Opis:
Prevalence of multifocal fungal infections in patients undergoing permanent immunosuppresion alter renal transplantation. The aim of the study was to estimate the prevalence of fungi in monofocal and multifocal infections in renal transplant recipients. 32 renal transplant recipients were examined for presence of fungi in genital organs, oral cavity and rectum. Fungal strains were recovered in 66 out of 96 samples (68.2% of all renal transplant recipients) in oral cavity (65.6%), rectum (37.5%) and genital organs (25.0%). Monofocal mycosis was found only in 21.9% of the patients. Multifocal infections occurred in 68.2% and contained ontocenoses of oral cavity and rectum (34.4%) the most frequently. Trifocal infection (genital organs - oral cavity - rectum) occurred in 12.5% of all examined renal transplant recipients. The following fungi were found: Candida albicans (31 strains), C. glabrata (5), C. guilliermondii (2), C. krusei (20, Saccharomyces cerevisiae (1).

In 77 diabetic patients the detection of fungi was carried out in the oral cavity, digestive tract, genital organs and skin lesions. Patient age ranged from 39 to 82 years, with the Body Mass Index (BMI) from 22 to the highest values of 46.5. Diabetes control was assessed by determination of the glucose level in serum after overnight fasting and after lunch, as well as by determination of the НБА | с level. Fungi were diagnosed in 61 patients (79.6%). The total of 111 fungal strains were isolated in 319 cultures evaluated from 12 biological materials. Fungal strains were detected in the highest percentage in oral cavity (77.9%), in the lower percentage in anus (33.8%), vagina discharge (11.6%) and the region of vulva (14.3%). Fungi were observed in one focus — exclusively in the oral cavity — in 28 (36.4%) patients, whereas they occurred in multifocal infections in 33 (42.9%) patients. The isolated fungal strains were classified into 4 genera (Candida, Saccharomyces, Trichosporon, Aspergillus) and 12 species. The most frequently detected fungi were C. albicans (55.2%), followed by C. glabrata (12.4%), C parapsilosis (10.5%) and С. tropicalis (9.5%). Relatively low percentage of fungi identified in vagina was associated, among others, with the fact that most of the female patients with diabetes were at the postmenopausal period. In all the patients in whom fungi were isolated from the vagina contents, these fungi were also found in other ontocenoses. The increase in percentage of fungal strains from genera other than Candida in patients with a higher risk is of clinical importance, as some of them are resistant in vitro to azole derivatives e.g. fluconazole. The digestive tract is thought to be the most frequent fungal habitat. A considerable prevalence of fungi, multifocal infections, particularly related to oral cavity and anus, and occurrence of high percentage of strains different from C. albicans species of Candida genus in female patients with type 2 diabetes indicate the necessity of the special mode of diagnostic and therapeutic management.

The present study deals with the ecology of Lyme borreliosis in Northern Poland. The complexity of environmental conditions are: (1) the presence of certain rodent hosts for the Borrelia, (2) the seasonal patterns of abundance and infection rate of /. ricinus in the population of rodents and on vegetation, (3) the prevalence of B. burgdorferi s.1., B. garinii and B. afzelii. The longterm studies were carried out on a monthly basis from April to October (1998-2001) in woodlands at Urwitałt near Mikołajki on the Mazury Lakes. Two methods of spirochate detection were used: indirect immunofluorescence assay (IFA) and polymerase chain reaction (PCR). The dynamics of ticks infestation, differences in infestation rates between years, months, age and sex for two species of rodents (Clethrionomys glareolus, Apodemus flavicollis) were analysed using the software package Statgraphics version 7 and GLIM 4 (statistical system for generalized linear interactive modeling, Royal Statistical Society 1993). We were able to demonstrate that in selected habitat of Mazury Lakes the spirochaetal agent of Lyme borreliosis occurs quite frequently, both in rodents and in vector — /. ricinus. Comparing C. glareolus and A. flavicollis, each rodent species showed different, seasonal and sex dependent, host infection with Borrelia. Due to the courses of prevalence of infection, both for partially engorged ticks and questing ticks, the risk of human infection seems to be highest in spring. The infection rate of engorged larvae was much higher than in questing larvae. We conclude that both species of rodents, highly abounded in the Mazury Lakes district, may represent significant zoonotic reservoir for B. burgdorferii sl., B. garinii and B. afzelii.

The aim of this study was to detect fungi in three ontocenoses of 947 women (aged 20-45). The prevalence of multifocal fungal infections in women varied from 62.2 + 2.99% to 81.0 + 3.92% in different ontocenoses. They assumed the following mean values: 38.4 + 1.77% for the vagina, 42.2 + 1.80% for the oral cavity, and 25.0 + 1.58% for the anus. The clinical manifestations of sexual organs inflammations were statistically significantly (p < 0.01) more frequent in women infected with fungi than in those without fungi in the vaginal ontocenosis. The highest convergence of the fungi presence with subjective and objective symptoms (assessed using the Yule Q index value) concerned: pruritus of the vulva or/and vaginal walls, burning sensation or painful urethra, as well as oedema of the external opening of the urethra, vulva or/and the vagina (Q from +0.808 to +0.970). However, there was no statistically significant convergence between occurrence of the fungi and vaginal discharge and painful hypogastrium. Based on more than 40 phenotypical features (morphological, biochemical) of the axenic strains originating from vaginal, oral, and anal ontocenoses, a total of 12 species of fungi were identified: Candida albicans, C. guilliermondii, C. krusei, C. tropicalis, C. kefyr, C. utilis, C. humicola, C. viswanathii, C. glabrata, C. parapsilosis, Rhodotorula mucilaginosa, and Geotrichum candidum. For intraspecific features, depending on the profile of the compounds used for auxanogram, 9 codes were read for Candida albicans and only one code for each of other species was detected. In API ZYM test based on assessment of 19 hydrolase activities the enzymograms of species included from 6 to 11 enzymes.
