Background: Osteoporosis is a growing health concern across the world. Some epidemiological data suggest that cadmium increases risk for development of osteoporosis and lead to higher rate of fracture incidents even on low environmental exposure level. Material and methods: Cadmium in urine and bone resorption markers – total fraction of the urinary pyridynoline (Pyr) and deoxypyridinoline (DPyr) – were determined in 36 patients, who were examined for toxic effects of cadmium exposure. Additionally calcium in urine was determined. Associations between cadmium exposure and factors related to bone metabolism were estimated and Pyr and DPyr excretion were compared in three groups categorized across cadmium concentrations. Results: In the investigated group there were significant positive correlations between cadmium levels in urine and Pyr and DPyr excretion. None of the other variables correlated significantly with examined bone resorption markers excluding calcium excretion in urine. Excretion of Pyr and DPyr differed significantly between group with the lowest cadmium concentration (<1.2 μg/g creatinine) and group with the highest cadmium concentration (>1.9 μg/g creatinine), where median values of Pyr and DPyr increased by 49.8% and 37.5%, respectively. Conclusion: The results suggest that cadmium increases bone resorption processes and induce osteotoxic effects in environmental exposure level.
Wstęp: Osteoporoza jest rosnącym problemem na świecie. Niektóre badania epidemiologiczne sugerują, że kadm zwiększa ryzyko rozwoju tej choroby i prowadzi do zwiększenia ryzyka złamań kości nawet przy niskim narażeniu środowiskowym. Materiał i metody: Kadm w moczu oraz biomarkery resorpcji kostnej – całkowite frakcje pirydynoliny (Pyr) i dezoksypirydynoliny (DPyr) w moczu – były oznaczane u 36 pacjentów badanych pod kątem toksycznego działania kadmu. Dodatkowo oznaczano zawartość wapnia w moczu. Oszacowano zależności pomiędzy narażeniem na kadm a czynnikami powiązanymi z metabolizmem kości oraz porównano stężenia Pyr oraz DPyr pomiędzy grupami skategoryzowanymi względem stężenia kadmu w moczu. Wyniki: W badanej grupie wykazano statystycznie istotną pozytywną korelację pomiędzy poziomem kadmu w moczu, a ilością wydalanej Pyr i DPyr z moczem. Żadna z innych badanych zmiennych nie korelowała znacząco z markerami resorpcji kości oprócz wydalania wapnia z moczem. Zróżnicowanie wydalania Pyr i DPyr było statystycznie istotne pomiędzy grupami o najniższym (<1.2 μg/g kreatyniny) i najwyższym stężeniu kadmu w moczu (>1.9 μg/g kreatyniny), w której mediany wartości stężeń Pyr i DPyr wzrosły odpowiednio o 49,8% i 37,5%. Wnioski: Wyniki badań sugerują, że kadm wzmaga procesy resorpcji kości i działa osteotoksycznie również w narażeniu środowiskowym.

Introduction and objective. Bone sialoprotein (BSP) is present in a non-collagenous fraction of bone matrix, in dentine, cement and calcified cartilage. The characteristic place where sialoprotein is produced makes it highly specific in relation to the bone tissue and the possibility of using it as a marker of bone metabolism. The study aimed to assess the BSP level in the saliva of female patients at the menopausal and postmenopausal period. Materials and method. The study included 71 women aged 45–74 years. In the examined subjects, bone mineral density and bone sialoprotein levels in saliva were determined. Patients also responded to survey questions about place of residence and the frequency of consumption of foods rich in calcium. The obtained results were statistically analyzed. Results. In the study population of women, BSP saliva concentration ranged from 0.40–14.97 ng/ml. The highest BSP values were observed in the control group (average – 5.66 ng/ml, median – 6.73 ng/ml,), the lowest in the osteopenia group (average – 3.09 ng/ml, median – 1.71 ng/ml). This relationship was close to statistical significance (p = 0.056). Place of residence of the surveyed women had neither effect on BSP concentration in saliva nor on the value of the T-score ratio. Similarly, there was no statistically significant relationship between the frequency of consumption of foods rich in calcium and BSP and T-score. Conclusions. It was found that the BSP level can be determined in saliva. Biochemical tests of saliva considering bone metabolism are an issue that should be continued in future, as the availability of material (saliva) for laboratory analyses, carries potential diagnostic opportunities.

Introduction. Osteoporosis is a chronic condition with serious sequela, including primarily bone fractures, and impacts on almost all spheres of human life. It is important for patients undergoing treatment to accept their health status and feel satisfied with life. Objective. The aim of the study was to determine the relationship between socio-demographic factors, duration of the illness, self-assessment of the patient’s knowledge of osteoporosis, and also between illness management support and acceptance of the illness and life satisfaction among females with chronic osteoporosis, resident in rural areas. Materials and method. The study, conducted from September 2016 – June 2017, involved 207 patients of the Clinic of Orthopaedics and Rehabilitation and the Clinic of Gynaecology at the Independent Public Teaching Hospital No. 4 in Lublin, eastern Poland. The study used the Acceptance of Illness Scale (AIS) and the Satisfaction With Life Scale (SWLS). Statistical analysis was performed with Student’s t-test, analysis of variance (ANOVA), Tukey’s range test, and chi-squared test. A p-value of <0.05 defined statistical significance of differences. The analysis was performed using commercial SPSS Statistics 19 software (IBM Corp., Armonk, NY, USA). Results. The respondents showed average acceptance of their illness (22.2±6.9) and relatively low satisfaction with life (14.7±5.6). Older age, low level of education, poor living conditions, prolonged illness, and frequent ailments are all associated with lower acceptance of the illness and poorer life satisfaction. Conclusions. As part of their treatment, chronically ill patients should be provided with appropriate support, in particular from healthcare personnel, health education and improved living conditions.

Opis:
Objective. The aim of the study was to determine the effect of nesfatin-1 on bone properties in female rats in the conditions of developing osteopenia induced by ovariectomy (OVX). Materials and method. The experiment was performed on 21 female Wistar rats assigned to 3 groups receiving intraperitoneally physiological saline (SHO, OVX-PhS) and nesfatin-1 in dose 2 μg/kg BW of (OVX-NES) once a day for 8 wks. At the end of the experiment, the rats were scanned using the DXA method to determine the body composition, tBMC, and tBMD. The isolated femora and tibia were tested with the DXA method for BMD and BMC, and with the pQCT method for separate analysis of the cortical and trabecular bone tissue. The bone strength parameters were also determined. The immunohistochemical method was used for determination of nesfatin-1 localization in growth cartilage. Bone metabolism markers (osteocalcin, bALP, and NTx) were identified using an ELISA kit. Results. OVX exerts a negative effect on bone tissue. The nesfatin-1 administration influenced positively the DXA parameters of tibia. TvBMD and TbvBMD measured by pQCT in metaphysis of bones were significantly higher in the OVX-NES group than in OVX-PhS. No differences were found in the values of bone strength parameters between SHO and OVX-NES females. Extra- and intracellular immunohistochemical reaction for nesfatin-1 was observed in all zones of growth cartilage, with the strongest reaction detected in the calcifying zone. Nesfatin-1 administration caused a significant increase in the osteocalcin and bALP concentration in relation to the OVX-PhS animals. Conclusion. The results of the experiment indicate that nesfatin-1 exerts a protective effect on bone tissue properties and can be used in the prevention of osteoporosis.
