Introduction. The problem concerning the impact of pregnancy on diabetic complications is a matter for discussion as there is some evidence suggesting that pregnancy may trigger development or progression of diabetic chronic complications. However, currently available data concerning this issue is still controversial. Objective. The aim of the study was to evaluate the impact of obstetric history on the development of chronic microangiopatic and macroangiopatic complications in type 1 diabetic women. Materials and Method. The retrospective study comprised 226 white Caucasian type 1 diabetic women, including 190 parous and 36 nulliparous women. Anthropometric data, information concerning the course of the disease, including metabolic control and chronic complications, together with obstetric history, were registered. Results. Parous women were older (p<0.001), but did not differ significantly regarding metabolic control in the course of the disease (p>0.05) and diabetes duration (p>0.05) from nulliparous subjects. There were no significant differences in the incidence (p>0.05) nor onset (p>0.05) of chronic diabetes complications between the groups. The number of deliveries did not correlate with either the incidence nor the onset of chronic complications. Longer DM duration at the moment of first delivery was related to the higher incidence of retinopathy (p<0.01), nephropathy (p<0.05) and neuropathy (p<0.001). Conclusions. The incidence of chronic diabetic complications does not differ between parous women and the subjects that were never pregnant, and is not related to the number of pregnancies.

Introduction. The renin-angiotensin-aldosterone system (RAAS) plays an important role in the pathogenesis of hypertension, cardiovascular diseases (CVDs) and chronic kidney disease (CKD). Drugs affecting the RAAS system, such as angiotensinconverting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs), are commonly used in the treatment of hypertension and heart failure. These drugs are also effective in reducing proteinuria and may, at least, delay end-stage renal disease in both diabetic and non-diabetic proteinuric CKD. However, novel drugs are needed to more effectively suppress the RAAS system and the progression of CKD. Aliskiren is the first direct renin inhibitor which has been approved for the treatment of hypertension. Additionally, a number of clinical studies have shown the antiproteinuric effect of aliskiren. Objective. This review focuses on the antihypertensive and antiproteinuric effects of aliskiren in patients with CKD. The pharmacology of aliskiren in these patients is also provided. Conclusions. Aliskiren-based hard endpoint large trials in non-diabetic nephropathy are needed in order to clarify the use of aliskiren in CKD patients.

Introduction. Myeloid sarcoma (MS) is a rare extramedullary tumour which may precede or occur concomitantly with bone marrow involvement in acute myeloid leukemia, myelodysplastic syndrome, or blast crisis in chronic myeloproliferative disorder. Myeloid sarcoma is most commonly found in lymph nodes, skin, subcutaneous tissue and gums, while it is less common in bones, the retroperitoneal space and eye socket. Case Report. The case is reported of a 65-year-old woman with chronic myelogenous leukemia treated for about 20 years with hydroxyurea, 6-mercaptopurine and tyrosine-kinase inhibitors. During the treatment, the general condition of the patient progressively deteriorated, lymphadenopathy and splenomegaly worsened, and blast crisis was diagnosed. After the first cycle of induction chemotherapy, the patient’s lymph nodes were swollen and painful. One of the lymph nodes was subjected to histopathology, on the basis of which a diagnosis of MS was made. As the patient showed no response to the treatment, palliative care was initiated. Three months after the diagnosis of MS, the disease progressed. The patient died of infectious complications. Conclusions: A diagnosis of MS, which is considered an adverse prognostic factor, significantly reduces the chances of remission and overall survival in patients with acute myelogenous leukemia or blast crisis inchronic myeloproliferative disorders. It seems that early confirmation of the diagnosis and initiation of the treatment adjusted to the patient’s clinical condition may improve the prognosis and increase the response rates.

Introduction. Despite significant recent advances in the treatment of chronic lymphocytic leukaemia (CLL), most cases of the disease are still incurable. Treatment with monoclonal antibodies, such as ofatumumab, is one of the new therapeutic options. Objective. Retrospective analysis of the efficacy of ofatumumab in patients with chronic lymphocytic leukaemia (CLL) treated in the Haematooncology and Bone Marrow Transplantation Department of the Medical University of Lublin, Poland, during 2011–2013. Materials and method. The analysis included 5 patients (3 women and 2 men), aged 47–65, with Rai stage II-IV CLL, after a few lines of treatment. Three patients received ofatumumab in monotherapy and 2 patients received ofatumumab in combination with cyclophosphamide (50 mg/day) and dexamethasone (40 mg/day). All patients included in the study were diagnosed with an active form of leukaemia with symptoms such as lymphocytosis or massive lymphadenopathy. Results. All patients responded to the treatment. Within the first 8 weeks of the treatment, levels of white blood cells returned to normal in patients with baseline lymphocytosis (3 patients). An increase in platelet levels was reported in 3 patients. Haemoglobin levels were higher or comparable to the baseline values in all studied patients after the completion of immunotherapy. In the patient with massive lymphadenopathy and hepato- and splenomegaly, the size of the lymph nodes, spleen and liver decreased and neutrophil levels increased. Time of progression was 5–12 months, and in one patient partial remission has been maintained. The treatment was well-tolerated in most cases. Asymptomatic neutropenia and an infection with Candida glabrata were observed. Conclusions. Ofatumumab may be a new and safe therapeutic option for patients with CLL after a few lines of treatment.

Introduction and objective. Paediatric chronic rhinosinusitis (PCRS) is a common disease in children. It is defined as an ongoing inflammation of the nasal cavity and sinuses lasting for over 12 weeks. Due to prevalence in the population, PCRS is associated with a high healthcare and financial burden. During the examination, attention should be paid to symptoms that may suggest chronic rhinosinusitis (CRS), because it may be underdiagnosed. Due to the chronicity of the inflammatory process and the similarity of symptoms to allergic rhinitis and adenoid hypertrophy, diagnosis and treatment are difficult. In this review, guidelines from The European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) 2020 and recent literature reports from Medline and Scopus database are analysed. Materials and methods. Information was obtained through a review and analysis of the PubMed Medline and Scopus databases. Brief description of the state of knowledge. Diagnostic difficulties with CRS can be caused by many factors. The EPOS guidelines recommend flexible or rigid nasal endoscopy as a preliminary objective method to assist in the recognition of CRS. Computed tomography (CT) is not recommended for routine management; therefore, CT is the gold standard for imaging when establishing a PCRS diagnosis or preparing for sinus surgery. The use of conventional X-ray imaging for chronic rhinosinusitis is limited and not recommended. Conclusions. Understanding the symptoms of chronic rhinosinusitis is of utmost importance in daily practice. Early initial assessment of symptoms in children with a high probability of CRS allows for early diagnosis and reduces the need for endoscopic and imaging examination.

Richter’s syndrome (RS) is a rare complication in which chronic lymphocytic leukaemia (CLL) or small lymphocytic lymphoma (SLL) transforms into a more aggressive type of lymphoma – diffuse large B cell lymphoma (DLBCL), or Hodgkin’s lymphoma (HL). The review describes the clinical case of a patient with CLL and RS diagnosis. A computed tomography (CT) scan of the abdominal cavity detected numerous normodense areas in the liver. Simultaneously, ultrasound examination (USG) of the thyroid revealed the presence of a solid hypoechogenic lump. The material sampled from closed biopsies of liver and thyroid in both cases allowed the diagnosis of diffuse large B cell lymphoma (DLBCL). The liver and the thyroid are particularly rare locations of RS. However, those cases allowed the conclusion that RS may occur even in a very unexpected and less probable location.

Introduction and objective. The rapid aging of the human population is an increasing challenge to public health. Effective strategies are required to prevent disability and dependency of the elderly. This study aimed to evaluate the effect of body weight on the prevalence of disability and chronic diseases among 60–80-year-old people living in south-eastern Poland. Materials and method. The study included 1,800 randomly selected people aged 60–80 years living in the Podkarpackie region of south-eastern Poland. Respondents, holders of a – personal identification number (PESEL), were randomly drawn by the Ministry of Interior and Administration (MSWiA) in Poland. The study was conducted in the form of a face-to-face interview at the respondent’s residence. The WHODAS 2.0 questionnaire was used to assess disability and functioning. Socio-demographic data were also collected, and the body weight measured in 5% of the respondents after completion of the study. Statistical analysis was performed using Statistica 10. Results. Respondents with Body Mass Index (BMI)<18.5 and BMI ≥35.0 had significantly higher disability levels than those in the normal weight and overweight categories. The greatest limitations were found in participating in everyday life, household activities, getting along and mobility. There was also a statistically significant relationship between BMI and the number of chronic diseases (p<0.001). Conclusions. When planning a healthcare strategy for people aged 60–80 living in Poland, additional support should be provided to those at risk in the categories of underweight and obesity. The obtained findings indicate that the health behaviour of seniors should be assessed – especially regarding their diet and eating habits, physical activity, and participation in social life – in order to tailor prevention programmes specifically to their needs.

Multiple sclerosis is a chronic inflammatory disease which, due to the destruction of the fibres of the central nervous system in the process of demyelination, leads to numerous neurological symptoms and progressive disability. The disease is autoimmune, which means that myelin is destroyed by the patient’s own cells, caused by improper functionong and regulation of the immune system. Multiple sclerosis affects more and more people and is therefore a significant clinical problem which, to a large Entent, affects women, especially in childbearing age This presents a big challenge for carrying out pregnancy while continuing the therapy, ensuring the safety of the foetus and simultaneously achieving the best possible therapeutic effect. The decision whether the therapy should be continued or whether it should be eliminatedis usually made according to assessment of the possible gains and losse. Despite the lack of clear indications, there are many studies proving the relative safety of the use of individual but not all the drugs during pregnancy. Pregnancy, however, has a fairly good impact on the development of multiple sclerosis, and that safety considerations, especially those concerning the growing foetus, force a decision to change or completely suspend the therapy. In-depth research on the already available and emerging therapeutic pathways in multiple sclerosis bring hope for increasingly better results in the future in the treatment of pregnant patients with multiple sclerosis.

Chronic kidney disease (CKD) is an important challange for healthcare systems wordwide because of its high prevalence and serious late complications. The results of recent studies suggest an association between CKD development and genetic variation within the uromodulin gene (UMOD). The aim of this study was to investigate associations between two common single nucleotide polymorphisms – rs12917707 and rs11864909, located in the region of UMOD and chronic renal disease. The study group consisted of 109 patients with chronic kidney disease, caused by chronic renal glomerulonephritis or chronic tubulointerstitial nephritis, and 109 pairs of their biological parents. Genotyping for rs12917707 and rs11864909 was carried out using the TaqMan Pre-designed SNP Genotyping Assay. In the transsmission disequilibrium test, allele C of rs11864909 was preferentialy transmitted from parents to the children with chronic tubulointerstinal nephritis. The rs12917707 was not associated with CKD. Neither of the investigated polymorphisms was associated with the progression of chronic kidney disease. The obtained results suggest an association of rs11864909 with chronic kidney disease secondary to chronic tubulointerstinal nephritis.

Introduction and objective. Pain is an integral element of the pathogenic process and sometimes determines its course. Disorders in pain sensation, as well as its lack, the pain threshold, and variability in sensation of the same pain stimuli as more or less intensive by different persons, may be genetically conditioned. The aim of the study is to examine genes in pathogenesis of chronic pain. Materials and method. The study was conducted in a specially selected group of 31 persons: study group – 20 patients with chronic pain, and control group – 11 healthy individuals who did not experience pain. The control group of 11 healthy persons, compared with the study group, was the catalyst for determining the relative quantification (RQ) of gene expression. Biological material in the form of venous blood was collected from the study participants into the tubes containing anticoagulant EDTA KE/2.7 ml (ethylenediaminetetraacetic acid), preventing extracorporeal blood clotting. Results. Analysis of expression of the examined genes showed over-expression of the DRD1 gene in patients experiencing chronic pain, which means that in these patients an increased number of dopamine D1 receptors encoded by this gene should be expected. The dopamine D1 receptor is a G-protein-coupled receptor which regulates (stimulates or inhibits) adenyl cyclase – the enzyme responsible for synthesis of cyclic AMP (cAMP). An increase in the concentration of cAMP in neurons enhances the sensation of pain. Conclusions. The genes (DRD1, COMT, OPRK1, HCN2) have a significant role in the pathogenesis of chronic pain in various diseases; they can also influence the perception of pain. Knowledge of these genes can contribute to the development of effective methods of combating pain.

Introduction and objective. The family is a reliable and permanent source of support for every human being. It is the key link in the welfare system. The aim of the study is to assess parents’ attitudes towards the occurrence of a difficult situation resulting from a child’s chronic illness. Material and methods. The study involved randomly selected parents of children with chronic diseases (N=107). The study was carried out at the Dental Clinic of the ‘Little Prince’ Children’s Hospice in Lublin. Results. The study revealed that for parents a difficult life situation is mainly associated with the disability of a family member or a friend, whereas the action approach is the most frequently chosen attitude towards a child’s chronic disease. In the examined group of parents, a significant correlation was observed between the age of the parents and the choice of a specific parental attitude towards the child’s chronic disease (in terms of intellectual approach). However, no significant correlations were found between the parents’ place of residence and the choice of a specific parental attitude towards the chronic disease of a child. Conclusions. During the diagnosis of chronically ill children, parents should also be educated in this regard. Therapy and psychological help, as well as directing parents to relevant support groups, organizations or associations, will prove immensely valuable while coping with a difficult life situation.

Objectives. The airborne contaminant exposure levels experienced by poultry farmers have raised concerns about the possible health hazards associated with them. Thus, a longitudinal project was instituted in France to monitor these exposures in poultry workers and to evaluate the long-term effect on health. Method. Sixty-three workers in two different poultry housing systems were included (33 from floor-based systems and 30 from cage-based systems). Personal dust concentrations (over 2 days) and activity patterns (over 14 days) were collected and then modeled to obtain average long-term estimates. Health data were collected by questionnaire. Results. The mean daily time spent in the cage system was more than 2 hours longer than in the floor system. Two main common tasks accounted for ~70% of this time. Dust concentrations were higher in the floor system than in the cage system. The concentrations for the 14 days of known activity patterns estimated using the statistical model agreed well with the measured values. Several chronic respiratory symptoms were significantly associated with the high levels of long-term exposure estimated by the model. The highest risk was for chronic bronchitis symptoms (>4-fold higher for exposures of 0.1 mg/m3 of respirable dust). Conclusion. The presented modeling strategy can be used to estimate the long-term average personal exposure to respirable dust, and to study the association between dust exposure and chronic respiratory symptoms. This population of workers will be followed-up in subsequent examinations (3 years later) to determine whether the predictive model is valid, and whether long-term dust exposure is related to the incidence of respiratory symptoms and changes in pulmonary functions.

Patient care requires a whole system compatible to the needs of organizational and technical solutions. The purpose of this study is to present a new model of patient-oriented care, in which the use of information and communication technology (ICT) can improve the effectiveness of healthcare for patients with chronic diseases. Material and methods. The study material is the process of healthcare for chronically ill patients. Knowledge of the circumstances surrounding ecosystem and of the patients’ needs, taking into account the fundamental healthcare goals allows us to build a new models of care, starting with the economic assumptions. The method used is modeling the construction of efficient healthcare system with the patient-centered model using ICT tools. Results. We present a new systemic concept of building patient’s environment in which he is the central figure of the healthcare organization – so called patient centered system. The use of ICT in the model of chronic patient’s healthcare can improve the effectiveness of this kind of care. The concept is a vision to making wide platform of information management in chronic disease in a real environment ecosystem of patient using ICT tools. Conclusions. On the basis of a systematic approach to the model of chronic disease, and the knowledge of the patient itself, a model of the ecosystem impacts and interactions through information feedback and the provision of services can be constructed. ICT assisted techniques will increase the effectiveness of patient care, in which nowadays information exchange plays a key role.

Introduction and objective. COPD is a medical state characterized by chronically poor airflow, and typically worsens over time. Farmers have an increased risk of COPD because of being exposed to ammonia, hydrogen sulfide, inorganic dust, and organic dust. The quality of life of the ill depends on biomedical as well as psychosocial factors, the impact of which has not been a frequent subject of studies among COPD patients. The aim of the study was to indicate the factors that have negative and positive influence on the quality of life of farmers suffering from COPD. Materials and method. The study was conducted among 84 farmers treated for COPD in the Department of Pneumology, Oncology and Allergology of the Medical University in Lublin, Poland. The differences between the farmers concerned: severity of the disease, level of education and income, frequency of smoking tobacco and drinking alcohol, kinds of support they receive from their families, and the level of depression and anxiety experienced by the patients. Results. The study revealed that most patients suffered from depressive and anxiety disorders, and the level of depression higher among the patients who smoked more. Lack of family support had significant influence on exacerbation of the patient’s depressive and anxiety symptoms. Patients who had recently experienced a critical situation presented with more severe COPD symptoms, lower quality of life and a higher level of depression. Higher income of the patients had positive influence on their quality of life. Farmers addicted to alcohol suffered from a higher level of anxiety.
