Opis:
Schwann cell tumours are a rare group of benign neoplasms that origin form peripheral nerves. 0.1%–1.5% develops in the larynx causing hoarseness, difficulty swallowing and dyspnoea. The treatment of choice is surgical resection of the tumour during open type surgery or endoscopic procedure. The chosen surgical technique depends on tumour location, size observed in clinical examination and imaging. Also, clinical manifestation of the disease is included in the decision-making process.

Introduction: Ischemic heart disease is the most common cause of death in the world. The lives of patients with vascular defects can be saved by coronary artery bypass grafting (CABG). However, it is associated with an increased risk of developing depression after surgery. Meterial and Methods: The aim of the study is to present the results of the latest research on postoperative depression after CABG, including studies describing the course of the disease, its consequences for the patient’s prognosis and treatment. The publications available on the PubMed platform published after 2011 were reviewed. Results: Depression before and after CABG affects 30–40% of patients, mostly women. Established after surgery and untreated, it persists for many years. The level of anxiety in patients decreases systematically after surgery. Indicators that may correlate with the patient’s postoperative depression, including cortisol, high sensitivity C-reactive protein (hsCRP) and oxidative stress biomarkers, are being investigated. The occurrence of depression in patients after CABG has a number of negative consequences. Those include: weaker response to treatment, greater chance of relapse, and increased readmission frequency and mortality. Treatment of patients with this disorder involves the use of antidepressants (most often SSRIs – selective serotonin reuptake inhibitor) and/or various types of psychotherapy with cognitive behavioral therapy (CBT) at the forefront. Conclusions: Depression following CABG decreases the quality of life and worsens patient prognosis. It is necessary to detect this condition early after surgery and to apply treatment, taking into account the cardiological disorders of the patient.

Introduction: Proliferative-inflammatory pathologies may occupy the temporal bone, resulting in: hearing loss, vestibular dysfunction, and neuropathies from cranial nerve compression. Although their occurrence is episodic, the appropriate diagnostic procedure is extremely important to achieve expected therapeutic effect. Aim: The aim of study was characterization of selected proliferative-inflammatory pathologies that may occupy the temporal bone: fibrous dysplasia, inflammatory pseudotumor, osteoradionecrosis, and presentation of diagnostic methods for the differentiation of these diseases as well as discussion on appropriate therapeutic options. Fibrous dysplasia (FD) is a slowly progressive, benign bone disorder of unknown etiology characterized by abnormal proliferation of fibrous tissue. Inflammatory pseudotumor (IPT) is a rare, non-malignant inflammatory process of unknown etiology, characterized by connective tissue proliferation and infiltration of inflammatory cells. Osteoradionecrosis of the temporal bone (TB-ORN) is a rare but potentially fatal complication of radiotherapy for head and neck cancer. Results: Due to the similarity of symptoms with typical inflammatory conditions of middle ear (pain, otorrhea, hearing loss), selected disorders may be a dilemma regarding the diagnosis and proper further treatment. The clinical examination is mandatory, however radiological imaging may demonstrate the presence of specific changes and direct the diagnosis. The computed tomography (CT) of fibrous dysplasia shows the abnormal organization of the bone structure. Magnetic resonance (MRI), as the most sensitive for inflammatory pseudotumors, visualizes inflammatory infiltration in soft tissues. The CT of temporal bone identifies the erosion in the course of osteoradionecrosis. However in all cases the final diagnosis may be establish using histopathological examination and after exclusion of the neoplastic process.

Opis:
Objective: Recurrent respiratory papillomatosis (RRP) is caused by types 6 and 11 human papilloma virus and occurs in both children and adults. It is characterized by proliferation of benign squamous cell papillomas within the aerodigestive tract. The problem with recurrent respiratory papillomatosis treatment is high recurrence of papilloma growth following surgical removal. Method: A literature review was carried out via surveys based on electronic data in public domains: MedLine (USA National Library of Medicine), PubMed and SciELO, using keywords such as: recurrent respiratory papillomatosis, adjuvant treatment, cidofovir, and bevacizumab. All types of papers written in English were included (cross-sectional, prospective and retrospective clinical trials, review papers, and case reports). Results: In the recent literature, several types of treatment such as surgery with mechanical debulking or laser and adjuvant therapies are mentioned. Intralesional bevacizumab and cidofovir treatment may increase the interval between surgical procedures and decrease the number of procedures per year. Conclusions: There is still an ongoing discussion within the European Laryngological Society regarding the balance between effectiveness and side effects of RRP adjuvant treatment, however, recent results show promising long-term effects. Bevacizumab and cidofovir in aggressive RRP give hope for improved treatment outcomes.

Solitary fibrous tumour (SFT) is a rare soft tissue tumour which belongs to fibroblastic/myofibroblastic group of tumours. The most often it appears in pleura, also in pericardium, internal organs, peritoneum and extraperitoenal space. SFT was first described as a new type of pleura’s tumour by Klemperer and Rabin in 1931. The histogenesis of SFT’s has been discussed for years suggesting its mesothelial origin. Recently, SFT has been classified as a mesenchymal fibroblastic tumour. We report a very rare case of 71-year old man suffering from gigantic solitary fibrous tumour of extraperitoneal space who underwent curative surgery in the Department of General, Gastroenterological and Oncologic Surgery in 2011

Opis:
Ligation of intersphincteric fistula tract in treatment of anal fistulas (LIFT) is being said to have satisfactory results in short and long follow up, with low risk of complications. This study was designed to evaluate the results in patients with complex and recurrent fistulas in comparison with simple transsphincteric anal fistulas. The aim of the study was to present a single-center experience in LIFT procedure in treatment of both simple and complex anal fistulas, including recurrent fistulas, in comparison with a review of current literature. Material and methods. A series of 17 patients were qualified to LIFT procedure. 5 patients were treated for simple transsphincteric, 6 for complex fistulas, 6 with fistulas recurrent after fistulotomy. Median age was 47, most of the patients were male (16/17). Mean follow up was 11 months. Results. Mean operating time was 55 minutes counting from surgical site disinfection to final dressing of the wound. Of the 17 patients the overall success rate was 53%. As expected, best results were achieved in patients with simple fistulas (80% success rate), then complex (50%), and recurrent fistulas (only 33%). There were no early nor late complications of the surgery. Conclusion. As expected, in simple transsphincteric fistulas the results were satisfactory, taking into account low complication rate. Complex and recurrent fistulas seem to be risk factors of LIFT failure. The results are consistent with data published by other authors, based on the review of the current literature, and it seems there is still room for improvement, so further research is required.

Mucosal melanoma constitutes 1,3–1,4% of all melanomas. 25–50% of these cases occurs in head and neck region. Primary melanoma of the head and neck mucous membranes is most often diagnosed in the nasal cavity (50%), maxillofacial region (20%), oral cavity (17%), and then nasopharynx, casuistic in the larynx. Recurrent nasal bleeding and persistent nasal obstruction are the most frequently presented symptoms. In comparison to skin melanoma, mucosal melanoma is diagnosed 1 decade later and has different etiology. It is usually diagnosed at an advanced stage when metastases are already present. Approximately 20% of patients have metastases to the lymph nodes at diagnosis and 10% distant metastases. Histopathological diagnosis is based on the detection of malanocyte markers: S-100, Melan-A, HMB-4, MITF-1 and vimentin. Mucosal melanoma can be misdiagnosed as olfactory neuroblastoma. In ⅓ of the cases, melanoma of mucous membranes occurs in the amelanotic form. Treatment of choice is radical resection and adjuvant radiotherapy. In the head and neck region radical disscection may be a challenge for operating surgeon because of closeness of anatomical structures and preservation of function. In advanced stadium immunomodulatory treatment can be applied. Overall 5-year survival is estimated for 12–44%. Based on presented case authors note the necessity of inclusion additional factors in geriatric oncology and generally discuss the diagnosis and treatment of mucosal melanoma.
Czerniak błony śluzowej stanowi 1,3–1,4% wszystkich czerniaków. 25–50% z nich występuje w regionie głowy i szyi. Pierwotny czerniak błony śluzowej głowy i szyi najczęściej rozpoznawany jest w: jamie nosowej (50%), masywie szczękowo-twarzowym (20%), jamie ustnej (17%), następnie w nosogardle oraz kazuistycznie w krtani. Zazwyczaj prezentuje się następujące jego objawy: nawracające krwawienie z nosa i utrzymujące się upośledzenie drożności. W porównaniu do czerniaka skóry, czerniak błony śluzowej występuje o dekadę później; ma też inną etiologię. Zwykle rozpoznaje się go w zaawansowanym stadium, gdy obecne są przerzuty. U około 20% pacjentów w momencie rozpoznania stwierdza się przerzuty do węzłów chłonnych, a u 10% – odległe przerzuty. Rozpoznanie histopatologiczne opiera się na wykryciu markerów malanocytów: S-100, Melan- A, HMB- 45, MITF-1 i wimentyny. W ⅓ przypadków czerniak błony śluzowej występuje w formie amelanotycznej. Może być on mylony z nerwiakiem węchowym zarodkowym. Leczeniem z wyboru jest radykalna resekcja z następową radioterapią. Z uwagi na bliskość anatomiczną ważnych struktur, usunięcie guza zlokalizowanego w obrębie głowy i szyi w całości z zachowaniem funkcji czynnościowej stanowi wyzwanie dla operatora. W zaawansowanym stadium stosuje się leki immunomodulujące. Ogólne 5-letnie przeżycie kształtuje się na poziomie 12–44%. Na podstawie studium przypadku, autorki zwracają uwagę na konieczność uwzględnienia dodatkowych czynników w onkologii geriatrycznej oraz ogólnie omawiają diagnostykę i leczenie czerniaka błony śluzowej.

Introduction: Inflammatory myofibroblastic tumor (IMT) is a rare condition that can mimic potentially more dangerous states such as malignant tumors. The tumor itself can also show local malignancy as well as malignant transformation. The paranasal sinus IMT is quite a rare case in the literature. The manifestation of the disease can include face swelling, nasal obstruction, epistaxis, vision acuity worsening, numbness of the face, pain. Etiology of this type of lesion still remains uncertain but there are a few assumptions on the issue: viral and genetic, as well as posttraumatic and postinflammatory. We report a case of an adult woman with IMT detected in the right maxillary sinus after endoscopic sinus surgery. Wstęp: Zapalny guz miofibroblastyczny (IMT) jest rzadkim stanem, który może naśladować potencjalnie bardziej niebezpieczne choroby, takie jak nowotwory złośliwe. Sam guz może również wykazywać miejscową złośliwość, a także złośliwą transformację. IMT zatok przynosowych jest dość rzadko opisywany w literaturze. Objawem choroby może być obrzęk twarzy, niedrożność nosa, krwawienie z nosa, pogorszenie ostrości wzroku, drętwienie twarzy, ból. Etiologia tego typu zmian nadal pozostaje niejasna, ale rozważa się następujące przyczyny powstawania guza: wirusową, genetyczną, a także pourazową i pozapalną. Niniejszy opis przypadku dotyczy dorosłej kobiety z IMT wykrytym w prawej zatoce szczękowej po endoskopowej operacji zatok. Introduction: Fibroadenomas are one of the most common benign tumors of the breast in the adolescent females accounting for about 2/3rd of all the breast lumps and more than half of all the biopsied breast lesions. They come into being due to overgrowth of glandular tissue under the influence of hormonal changes that the girls undergo at the time of puberty. Due to the wide prevalence of fibroadenomas and the psychosocial morbidity associated with the finding of a breast mass, it is imperative for physicians treating adolescent patients to be thoroughly familiar and updated with this disease. Aim: The article aims at providing a brief review of the classification, presentation, diagnosis, and update on the management of breast fibroadenomas on the basis of recent literature.

This paper presents a case of a patient with invasive ductal breast cancer following breast augmentation. Following breast implants rupture in March 2013 the breast implants have been removed – histopathological examination revealed leaked silicone with inflammatory infiltration, without evidence of cancerous lesions. Diagnostic imaging revealed multiple encapsulated silicone particles and clusters of post-inflammatory macrocalcifications in both breasts. In January 2014 the patient presented with symptoms of massive inflammation of the left breast. Following surgical consultation the patient had undergone radical left-sided mastectomy with lymphadenectomy. Postoperative histopathological examination revealed a multifocal advanced invasive ductal cancer G3 pT3pN3a (vascular invasion, metastases in 11 of 12 examined axillary lymph nodes). Following surgery the patient was qualified for further treatment – chemotherapy, radiotherapy, hormone therapy. The discussion includes a review of literature on the risk evaluation of co-occurrence of breast cancers in women with silicone breast implants and presents diagnostic challenges of breast cancer in this patient group.

Neuroendocrine carcinoma is a rare neoplasm, and its most common location in the head and neck area is the larynx, especially the epiglottis and the supraglottic region. The first case of neuroendocrine carcinoma of the larynx was reported in 1969 by Goldman et al. Expression of the two crucial markers, synaptophysin and chromogranin, along with neural cell adhesion molecule (CD56) should be mentioned among the neuroendocrine features detected on immunohistochemistry. Human papilloma virus (HPV16/18) infection in the laryngeal neuroendocrine carcinoma can be one of the causal factors, and the detection of HPV should be considered as a standard procedure for the diagnosis and treatment.

Involuntary falling asleep at the wheel is the cause of many road accidents and is an important social problem. For this reason, it seems important to explore the physiological processes associated with human falling asleep. Particularly interesting are those physiological parameters, which changes accompany the transition from the state of full consciousness to sleep. Monitoring them could be used eg. in determining the level of driver drowsiness and therefore assesment of the risk of falling asleep at the wheel. In this article we present a summary of knowledge on changes in selected physiological parameters that occur on the border of allertness/ sleep and deep sleep. Significant changes are particularly evident in the incidence of brain waves in the EEG. In addition, decrease in the activity of the autonomic nervous system leads to a decrease in heart rate and blood pressure. Also respiratory rate decreases and tidal volume increases. Spectral analysis of heart rate variability (HRV) shows decrease in the amplitude of the power spectral density function in the low frequency range. Also transition of eye movement from saccade to slow wave occurs during sleep which correlates with EEG changes. In addition, eyelids vibration disappear. Electromiography (EMG) shows gradual muscle tension fall. Warming in distal parts of limbs is also observed when falling asleep. All these parameters tend to be useful for evaluating the fatigue of the driver. In this article, we also try to determine which of them are suitable for drivers drowsiness monitoring

Opis:
An enteric fistula that occurs in an open abdomen is called an enteroatmospheric fistula (EAF) and is the most challenging complication for a surgical team to deal with. The treatment of EAF requires a multidisciplinary approach. First of all, sepsis has to be managed. Any fluid, electrolyte and metabolic disorders need to be corrected. Oral intake must be stopped and total parenteral nutrition introduced. The control and drainage of the effluent from the fistula is a separate issue. Since there are no fixed algorithms for the treatment of EAF, surgeons need to develop their own, often highly unconventional solutions. We present the case of a 24-year-old man who developed enteroatmospheric fistula after laparotomy and relaparotomy due to acute necrotic pancreatitis. Both the laparostomy and the fistula were successfully managed using modified negative pressure wound therapy. The literature regarding this issue was also reviewed.

Cervical branchial cleft cysts are relatively common tumors of the neck that should be distinguished from an epidermoid cyst, hygroma, hemangioma, lymphangioma, lymphadenitis, and metastatic papillary carcinoma of the thyroid gland. Infected cysts might be misdiagnosed as a recurrent abscess. The aim of the study was to present current views concerning diagnosis and treatment of cervical branchial cleft cysts. Material and methods. Data and histopathological results obtained from 49 patients (18 women and 31 men) admitted to the Department of Cranio-Maxillofacial Surgery, due to lateral cervical cysts during the period between 2005 and 2009 were subject to retrospective analysis. Results. Most patients were in their third decade of life. The clinical examination showed a painless, slowly growing tumor on the lateral surface of the neck, more often on the right side, and in 30 cases with a concomitant infection. Initial diagnosis on the basis of the clinical examination, radiology and biopsy was confirmed in 48/49 cases (98%). All patients were subject to surgical treatment. During the 3 to 7 year follow-up period recurrence was not observed. Conclusions. Initial diagnosis of a cervical branchial cleft cyst on the basis of the clinical examination should always be confirmed by means of ultrasonography. In case of suspicion of a coexisting infection, fine-needle aspiration biopsy under ultrasound control is recommended. If there is concern that the lateral neck lesion is not a branchial cyst or its dimension is large, computed tomography of the neck or magnetic resonance should be performed. Complete excision of the tumor under general anesthesia is the treatment of choice, being associated with the low risk of local postoperative complications
