Neuropathic pain and chronic pain constitute an interdisciplinary problem on the border of medicine, psychology, sociology and economics. While it seems to be underestimated, the scale of this problem will continue to increase due to the population aging and the growing incidence of lifestyle disorders. People employed in various occupational sectors may also wrestle with these disease units, which affect the quality of their life, mental health and work productivity. A narrative review provided an overview of neuropathic pain and chronic pain, and their relationship to such factors as job type, work absenteeism and productivity decline, as well mental well-being. A systematic literature search was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines to identify appropriate literature by searching the electronic databases: PubMed/MEDLINE, Pain Journal and the Cochrane Database of Systematic Reviews. Studies were published in Polish, English and French. Research shows an increasing number of musculoskeletal diseases in professionally active people, which lead to disability or provoke work absences. However, sickness presenteeism and/or absenteeism caused by pain not only leads to economic burdens, but also to burnout, fatigue and depression syndromes in employees. These disorders may require specialized effective interventions to support the return to work or maintaining employment despite experiencing pain. Every patient with chronic or neuropathic pain should be correctly assessed to determine the best method of treatment and its effectiveness.

The authors aimed to characterize the current evidence base regarding musicians’ musculoskeletal symptoms (MSS), in order to identify gaps for future research. A systematic search was conducted to identify peer-review studies published in English in 2007–2016 that investigated musicians’ MSS. Narrative reviews, case reports, protocols, and questionnaire or program development papers were excluded. Data were synthesized descriptively in order to identify gaps in the current evidence base. Five systematic reviews and 153 primary studies (133 unique) were included in this review. The majority (71%) of studies investigated professional musicians and/or university music students, with orchestral musicians being the most commonly investigated group. The majority of studies investigated the extent of the problem (68%) and/or associated factors (54%). Eight studies compared the prevalence of MSS outcomes with other populations. A range of risk factors were investigated; however, few studies used longitudinal designs. A total of 16 intervention studies were identified (3 clinical, 13 public health), with 12 investigating education or exercise programs. There is a need for research into musicians beyond classical university music students and professional orchestral musicians, and these musical sub-groups should be compared to determine the most at risk groups of musicians. Studies looking at potential risk factors should move towards longitudinal designs so that the temporal relationship of these factors and MSS could be established, where cross-sectional designs have indicated that an association exists. Intervention studies should be based upon the risk factors identified, and extend beyond education and exercise programs. Int J Occup Med Environ Health. 2019;32(3):291–331

Objectives: Low back pain in pilots of military helicopters is widely discussed in the world literature. Vibrations and improper seat configuration cause that cervical, thoracic and lumbar spine might be exposed to overloading. The aim of the study was to determine the incidence and intensity of pain in the spine as well as to identify subjective risk factors of back pain and its effect on the pilot’s actions while flying a helicopter. Materials and Methods: 112 pilots, aged 25−56 years (mean age: 34.8±6.3 years), actively flying helicopters, participated in the questionnaire survey. The questionnaire containing five groups of questions, elaborated by the authors, was used. Results: 70% of the pilots reported pain complaints. Pain was localized in different parts of the spine. Uncomfortable body posture during prolonged flights resulting from the lack of lumbar support exerted the highest effect on pain provocation. Conclusions: This questionnaire survey enabled to determine the prevalence of pain, its type and intensity. Knowledge on low back pain imposes the necessity to eliminate the negative environmental effect on helicopter pilotage. Elimination of risk factors is possible through appropriate physical exercises and adjustment of pilots’ seats. The authors plan to conduct – in the future – detailed annual examinations among pilots with back pain and, based on the results, to elaborate a prophylactic program containing simple forms of isometric and isotonic training involving paravertebral and deep abdominal muscles designed specifically for pilots of military helicopters.

Background Spinal overload among blue-collar workers occurs most frequently in the lumbar spine. Long-lasting spinal overload results in pain syndromes leading to limitations in everyday and professional life. Material and Methods The research included 106 adult males working as farmers for at least 10 years. The control group included 50 male white-collar workers. Four research tools were used: an interview questionnaire, a questionnaire assessing pain intensity and limitations in everyday functioning of study participants, a Numeric Rating Scale (NRS) measuring pain intensity and Oswestry Disability Index. Results The research revealed that as many as 86% of the farmers (91 individuals) experienced low back pain. In the control group only 64% of all the white-collar workers complained of pain in this part of the spine. The farmers defined their low back pain as constant pain or stiffness significantly more often. In addition, they reported hip pain radiating to one or 2 legs and experienced chronic and acute back pain more frequently. The total time in which they experienced this pain was usually 11–20 years. The correlation between pain intensity measured on a numeric scale and the level of disability of farmers was revealed. Conclusions The research revealed that individual farmers complained of low back pain significantly more often. The pain was usually defined as constant pain, chronic pain, acute pain or hip pain radiating to legs. Med Pr 2018;69(4):355–364

ObjectivesHairstylists form an occupational group whose tasks involve repetitive and forced movements of hands and wrists, thus posing a risk of developing carpal tunnel syndrome (CTS). This study assessed the prevalence of and factors associated with CTS symptoms among hairstylists in Gaborone, Botswana.Material and MethodsA cross-sectional study was conducted using a self-administered questionnaire distributed among randomly selected hairstylists. The questionnaire gathered information on demographic characteristics, lifestyle, work-related characteristics and psychosocial factors. The Boston Carpal Tunnel Syndrome Questionnaire was used to determine the severity of CTS symptoms and its functional effects. Data were then analyzed using χ2 and logistic regression models. The level of significance was determined at p < 0.05.ResultsA total of 165 hairstylists took part in the study, with 92 (56%) of the respondents being females. The mean age (M±SD) of the respondents was 35.05±7.54 years with an age range of 22–63 years. Seventy-three (42.2%) hairstylists reported CTS symptoms, with the majority (73%) being females. Out of all the CTS cases, 53 (72.6%) and 16 (21.9%) had mild and moderate symptoms, respectively. Over 80% of the hairstylists did not know about CTS. Among individual factors, CTS symptoms were associated with being female (the odds ratio [OR] of 9.99, and the 95% confidence interval [CI]: 3.64–27.44), increasing age (OR 9.84, 95% CI: 2.74–35.36), the length of employment (OR 3.73, 95% CI: 1.39–9.95), hair washing (OR 2.88, 95% CI: 1.41–5.85), an awkward posture (OR 2.52, 95% CI: 1.03–6.19), and the use of a great muscular effort when performing a task (OR 2.39, 95% CI: 1.01–5.72). Perceived heavy workload and stressful work were also risk factors.ConclusionsThe results suggest a high prevalence of CTS among female hairstylists in Gaborone, and also point out that individual, work-related and psychosocial factors are associated with this syndrome. Future large-scale research is needed to establish the extent of CTS countrywide to influence policy-making. Currently, CTS is not listed amongst occupational health diseases in Botswana.

Objectives To prospectively investigate if the risk of chronic neck/shoulder pain is associated with work stress and job control, and to assess if physical exercise modifies these associations. Material and Methods The study population comprised 29 496 vocationally active women and men in the Norwegian Nord-Trøndelag Health Study (HUNT Study) without chronic pain at baseline in 1984–1986. Chronic neck/shoulder pain was assessed during a follow-up in 1995–1997. A generalized linear model (Poisson regression) was used to calculate adjusted relative risks (RRs). Results Work stress was dosedependently associated with the risk of neck/shoulder pain (p trend < 0.001 in both sexes). The women and men who perceived their work as stressful “almost all the time” had multi-adjusted RRs = 1.27 (95% confidence interval (CI): 1.1–1.47) and 1.71 (95% CI: 1.46–2), respectively, referencing those with no stressful work. Work stress interacted with sex (p < 0.001). Poor job control was not associated with the risk of neck/shoulder pain among the women (RR = 1.04, 95% CI: 0.92–1.19) nor the men (RR = 1.09, 95% CI: 0.95–1.26). Combined analyses showed an inverse dose-dependent association between hours of physical exercise/week and the risk of neck/shoulder pain in the men with no stressful work (p trend = 0.05) and among the men who perceived their work as “rarely stressful” (p trend < 0.02). This effect was not statistically significant among the women or among men with more frequent exposure to work stress. Conclusions Work stress is an independent predictor of chronic neck/shoulder pain and the effect is stronger in men than in women. Physical exercise does not substantially reduce the risk among the persons with frequent exposure to work stress.

Objectives: This study aims to assess muscle fatigue for kitchen workers during various tasks in a kitchen. Material and Methods: This study consists of 2 phases: a) surface electromyography (EMG) to determine muscle fatigue of trapezius and erector spinae muscles bilaterally for 12 healthy male volunteers during 30 min of Indian flatbread (chapati) preparation, b) self-administered questionnaire study after 30 min preparation to determine the impact on performance. Results: Surface EMG results show that the right trapezius muscle had significantly higher fatigue during kneading, rolling and roasting tasks. Similarly, right erector spinae muscle showed significantly higher fatigue during rolling and roasting tasks. Conclusions: Both phases of the study conclude that, even though the dynamic postural adjustment may reduce fatigue during the work schedule; the combination of risk factors like the repetitive task, forceful exertion and prolonged standing during working results in a high risk of developing muscle fatigue. Objectives: The aim of the paper was to prove that job conditions impact the state of the afferent part of the somatosensory system in miners. Materials and Methods: Data analysis of the electrophysiological examination of the syndrome in 148 patients, aged from 28 to 55 years, with a mild, moderate and severe degree of the pain syndrome was performed. The control group included 28 people without any pain symptoms. The method used was that of somatosensory stimulated potential (SSP) with the potentials amplitude and latency main components taken into consideration. Results: It was proven that the true decrease of the somatosensory stimulated potential SSP N22 (p < 0.05) component amplitudes by 41%; N30 component amplitude tend to decrease by 26%. This proves that the true N22 (p < 0.01) component latency increase by 63.8% corresponds to afferent excitation wave conductibility under the pain syndrome of vertebral pathology through sensitivity pathways mainly in the posterior spinal cord columns and then, through the parts of the brain stem, involving the cerebral cortex, which is confirmed by the fact that the P38 and P46 components amplitudes tend to decrease. In addition to this, the proven N10–N13 (p < 0.05), N13–N20 (p < 0.05), N10–N20 (p < 0.05) intervals increases by 43.5–41.8–38.7%, respectively, correspond to the nervous impulse conductibility through the peripheral nervous system structures and allow to reveal the subclinical slowdown of impulse conductibility, which indicates that the conducting system is changed even under a mild pain syndrome. Conclusions: It was found that the data obtained allow for the better understanding of how the neuropathological pain syndrome under vertebral spine pathology is formed.

Background: Low back pain is a common health problem among hospital nurses. However, the prevalence, characteristics, and work-related risk factors of low back pain have not been widely investigated in Taiwan. Materials and Methods: This study used a cross-sectional survey of 217 hospital nurses to gather self-reported information on the prevalence of back pain, demographic and pain characteristics, and work-related risk factors from 178 respondents who indicated a past history of back pain. The association between the characteristics of back pain and work-related risk factors was also examined. Results: The lifetime prevalence of back pain was 82.03%, and the point prevalence of back pain was 43.78%. The mean pain score is 41.67. The number of years at work was significantly associated with the pain score for an individual’s most recent episode of back pain, the extent of bothersomeness of back pain and leg pain, and the extent to which back pain interfered with normal work. Conclusion: Back pain is common among hospital nurses in Taiwan. Years at work are significantly associated with pain severity and disability caused by back pain.

Objectives This paper evaluates the efficacy of using the McKenzie and Vojta methods for patients with low back pain and the use of the DIERS Formetric 4D system as an objective diagnostic tool. Material and Methods The study enrolled 28 patients aged 15–17 years old. The patients were hospitalized at the Department of Orthopedics and Traumatology of the Świętokrzyskie Center for Pediatrics in Kielce with a diagnosis of back pain associated with a discopathy. The patients were rehabilitated according to the McKenzie and Vojta methods. Assessment by means of the DIERS Formetric system had taken place before the first therapy session and on the day that pain was eliminated to evaluate trunk inclination, angle of thoracic kyphosis, angle of lumbar lordosis, lateral deviation, trunk torsion, surface rotation and pelvic obliquity. Pain intensity and change in pain intensity over time were assessed by means of a numerical rating scale. Results Pain intensity was reduced to 0 over 3–12 days. The study participants demonstrated reduction in anterior trunk inclination of the mean value at 1.83°. The angle of thoracic kyphosis was also reduced by 7.95°. The angle of lordosis increased by 7.6°. The lateral spinal curvature was reduced by 8.92 mm. There was a reduction of 4.64° in trunk torsion. Surface rotation was reduced by 1.61° and pelvic obliquity was reduced by 3.78°. Conclusions In discopathic patients, postural parameters comprising trunk inclination, angle of thoracic kyphosis, angle of lumbar lordosis, lateral deviation, trunk torsion, vertebral rotation and pelvic obliquity fail to reach Hartzmann’s physiological reference ranges. A therapeutic intervention based on the Vojta and McKenzie methods may normalize the posture to physiological reference ranges and is effective in the treatment of patients with back pain. The DIERS system is an objective tool for tracing the effects of therapy in patients with back pain. Int J Occup Med Environ Health. 2019;32(1):33–41

Objectives: Musicians represent a very specific professional group, which due to some occupational hazards is exposed to different health problems known as playing-related musculoskeletal disorders (PRMDs). The aim of this study was to assess correlations between the level of physical activity, and the occurrence of musculoskeletal pain among young instrumentalists. Materials and Methods: Total of 225 Polish musical school students were investigated. To assess the study participants' physical activity level, the moderate to vigorous physical activity (MVPA) index was used. To assess pain complaints on the side of the musculoskeletal system, Nordic Musculoskeletal Questionnaire (NMQ) was applied. The NMQ questionnaire was complemented by adding a visual-analog scale (VAS), which also allowed to assess pain intensity on a numerical scale 1-10. Results: The pain localized in neck, shoulders, upper and lower back was reported significantly more often by the participants who did not meet standard criteria for the recommended or minimal physical activity level. Conclusion: Performing the recommended health-oriented physical activity may reduce the prevalence of musculoskeletal pain in young musicians.

Objectives: The paper describes the case of a physical therapist with acute Low Back Pain (LBP) due to patient handling and the efficacy of Kinesio Taping (KT) around the trunk in the treatment of this occupational LBP. Materials and Methods: KT was applied around the trunk for 3 days, for an average of 10 h/day. Kinesio tape was applied with 130–140% stretch to the rectus abdominis, internal oblique, erector spinae, and latissimus dorsi muscles, which are activated in the process of lifting. Results: Following the KT application, the ‘Visual Analog Scale’ and ‘Oswestry Disability Questionnaire scores’ gradually decreased and active trunk range of motion limited by the LBP increased progressively. The physical therapist no longer complained of LBP and was able to handle patients without any pain. Conclusions: Hence, continuous application of KT around the trunk may be a supplementary treatment method for acute LBP in physical therapists and enable continuous patient handling without any loss of work time due to occupational LBP. In addition, KT may also be applicable for the prevention and treatment of occupational LBP in other professions involving lifting heavy objects.

Objectives: The purpose of this study was to investigate the relationship between daily physical activity (PA) level and low back pain (LBP) in young women. Material and Methods: Two hundred forty three female, desk-job workers aged 20–40 voluntarily participated in the study. The participants were assessed by the use of Oswestry Disability Index for measuring LBP disability and by the use of the short version of the International Physical Activity Questionnaire for PA assessment. The 1-way ANOVA test was used for comparing the mean values according to the physical activity level groups. Correlations between the average LBP disability score and all the other variables were obtained using Pearson’s correlation analysis. The level of statistical significance was p < 0.05. Results: Significant differences were found for LBP disability score between the results of 3 different PA groups (p < 0.05) (low, moderate and high PA groups). The correlation between the average LBP disability score and body weight (r = 0.187, p < 0.01), body mass index (r = 0.165, p < 0.01), vigorous MET score (r = 0.247, p < 0.01) and total PA MET score (r = 0.131, p < 0.01) were significant. Conclusions: The main finding of this study is that there is a U-shaped relationship between PA and LBP disability score in young women. A moderate level of daily physical activity and preventing body weight and fat gain should be recommended in young, female desk-job workers in order to prevent and manage low back pain.

Objectives: This study aimed to investigate the prevalence of Low Back Pain (LBP) among female nursing staff and explore the potential risk factors associated with LBP. Methods: An analytical cross-sectional study was conducted on randomly selected female nurses using payroll as a sampling frame in all public hospitals in Sana'a City, Yemen. Data was collected through face-to-face interview using a structured, pre-coded questionnaire that was available in Arabic and English. Weight and height of the nurses were measured using weight and height scales and body mass index was calculated. Multiple logistic regression was used to identify the factors associated with LBP. Results: Out of 696 female nurses selected, 687 (98.7%) responded. The life-time, the 12-month and one-week prevalence rates of LBP among female nurses were 512 (74.5%; 95% CI: 71.1-77.7%), 411 (59.8%; 95% CI: 56.0-63.5%) and 249 (36.2%; 95% CI: 32.6-39.9%), respectively. The prevalence was significantly lower in Indian nurses compared to other nurses. Three out of every 10 nurses with LBP had sick leave because of LBP in the last 12 months. Factors that showed significant association with LBP among nursing staff in the multivariate analysis were age, nationality, menstrual disorders and stress level at work. Conclusion: LBP is common among female nurses in Yemen. The role of menstrual disorders in developing LBP among female nurses seems to be important. Although sharing the same working conditions, Indian nurses were less likely to report LBP, which highlight the importance of cultural differences in willingness to report LBP.

Background Musculoskeletal disorders mainly affecting the spine constitute one of the major social problems. Frequently, they constitute the cause of prolonged absence at work. Working in a fire brigade is specific because of the physical strain, working time under adverse conditions or stress related to rescue operations. Material and Methods The study group comprised 61 firefighters, 60 men (98.36%) and 1 woman (1.64%). The age of examined firefighters ranged 20–56 years old. The average age of the examined persons was 33.77 years old and the average working service in a fire brigade was 9.59 years. The following research instruments were applied as the study methods: own survey, the Numerical Rating Scale, the International Physical Activity Questionnaire (IPAQ) – long version and the Oswestry Disability Index (ODI) questionnaire. The Pearson correlation coefficient at significance level of p < 0.05 was used for determining the dependencies among the variables. Results Knowledge of ergonomic lifting and handling principles concerning heavy loads was reported by 84% of the group, the remaining 16% declared ignorance of such rules. The dependence of pain on the length of service of the examined was insignificant and amounted to 0.281 Pearson’s r. The results of the ODI questionnaire did not significantly change statistically with age. Conclusions The IPAQ confirmed the high level of physical activity of the study population. Even though the study group was small, the results of ODI showed a minor degree of disability. Med Pr 2018;69(4):365–373
