Opis:
Regenerative medicine is focusing on searching for stem cells, which can be efficiently and safely used for regeneration of damaged tissues and organs. Pluripotent stem cells would be ideal for this purpose. It is because they have the ability to differentiate into cells of all three germ layers (ecto-, meso- and endoderm). One of the sources of their isolation are embryos. For many years, they are made unsuccessful attempts to use of very controversial embryonic stem cells that are isolated from embryos. So strong ethical controversy forced scientists to look for other, undoubted ethically, sources of pluripotent stem cells. Induced pluripotent stem cells are proposed, as a more promising alternative to cells isolated from embryos. Unfortunately, both embryonic stem cells and induced pluripotent stem cells tend to genetic instability leading to the formation of teratomas. In parallel studies scientists try to use of stem cells isolated from adult tissues (e.g. bone marrow cells or adipose tissue) in the regeneration of parenchymal organs. Unfortunately, there is no convincing evidence for most of these cells that can regenerate damaged parenchymal organs. Regenerative medicine more frequently is employed in the otorhinolaryngological therapies. More and more researchers’ efforts are put into the development of an effective method of stimulation (in vitro) of pluripotent stem cells isolated from adult tissue for differentiation of the renewable progenitor stem cells which can keep their potential after transplantation into the recipient (e.g. in the treatment of imbalances or hearing loss). Moreover, there are promising methods for employing of the stem cells potential in tissue engineering as they are more effectively introduced as a clinical therapies.

Opis:
Treatment of perianal fistulizing Crohn’s disease is demanding and burdened with a high percentage of failures, which forces clinicians to search for new, more effective therapeutic options. One of these options is the use of adipose-derived mesenchymal stem cells in local administration. Due to their multipotentiality and complex mechanism of action, stem cells are the promising new therapeutic approach for the treatment-refractory complex perianal fistulas – demonstrating both high efficacy and a favorable safety profile. The paper presents current knowledge on the mechanisms of action and manner of administration of mesenchymal stem cells, as well as the effectiveness and safety of their use in the treatment of perianal Crohn’s disease based on available literature.

Background: Colorectal cancer (CRC) is one of the most common malignancies in the world. The cancer stem cell (CSC) markers are associated with aggressive cancer types and poor prognosis. The objective of the study was to evaluate the CD133 expression and to correlate it with clinicopathological features in patients with CRC. Material and Methods: Our study included ninety patients with CRC who underwent curative surgical resection from 2012 to 2017 at the University Clinic for Digestive Surgery, Skopje, North Macedonia. Tumor samples were first analyzed with standard histopathological methods and then the CD133 expression was investigated immunohistochemically. The level of expression of CD133 was classified semiquantitatively. Low positivity was defined as positive immunoreactivity in <50% of tumor glands, and high positivity was defined as positive immunoreactivity in ≥50% of tumor glands. Furthermore, clinicopathological features of patients were retrospectively reviewed. Results: High expression of CD133 was found in 47.8% of patients’ CRC samples. In 69.6% of patients with metastatic lesions in visceral organs we found high expression of CD133. We found statistically significant differences in the expression of CD133 between patients with and without visceral metastatic lesions (P = 0.0153), between patients with a different T category (P = 0.0119), N status (P = 0.0066) and grade (G) (P = 0.0115). Our results showed that the stage of disease has the greatest impact on expression of CD133 (P < 0.00001). Conclusion: High expression of CD133 is a useful marker for prediction of the clinically aggressive type of CRC and can be routinely implemented in standard pathohistological diagnostics.

Perirectal fistulas in the course of Crohn’s disease (CD) constitute an important problem in this group of patients. They are observed in a vast majority of patients with involvement through colorectal inflammation. Perirectal fistulas in CD present a great diagnostic and therapeutic challenge due to the intensified clinical symptoms and worse prognosis than in the case of crypt originating fistulas. The condition for implementation of effective treatment of perirectal fistulas in the course of CD is the correct diagnosis, defining the anatomy of fistulas, presence of potential stenoses and inflammation in the gastrointestinal tract. Treatment of these fistulas is difficult and requires close cooperation between the colorectal surgeon and the gastroenterologist. The combination of surgical and pharmacological treatment has higher efficacy compared to surgical treatment or pharmacotherapy alone. In conservative treatment, aminosalicylates and steroids are of minor importance, while chemotherapeutics, antibiotics, and thiopurines find application in daily clinical practice. TNF-α neutralizing antibodies such as infliximab (IFX), adalimumab (ADA) or certolizumab (CER) prove to be the most effective. Surgical treatment may be provided as ad hoc; in this case, drainage procedures are recommended, usually with leaving a loose seton. Planned procedures consist in the excision of fistulas (simple fistulas) or performing more complex procedures, such as advancement flaps or ligation of the intersphincteric fistula tract Surgical measures can be complemented by the use of video technology (video-assisted anal fistula treatment VAAFT) or vacuum therapy. In extreme cases, it may be necessary to create the stoma. Treatment of perirectal fistulas includes adhesives or so-called plugs. High hopes may be associated with the introduction of stem cells into clinical practice, which is the administration of non-hematopoietic multipotent cells to the fistulas to induce the phenomenon of immunomodulation and tissue healing.
