Objectives: To assess the effect of exposure to flour dust on pulmonary function tests, prevalence of symptoms (respiratory, allergic/irritating) and parameters of allergic sensitization in terms of skin prick test, and bronchial hyper-responsiveness. Material and Methods: 43 bakers (with at least 2 years of occupational exposure) working at different bakeries in Ismailia city, Egypt; and 64 control subjects of comparable socio demographic characteristics were compared. All participants were subjected to an interview questionnaire, clinical chest examination, skin prick test, bronchial hyper-responsiveness test and measurements of pulmonary function parameters. Results: All respiratory and allergic symptoms were more prevalent among bakers compared to the control group, with the highest odds ratio for allergic symptoms (OR = 6.9; p < 0.0001) and dyspnea (OR = 6.3; p = 0.0004). Bakers had a higher percentage of SPT positive results with statistically significant difference between the two groups (p < 0.0001). Bakers had lower observed values compared to the control group with statistically significant difference for FVC, FEV₁/FVC ratio, $ \text{FEF}_\text{75%} $, and $ \text{FEF}_\text{25-75%} $ parameters. Conclusion: The present study suggested that occupational exposure to flour dust may affect respiratory irritation and sensitization, and reduce the pulmonary function tests such as FVC, FEV₁, and FEV₁/FVC ratio and $ \text{FEF}_\text{25-75%} $.

Introduction: Information on the changes of pulmonary function and the right cardiac structure in patients with coal worker’s pneumoconiosis in China is very scarce. This study was performed to clarify the changes of pulmonary function and right cardiac structure in patients with coal worker’s pneumoconiosis in China. Material and methods: Pulmonary function, pulmonary artery systolic pressure, and the right cardiac structure were evaluated by spirometry and color Doppler echocardiography. Results: The pulmonary artery systolic pressure of patients with coal worker’s pneumoconiosis was increased with disease severity. Patients with coal worker’s pneumoconiosis also exhibited an impaired pulmonary function and altered right cardiac structure compared with control subjects. A significant linear correlation of the variables of pulmonary ventilation and diffusion function with the indicators of the right cardiac structure was found in patients with coal worker’s pneumoconiosis in China. Conclusions: This study elucidated a deterioration of pulmonary function and right cardiac structure in patients with coal worker’s pneumoconiosis in China.

Isoprostanes are a recently discovered group of prostaglandin isomers. Results of previous studies suggest that they can be used as oxidative stress markers, because in a number of cardiovascular, pulmonary and neurological diseases their levels in biological samples considerably increase. It has been found that people suffering from diabetes, obesity, homozygous familial hypercholesterolemia, moderate hypercholesterolemia, and smokers have higher levels of isoprostanes in urine. The same refers to patients with asthma, Alzheimer disease and Down syndrome. This paper reviews the results of relevant studies.

Opis:
Ethylene phosphorodifluoridite (C2H4F4O2P2) (CAS No. 3965-00-2) is a colorless corrosive fuming liquid that is used as a stabilizer in the electrolyte of rechargeable batteries. There are no previous reports of toxic effects following exposure to this compound. A 28-year-old male complained of respiratory distress after accidental inhalation of ethylene phosphorodifluoridite for 30 min. The patient developed acute lung injury with noncardiogenic pulmonary edema and was treated with supportive management. The patient fully recovered and was discharged after 7 days without any significant sequelae. The patient’s symptoms were attributed to non-cardiogenic pulmonary edema caused by ethylene phosphorodifluoridite exposure. The case study showed that ethylene phosphorodifluoridite should be added to the list of chemicals that can cause acute lung injury.

ObjectivesSilicosis is a chronic occupational lung disease. As was previously found by the authors, some proteins increased in the lung tissue of activated rats, and protein tyrosine phosphatase non-receptor type 2 (PTPN2), factor B, and vaccinia-related kinase 1 (VRK1) showed highly differential expressions.Material and MethodsIn this study, serum and bronchoalveolar lavage fluid samples were collected from patients with silicosis and healthy people to verify the expression of PTPN2, factor B, and VRK1. The diagnostic value of differentially expressed proteins for silicosis was judged.ResultsThe expression levels of serum PTPN2, VRK1, and factor B in patients with silicosis were significantly higher than those in the control group (p < 0.01). Higher serum PTPN2 and factor B concentrations significantly and negatively correlated with the ratio of forced expiratory volume in 1 s to forced vital capacity (FEV1/FVC), maximum vital capacity (VCmax), FEV1, and FVC, suggesting that the high expression of PTPN2 and factor B is associated with decreased pulmonary ventilation function and restrictive ventilatory impairment in patients with silicosis. All area under curve (AUC) measurements generated from single detection events were >0.744, with PTPN2 reaching the highest value (0.858). The AUC, sensitivity, and specificity for the combined diagnosis using factor B and PTPN2 were 0.907, 86.91% and 85.07%, respectively, for factor B and PTPN2. The 3 differentially expressed proteins are potential classes of predictive biomarkers for silicosis.ConclusionsRegarding the economy and test practicality, the best diagnostic combination is factor B and PTPN2 for the analysis of AUC, sensitivity and specificity.

Background The photocopying occupation contributes to various health issues in workers. Elevated levels of oxidative stress have been found to be associated with respiratory problems in occupational workers. The intention of the current research is to evaluate the association between the pulmonary function parameters, a biomarker of oxidative stress, and the photocopying occupation in Lahore, Pakistan. Material and Methods The study was performed on a group of photocopying operators (N = 100) and a control group (N = 100). A structured questionnaire was developed to gather the demographic attributes. Lung function testing was done with the spirometer. The blood lipid peroxides level, as thiobarbituric acid reactive substances (TBARS), was assessed spectrophotometrically. Results A statistically significant reduction was found in the pulmonary function parameters as compared to the control group. In the study group, the prevalence of respiratory symptoms, including cough (31%), sneezing (18%), wheezing (9%) and breathing issues (19%), was high as compared to the control group. A significantly higher plasma TBARS concentration was observed in the photocopying workers. A negative correlation prevailed between FVC (−0.081), FVC% predicted (−0.038) and FEV1 (−0.043), and the duration of exposure. Conclusions The study showed that the occupational exposure could have a significant effect on the normal pulmonary function and the oxidative potential of well-being. The profound emissions in the vicinity of photocopying centers may be responsible for the variations in the pulmonary function parameters and oxidative stress. Med Pr. 2019;70(4):403–10

Introduction Increasing evidence suggests that idiopathic pulmonary fibrosis (IPF) occurs more often in subjects previously exposed to wood dusts than in non-exposed subjects. Here we report 2 cases of the IPF among workers prolongedly exposed to high levels of hardwood dusts. Case report The case No. 1: An 83 year-old male former smoker, retired joiner developed mild dyspnoea and chronic dry cough over the period preceding the examination. Pulmonary function tests (PFT) identified a mild restrictive pattern and diffusion capacity for carbon dioxide (CO₂) that was severely impaired (57% of predicted value). High resolution computer tomography (HRCT) identified bilateral, subpleural basal reticular opacities in honeycombing, without any nodules or ground-glass opacities. The case No. 2: A 73 year-old male retired joiner, never smoker, presented a 3-year history of progressive breathlessness and non-productive cough in mild hypoxemia. Pulmonary function tests suggested a moderate restrictive pattern in severely impaired diffusion capacity for CO₂ (54% of predicted value). High resolution computer tomography identified diffuse peripheral reticular opacities and honeycombing of lower fields, with apico-basilar gradient. Both cases received diagnosis of the idiopathic pulmonary fibrosis. Discussion The pulmonary fibrosis is a common feature of several diseases and may be induced by inflammatory disorders following inhalation of organic and inorganic dusts (e.g., asbestos, silica), and several reports suggest that many cases of the IPF may be in fact secondary to occupational dust exposure as in the case reports we present here. Conclusions Occupational exposure to wood dusts may be a risk factor for the IPF. Unfortunately, exposure reconstruction is frequently inconsistent and anamnesis often misses other causes of the pulmonary fibrosis (e.g., extrinsic allergic alveolitis). Med Pr 2015;66(5):739–747
Wstęp Coraz więcej dowodów wskazuje na częstsze występowanie samoistnego zwłóknienia płuc (idiopathic pulmonary fibrosis – IPF) u osób wcześniej narażonych na pyły drewna niż u osób nienarażonych na nie. Niniejszy artykuł przedstawia 2 przypadki IPF u pracowników długotrwale eksponowanych na wysokie stężenia pyłu drewna. Opis przypadków Przypadek nr 1: U 83-letniego mężczyzny, byłego palacza i emerytowanego stolarza, w ciągu 6 miesięcy poprzedzających badanie rozwinęły się lekka duszność oraz przewlekły suchy kaszel. Badania czynnościowe płuc wykazały łagodne zaburzenia wentylacji typu restrykcyjnego oraz poważne upośledzenie dyfuzji dwutlenku węgla (CO₂) (57% wartości prognozowanej), natomiast tomografia komputerowa wysokiej rozdzielczości – obustronne podopłucnowe siateczkowate zacienienia u podstawy płuc w postaci obrazu plastra miodu, bez żadnych guzków ani zacienień typu matowej szyby. Przypadek nr 2: Badania czynnościowe płuc 73-letniego mężczyzny, nigdy nie palącego emerytowanego stolarza, z postępującą od 3 lat dusznością, suchym kaszlem oraz łagodną hipoksemią, wykazały umiarkowane zaburzenia wentylacji typu restrykcyjnego oraz poważne upośledzenie dyfuzji CO₂ (54% przewidywanej wartości), natomiast tomografia komputerowa o wysokiej rozdzielczości – rozproszone obwodowe siateczkowate zacienienia oraz obraz plastra miodu w dolnych polach ze szczytowo-podstawnym gradientem. W obu przypadkach zdiagnozowano samoistne zwłóknienie płuc. Omówienie Zwłóknienie płuc jest wspólną cechą kilku chorób i może być wywołane przez procesy zapalne wskutek wdychania organicznych lub nieorganicznych pyłów (np. azbestu, krzemionki). Wiele doniesień, w tym przedstawione w niniejszej pracy, wskazuje na liczne przypadki IPF, w których etiologii narażenie zawodowe na pyły miało charakter drugorzędny. Wnioski Narażenie zawodowe na pyły drewna jest czynnikiem ryzyka występowania IPF. Niestety odtworzenie rzeczywistej ekspozycji zazwyczaj jest trudne, a przeprowadzany wywiad często pomija pozostałe przyczyny powstania zwłóknienia płuc (np. alergiczne zapalenie pęcherzyków płucnych). Objectives Idiopathic pulmonary fibrosis (IPF) (J84.1 by the International Statistical Classification of Diseases and Related Health Problems, 10th revision – ICD-10) is a rare disease of unknown cause. Among many risk factors, occupational exposure to metal dust is enumerated. Because of metal industry concentration in the Silesian Voivodeship, Poland, an attempt of spatial and temporal variability of the idiopathic pulmonary fibrosis occurrence evaluation in 2006–2010 is made. Material and Methods The research is a descriptive analysis. Data of the number of new treated cases and firsttime hospitalization due to idiopathic pulmonary fibrosis in the years 2006–2010 was obtained from the National Health Found (Narodowy Fundusz Zdrowia – NFZ). We also assessed spatial and temporal variability of both rates. Data concerned adults aged ≥ 19 years old inhabited the Silesian Voivodeship. Results The standardized incidence rate due to idiopathic pulmonary fibrosis increased in the study period and was in the range of 2.9–3.8/100 000 population. The highest values of incidence were observed in districts localized in the centre of the Silesian Voivodeship. Conclusions Incidence of idiopathic pulmonary fibrosis is not so high in the Silesian Voivodeship but the number of newly treated cases slightly increased in 2006–2010. Spatial and temporal variability of incidence rates for treated registered cases of idiopathic pulmonary fibrosis in the Silesian Voivodeship was observed. It could be an effect of occupational exposure to metal dust, however, final conclusions need more precise studies to the extent of analytical epidemiology. Objectives: Chronic obstructive pulmonary disease (COPD) may be work-related. It has been estimated that 15% of the population burden of COPD is attributable to occupational exposure. However, in Poland COPD is rarely recognized as an occupational disease. The aim of the study has been to analyze the causes of the low prevalence of work-related COPD in the context of the existing criteria as well as to analyze which part of the assessment – clinical or hygienic one – is responsible for such a low rate of occupational COPD recognitions. Material and Methods: The study group included 150 patients hospitalized with a suspicion of occupational COPD. Each patient underwent a clinical examination, spirometry and reversibility test using bronchodilator. Moreover, hygienic evaluation of work conditions was performed in all the considered cases. Results: In the case of the patients who fulfilled the criteria for COPD diagnosis in accordance with the Global Initiative for Chronic Obstructive Lung Disease (GOLD) occupational origins of the disease, the disease was not recognized because 24.1% of the individuals did not meet spirometric criteria included in a definition of COPD in the Polish list of occupational diseases, while 27.8% of the individuals did not fulfill the criterion of a documented exposure to dusts and irritant gases. None of these criteria was fulfilled by 42.6% of the patients. Conclusions: In our country, both clinical and hygienic criteria result in limitations in recognition of occupational COPD. There is the need to establish new guidelines for the recognition of COPD as a compensable disease in Poland. Int J Occup Med Environ Health 2018;31(2):139–150

Occupational hypersensitivity pneumonitis (OHP) is an interstitial lung disease caused by sensitization to an inhaled antigen. Polyethylene terephthalate (PET) is mainly used for disposable beverage bottles. A clinical case of hypersensitivity pneumonitis (HP) in a 66-year-old patient in the follow-up as a worker formerly exposed to asbestos is presented. At the first visit in 2012 a diagnosis of asbestosis and pleural plaques was formulated. In 2017 the high resolution computed tomography was performed demonstrating a slight progression of the pulmonary fibrosis, while physical examinations revealed inspiratory crackles on auscultation, and lung function tests showed a decreased diffusing capacity for carbon monoxide. The radiological and histological pictures were compatible with HP. From 1992 to 2013 the patient worked in a chemical company that produced PET for disposable beverage bottles. A diagnosis of OHP was made, and the most likely causative agents were terephthalic acid and dimethyl terephthalate. To the best of the authors’ knowledge, this is the first report of an OHP case in PET production.

Pulmonary alveolar proteinosis (PAP) is a rarely seen disease of the alveoli, characterized by accumulation of proteinous material, which stains positive with periodic acid Schiff, in the alveoli. Secondary PAP may develop as a result of occupational exposure to materials such as silica and indium. In the paper, together with a review of the relevant literature, we present an uncommon case of a 47-year old male, marble worker who was diagnosed with PAP associated with a 12-year history of exposure to marble dust. Opis:
The natural course of idiopathic pulmonary fibrosis (IPF) is unpredictable at the time of diagnosis. Some patients may experience episodes of acute respiratory worsening that have been termed acute exacerbations. A 58-year-old male was admitted to our Emergency Department due to progressive and intense dyspnea and dry cough after accidental inhalation of waterproof’s vapor containing siloxanes. Chest high resolution computed tomography (HRCT) scan showed diffuse and bilateral ground glass attenuation, basal predominant reticular abnormalities and subpleural honeycombing. The patient didn’t know that he suffered from IPF and siloxanes’ inhalation triggered an acute exacerbation of his disease. Clinical course after the inhalation was aggressive and, despite steroids and cyclophosphamide therapy, the patient died 3 months after due to a respiratory failure. Inhalation of water repellents has been associated with an acute onset of respiratory symptoms and acute lung injury; usually, however, the prognosis is commonly good with a complete recovery. Our case is an example of an extremely negative reaction probably because of pre-existing and misdiagnosed IPF. Currently, no literature concerning water repellent inhalation as a trigger of acute exacerbation of IPF is available.

Objectives: To explore the rate of pneumoconiosis in dental technicians (DTP) and to evaluate the risk factors. Material and Methods: Data of 893 dental technicians, who were admitted to our hospital in the period January 2007–May 2012, from 170 dental laboratories were retrospectively examined. Demographic data, respiratory symptoms, smoking status, work duration, working fields, exposure to sandblasting, physical examination findings, chest radiographs, pulmonary function tests and high-resolution computed tomography results were evaluated. Results: Dental technicians’ pneumoconiosis rate was 10.1% among 893 cases. The disease was more common among males and in those exposed to sandblasting who had 77-fold higher risk of DTP. The highest profusion subcategory was 3/+ (according to the International Labour Organization (ILO) 2011 standards) and the large opacity rate was 13.3%. Conclusions: To the best of our knowledge, it was the largest DTP case series (N = 893/90) in the literature in English. Health screenings should be performed regularly for the early diagnosis of pneumoconiosis, which is an important occupational disease for dental technicians.

Objectives A water pipe (hookah) is a tobacco smoking tool which is thought to be more harmless than a cigarette, and there are no adequate studies about its hazards to health. Water-pipe smoking is threatening health of the youth in the world today. The objective of this study has been to investigate the carbon monoxide (CO) levels in breath, examine the changes in pulmonary function tests (PFT) and to assess the change of the oxidative stress parameters in blood after smoking a water pipe. Material and Methods This study is a cross-sectional analytical study that has included 50 volunteers who smoke a water pipe and the control group of 50 volunteers who smoke neither a cigarette nor a water pipe. Carbon monoxide levels were measured in the breath and pulmonary function tests (PFTs) were performed before and after smoking a water pipe. Blood samples were taken from either the volunteer control group or water-pipe smokers group after smoking a water pipe for the purpose of evaluation of the parameters of oxidative stress. Results Carbon monoxide values were measured to be 8.08±7.4 ppm and 28.08±16.5 ppm before and after smoking a water pipe, respectively. This increment was found statistically significant. There were also significant reductions in PFTs after smoking a water pipe. Total oxidative status (TOS), total antioxidant status (TAS) and oxidative stress index (OSI) were found prominently higher after smoking a water pipe for the group of water-pipe smokers than for the control group. Conclusions This study has shown that water-pipe smoking leads to deterioration in pulmonary function and increases oxidative stress. To the best of our knowledge this study is the only one that has shown the effect of water-pipe smoking on oxidative stress. More studies must be planned to show the side effects of water-pipe habit and protective policies should be planned especially for young people in Europe. Wstęp Śródmiąższowe choroby płuc to grupa chorób przewlekłych wpływająca na znaczne pogorszenie jakości życia. Postępowanie terapeutyczne w przypadku tych chorób wymaga także rozpoznania i leczenia chorób współistniejących, dlatego jest obarczone wysokimi kosztami bezpośrednimi i pośrednimi. Brak danych epidemiologicznych dotyczących częstości występowania chorób śródmiąższowych w naszym kraju, jak również doniesienia o rosnącej zapadalności w innych krajach europejskich uzasadniają poznanie sytuacji epidemiologicznej w odniesieniu do tych chorób, z uwzględnieniem chorób współistniejących. Materiał i metody Na podstawie danych rejestrowych z lat 2006–2010, udostępnionych przez Narodowy Fundusz Zdrowia (NFZ), oceniono czasowe zróżnicowanie standaryzowanych współczynników opisujących liczbę nowych zachorowań oraz przedstawiono strukturę chorób współistniejących. Dane dotyczyły mieszkańców województwa śląskiego w wieku ≥ 19 lat. Wyniki Wykazano niewielki, lecz systematyczny, spadek wartości standaryzowanego współczynnika zachorowań na śródmiąższowe choroby płuc w zakresie 9,7–7,8/100 000 dorosłych mieszkańców. Wśród chorób współistniejących dominowały choroby układu krążenia, przewlekłe i zakaźne choroby układu oddechowego, choroby metaboliczne oraz choroby układu mięśniowo-szkieletowego i tkanki łącznej. Choroby towarzyszące występowały częściej wśród osób starszych (≥ 65 lat), z wyjątkiem sarkoidozy, w której przypadku liczba rejestrowanych chorób współistniejących była 4-krotnie większa u młodszych osób (19–64-letnich). Koszty leczenia wybranych chorób układu oddechowego w województwie śląskim sięgają ponad 50 mln złotych rocznie. Wnioski Wykazano niewielki spadek wartości standaryzowanego współczynnika zachorowań na śródmiąższowe choroby płuc u dorosłych mieszkańców województwa śląskiego. Wśród najczęściej rejestrowanych chorób współtowarzyszących znajdują się choroby układu krążenia oraz przewlekłe i zakaźne choroby układu oddechowego. Background Interstitial lung diseases form a group of chronic diseases associated with a significant worsening of the quality of life. Proper management of these diseases involves the recognition and treatment of comorbidities, so it implies high direct and indirect costs of therapy. The lack of epidemiological data on the total incidence of interstitial diseases in Poland, as well as of information on their increasing incidence in other European countries justify investigations into epidemiological situation in the Silesian voivodeship (the southern region of Poland). Material and Methods In a descriptive study registered data provided by the National Health Found in 2006–2010 were used to assess the temporal variability of standardized incidence rates. A data set included interstitial lung diseases and comorbidities in adults aged ≥ 19 years, residents of the Silesian voivodeship. Results In the period under study standardized incidence ratios for interstitial lung disease declined from 9.7/100 000 adult population to 7.8/100 000 adult population. The most prevalent comorbidities included cardio-vascular diseases, chronic and infectious respiratory diseases, metabolic diseases and musculoskeletal and connective tissue diseases. Comorbidities were found more frequently in older people ≥ 65 years, except for sarcoidosis, which was four times more prevalent in younger people (19–64 years) compared to older patients. The estimated costs of treatment of the analysed diseases reached more than 50 mln zlotys (12 mln dollars) per year. Conclusions A small but systematic decrease in the value of the standardized incidence rate for interstitial lung disease has been shown within the range of 9.7–7.8/100 000 adult inhabitants. The most frequent comorbidities included cardiovascular or chronic and infectious respiratory diseases. The high cost of therapy of interstitial lung diseases was largely related to simultaneous treatment of comorbidities. Med Pr 2016;67(6):751–763
