Introduction and objective. This study aimed to determine the environmental conditions for the occurrence of carpal tunnel syndrome (CTS) in a non-industrial area, and patient-reported outcomes after surgical release. Materials and method. This observational study utilized convenience sampling to screen 100 consecutive patients for carpal tunnel syndrome at the Orthopedic Clinic, using two questionnaires. Data were collected from the Disability of Arm Shoulder and Hand (DASH) questionnaire, and the PROMIS® (Patient-Reported Outcomes Measurement Information System) Upper Extremity and PROMIS® SF 3a questionnaire (Pain Intensity). The relationship of various repetitive musculoskeletal disorders to CTS was validated by questionnaire scores, PROMIS® T-score, and correlation coefficients. Results. Finally, CTS was confirmed by electromyography in 69 patients (55 females and 14 males; average age: 47.5 years). Aging significantly influenced the occurrence of symptoms associated with pain (neck, thoracic, lower back, shoulder, and CTS (p<0.001)). Those employed for longer more frequently declared performing exercises to prevent overload pain (p<0.001). DASH results significantly correlated with the PROMIS Upper Extremity score (r = -0.64; p<0.05). Conclusions. Geographical and environmental conditions indicate that even though working with a computer is described as an essential risk factor for CTS, the study group showed a predominance of elements that were unrelated to working at a computer. CTS also occurred among people working physically, and even among unemployed individuals. The existence of a statistically significant, strong, negative correlation (r=-0.64; p < 0.05) between the results obtained in the questionnaires DASH and PROMIS Upper Extremity has been demonstrated.

Introduction and Objectives. One of the features of systemic transformation are its social costs. This is also the case with the Polish transformation initiated in 1989. Social processes connected with it are a kind of accelerator which increases the range of health needs realized outside the medical system. Utilization of non-medical healing methods may also be perceived as a way of coping with negative, i.e. sociopsychological consequences of transformation, including the fact that many people’s health needs were not met within institutional medicine. Such a situation results in a deepening of social inequalities in health. This problem will be presented from the perspective of ‘ordinary people’ in accordance with the leading research directive in medical sociology. Following the directives of humanistic sociology, the sources of information on the subject were personal documents. Materials and Methods. The paper presents selected results of the sociological qualitative analysis of 1,311 letters received by the editorial section of public Polish Television’s Channel Two in 1991 in connection with the broadcasting of a series of programmes conducted by the unconventional therapist Anatoly M. Kashpirovsky, who had a viewership of eight to nine million, on average. The presented and commented-on material consists of spectators’ statements on the adverse consequences of the systemic transformation, concerning health and illness. Results. The post-1989 political-system transformation is the most thorough-going social change in post-war Poland. It triggered off a number of both positive and negative processes. The negative ones include the deepening of social inequalities in health as a result of, inter alia, the progressive pauperization of society and also the growing utilization of non-medical healing methods. The negative effects of transformations are especially felt by typical ‘clients’ of Anatoly M. Kashpirovsky: poorly educated, indigent, residents of villages and small Polish towns: ‘the transformation process losers’. Conclusions. Systematic sociological knowledge on the ways of description and interpretation of health and illness by ‘lay people’ enables a more complete understanding of phenomena related to inequalities in health, including their social and structural causes.

Introduction and objective: The presented analysis is a reconstruction of the origins, inspirations for development, and theoretical foundations of the critical and unmasking trend in Polish and Western medical sociology. Abbreviated description of the state of knowledge: As a part of the critical medical sociology initiated in Poland by Professor Magdalena Sokołowska, a diagnosis of the (dys)functionality of contemporary medicine is carried out, emphasizing pathologies in the realization of its basic social functions, both at the level of systemic and institutional solutions, as well as stressing their consequences which include inter alia social health inequalities. Within the critical sociomedical research orientation, the diagnoses of the social role of medicine and distortions in the ways it is exercised are placed in the broad structural, political, and cultural contexts, which makes it possible to point to the principal causes of the analyzed phenomena. Summary: The crucial ‘value added’ of critical sociological analyses of medicine and health policy are directives intended to humanize medicine and health systems in contemporary societies, taking social and cultural realities into consideration. We understand the humanization of medicine in terms of its better adjustment to human needs that emerge in the situations of illness and being ill, with the simultaneous guarantee of universal and equal access to medical services.

Tobacco smoking is a major avoidable single cause of premature mortality in Poland. Almost one in three Polish males do not live to 65 years of age, and almost half of this premature mortality can be traced back to the much higher smoking prevalence in Poland than in Western Europe – every third Polish male and every fourth Polish female smokes daily. However, the current health situation in Poland is much better than two decades ago when the country entered a period of political and economic upheaval. In the early 1990s, the state of health of the Polish population was catastrophic and its tobacco consumption levels the highest in the world. In the early 1990s, the probability of a 15-year-old Polish boy living to the age of 60 was not just twice lower than in Western Europe, but also lower than in China or India. The health policy of limiting the health consequences of smoking conducted by the European Union and, in the last two decades, by the Polish parliament and government, helped to stop this health catastrophe. In Poland, cigarette consumption has decreased by 30% since 1990, as did lung cancer mortality among males. Despite this progress, tobacco smoking remains the most serious health problem in Poland. Therefore, comprehensive tobacco control policy should not only be continued, but expanded and accelerated. The EU Tobacco Products Directive proposes a package of actions for reducing tobacco-related health harm in Europe. The Directive proposal is rational, science-and-evidence based, and grounded on the best practice examples from other countries. Both the Polish tobacco control law and the WHO Framework Convention on Tobacco Control (FCTC), ratified by Poland in 2006, oblige our country to support tobacco control, including all the initiatives taken by the European Union.

Introduction. Depression is a major public health problem. Magnesium (Mg2+) is involved in many metabolic processes as an activator of over 300 different enzymes. For the last 60 years lithium (Li+) compounds have been used in psychiatry. Li+ salts are regarded as the first choice medicine in the treatment of affective disorders and are also applied as an adjuvant intensifying the therapy in drug-resistant depression patients. Objective. The objective of the study was an analysis of the relationship between the levels of magnesium, lithium, and education and place of residence of patients hospitalized due to depression. Material and methods. Patients with bipolar affective disorders undergoing lithium therapy during their stay in the Department of Psychiatry at the Medical University in Lublin were examined. Patients were divided into three groups according to education level and were also analyzed according to place of residence. Results. In the group of patients in the study, a significantly lower level of magnesium was found (p=0.02) in blood plasma of patients with secondary education level, compared to those who had elementary education. There was also a significantly higher level of magnesium (p=0.01) in blood plasma of patients who lived in urban areas, compared to rural inhabitants. No statistically significant differences were noted between lithium level in plasma, and the patients’ place of residence (p=0.34). Conclusion. Significantly higher plasma magnesium levels were observed among city than village inhabitants, there was also a relationship between type of education and magnesium level in blood plasma of the patients in the study. Further studies including larger groups of patients should be performed to enable a final conclusion.
