As part of the European Public Health project IMCA II validity and practicability of “air pollution” as a respiratory health indicator were analyzed. The definitions of air quality as an indicator proposed by the WHO project ECOEHIS and by IMCA I were compared. The public availability of the necessary data was checked through access to web-based data-bases. Practicability and interpretation of the indicator were discussed with project partners and external experts. Air quality serves as a kind of benchmark for the good health-related environmental policy. In this sense, it is a relevant health indicator. Although air quality is not directly in the responsibility of health policy, its vital importance for the population’s health should not be neglected. In principle, data is available to calculate this IMCA indicator for any chosen area in Europe. The indicator is relevant and informative, but calculation and interpretation need input from local expert knowledge. The European health policy is well advised to take air quality into account. To that end, an interdisciplinary approach is warranted. The proposed definition of air quality as a (respiratory) health indicator is workable, but correct interpretation depends on expert and local knowledge.

Objectives The main aim of this study was to examine the health behavior patterns of soldiers in the Hungarian Defense Forces and to introduce health behavior profiles according to the cluster analysis of lifestyle factors. Material and Methods The soldiers (N = 5475) who underwent health tests in 2011–2015 participated in this cross-sectional study. The factors included in the analysis are the following: age, sex, diseases diagnosed, the body mass index, eating habits, the smoking status, daily physical activity, sporting habits, the presence of psychosomatic symptoms, mental toughness and sleep apnea. The response options for each factor were scored on a linear scale; the minimum number of points available was –47.5 pts and the maximum number was 48.5 pts according to the 24 factors. Finally, the authors created health profiles typical of the pattern with the cluster analysis of the data. Results As a result of the cluster analysis, 16 distinct profiles were found, 10 of which differed significantly (p < 0.05) from each other. The lowest point value achieved was 3.1 pts and the highest was 26.2 pts. The lowest number of points was achieved by the cluster, 1.8% of the sample, with the highest average age (43.5±7.2 years) in which women showed the highest participation (46%). The 2 clusters with the highest numbers of points, 2.9% and 5.5% of the sample, were the 2 groups with the lowest average age (33.7±7.1 years and 34.3±7.9 years). Conclusions The significance of the health profiles obtained during this examination with the Hungarian Defense Forces is that the health promotion intervention opportunities may be determined by clusters, the health behavior factor with which the authors can reach higher health benefits can be chosen and the effectiveness of the interventions carried out can be traced easily. This review includes information about the number of private health insurance and medical subscriptions in Poland, as well as the level of increases in premium rates, and the scale of health promotion and prevention conducted by Polish employers. The aim of this article is to present the current situation on the supplementary private medical market, which is involved in employees’ healthcare, and the challenges that both employers and medical providers with health insurers are now facing, and also to present the reasons why prevention and health promotion are important factors of healthcare under private health insurance. For the purpose of this review, scientific publications devoted to health promotion at the workplace were used, along with statistical data presented in the studies released by the Polish Insurance Association, the Polish Social Security Institution, the World Health Organization and the Organization for Economic Co-operation and Development. The literature search was carried out using the electronic databases of PubMed. Search terms included medical subject headings and free text words. No year of publication restriction was imposed. The conducted analysis shows that the demand for private medical care is rising. Employers are willing to invest in private medical care for their employees, although the scale is much lower among small and medium-sized entrepreneurs due to fiscal burdens. Given the rising demand and an insufficient number of specialists, access to medical services is deteriorating, and the premium rates and costs are rising. More employers are investing in health promotion at the workplace in order to decrease absenteeism and presenteeism, lower the utilization of medical packages, and improve their attractiveness on the market. Although the interest in private medical care is rising, employers should focus on health promotion and prevention at the workplace, and adapt their actions to the employees’ current needs and health issues. Introduction: In an average Polish person aged 35–44, more than 16 teeth have or had been affected by dental caries. Of that number, almost half of the teeth have already been extracted. Oral health behaviours contribute to this civilization disease in 50%. Such poor oral health status limits the ability of the affected people to take many social and professional roles. Objectives: To evaluate current oral health behaviours and their trends among 35–44 year old Polish people during the period of recent 30 years. Material and Methods: The data were obtained from the International Collaborative Studies conducted in 1978 and 1988 at the Nofer Institute of Occupational Medicine, Łódź, Poland, under the auspices of the World Health Organization (WHO) as well as from 3 stages of the study on Nationwide Monitoring of Oral Health Status and Its Conditioning performed in 1998, 2002 and 2010. The researchers evaluated oral health behaviours and oral health condition of 5425 subjects. Results: Despite a noticeable improvement, poor oral health behaviours are observed in 30%–40% of the adults. In the analysed period, the number of people brushing their teeth at least twice a day increased by more than 10% and the number of people using dental floss increased by 38%. Only 60% of the adults visited a dentist at least once a year. Reduced accessibility of state-run, free-of-charge dental care has caused that over 58% of Poles paid for their dental services. Every 3rd person of working age has not visited a dentist for longer than 2.5 half years, primarily due to behavioural and financial reasons. Oral health behaviours of Polish people are among the poorest in Europe. Conclusions: Despite a noticeable improvement of the behaviours, gap between the Poles and citizens of other highly developed countries is around 20 years. A health promotion programme including oral health issues, if implemented in workplaces, might considerably reduce this gap.

Objectives: To assess relationship between physical activity (PA) and perceived work ability amongst teachers from the Upper Silesia, Poland. Material and Methods: The study involved 171 teachers (129 women, 42 men) of primary and secondary schools of the Upper Silesia, Poland. Physical education teachers were excluded from the study. The level of PA was estimated using the International Physical Activity Questionnaire short version, and perceived work ability was estimated using Work Ability Index (WAI). Results: Male teachers had significantly higher levels of vigorous-intensity PA, moderateintensity PA, and total weekly PA than female teachers. The recommendations of the World Health Organization (WHO) met 46% of studied women and 74% of men. Work ability did not differ between male and female teachers. Work ability was related to age, body mass index (BMI), and PA (vigorous-intensity PA, moderate-intensity PA, total weekly PA). The female teachers with excellent or good WAI had significantly higher levels of vigorous-intensity PA, moderate-intensity PA and total weekly PA than female teachers with moderate or poor WAI. Conclusions: The teachers involving in high or moderate intensity PA could improve their work ability. Further studies should focus on relation between physical activity and work ability among teachers of various age and seniority, from both, urban and rural schools. Objectives The aging of the workforce is an issue that calls for concrete measures to promote the health of older workers. This study reports on the results of the European “ProHealth65+” research project interventions conducted in Poland in relation to workplace health promotion for older workers (WHPOW) and the institutions involved in these programs. Material and Methods A three-stage search of peer-reviewed and grey literature on the WHPOW in Poland. Results A total of 59 WHPOW programs were retrieved in Poland in the observation period (2000–2015). Most of these aimed at improving the Qualification and Training or at the Work Climate and attitudes toward older workers. The promotion, organization, and funding of these activities were carried out mainly by supra-national and governmental bodies, enterprises and employers, and educational and trainee institutions. Conclusions Although there is great commitment to the medical surveillance of workers on the part of the Polish occupational health service, our search detected a relatively low number of the WHPOW initiatives. ObjectivesHealth care services in Poland are delivered by public and private providers. The aims of this study were to assess the attitudes towards private and public health care services in Poland and to identify differences between them, in the opinion of physicians.Material and MethodsA questionnaire-based survey was carried out among physicians attending mandatory courses delivered at the School of Public Health, the Centre of Postgraduate Medical Education in Warsaw, Poland. The questionnaire included 29 questions concerning private and public health care services.ResultsCompleted questionnaires were obtained from 502 physicians (67.7% females), aged 42.1±10.8 years, with a response rate of 77.2%. In the opinion of the surveyed doctors, the major advantages of private health care units, in comparison with public ones, are short waiting times for an appointment (88.2%), an efficient on-site service (78.6%) and convenient appointment times (75.7%). The respondents gave high scores to items such as relations with patients (p < 0.001), superiors (p < 0.001) and colleagues (p = 0.03) when working in private, rather than public, institutions. In the opinion of physicians, public health care institutions guarantee better employment conditions (44.4% vs. 13%; p < 0.001) and security (29.1% vs. 11.1%; p < 0.001) than private ones. The respondents did not observe any differences (p > 0.05) between public and private facilities in terms of the involvement of medical staff and infrastructure. There were significant differences (p < 0.001) in the perception of working conditions in public and private health care institutions depending on the medical education level and the place of primary employment. Conclusions: Among physicians in Poland, private medical institutions are perceived as better organized and granting faster as well as more comprehensive access to health care services when compared to public ones. Closing the gaps between working conditions in public and private units could encourage physicians to practice in the public health care sector.

ObjectivesCompanies that understand the collective health risks of their employees, as well as worker productivity as it relates to health risks can provide more effective and necessary interventions. Health Risk Assessments (HRAs) are an important source of information for understanding overall health risks of a company’s employees. However, HRA data tend to represent only a subset of employees, thereby providing employers an incomplete picture of employee health risks. The current study identifies the representativeness of those completing a worksite HRA by selected demographics, health care costs, and health risks in a large US company.Material and MethodsA retrospective analysis of employees in a large US company during 2017–2019, with statistically significant results reported, adjusted for sex, age, and year.ResultsThe percentage of employees completing the HRA increased from 23.9% in 2017 to 28.4% in 2018 to 32.3% in 2019. These employees were more likely women, middle aged, have lower health care costs, remain employed from year to year, and have better health behaviors and biometric scores. If all employees looked like employees completing the HRA, total medical costs would be 17% lower. If all employees looked like employees completing the HRA who had a health perception rating (1–10 [excellent]) of 7–8 (57.4%) or 9–10 (25.2%) vs. 1–6, total medical costs would be 21.9% and 25.6% lower, respectively.ConclusionsA minority of employees completed the worksite HRA. Basing overall employee health risks on the HRA underestimates health risks and can result in a poorly representative health intervention program.

Objectives: This paper is a report on the relationship between a sense of coherence (and its components) and work related patterns of behaviour in the group of nurses. Materials and Method: Data were collected by two questionnaires (Antonovsky’s Orientation Questionnaire and Work Related Patterns of Behaviour and Experience Questionnaire) in the sample of 331 volunteer nurses employed in three Polish general care hospitals. Results: The analysis of r-Pearson’s correlation and the linear multiple regression analysis revealed a strong relationship between the sense of coherence and healthy type of behaviour, and a strong negative relationship with a burnout type of behaviour. Conclusion: The results obtained in this study provide grounds for the conclusion that the sense of coherence correlates with healthy functioning at work in the group of nurses.

Background: The level of health literacy possessed by an individual (functional, communicative, critical) determines their ability to effectively self-manage a chronic disease. The aim of the study was to assess the level and functional, communicative, and critical determinants of health literacy in the group of chronically ill older adults. Material and Methods: The study was conducted on a representative sample of chronically ill people (N = 400) aged ≥65 years, living in Poland. Three levels of health literacy were measured on the basis of the Functional, Communicative and Critical Health Literacy scale (FCCHL), employed in Polish social research for the first time. Results: The conducted research showed that the average health literacy score of the surveyed older adults, measured on the basis of the FCCHL scale, was 2.81±0.71 (M±SD), whereas for the functional subscale of health literacy it was 3.06±0.58 (M±SD), for communicative health literacy it was 2.82±0.86 (M±SD), and for critical health literacy – 2.71±0.92 (M±SD). In terms of functional health literacy, the surveyed seniors most frequently reported problems with reading health-related information, resulting from the use of inadequate font size in information leaflets and brochures. Questions included in the subscale representing communicative health literacy revealed that the majority of the respondents understood the provided information about diseases, nevertheless they were reluctant to share thoughts about their health with other people. Within the framework of critical health literacy, one-third of the respondents did not search for any additional information, trusting that the information obtained was accurate and reliable. Conclusions: The surveyed elderly people rated worse their level of communicative and critical health literacy compared to the functional level. As a consequence of the low level of communicative and critical health literacy of the surveyed older adults, a certain deficit in their self-managing a disease entity is forecast.

Objectives: The aim of this article is to assess the health behaviors of Polish Army soldiers participating in the National Health Programme 2016–2020 in relation to types of armed forces, BMI and selected demographic factors. Material and Methods: Anthropometric and questionnaire data were obtained in a group of 1229 soldiers of the Polish Armed Forces (PAF) from military units from all over Poland. Health behaviors patterns were assessed using the Health Behavior Inventory (HBI), developed by Juczyński. Results: The HBI of PAF soldiers was 79.9±12.68, indicating an average score. Statistically significant differences were found in the HBI values and in the individual categories of health behaviors according to the type of armed forces. The highest scores in each category of health behaviors were obtained by the Air Force (AF) soldiers. The lowest HBI score was obtained by the Land Forces and Territorial Defence Forces soldiers. Soldiers with higher education obtained significantly higher scores in such health categories as proper eating habits, preventive behaviors and health practices, compared to respondents with secondary education. Soldiers residing in cities had statistically significantly higher health behaviors intensities in all categories, compared to village residents. A significantly higher score in all health categories was noted in soldiers with normal body weight compared to those with diagnosed obesity. There was no significant relationship between the age of the respondents and health behaviors. Conclusions: The study found that factors such as type of armed forces, BMI, place of residence and education level were significant for the adoption of health behaviors by PAF. The level of health practices was significantly higher among AF soldiers compared to other types of armed forces. It seems necessary to further disseminate education on pro-health behaviors, especially among soldiers with obesity through participation in organized training and psychodietetic consultations as part of the National Health Programme.

BackgroundEpidemiological studies show that an inappropriate healthy lifestyle is a major incidence factor, inter alia, for cardiovascular diseases, cancer, diabetes, obesity, as well as premature deaths, especially among men.Material and MethodsIn order to check the attitudes of men towards health and health behaviors, a questionnaire-based research was carried out among 600 men active on the labor market. Several standard questionnaire tools were used: the Positive Health Behaviors Scale (PHBS), the List of Personal Values for measuring the place of health in the value hierarchy; the Multidimensional Health Locus of Control Scale, the Work Ability Index, the Psychological Sex Inventory, and the Work–Life Balance Subscale of the Copenhagen Psychosocial Questionnaire (COPSOQ II). Additionally, an independent questionnaire was developed.ResultsIn the PHBS, men could score 0–111 pts; the average score was 70.98 pts. A high level of care for health expressed in the scores ranging 80–111 pts was achieved by less than one-third of the respondents. One of the 4 groups (referred to as the “Active”) achieved the best result according to PHBS, with an average score of 77 pts. The worst group (referred to as the “Frustrated”) achieved an average of 54.5 pts. The latter performed physical or mixed work, and half of them worked shifts, including nights.ConclusionsThe selected 4 groups were not found to differ from one another as much as the authors had expected, but they pointed to a very important aspect determining health care, namely socio‑‑economic factors. There is a great need to conduct health education among men in Poland, targeted especially at young, low-educated and blue-collar workers. Since occupation is a major determinant of health, traffic police personnel face multiple occupational hazards. They are continuously exposed to vehicular emissions and work in a noisy and polluted environment. The objective of the present review is to explore the impact of occupational health hazards on the health of traffic police personnel. Published research papers on traffic police reporting occupational health issues were accessed and reviewed. Attempts were made to access papers that reported negative associations in order to present a balanced review. The majority of the studies have reported a decrease in the lung function and increased respiratory morbidity. The research on the cytogenetic abnormalities or genotoxic effect of vehicular emissions arising due to long-term exposure to benzene and other polyaromatic hydrocarbons has provided conflicting results, since more or less equal numbers of studies have given evidence for and against the causal association. There is a vast accumulation of epidemiological evidence on the casual association between vehicular pollution and its carcinogenic effect. Multiple studies have concluded that traffic police are highly stressed. A number of occupational factors have been attributed to stress among traffic police. Occupational health studies help us to understand the effects of vehicular pollution and its adverse influence on workers. They also provide opportunity for defined exposures measurements and precise risk assessment. The findings from these studies are easily generalizable and can help us understand the impact of air pollution on the general population.

Objectives In the contemporary Western culture, working population health issues, stressed in various publications, have been perceived as a crucial part of public health. To deal with occupational health issues properly, occupational health services (OHS), aiming at the protection of the workers’ health, are organized in many countries. The survey was to find the differences in occupational health systems that the European Economic Area countries use in order to execute prophylactic activities focused on diseases of affluence and how the OHS are used for health promotion actions. Material and Methods The survey was conducted with the use of an on-line expert questionnaire. No deep statistical analysis was performed as for the designed aims of the study simple statistics were sufficient. Results All the reviewed countries have organized OHS. The funding mechanisms are based mainly on the financial involvement of employers. In the majority of countries, the main goal of the OHS is certification of the ability to work. Workplace surveillance aiming at the occupational risk assessment and health promotion activities that focus on work-related and/or occupational diseases are the most popular services provided within national systems. Conclusions Occupational health professionals are well placed to perform actions directed at health promotion and prophylactics of a wide range of diseases – not only occupational/work-related, but also non-communicable, particularly life-style-related diseases of affluence. The engagement of the occupational medical services (OMS) in prevention of the non-work-related health issues would be valuable. Work for the Prison Service: selected health consequences – investigating the role of personal resources, job demands, work stress, and burnout

Objectives The specific job demands of the Prison Service (PS) may affect the health of officers. The job demands-resources model (JD-R) model was used to design a survey of the consequences of working subject to particular job demands. The aim was to gain an insight into the relationship between job demands, personal resources, occupational stress and burnout and selected health consequence indicators (such as behaviors associated with the consumption of alcohol, stress symptoms). Material and Methods A total of 1732 PS officers in Poland were surveyed. The following tools were used as part of the survey: the Copenhagen Psychosocial Questionnaire (COPSOQ II), the Multidimensional Inventory for Assessing Coping Responses (COPE), the Alcohol Use Disorders Identification Test (AUDIT) and a form with a respondent’s particulars. Path analysis using partial least squares structural equation modelling (PLS-SEM) was performed. Results The assumed hypotheses were partially confirmed by the results. Out of 4 job demands categories only work pace turned out not to be a significant predictor of burnout and stress. For alcohol related behaviors, stress level was the only significant predictor, both as a direct and indirect effect taking into account job demands. It transpired that support from superiors rather than support from colleagues or self-efficacy was a significant moderator in the emotional demands – stress relationship. Limitations of the study and perspectives for its continuation are also presented herein. Conclusions Based on the obtained results it may be concluded that job demands and support from superiors do have an impact on stress in the PS group. This is also consistent with available reports in literature. At the same time stress is a significant predictor of alcohol related behaviors. Coping through the use of psychoactive substances was not a significant factor in statistical analyses and it has still not been subject to sufficient scientific analysis.
