Opis:: This paper analyses the causes of fires on board merchant ships and fishing vessels. The study attempts to identify the causes of fire occurrence on board ships and to determine their interrelations using the relations diagram, also known as the interrelationship diagram or digraph. This diagram identifies relations not only between causes and effects but also between two or more causes. Elements of the diagram, which have the greatest number of directed connections from/to, are the starting points for further ship safety analysis. The relations diagram is used as a basis for planning corrective measures and actions directly increasing shipping safety at sea. Knowing the various causes of fire occurrence, it is possible to eliminate or to reduce their number in the future by revising and drawing up relevant maritime transport regulations. This, in turn, can enhance shipping safety.

Opis:: This article aims to compile, describe and compare three different models taken from the literature describing the causes of explosions in the crankcases of marine engines. Each of the models has a different level of detail and was prepared with a different purpose. However, the same process, explosions in crankcases, was analyzed in all cases. A statistical evaluation of the frequency of events leading to explosions, a model built using failure mode and effects analysis (FMEA) and a model based on fault tree analysis (FTA) are described in turn. The FTA model drawn from the literature formed the basis for further analysis. Values of important measures of all elementary events of the fault tree were calculated using the Birnbaum reliability measure, Vesely-Fussell measure, Birnbaum structural measure, criticality measure and improvement potential. The percentage importance values of all events determined using these importance measures were compared. The results obtained from the application of each model were evaluated. The results of the models were compared with each other, and an approach using all three models supplemented with diversion analysis was proposed.

Opis:: This paper presents the process of quantitative assessment of selected factors influencing the probability of grounding. All grounding accidents that occurred during one year (2013) in the Baltic Sea area were analysed. The research area was chosen because of very high traffic intensity and a large number of narrow passages that make it difficult for navigation. The research period was limited by the latest data reported by HELCOM up to the year 2013. The Method of Grounding Probability Coefficient (GPC) calculation is proposed to reach the assumed goal. Either internal factors such as the ships size and ship type or external factors such as time of day, wind force and season were taken into account. As a result the values of coefficients describing the impact of various factors on the probability are evaluated. This coefficients, taking into consideration coupling effect, can be used in future in the navigational safety simulation models based on the assumed value of accident probability.

The article presents some problems with O-ring type seals that are in common use in marine machines. Typical failures of elastomers and their causes are discussed. Attention is drawn to correct assembly of o-ring seals in static and movable joints.

The author takes a stance on the role of reliability of port cargo-handling facilities. An analysis has been performed of accidents during cargo-handling operations in the port of Szczecin in a five-year period, with particular consideration of accidents where cargo-handling facilities were involved. An example of the port of Szczecin is given in determining the share of accidents caused by failure of cargo-handling facilities.
