The aim: of the study was to evaluate the usefulness of D-dimer evaluation in the diagnosis and prediction of venous thromboembolism (VTE) of lower extremities in patients operated on for abdominal tumors depending on the chosen cut-off point for this parameter. Material and methods: We included 150 patients operated on for abdominal cancer in our department between October 2014 and June 2016. In these patients, concentration of D-dimer was determined, medical histories were taken, and physical examinations were performed. Ultrasound exams of the veins of the lower limbs were performed three times in every patient in order to confirm or exclude VTE. Results: When a standard cut-off point (500 ng/ml) was used, in 46% of cancer patients D-dimer values were elevated despite the lack of VTE. We did not detect any influence of cancer stage on the value of D-dimer. However, if cut-off point was 1440 ng/ml, which has been suggested in the literature, only 14% of patients were false positive. When the upper cut-off value for D-dimer was raised, the effect of cancer stage on the value of this parameter could be seen. Conclusion: The concentration of D-dimer is often elevated in patients with active cancer, but is not a sufficient criterion for diagnosis of VTE. The concentration of D-dimer before surgery does not determine the risk of postoperative thromboembolic complications. This is undoubtedly related to the widespread use of effective thromboprophylaxis. According to the literature, ultrasound is the optimum method for detection of VTE in surgically treated cancer patients. The effect of cancer stage on the value of D-dimer is revealed only when the cut-off point in this group is 1440 ng/ml, instead of 500 ng/ml which is used for the general population.

The breast cancer is the most common cancer in women, both in Poland and in the world. Consequences entail a disruption in the physical, psychological and social functioning. The aim of the study was to assess the acceptance of illness by patients treated for breast cancer in the early postoperative period. Material and methods. The research was conducted on the group of 100 consecutive patients aged 32‑80 years (median 56 years) who underwent surgery for breast cancer in the Centre of Oncology in Bydgoszcz w 2014 roku. 68 of women had mastectomy, 32 of women had conservative surgery. Polling was conducted in the early period after surgery. The original questionnaire containing closed questions the scale of acceptance of the disease (AIS) as well as mental adaptation to cancer (Mini-Mac) was used in the study. Results. 38% of patients had high acceptance of the disease, 48% averageand 14% had low acceptance. Patients after conservative surgery had a higher average values for the mental strategies to cope with the disease, for the fighting spirit (23.1), helplessness and hopelessness (13.5), positive revaluation (23), the patients had a lower average (16.5) in the strategy to absorb anxiety. Patients after conservative surgery had a higher average for constructive style (2.6) but lower for destructive style (1.5). High level of mental coping with the disease was observed in 53%of patients with constructive style and 4% of patients with destructive style. While, a low level of mental coping with the dosease was observed in 5% of patients with constructive style and 46% of patients with destructive style. Conclusions. Almost half of women after mastectomy or conservative surgery had an average acceptance of the disease. The disease was accepted best by educated women living in the cities, whitecollar workers with a good economic situation. The following factors were affected the better management of the disease, in order: age, education, current occupation and economic situation, while the type of surgery did not affect better management. More than half of women, regardless of the type of surgery reflected the high level of constructive style.

Opis:
Satisfaction with life and disease acceptance by patients with a stomy related to surgical treatment of the rectal cancer depend on multiple factors. Such factors as social support, life conditions and time that elapsed after stomy creation, are very important in this context. The aim of the study was to conduct an early evaluation of life satisfaction and disease acceptance by patients with a stomy related to surgical treatment of the rectal cancer. Material and methods. The study was conducted at Dr. Jan Biziel University Hospital No. 2 in Bydgoszcz and at the prof. F. Łukaszczyk Oncology Centre in Bydgoszcz in 2014. The final analysis included 96 subjects aged 41-87 years (median 59 years). Satisfaction With Life Scale (SWLS) and Acceptance of Illness Scale (AIS) adapted by Zygfryd Juczyński, were used in this study. Results. Most patients had satisfaction with life score of 5 or 6, 23 (24%) and 28 (29.2%) subjects, respectively. Twenty nine (30.2%) study subjects had low satisfaction level, while 16 (16.7%) had high satisfaction level. Average disease acceptance score was 23.2 points. Most patients, 71 (74%) had a moderate disease acceptance score, while the lowest number of subjects, 9 (9.4%), had high disease acceptance score. None of the study subjects who were under the care of a psychologist (14/100%) did not have a low acceptance level. Conclusions. Half of the study subjects had a moderate level of satisfaction with life. Most patients with stomy related to surgical treatment of the rectal cancer in an early postoperative period had moderate level of the disease acceptance. Patients with high level of satisfaction with life, accept the disease better. Few patients who used help by a psychologist, were two- and three-fold more likely to have higher level of satisfaction with life and disease acceptance, respectively.

Opis:
The aim of the study was to answer the question whether there are differences in the progression of the disease and its morphology, dependent of the patient’s age, as well as show colorectal cancer differences between young and elderly patients. Material and methods. During the period between 2009 and 2011, 747 patients with rectal carcinoma, and 478 with colon cancer underwent surgery at the Department of General and Colorectal Surgery. The study group comprised patients under the age of 40 years (56 patients) and >80 years (90 patients). The following were subject to analysis: gender, tumor location, percentage of radical and palliative procedures, clinical and histopathological staging, as well the differences in the morphology of the tumor. The χ2 test and Yates’ correction were used for statistical analysis. p<0.05 was considered as statistically significant. Results. A higher incidence of rectal carcinoma was observed in younger patients (p=0.004). Amongst the younger patients 30.3% were diagnosed with stage III cancer, while 35.7% with stage IV. In case of elderly patients 30.6% were diagnosed with stage III cancer, while 30.7% with stage IV. In 53.3% of young patients, local lymph node metastases were observed, while in the elderly 41.8%. No statistically significant difference was observed, considering the total number of lymph nodes metastases. However, in case of stage N2, results were unfavorable for young patients 31.8% vs 17% in case of the elderly. Mucogenic adenocarcinoma incidence was similar in both groups. In case of patients< 40 years we observed a higher incidence of poorly differentiated tumors, as compared to the elderly patients (>80 years). Conclusions. 1. It is necessary to create appropriate prevention programs for young community, and improvement of public awareness in this age group. 2. Colorectal cancer (CRC) in young patients is often recognized in advanced stage. 3. A high percentage of unresectable tumors in group of elderly patients shows that CRC is still diagnosed too late in Poland.

Opis:
Breast cancer has high metastatic potential with distant metastases involving mainly lungs, liver and bones. Less frequently it gives distant spread to other organs. Herein we would like to present a very rare case of an acute cholecystitis which turned out to be a metastatic breast cancer in previously healthy woman. A female patient, 64-years old, presented to the emergency department with symptoms of biliary colic and acute abdomen. During the emergency cholecystectomy, we diagnosed the gallbladder empyema with thickened wall. There were also multiple metastatic nodules in the peritoneal cavity and an excessive amount of free fluid. The emergency physicians diagnosing female patient with the acute abdominal symptoms and a breast cancer history might suspect malignant spread into abdominal organs including gallbladder. On the other hand, acute cholecystitis symptoms might be the first symptoms of metastatic process in the gallbladder from the unknown primary source, which may be breast.
