blood and manifestation of obesity, and potential mechanisms which affect the concentration of the vitamin in conditions of an excessive accumulation of adipose tissue. Several variables are distinguished which can affect the status of vitamin D in obesity, but the key role in this respect is ascribed to the metabolic activity of visceral adipose tissue. Among others, the activity favours sequestration and modulation of calcitriol turnover. On the other hand, the effects of vitamin D on the process of adipogenesis and its involvement in remodelling of adipose tissue are pointed out. Also, several factors of an environmental nature (e.g. time of year/day, dietetic supply of vitamin D), genetic nature (e.g. genetic polymorphisms) and other conditioning (e.g. coexisting diseases, age, content of melanin in skin) cannot be bypassed as they may affect the concentration of vitamin D. Nevertheless, it still remains unresolved to what extent hypovitaminosis D represents the cause and to which it is the effect of obesity.

The rising prevalence of overweight and obesity in children and teenagers is a major challenge for public health. Obesity is a complex and heterogeneous disorder, affected by many interacting genetic and non-genetic factors. The aim of this article was to focus on the environmental risk factors for childhood obesity. Among different factors contributing to an increase in BMI, we highlighted the role of exposure to cigarette smoke, DDT, bisphenol A, pesticides, and noise. The correlation between exposure to environmental toxins during prenatal period and obesity development in later life was underlined. According to obesogenic environment hypothesis, some features of distal and proximal neighbourhood also have a pivotal impact on children's behaviour and may contribute to increasing the risk for overweight. The area of residence (urban or rural) may affect access to sports facilities or other opportunities for physical activity. Therefore, for designing adequate prophylaxis, it is essential to take into account modifiable risk factors present in residential neighbourhood. Prevention of childhood obesity should integrate activities for both micro- and macro-environment surrounding the child.
Wzrastająca częstość występowania nadwagi lub otyłości u dzieci i nastolatków stanowi poważne wyzwanie dla zdrowia publicznego. Otyłość to złożone i heterogenne zaburzenie wynikające z wielu interakcji pomiędzy czynnikami genetycznymi i pozagenetycznymi. Celem artykułu było skupienie się na środowiskowych czynnikach ryzyka dziecięcej otyłości. Spośród różnorodnych czynników przyczyniających się do wzrostu BMI na- świetliliśmy rolę ekspozycji na dym tytoniowy, DDT, bisphenol A, pestycydy oraz hałas. Podkreślony został związek pomiędzy narażeniem na środowiskowe toksyny w okresie prenatalnym a rozwojem otyłości w późniejszym życiu. Zgodnie z hipotezą obesogennego środowiska niektóre cechy dalszego i bliższego sąsiedztwa mają bardzo ważny wpływ na zachowania dzieci i mogą przyczynić się do wzrostu ryzyka nadwagi. Miejsce zamieszkania (wieś lub miasto) może warunkować dostępność do obiektów sportowych czy innych możliwości wykonywania aktywności fizycznej. Dlatego też, niezwykle ważnym jest uwzględnienie przy projektowaniu odpowiedniej profilaktyki modyfikowalnych czynników ryzyka obecnych w lokalnym środowisku. Prewencja dziecięcej otyłości powinna integrować działania na rzecz mikro- i makrośrodowiska otaczającego dziecko.

Polycystic ovary syndrome is a disorder which affects 5–10% of women in reproductive age. PCOS is a cause of hyperandrogenism, menstrual disorders and infertility. The most common clinical symptoms are hirsutism, acne and obesity. Patients often suffer from metabolic disorders: insulin resistance, hyperinsulinemia, dislipidemia, leading to atherosclerosis and others irregularities of the metabolic syndrome. Patients are in the high risk group for cardiovascular diseases (CVD) development because of the metabolic abnormalities. Obesity is observed in 35–60% of women with PCOS. Lean women with PCOS are also exposed to a greater risk of glucose intolerance development and abnormalities in lipid profile than women without PCOS with comparable BMI. Adipocytes are the source of many compounds of the paracrine and endocrine activity. Some of them are also markers and mediators of inflammation. Increased levels of proinflammatory cytokines in blood can promote atherosclerosis and cardiovascular disease. Markers: IL-18, TNF, IL-6 and hs-CRP are often elevated in patients with polycystic ovary syndrome. An increase in inflammatory markers may be an early indicator of the risk of developing insulin resistance and atherosclerosis, and may become a useful prognostic and therapeutic tool for monitoring patients with PCOS: lean and those with overweight and obesity. Assessment of the concentrations of inflammatory markers may become a very useful test in evaluating the risk of developing atherosclerosis and cardiovascular disease, long before their clinical manifestation. It will also allow for the appropriate prophylaxis.

Poland is high on a list of countries affected by epidemics of obesity, a problem that has especially suddenly increased in the post-transformation period. Materials and method. Documentation of 115 subsequent patients diagnosed with grade 3 obesity, considered eligible for surgical treatment, was analysed since 2015. A subgroup of 68 patients was selected from this group and the included patients who decided to cancel their treatment at various stages. Their history since presentation at the obesity treatment centre, BMI with its history, age, education and place of residence were analysed, followed by a telephone survey. 42 patients were finally contacted. Special attention was placed on the analysis of patients taking into account their place of residence. Patients were divided into inhabitants of rural and urban areas. Results. In the study group of 68 patients, 19 (27.9%) were inhabitants of rural areas and 49 (72.1%) of urban areas. Women accounted for 67.6%, and men for 32.4%. The mean age of patients when they presented for treatment was 43 years. On average, men presented 5 years later compared to women. The mean BMI on qualification for treatment of obesity was 47.6 kg/m2. The mean BMI max – 49.6 kg/m2. There were no differences regarding education in both populations. Despite the lack of differences regarding changes in the BMI since withdrawal from treatment, as many as 63% of patients from rural areas reached the BMI max in this time. Conclusions. There were demographic differences between the degree of obesity observed among patients receiving bariatric treatment, and inhabitants of urban areas were favoured. Patients from rural areas who withdrew from bariatric treatment and were left without medical care significantly more often achieved their maximum body weight, when compared to those living in urban areas. The active participation of physicians, both specialists and general practitioners, in the life of obese patients is imperative.

Introduction. Obesity is a serious health problem in nearly all regions of Europe. During the last four decades, the percentage of obese people increased four times among men and twice among women, reaching a global epidemic scale as the biggest health-related issue in the developed and developing countries. Objective. The aim was to assess the epidemiological situation of obesity, according to gender among adults in Europe, compared with other regions and selected worldwide countries, as well as to devise a forecast for the near future based on analysis results. Materials and method. An overview was performed of the World Health Organization (WHO) statistical database in 1999–2016, concerning obesity prevalence among people over the age of 20 across 28 countries of the European Union. A forecast for the years 2020 and 2030 was prepared by evaluation of gradients for trendline equations. Moreover, some of the world regions, as well as individual countries, five per region, were collated and analyzed to prepare the forecast. Results. The percentage of obese people in the countries of the European Union continues to increase every year. The dynamics of obesity growth in Europe is greater for men than for women (3.09% per year vs. 1.92% per year). With the growth rate remaining at the estimated level, in 2030 there will probably be more obese men (38.1%) than women (32.7%) in Poland, and in Europe likewise – 36.6% and 32.0%, respectively. Conclusions. Obesity has become a major epidemiological problem and an undisputable health problem not only in Poland and other European countries, but worldwide.

Introduction and objective. The nursing profession entails many negative factors and high risk of chronic diseases, including overweight and obesity. The aim of the study is to investigate the prevalence of overweight and obesity and health enhancing behaviours in Polish female nurses, and associations between overweight and 4 groups of such behaviours, age and shift work. Materials and method. The analysis covered data obtained through cross-sectional survey carried out in a group of 994 nurses with an average age of 43. The analysis covered answers about 29 health-enhancing behaviours divided into 4 categories (subscales): 1) nutrition, 2) physical activity, 3) sleep, rest and behaviours related to mental health, 4) preventive behaviours. They were analysed through the validated Positive Health Behaviours Scale for adults. The analysis also covered answers about avoiding drinking large amounts of alcohol in one go, not smoking, avoiding passive smoking, not abusing unprescribed drugs. Results. The prevalence of overweight and obesity was 44%. Of 29 health behaviours concerned with nutrition, physical activity, sleep, rest, and mental health, preventive behaviours, 3 were exhibited always or almost always by over a half of the nurses. Health behaviours were more common in nurses whose BMI was normal than in nurses with BMI ≥ 25.0. Age and low physical activity levels were related to overweight and obesity in nurses. Conclusions. The study revealed a high prevalence of overweight and obesity and many deficits in health behaviours. Obesity and health behaviour deficits pose risks to the health of nurses, limit their involvement in prevention and treatment of patient obesity and impact as health educators. It is essential to engage in actions for health promotion among nurses.

Introduction and objective. Obesity and overweight among teenagers and young adults pose serious problems for the Polish health care system. The aim of this study was to assess the relationships between the prevalence of overweight and obesity and the level of urbanization, consumption of selected food products and socio-economic development in the provinces in Poland. Materials and method. The material used in this study is based on data published by the Central Statistics Office (GUS) for population, components of the Human Development Index (HDI), urbanization level (URBI), average monthly consumption of selected food products per person, and the percentage of obese and overweight people in the Polish provinces. The prevalence of overweight and obesity in all 16 provinces was analysed in the context of the URBI, HDI, and the average monthly consumption of selected food products. Results. There was no a statistically significant correlation between the number of overweight and obesity cases and the levels of HDI and URBI. The negative correlations were found between the consumption of sugar and URBI (-0,710, p=0.002), bread and grain product consumption (-0.642; p=0.007) and fresh milk consumption (-0.594; p=0.015). Fish consumption correlated with URBI at a moderate level (0.550; p=0.027). The consumption of oils and fats strongly correlated with HDI (-0.788; p < 0,001). In the group of overweight woman, a moderate negative correlation was noted (-0.511; p=0.043) in cheese and curd consumption. Conclusions. The high level of economic development and urbanization have no direct impact on the prevalence of overweight and obesity in Poland. The results of the study indicate existing dependencies of the consumption of some of selected products with respect to the level of urbanization.

Introduction and objective. There is an evidence suggesting that obesity may influence the timing of puberty and growth patterns. There are few research on the reactions of the craniofacial complex to an overall increase in the body mass. Thus the objective of the study was to investigate if overweight or obesity may influence craniofacial morphology in adolescent orthodontic patients. Material and methods. In total, 77 patients aged 11–16 years were selected for the retrospective analysis. The study group comprised 37 overweight or obese individuals (mean age: 13,45±2,15). The control group included 40 normal-weight subjects (mean age: 13.79±1.81). Body mass index (BMI) percentile of each patient was assessed with the use of BMI score and age- and sex specific growth charts in accordance with International Obesity Task Force (IOTF). Craniofacial measurements included in the study were performed on the tracings of lateral cephalometric radiographs. The data was analyzed by STATISTICA 10 for Windows Software. Results. Both females and males of the study group exhibited significantly greater mandible length (Cd-Gn), corpus length (Go-Pg), midfacial length (Cd-A) and lower anterior facial height (Ans-Me) (p<0.05), as well as SNB, SNPg and ML/SN angles (p<0.05) compared to the normal-weight controls. Moreover, high BMI females showed greater maxillary length (Pm-A) and SNA angle (p<0.05), whereas males exhibited greater posterior facial height (S-Go) (p<0.05) compared to the controls. Conclusions. Weight status is an important factor that can affect craniofacial growth pattern and should be taken into consideration when planning orthopaedic treatment in adolescent patients.

Objective. The aim of the study was to define whether in a group of young women the place of residence in their childhood and adolescence, as well as moving from a rural to an urban area, have affected their biological condition. Materials and method. The data were obtained from 1,305 unmarried and childless women aged 18–25. The respondents were university students. Each person’s height, weight and waist circumference were measured, and the BMI, WHR, and WHtR indices were calculated. A survey was used to collect data on the females’ socio-economic status, their health condition and course of menstruation cycle. Results. In the whole sample, apart from WHR, there was no significant difference in anthropometric characteristics depending on the urbanization degree of the place of residence before commencing studies. In comparing the two groups of women, changing place of residence from a village to a city and from a city to another city, more differences were visible. Women of rural origin presented significantly higher values of BMI, waist circumference and WHtR than those from the city. They were also more frequently characterized by overweight and abdominal obesity. The women from other cities were characterized by underweight and too low amount of abdominal adipose tissue. Health problems were found in both groups. Students of rural origin more rarely declared changes in body weight and health condition after beginning studies. Students of urban origin were more often affected by sleeping problems and spine pains. Conclusions. The results of the study do not indicate that a change of place of residence has a stronger effect on the biological condition of rural women than their urban peers.

Introduction. Overweight and obesity, as well as a gonadal function, are pivotal factors influencing bone tissue metabolism. Materials and method. The aim of the study was to determine the effect of dietary induced obesity (DIO) on bone tissue metabolism in sham-operated (SHO) or ovariectomized (OVX) adult female Wistar rats. Additionally, the influence of DIO in SHO or OVX on the concentration of sclerostin in the blood serum was analyzed. After SHO or OVX, the rats were placed in groups (n=8) and either received a standard diet (11.5 MJ/kg) (SHO-CON; OVX-CON) or a high-energy diet (17.6 MJ/kg) (SHO-FAT; OVX-FAT). The experiment lasted for 90 days and allowed for the establishment of osteopenia in OVX females and obesity in the rats that had received the high-energy diet. Results. The results of the study demonstrate that obesity or/and ovariectomy increases the resorption of femora and tibiae, hence decreasing the densitometric and mechanical parameters affecting the bone structure in adult females rats. The strongest osteodegenerative effect was seen in the OVX-FAT females. Interestingly, the degree of bone tissue degradation caused exclusively by ovariectomy was similar to that found in the obese sham-operated rats. Conclusions. Bone losses invoked by DIO seem to be independent from the Wnt/β-catenin pathway inhibition induced by sclerostin. While further study is necessary, the obtained results suggest that the usage of sclerostin anti-body in the treatment of osteoporosis can be ineffective, and in obese patients the undertaking of such therapy should be reassessed.

In the light of up-to-date epidemiological data concerning the prevalence of infertility, and also considering the scale of the problem of obesity and proven cause-effect relationship between abnormal body weight and infertility, it is justifiable to undertake the task of systematization of the state of knowledge concerning nutritional correlates of infertility, in order to create a scientific basis for the formulation of the assumptions of a fertility diet. The study is an attempt to systematize the current state of knowledge concerning the importance of energy and structural (qualitative) aspects of nutrition in the prevention and treatment of infertility.

Introduction. Elevated uric acid (UA) is associated with arterial hypertension (AH), obesity, dyslipidemia and insulin resistance. However, its association with body components has not been previously investigated. Objective. The aim of this study was to evaluate the relationship between UA and cardiovascular risk factors, anthropometric parameters and body composition in patients with AH. Materials and method. In 138 patients with AH the following parameters were evaluated: UA, low and high density lipoproteins (LDL-C, HDL-C), triglycerides (TG), fasting glucose (FG), creatinine; body mass index (BMI), waist circumference (WC), fat mass (FM), fat free mass (FFM) and total body water (TBW). Results. Positive correlations were shown between UA and LDL-C (p=0.041), TG (p<0.001), FG (p=0.025) and creatinine (p<0.001) and negative between UA and HDL-C (p<0.001). Significant associations between UA and anthropometric parameters and body components, such as WC (p<0.001), BMI (p<0.001), FFM (p<0.001) and TBW (p<0.001), were also observed. In the multiple regression model, independent predictors of UA concentration were serum creatinine and TBW (R2=0.45; p<0.001). Conclusions. In patients with AH, uric acid was significantly related to cardiovascular risk factors, including obesity. However, the main anthropometric determinant of plasma UA concentration is FFM. The consideration of body composition in the interpretation of UA concentration appears to be justified, but the verification of this hypothesis requires further studies.
