Opis:
Transfacial surgery with the use of microscopic end endoscopic techniques has been used for resection malignant tumors of the paranasal sinuses and anterior skuli base. After tumor resection the dura mater injuries should be carefully closed to avoid the most common postoperative complications (encephalocele, meningocele, meningitis, intracranial abscess and pneumo-encephalus). Authors analyzed cases of tumors of nasal cavity and paranasal sinuses with extension to anterior scull base treated in ENT Department of Medical University of Warsaw. In case of smali injuries to 10 mm in diameter the reconstruction was usually performed with multi-layer technique. In cases of bigger defects the dural piane was rebuild with fascia lata flap by suturing it to the dura mater. Authors conclude that malignant tumors of the paranasal sinuses and anterior skul) base can be resected entirely, together with dura mater by the extracranial approach through a lateral rhinotomy incision.

Introduction: The paper aims to demonstrate the Clinic’s and own research in the treatment of major salivary gland tumors. Material and method: In the years 2013–2019, there were 95 salivary gland surgeries, including 45 performed in women (47.36%) aged between 24 and 82 and in 50 men (52.64%) aged 29 to 86. Diagnostics of major salivary glands included: patient history, otolaryngological examination, ultrasound and fine needle aspiration (BACC) (of tumor), laboratory tests (morphology, CRP) and computed tomography (CT) or magnetic resonance imaging (MRI) to accurately assess salivary glands, location of lesion and lymph nodes prior to elective surgery. Research results: In the analyzed material patients with major salivary gland tumors were most frequently operated on at the age of 61–70 (34.7%) and 51–60 years (23.2%). Most patients with tumors were hospitalized in the last 4 years between 2016–2019, which accounted for 77.8% of all operated cases. Among the operated lesions, benign neoplasms and tumors occurred in 93.7%, of which the most frequent findings in histopathological examination were Warthin tumor in 50.5%, and multiform adenoma in 26.3%. The remaining tumors and benign neoplasms constituted only 16.9% and were observed in isolated cases, including eosinophilic adenoma, lymphoepithelial cyst, myoepithelial adenoma, lipofibrosarcoma, congestive cyst, air cyst, tubular adenoma, basal cell carcinoma, cyst with squamous metaplasia features, vascular malformation. Malignant salivary gland tumors were found in 6.3%, including the following malignant tumors were revealed histopathologically: adenocarcinoma, mucoepidermal carcinoma, acinocellular carcinoma, epithelialmyoepithelial carcinoma, ductal carcinoma. Conclusions: The majority of operated salivary gland tumors were benign (93.7%), of which the most common histopathologically were: Warthin tumor (50.5%) and multiform adenoma (26.3%), while malignant tumors occurred in 6.3%. Tumors were localized mainly in the parotid gland in as many as 93.7% cases, of which the most common localization concerned the superficial lobe in 56.8%. The most frequent surgical technique used in the Clinic was extracapsular dissection of the tumor in 88.4%, and other methods were used rarely.

INTRODUCTION: Sinonasal inverted papillomas (IPs) are a major challenge for ENT specialists. In the last decades, the number of endoscopic procedures has increased, while the percentage of external or double approach procedures has decreased. The aim of this study was to evaluate long-term IP treatment outcomes according to the applied surgical method. MATERIAL AND METHODS: the retrospective study included 69 patients (28 women and 41 men) treated during 2002–2012 in the Department of Otolaryngology and Laryngological Oncology of Poznań University of Medical Sciences. Of these patients, 47 were operated endoscopically, 16 received a double procedure and six received an external procedure. RESULTS: The recurrence rate in patients treated endoscopically was 32%, while patients treated by the external/double approach showed 64% (p = 0.03306). The recurrence rate after endoscopic surgery was 75% during 2002–2006, but was 28% during 2007–2012. There was no correlation between recurrence rate and age (p = 0.087686), gender (p = 0.42810), type of symptoms (p = 0.20955), or Krouse staging (p = 0.23658). A significant impact on relapses was the position to Ohngren’s plane (p = 0.004768) and tumor location in the anterior ethmoid (p = 0.00416). DISCUSSION: Endoscopic procedures are an effective method of IP treatment, however reasonable indications and surgeon experience should be taken into consideration.

Introduction: The aim of the study was to analyze the profile of patients who reported for free screening examinations during the 4th European Head and Neck Cancer Awareness Week to the Department of Otolaryngology, Miedzylesie Specialist Hospital, Warsaw, Poland. Material and methods: We enrolled 225 patients, aged 26-92 years (mean, 63 years). There were 149 women (66%) and 76 men (34%). All patients filled out a survey regarding risk factors of head and neck cancer (HNC), including symptoms and lifestyle characteristics. Subsequently, all participants underwent a complete laryngological examination. Results: Among the studied patients, we found the following lifestyle-related risk factors of HNC: tobacco use (22%), alcohol use more than 1 time per week (12%), oral sex (17%), multiple sexual partners (10%), rare dental checkups (24%), and dental prostheses (45%). The most frequent symptoms that prompted the patients to report for screening exams were as follows: chronic hoarseness (64%), xerostomia (39%), dysphagia (37%), nasal congestion (6%), and neck tumor (5%). Further imaging studies were ordered in 15 patients (7%), fiberoscopy in 25 (11%), and lesion excision in 18 (8%). Among the studied patients, 17 were referred for further oncological treatment, of whom 14 had benign tumors and 4 malignant tumors. Conclusions: The Head and Neck Cancer Awareness campaign increased the awareness of HNC, and it should systematically encompass people at increased risk. By implementing this program in our department, we were able to detect and initiate early treatment in 7.5% of the screened people. However, appropriate workup and treatment require funding and therefore systemic measures should be taken to enable a wide implementation of such screening programs.

The authors propose a set of rehabilitation exercises concerning the muscles which are responsible for movement of the eyeball. After surgical treatments of retrobulbar tumors, the function of the eyeball muscles is often inadequate. Some compensation should be created at the level of the central nervous system, which means trigging adaptation, substitution and habituation. The exercises should be started just after the patient is awakened: first in the horizontal position, then sitting position and finally standing position. The highest number of exercises should be done in the direction of extreme diplopia
