Introduction and objective. Currently, most women with less advanced breast cancer are offered operations with breast conserving treatment; however, if it is necessary to completely remove the breast, the patients may benefit from free breast reconstruction. The aim of the study was to determine the motives for breast reconstruction after mastectomy or decisive reasons for rejecting this treatment. Material and methods. The study included 241 women hospitalized in Polish oncological hospitals who were divided into two groups: 55.19% (n=133) were women after mastectomy; 44.81% (n=108) after breast reconstruction. Quantitative approach with the use of survey method was utilised. Results. Women who wished to undergo breast reconstruction were generallyy younger, better educated and more often professionally active. For women who wished to undergo breast reconstruction, the most important aspect was the willingness to improve their body image, their mental well-being, the inconvenience connected with wearing the prosthesis, better sexual relations with husband/partner, and the possibility of exposing the neckline without discomfort. The strongest correlation was found between women’s young age and their functioning in a relationship. Women after mastectomy, who decided not to undergo a breast reconstruction, were afraid of another operation, suffering, and they believed that there were more important issues. Conclusions. The study revealed a relationship between the lack of knowledge, in which hospital breast reconstruction procedure can be performed and the lack of motivation for undergoing the procedure. Women who have had knowledge of the possibility of a free breast reconstruction were younger, better educated and more often lived in the city.

Breast and cervical cancer represent a significant health and economic issue for Polish society, although if detected early, both can be cured successfully. For this reason, since 2006, according to the National Cancer Combat Programme, populationbased screening programmes have been implemented, aimed at reducing the mortality and morbidity for breast and cervical cancer. The aim of this study is to determine which of the selected four environmental factors affect attendance for screening mammography and cytology. Analysis included data from questionnaires filled in during mammography by 582,959 women aged 50–69 years, and 288,142 women during cytology, aged 25–59 years, in 2007–2012 in the Wielkopolska Province of Poland. It was found that the impact of medical staff on the attendance for cytological screening was the strongest statistically significant factor (p = 0,0001). Invitation by name (p=0,001) and other factors (p= 0,0001) also affected the attendance. In the cytological screening, medical staff was the factor that had the greatest impact on attendance. Other factors, such as self-reporting, increased participation in the next screening rounds, although the factors that affect attendance changed over time. Their constant analysis is essential for the efficient and effective evaluation of screening programsme.

Introduction. Breast cancer (BC) makes up nearly 26% of malignant tumours worldwide and is the leading cause of cancerrelated deaths in European women. With approximately 18,000 new cases of BC diagnosed in Polish women annually, breast cancer liver metastasis (BCLM) is respectively an increasing issue. Recent data found in literature indicates improved survival following liver resection with systemic therapy. Objective. The aim of study was to evaluate surgical treatment in patients with isolated BCLM. Materials and method. During 2009–2013, a retrospective study was undertaken and 30 cases analysed. From nearly 2,000 liver resections performed, 11 female patients at the mean age of 59.18 years with BCLM were qualified for surgery. Results. The median time between primary and secondary treatment was 3.5 years (1–7). One patient (9.1%) presented an extrahepatic lesion – bone metastasis. The left lobe, right lobe and both lobes of the liver were affected, respectively, in 3 (27.3%), 4 (36.4%) and 4 (36.4%) patients. 5 patients (45.5%) presented single hepatic lesion, in contrast to the maximum number of lesions which equalled 6 in the right lobe. Average hospitalisation period was 13.27 days and discharge on the 11.3 postoperative day. One-year survival was 72.7% (8 patients); therefore, three-year survival was 36.4% (4 patients). Conclusions. Oncological centres should assess BCLM patients more openly and qualify them for hepatic resection along with adjuvant systemic treatment in order to improve overall survival. This, however, needs to be studied in a multicentre randomized trial.

Introduction and objectives. Breast cancer (BC) is the most common malignancy and the leading cause of cancer death among women. About 15–20% of all BCs are HER2-positive. Proper assessment of HER2 status is crucial to choose appropriate treatment. The review summarizes data on anti-HER2 drugs used to treat HER2-positive BC and provides basic information on new agents under investigation. Brief description of the state of knowledge. Specific HER2-targeting drugs are available or are being evaluated in clinical trials. Anti-HER2 agents include: monoclonal antibodies, tyrosine kinase inhibitors, antibody-drug conjugates, bispecific antibodies, PI3K/AKT/mTOR inhibitors and heat shock protein 90 inhibitors, HER2-targeting vaccines and CDK4/6 inhibitors. The advent of anti-HER2 therapies increased the time of progression free survival and overall survival in BC patients. Results. Final analysis of the CLEOPATRA trial shows that the combination of trastuzumab, pertuzumab and taxane significantly improved outcomes in metastatic HER2-positive BC and it is currently preferred first-line treatment. The recommended second-line treatment is based on trastuzumab emtansine or on the combination of lapatinib and capecitabine. Some promising agents such as margetuximab or trastuzumab deruxtecan are still under investigation. Conclusions. Anti-HER2 directed treatment undoubtedly improves outcomes among patients with HER2-positive BC. Access to drugs such as trastuzumab, pertuzumab, lapatinib and T-DM1 improves prognosis even in patients with advanced disease. Further studies and clinical trials on novel anti-HER2 therapies are required. Nevertheless, BC treatment is becoming more effective and, hopefully, one day it may be possible to cure patients even with metastases.

Introduction. The results of breast cancer treatment depend mainly on better detection in mammography screening and, consequently, a higher proportion of women with early stage of the disease. They depend also on a better access to health care services and the effectiveness of oncological treatment. One of the methods of breast cancer control is a breastconserving surgery. With a proper patients’ classification for the treatment, the results of the breast-conserving surgery do not differ in relation to mastectomy. That’s why, the availability of breast-conserving surgical methods is particularly important, especially in a population in which a population screening is conducted. Objective. The analysis of the selected aspects of the breast cancer treatment’s standard in Podlaskie Voivodeship. Patients and methods. In years 2001–2002, 709 cases of women with breast cancer were reported to the Voivodeship Cancer Registry in Bialystok. 659 women were diagnosed with a primary invasive breast cancer. Based on a cohort of 499 women treated with radical methods the following indicators, recommended by WHO for the evaluation of the breastconserving surgery’s availability, were calculated: a percentage of patients with whom breast-conserving surgery was used, a proportion of breast cancer cases receiving post-operative breast radiotherapy after breast-conserving surgery and a proportion of breast-conserving surgery in pT1 cases. Results. The breast-conserving surgery has been used much less frequently in Podlaskie Voivodeship than in Western Europe, but more frequently than in Poland in general. Conclusion. It should be aimed to provide a surgical treatment with the use of breast-conserving surgical methods to the highest possible percentage of patients. The increase of the percentage of patients treated with breast-conserving surgery methods can be a result of an effective screening realisation.

Introduction. Despite great progress in the diagnostics and treatment, breast cancer still remains the most frequently diagnosed malignant cancer in women. The outcomes of cancer treatment depend, to a considerable extent, on the degree of acceptance of the illness. The objective of the study was determination of the level of acceptance of the disease among the examined women with breast cancer. Materials and method. The study covered a group of 231 women who received treatment for breast cancer in the Rev. Bronislaw Markiewicz Subcarpathian Oncology Centre in Brzozow, south-eastern Poland, and conducted by the method of a diagnostic survey, using an author constructed questionnaire and the standard Acceptance of Illness Scale, adapted by Z. Juczyński. Statistical analyses were performed using the software IBM SPSS v. 21.0.0.1. The p values p≤0.05 were considered statistically significant. Results. The mean level of acceptance of the illness in the examined group of women was 26.53 (SD = 7.71; Me = 26). The largest number of respondents (50.6%) obtained scores within the range of 20–29, according to the AIS Scale. From among the adopted variables, the level of acceptance of the illness significantly varied according to age and occupational activity. However, no such differences were confirmed according to the place of residence, marital status, education level, or the type of occupation performed. Conclusions. In the examined group of women, a mediocre level of acceptance of cancer was observed. The respondents’ age and occupational situation exerted a significant effect on the level of acceptance of the illness. The results of the study should be considered as satisfactory because the number of women with a high level of acceptance of breast cancer was more than twice as high as those ones with a low acceptance of the disease.

Objective. The aim of the study was evaluation of satisfaction with life among women ill with breast cancer, with consideration of selected demographic and social factors. Materials and method. In the study participated 121 patients from the Sub-Carpathian Oncology Centre in Brzozów, Poland, who had undergone surgical treatment due to breast cancer. The method of a diagnostic survey was used. The research instruments were an author-constructed questionnaire and the Satisfaction with Life Scale. Results. Women who received surgical treatment due to breast cancer evaluated their satisfaction with life on a mediocre level. Younger patients had a lower satisfaction with life than those who were older. Women living in urban areas evaluated their satisfaction with life in more positive terms than rural women. In turn, education level, marital status and material standard had no effect on the level of satisfaction with life among the women in the study. Conclusions. According to the SWLS, women with the diagnosis of breast cancer obtained a mean result of 5.64 sten scores Younger women evaluated their satisfaction with life in more positive terms than those who were older. Women who lived in urban areas obtained a considerably higher result according to the SWLS, compared to rural inhabitants. Marital status, education level, material standard and occupational activity of the women were insignificant in the respondents’ evaluation of satisfaction with life.

Introduction and objective. αB-crystallin belongs to the ubiquitous family of small heat-shock proteins. It was discovered as a physiological protein of the eye lens, maintaining its liquid-like property. Furthermore, αB-crystallin was proved to playa bipolar role in both physiological and pathophysiological conditions. This review discusses current knowledge about the biology and genetics of αB-crystallin, and summarizes recent advances in understanding its role in ophthalmic and neurological disorders, as well as breast cancer, renal cancer and other malignancies. State of knowledge. α-crystallins are established as important elements of the protein quality control network, and consequently their defects are related to multiple human diseases. New studies highlight αB-crystallin’s involvement in proliferative diabetic retinopathy angiogenesis and point out its therapeutic potential in age-related macular degeneration. αB-crystallin is thought to be associated with the disease-causing protein aggregates, leading to its connection with such neurological disturbances as anaplastic astrocytoma, Parkinson disease, aging deficits in the peripheral nervous system and multiple sclerosis. In breast cancer, it was proven to be a marker of aggressive behaviur and cerebral metastases. Strong expression of αB-crystallin promoted growth and migration of clear cell renal cell carcinoma cells and was correlated with lower overall survival rate. Considering other malignancies, its various roles were established in colorectal and gastric cancers, head and neck squamous cell carcinomas and osteosarcomas. Conclusions. Further studies concerning αB-crystallin seem to be enormously promising, as they might improve our understanding of common human pathologies as well as contemporary diagnostics and treatment.

Introduction and objective. One of the main factors determining the burden of breast cancer is the effectiveness of measures taken to combat this cancer including urban-rural differences. The aim of the study was to analyse the differences in breast cancer incidence and disease stage distribution among urban and rural women in the Świętokrzyskie Province as a part of a strategy for breast cancer control. Materials and method. The study of disease stage distribution included 483 female residents of Świętokrzyskie Province who were diagnosed in 2013 with invasive breast cancer, and reported to Świętokrzyskie Office for Cancer Registration. Urban-rural differences in breast cancer incidence in 2002–2013 were presented using Range Ratio (RR). Changes in incidence trends in urban and rural areas were analysed using joinpoint models. Annual Percentage Change (APC) of the rates was calculated for each time trend. Results. Breast cancer incidence rate in the urban female population was higher than in rural women with RR, amounting to 1.43. However, the analysis of trends showed that the pace and direction of change were developing negatively among inhabitants of rural areas. In 2002–2013, in rural women, the age-standardized rate (ASR) values increased by 2.8% per year (p<0.05). The course of ASR trends showed statistically significant urban-rural differences (p=0.004). Analysis of urban-rural differences in disease stage distribution revealed a non-significantly higher proportion of cases with localised stage in urban than in rural areas, amounting to 51.0% and 43.9%, respectively. Conclusions. Existing health inequalities indicate the need to intensify activities in rural areas and should be the starting point for making key decisions in combating breast cancer.

Introduction and objective. The γ-amino butyric acid (GABA) plays important role in the proliferation and migration of cancer cells. The aim of the study was to evaluate the level of GABA in breast cancer, in relation to clinical and epidemiological data. Materials and method. The study was conducted on 89 patients with breast cancer in stage I-II. GABA level was assessed using spectrofluorometric method in tumour homogenates. Immunoexpression of E-cadherin was evaluated histologically on paraffin fixed specimens. Overall and disease-free survival was assessed for a 15-year interval period. Results. Median overall survival was significantly longer (127.2 months) in patients with a high level of GABA (>89.3 μg/1), compared with a group with a low level of the amino acid (106.4 months). Disease-free survival was insignificantly different – 99 and 109 months, respectively. A significantly longer overall survival (131.2 months) was seen among patients with a high level of GABA and positive E-cadherin immunoexpression, compared with a group characterized by a low level of GABA and lack of E-cadherin immunorectivity (98.1 months). The co-existence of negative immunoexpression of E-cadherin and low GABA concentration resulted in a six-fold increase in the risk of death (HR=6.03). Conclusion. GABA has a significant prognostic value in breast cancer. Co-existence of a low level of GABA and loss of E-cadherin immune-expression seems to be a new, independent, and negative prognostic marker of the neoplasm.

Introduction. Breast cancer is the most common cancer among women and is the second cancer frequently occurring worldwide of newly-diagnosed cancers. There is much evidence showing the influence of life style and environmental factors on the development of mammary gland cancer (high-fat diet, alcohol consumption, lack of physical exercise), the elimination of which (primary prevention) may contribute to a decrease in morbidity and mortality. Secondary prevention, comprising diagnostic tests (e.g. mammography, ultrasonography, magnetic resonance imaging, breast self-examination, as well as modern and more precise imaging methods) help the early detection of tumours or lesions predisposing to tumours. Objective. The aim of this study paper is to review current knowledge and reports regarding primary and secondary prevention of breast cancer. State of knowledge. It is estimated that nearly 70% of malign tumours are caused by environmental factors, whereas in breast cancer this percentage reaches 90–95%. There are national programmes established in many countries to fight cancer, where both types of prevention are stressed as serving to decrease morbidity and mortality due to cancers. Conclusions. Cancer prevention is currently playing a key role in the fight against the disease. Behaviour modification, as well as greater awareness among women regarding breast cancer, may significantly contribute towards reducing the incidence of this cancer. Another important aspect is the number of women undergoing diagnostic tests, which still remains at an unsatisfactory level.

Introduction and objective. Phytoestrogens are one of nutritional factors exhibiting a chemoprotective action, potentially reducing vasomotor symptoms as well as cancerous lesions. The study was ocused on an association between the consumption of phytoestrogens in the diet, blood serum cholesterol concentration and urinary estrogen metabolites levels. Materials and methods. The study was conducted on 55 women in the perimenopausal period, who were characterized by an increased risk of hormone-dependent cancer. Energy and nutritive value of food rations, and especially the level of phytoestrogen consumption in the diets in the examined population, were analyzed using the Wikt Pro computer programme. Estrogen metabolites (2OHE, 16α-OHE1) were determined from 24‑urine samples by the ELISA method. The EMR index was calculated. Plasma total cholesterol concentrations was measured. Results and conclusions. The population with a higher consumption of phytoestrogens and dietary fibre in the diet, at the simultaneous slight limitation of energy value of the food ration, is characterized by an improved blood serum cholesterol concentration and an appropriate estrogen metabolism towards a significant increase in the estrogen index for the cancer lesions in the mammary glands.

Opis:
Introduction. The biological activity of VEGF depends on the presence of its specific receptors on the endothelial surface: VEGFR-1, VEGFR-2, and on their soluble forms sVEGFR-1 and sVEGFR-2. The binding of the membrane-bound receptors with VEGF affects the permeability, proliferation and migration of vascular endothelial cells. This creates the necessary conditions for the vascularisation of solid tumours and for the spread of remote metastases. The sVEGFR-1 and sVEGFR-2 receptors are believed to be natural inhibitors of VEGF. Objective. To determine the clinical usefulness of VEGF and the sVEGFR-1 and sVEGFR-2 receptors level assay in women with primary breast cancer. The assessment also took into account: patient’s age, stage of the disease, histological grade, status of the axillary lymph nodes and size of the primary tumour. Material and methods. The concentrations of VEGF, sVEGFR-1 and sVEGFR-2 were ascertained in 103 women with primary breast cancer. The concentrations of VEGF in the plasma, and those of the soluble receptors sVEGFR-1 and sVEGFR-2 in the serum, were assessed by ELISA, R&D Systems. Results. The study found significantly raised concentrations of VEGF, sVEGFR-1 and sVEGFR-2 in the serum of women with breast cancer, relative to the values obtained from the control group. It was found that with increasing clinical stages of the disease, the levels of VEGF and concentrations of sVEGFR-1 and sVEGFR-2 also increased. Similar findings were noted when assessing the degree of the histological grade of the tumours. Significantly higher values of VEGF protein and the assessed receptors were obtained from women with metastases to the axillary lymph nodes. A positive relationship, though without statistical significance, was noted between the concentration of sVEGFR-2 and the size of the tumour. Conclusions. The high concentrations of the VEGF cytokine and the sVEGFR-1 and sVEGFR-2 receptors in women with breast cancer are responsible for giving rise to the processes of tumour angiogenesis. The concentrations of the VEGF protein and the soluble forms of the receptors sVEGFR-1 and sVEGFR-2 in the serum of breast cancer patients showed positive correlations with the clinical stage of the disease. These results point to the usefulness of VEGF assessment and its soluble receptors in the clinical evaluation of patients with breast cancer.
