Multiple sclerosis is a chronic inflammatory disease which, due to the destruction of the fibres of the central nervous system in the process of demyelination, leads to numerous neurological symptoms and progressive disability. The disease is autoimmune, which means that myelin is destroyed by the patient’s own cells, caused by improper functionong and regulation of the immune system. Multiple sclerosis affects more and more people and is therefore a significant clinical problem which, to a large Entent, affects women, especially in childbearing age This presents a big challenge for carrying out pregnancy while continuing the therapy, ensuring the safety of the foetus and simultaneously achieving the best possible therapeutic effect. The decision whether the therapy should be continued or whether it should be eliminatedis usually made according to assessment of the possible gains and losse. Despite the lack of clear indications, there are many studies proving the relative safety of the use of individual but not all the drugs during pregnancy. Pregnancy, however, has a fairly good impact on the development of multiple sclerosis, and that safety considerations, especially those concerning the growing foetus, force a decision to change or completely suspend the therapy. In-depth research on the already available and emerging therapeutic pathways in multiple sclerosis bring hope for increasingly better results in the future in the treatment of pregnant patients with multiple sclerosis.

Association between coal and firewood combustion and hospital admissions and mortality in Chile 2015 - an ecological approach

Introduction and objective. Burning coal and firewood generates toxic emissions that are associated with respiratory illness, cardiovascular disease, and even death. The aim of the study is to evaluate the association between county-level prevalence of household coal and firewood use and health outcomes, including total, respiratory, and cardiovascular mortality, as well as total and respiratory hospitalization rates. Materials and method. The ecological study included data on the use of household coal and firewood in 139 counties obtained from the 2015 Chilean National Socio-economic Characterization Survey. Total, respiratory, and cardiovascular mortality, as well as total and respiratory hospitalization rates, were obtained from the Department of Health Statistics. Poisson models with robust error variance, Pearson linear correlation coefficients, and scatterplots were used to explore associations between household coal and firewood use and morbidity-mortality, stratifying by geographic zone. Results. Total, respiratory, and cardiovascular mortality and total and respiratory hospitalization rates were 5.7 per 1,000, 552 per 100,000, 157 per 100,000, 92.5 per 1000, and 8.8 per 1000 inhabitants, respectively. The median prevalence of coal use for residential cooking, heating, or water heating was 3.64%, while the median prevalence of firewood combustion was 12%. In southern counties, age- and gender-adjusted respiratory mortality increased 2.02 (95% CI: 1.17–3.50), 1.5 (95% CI: 1.11–1.89), and 1.76-fold (95% CI: 1.19–2.60) for each percentage increase in household coal and firewood use for heating, cooking and heating water, respectively. Conclusions. The prevalence of household coal and firewood used for heating and cooking was positively correlated with respiratory mortality and hospitalization in southern zone counties.

Introduction and objective. Infectious and parasitic diseases transmitted by ticks, such as Lyme diseases, granulocytic anaplasmosis and piroplasmosis, have been frequently reported in Europe, with increasing attention to them as an emerging zoonotic problem. The presented study was performed to assess the distribution and the density of questing ticks in three regional parks of Emilia-Romagna region of Northern Italy, and to seek molecular evidence of potential human pathogens in tick populations. Materials and Methods. In the period April-October 2010, 8,139 questing ticks were collected: 6,734 larvae, 1,344 nymphs and only a few adults – 28 females and 33 males. The abundance of Ixodes ricinus questing ticks was compared among different sampling sites and related to microclimate parameters. 1,544 out of 8,139 ticks were examined for the presence of pathogens: PCR was used to detect piroplasms DNA and Real time Taqman PCR for Anaplasma phagocytophilum and Borrelia burgdorferi s.l. Results. The predominant species was I. ricinus (overall abundance 1,075.9/100 m2); more rarely, Dermacentor marginatus (n = 37 – 0.45%), Scaphixodes frontalis (n = 13 – 0.16%), Hyalomma spp. (n = 6 – 0.07%) and Ixodes acuminatus (n = 3 – 0.04%) were also found. 28 out of 324 (8.6%) samples of ticks were PCR-positive for piroplasm DNA. 11 amplicons of 18S rRNA gene were identical to each other and had 100% identity with Babesia EU1 (Babesia venatorum) using BLAST analysis. Real time Taqman PCR gave positive results for A. phagocytophilum in 23 out of 292 samples (7.9%), and for B. burgdorferi s.l. in 78 out of 292 samples (26.7%). I. ricinus was the only species found positive for pathogens by molecular analysis; 16 tick samples were co-infected with at least 2 pathogens. Discussion. The peak of nymph presence was in May, and the higher prevalence of pathogens occurred in April-June, most often in nymphs; therefore, spring season could represent the higher risk period for the transmission of pathogens. These data could provide guidelines for the preventions of tick-trasmitted diseases in this region.

Introduction. In animal models, gestational exposure to inorganic arsenic has been associated with higher corticosterone concentration and consequent impairment of stress control in offspring. An equivalent association relating cortisol, a glucocorticoid hormone, in humans has not been previously studied. Objective. The aim of the study was to explore the association between prenatal inorganic arsenic exposure and salivary cortisol in infants from Arica, Chile. Materials and method. A cohort study of 168 mother-child dyads was recruited. In the 2nd trimester of pregnancy, urinary inorganic arsenic was assessed; 18–24 months after delivery, salivary cortisol was measured in the children. Maternal cortisol, maternal depression, stress, and socio-economic status were also evaluated. Results. The adjusted association was estimated with multiple linear regression after evaluating confounding through a directed acyclic graph. Median urinary inorganic arsenic in pregnant women was 14.1 μg/L (IQR: 10.4–21.7) while salivary cortisol in the children was 0.17 μg/L (IQR: 0.11–0.38). Among children from the highest income families (> 614 USD/month), arsenic exposure was associated with salivary cortisol. Children in the third quartile of arsenic exposure had -0.769 units of the logarithm of salivary cortiso, compared with those in the first quartile (p = 0.045). Conclusions. In this sample, prenatal exposure to arsenic was associated with salivary cortisol (third quartile of inorganic arsenic), only in infants belonging the highest income strata (> 614 USD). More studies are needed to confirm these preliminary results.
