Lower leg ulcers are characterized by a chronic and recurrent course, being considered as a difficult therapeutic problem. Based on epidemiological data chronic ulcers concern 0.2 to 2% of the Western European population. The above-mentioned problem affects women more often than men, and increases with age. Treatment and recurrence prevention methods remain under discussion and research.

The aim of the study was to examine the utility of dermatoscopy in plastic surgeons’ practice in pigmented and non-pigmented skin lesions management. Material and methods. The examined group consisted of 68 patients with 132 lesions (50 women and 18 men) aged from 12 to 75 years (the mean: 47.2 years ± 16.9 years), who underwent dermatoscopy. Dermatoscopic photographs were analysed according to the ABCD and 7-point scales and then, a further treatment (surgical excision, electro resection or regular follow-up and observations) was planned. Results. The mean score of all lesions according to ABCD scale was 2.34 while in 7-point scale it was 0.62. In male and female groups the number of lesions and their ABCD and 7-point scale scores were similar (p>0.05). Histopathological examination revealed that all excised lesions were benign (compound melanocytic nevi) which corresponded with dermatoscopic evaluation. Conclusions. Dermatoscopy seems to be helpful in surgeons’ dealing with skin lesions practise and in many cases it enables to choose less invasive technique of lesions’ removal (electro resection), which gives better aesthetic results.

Opis:
The great popularity of foreign excursions and travel in exotic directions, as well as increasing popularity of breeding exotic animals at home, is evidence that in daily practice one may observe injuries inflicted by animals atypically occurring in Poland. The study presented and described a rare case of a patient attacked by a camel living in an agro-tourism farm in our country. Thanks to the combination of surgical and conservative treatment complete wound healing was observed, including the skin grafts, with preserved motor function of the foot.

Loop stoma allows reducing the percentage of anastomotic leak and re-operation caused by this complication. Our department has performed the loop stoma on a skin bridge since 2011. The aim of the study was to evaluate the early results of treatment after the skin bridge loop stoma creation in comparison with the stoma made on a plastic rod. Both groups had 20 patients. Material and methods. The study involved 40 patients with ileostomy, operated 2010-2013. We evaluated 20 patients with a loop ileostomy on a plastic rod, compared to 20 other patients with a skin bridge ileostomy. The study included 24 men and 16 women. Median age was 68.3. All evaluated patients were previously operated due to rectal cancer. Results. It has been shown that the surgical site infection is more common in the group with a plastic rod (5 vs 1 patient). Inflammation of the skin around the stoma occurred in 18 patients (90%) in the first group, while no such complication was found in patients in the second group. The average number of exchanged ostomy wafers was 2,9 per week in the first group of patients, and 1,1 in the second group (p 0,05). Conclusions. The creation of the skin bridge stoma allows for tight fit of the ostomy appliance immediately after surgery completion. The equipment has stable and long-lasting contact with the skin, no skin inflammatory changes occur. Also the surgical site infection rates are lower in this group of patients. As perioperative patient does not require an increased number of ostomy appliance, the cost of treatment can be considered as an important aspect.

In this paper the authors would like to present a correct procedure in both surgical and hyperbaric treatment of patients with gas gangrene admitted to a surgical department during ER. Gas gangrene is not very common these days, but when it comes to dealing with gangrenous infection in the emergency it is quite likely to make errors in both diagnostic and therapeutic manners. When there is a gas gangrene in a patient at the emergency time plays crucial role and the proper application of procedures is vital for the patient’s survival. 10 cases made the study group here, all of them were patients diagnosed and treated surgically due to gas gangrene. As shown here, It is important to perform a revision of surgical wounds after few hours since primary surgery and to begin hyperbaric treatment as quickly as possible. The findings and suggestions included in this study are supported by own experience of The 2nd Department of General and Gastrointestinal Surgery of Medical University in Białystok, Poland.

Opis:
Pilonidal cyst disease mainly concerns young people, aged between 16 and 25 years. The disease causes significant pain, considerable discomfort and impairs daily functioning, often resulting in an inability to work and study. Many techniques have been introduced in the treatment of pilonidal cysts, but till today no gold standard has been established. The aim of the study was to present initial results considering the treatment of pilonidal cyst disease with the use of a skin flap by means of Limberg’s method. Material and methods. During the period between January, 2012 and March, 2013, 10 patients were operated due to pilonidal cysts using the Limberg flap procedure. All patients presented with the chronic form of the disease and were after previous surgical incisions. Pain considering patients was evaluated using the visual pain scale (VAS), one and 10 days after the operation. Patients reported for control 10 days, one month, and 6 months after the surgical procedure. Results. The planned procedure was safely performed in all cases. There were no complications during the procedure, nor late postoperative complications. We observed no wound infection or dehiscence in all operated patients. The mean follow-up time of the presented group was 13.32 months (ranging between 7.53‑21.57). During the follow-up period there was no episode of recurrence. Conclusions. The use of the skin flap by means of Limberg’s method after excision of the pilonidal cyst in the sacrum area is a good method, which provides a very low percentage of recurrences and complications. Fast return to daily activities, no significant pain in patients, and an acceptable cosmetic result are strong reasons in favor of the above-mentioned method. Preliminary observations and experience enable to recommend this method for the treatment of pilonidal cysts in the vicinity of the sacrum

Due to economic problems, sigmoid loop colostomy using glass rod may cause problems for our patients for finding glass rod and several visits. The aim of the study was to compare rod versus skin bridge colostomy. Material and methods. In this study, 42 cases who are candidate for colostomy were included. Cases were randomly placed in skin bridge and rod colostomy group. Independent sample t-test and Chi-square were used for comparison. SPSS version 16.0 (SPSS Inc, Chicago, IL, USA) was used for analysis. Results. Of 42 cases, 20 were male and 22 were female. Hirschsprung’s disease was the indication of colostomy in 33 cases. In nine cases, imperforate anus was the indication of colostomy. Mean time of surgery was 79.4 and 82.5 minute for the rod and skin bridge group respectively (P>0.05). Retraction was seen in 2 case of rod group, and no case of skin bridge group. Prolapse was seen in 2 (9.5%) case of rod group and 1(4.7%) case in skin bridge. There were no reports of necrosis, stenosis, and hernia in both groups. Conclusion. In the skin bridge group the rates of complications were lower but the groups are too small for statistical analysis. Colostomy with a skin bridge method may decrease number of revision and expenses and may be appropriate option. Sigmoid loop colostomy using skin bridge flap may be appropriate choice in developing country. Another study with more samples is recommended to better comparison of Skin Bridge versus rod colostomy.

Degloving injury poses a severe therapeutic challenge concerning both trauma and plastic surgery. The injury involves separation of skin and subcutaneous tissue from fascia and muscles. Treatment is often long-lasting and brings unsatisfying results due to the extent of damage, risk of infection and massive blood loss. In this article, we present the management and therapeutic outcomes of a patient admitted due to the degloving injury of the lower extremity caused by workplace accident. We described the complexity of treatment including surgical intervention as well as additional treatment, which combined brought good esthetic outcome.

Nearly 1% of population is affected with burn trauma annually. Among patients seeking advanced medical care almost 8% require hospital stay. Most burns are superficial and do not cause full thickness damage to the skin. Standard topical treatment of burn wounds with creams and ointments requires frequent dressings’ changes. The healing process is long and expensive. The technological revolution, as occurred in the last decade in the field of dressing care allows us to introduce the use of advanced material solutions. Modern materials allow to shorten the treatment time, reduce the expenses and diminish the patient discomfort leading to good outcome. The study presented a case of four patients treated due to the partial thickness burn wound. Hydrofiber dressing with silver ions were introduced in the treatment. We observed good short and long term results

The problem of hard-to-heal wounds concerns 1-1.5% of the total population and about 3% of the population above 60 years of age. The risk factors associated with impaired wound healing are diabetes, arterial and venous insufficiency, advanced atherosclerosis, obesity, and inadequate wound supply. As a result of these pathological processes may develop localized wound infection, disseminated infection, tissue necrosis, and even chronic inflammation carcinogenesis. In the group of patients with malignant tumors, there are wounds arising in the course of the underlying disease and as a result of medical treatment. Wound healing is a significant problem and is often complicated due to the patient’s general condition, comorbidities and complex treatment of cancer, which includes surgery, radiotherapy, and chemotherapy. Radiotherapy used for local-regional control of disease after surgical treatment has a negative effect on healing by causing fibrosis of tissues and blood vessels damage, while chemotherapy interferes with the process of cell proliferation.
