Objectives The specific job demands of the Prison Service (PS) may affect the health of officers. The job demands-resources model (JD-R) model was used to design a survey of the consequences of working subject to particular job demands. The aim was to gain an insight into the relationship between job demands, personal resources, occupational stress and burnout and selected health consequence indicators (such as behaviors associated with the consumption of alcohol, stress symptoms). Material and Methods A total of 1732 PS officers in Poland were surveyed. The following tools were used as part of the survey: the Copenhagen Psychosocial Questionnaire (COPSOQ II), the Multidimensional Inventory for Assessing Coping Responses (COPE), the Alcohol Use Disorders Identification Test (AUDIT) and a form with a respondent’s particulars. Path analysis using partial least squares structural equation modelling (PLS-SEM) was performed. Results The assumed hypotheses were partially confirmed by the results. Out of 4 job demands categories only work pace turned out not to be a significant predictor of burnout and stress. For alcohol related behaviors, stress level was the only significant predictor, both as a direct and indirect effect taking into account job demands. It transpired that support from superiors rather than support from colleagues or self-efficacy was a significant moderator in the emotional demands – stress relationship. Limitations of the study and perspectives for its continuation are also presented herein. Conclusions Based on the obtained results it may be concluded that job demands and support from superiors do have an impact on stress in the PS group. This is also consistent with available reports in literature. At the same time stress is a significant predictor of alcohol related behaviors. Coping through the use of psychoactive substances was not a significant factor in statistical analyses and it has still not been subject to sufficient scientific analysis.

Objectives This study integrates the Self Determination Theory and the Job Demands–Resource model in explaining motivational antecedents of 2 forms of excessive work: work engagement and workaholism. It specifically examines the relationship between job autonomy, situational work motivation, work engagement, and workaholism. Material and Methods The sample comprised 318 full-time employees of an international outsourcing company located in Poland. The mediation analysis was used for testing hypotheses about the mediation of autonomous and controlled motivation in the relationship between job autonomy, work engagement, and workaholism. Results The results have confirmed that autonomous motivation mediates the relationship between job autonomy and work engagement. The assumption about the mediation role of controlled motivation in the relationship between job autonomy and workaholism has not been confirmed; however, external regulation (i.e., controlled motivation) is a significant predictor of workaholism. Conclusions Giving employees more job autonomy might increase their intrinsic and identified regulation and may therefore lead to more energetic, enthusiastic, and dedicated engagement with their jobs. Workaholism may be predicted by external regulation, and work characteristics other than job autonomy may play an important role in enhancing this controlled type of motivation. Background The competencies of medical staff in the public health emergency system and evaluated the effects of system-based professional training were investigated. Material and Methods A competency model for individuals in a public health emergency management system was developed, which contained 33 items with 5 domains. A competency-based intervention was performed. A total of 68 participants from 4 health emergency teams in Xinjiang, China were recruited and randomly divided into 2 groups: the intervention (N = 38) and control groups (N = 30). Participants in the intervention group received competency-based training, while those in the control group received no training. All participants responded to the COVID-19 activities. The competencies of medical staff in the 5 domains were then analyzed in the pre-intervention, post-first training, and post-COVID-19 intervention using a self-designed questionnaire. Results Participants’ competencies were at the middle level at baseline. After the first training, competencies in the 5 domains significantly improved in the intervention group; in the control group, there was a significant increase in professional quality compared in the pre-training. After the response to COVID-19, the mean scores of competencies in the 5 domains significantly increased in both the intervention and control groups compared with those in the post-first training. Psychological resilience scores were higher in the intervention group than in the control group, whereas no significant differences in competencies were found in other domains. Conclusions Competency-based interventions provided practice and showed a positive effect on improving the competencies of medical staff in public health teams. Med Pr. 2023;74(1)

Objectives: A high percentage of professional drivers (PDs) often report feeling fatigue during their work, and falling asleep at the wheel (FAW) is a major contributing factor to the occurrence of near-miss or actual accidents. The aim of this study is to evaluate the prevalence of FAW among Italian PDs and the effect of fatigue on this occurrence (corrected for the main predictive factors already known). Material and Methods: We performed a cross-sectional questionnaire survey. Data from PDs (N = 497) were used for analyses. Logistic regression analyses were performed to assess the association of reported sudden-onset sleep at the wheel with working conditions and general lifestyle factors. Results: Forty-one percent of the interviewees experienced at least 1 episode per month of sudden-onset sleep at the wheel (4.7% per week). Predictive factors of self-reported FAW were: age > 55 years old (odds ratio (OR) = 4.91, confidence interval (CI): 1.79–13.50, p < 0.01), traveling more than 40 thousand miles per year (OR = 1.86, 95% CI: 1.08–3.22, p < 0.05), body mass index ≥ 30 (OR = 2.16, 95% CI: 1.01–4.64, p < 0.05) and Chalder Fatigue Questionnaire score > 22 (OR = 3.93, 95% CI: 1.90–8.14, p < 0.01). Conclusions: There are different work and human factors underlying FAW among PDs. The Chalder Fatigue Questionnaire might be useful in measuring fatigue in this group and in detecting PDs at high risk of experiencing FAW.

System ochrony zdrowia pracujących w Polsce, oparty na świadczeniach realizowanych przez służbę medycyny pracy, obejmuje opieką niemal 12,5 mln pracowników. Rocznie wykonuje się u nich ponad 4,5 mln obowiązkowych badań okresowych. Tak sprawowana opieka profilaktyczna sprowadza się jednak najczęściej tylko do wykonywania badań określonych przepisami Kodeksu pracy. Ich zakres nie jest ukierunkowany na kompleksową ocenę stanu zdrowia pracujących, lecz na ocenę tych układów i narządów człowieka, które są krytyczne dla zagrożeń występujących na stanowisku pracy. Tymczasem dane epidemiologiczne wskazują na wysoką chorobowość z powodu chorób przewlekłych, które wpływają na utrzymywanie aktywności zawodowej (takich jak nadciśnienie tętnicze czy cukrzyca), oraz na znaczny odsetek chorych nieświadomych swojej choroby lub nieuzyskujących odpowiednich efektów terapeutycznych. Na obligatoryjne badania lekarskie zgłaszają się osoby przekonane o swoim zdrowiu, niekorzystające na co dzień z opieki zdrowotnej, dlatego lekarz medycyny pracy ma unikalną możliwość wykrycia zaburzeń w stanie zdrowia pracownika na ich wczesnym etapie. Dzięki temu może zapobiegać rozwojowi powikłań z jednej strony obciążających zdrowie samego pracownika, a z drugiej powodujących dodatkowe obciążenie finansowe dla całego systemu ochrony zdrowia. Konkludując, w artykule wskazano potrzebę zaangażowania służby medycyny pracy w profilaktykę chorób przewlekłych i związaną z tym konieczność wprowadzenia dodatkowych (oprócz pracodawców) źródeł finansowania działań profilaktycznych, umożliwiających wczesne wykrywanie stanów chorobowych, których pacjent nie jest świadomy, lub kontrolę skuteczności leczenia chorób już zdiagnozowanych. Podniesiono także potrzebę zmiany ustanawiania i publikowania zakresu badań oraz wytycznych do orzekania o braku lub istnieniu przeciwwskazań zdrowotnych do pracy z dotychczasowej formy prawnej na rzecz standardów określanych i na bieżąco publikowanych przez instytuty badawcze i towarzystwa naukowe działające w obszarze medycyny pracy. The system of occupational health care in Poland, based on occupational medicine service, takes care of almost 12.5 million employees subjected to over 4.5 million obligatory periodic medical check ups. This form of providing prophylactic care comes down to examinations dictated by legal regulations, whose scope is not oriented towards a comprehensive workers’ health assessment, but to the examination of the systems and organs critical to work-related dangers. Simultaneously, epidemiological data indicate a large number of chronic diseases, which may influence the professional activity, like hypertension or diabetes and a high percentage of patients not aware of their illness. Since patients participating in obligatory examinations usually feel healthy and do not use health care services on a daily basis, an occupational medicine physician has a unique opportunity to detect health disorders at an early stage, which can prevent the development of health complications affecting the condition of the patient, limiting their professional activity, but also causing additional costs of the health care system. The authors have proven the need to involve occupational medicine services in the prevention of chronic diseases and the need to introduce additional sources of financing for procedures enabling early detection of diseases the patient may not be aware of or control of the effectiveness of already diagnosed illnesses. They addressed the need to change the current legal form of establishing and announcing the range of examinations and directives for certifying the lack or presence of health contraindications to work to the specified and updated standards prepared by scientific research institutes and occupational medicine societies.
