    Opis:
    Immunotherapy supports other therapeutic methods and is an important element in the fight against cancer. Its main tasks include stimulation and guidance of the immune system to fight cancer, inhibition of the mechanisms that block the immune system, and direct destruction of neoplastic cells. Different from chemotherapy or hormonal therapy, its mechanism of action is associated with a different profile of adverse events. HER2 receptor inhibitors may cause symptoms of heart failure, which usually recede, once the treatment has been discontinued. Bevacizumab, an anti-VEGF antibody, induces numerous cardiovascular complications, including arterial hypertension, arterial embolism, haemorrhage and haemoptysis as well as venous thromboembolism.

    Left ventricle systolic dysfunction manifesting during trastuzumab treatment is defined as cardiotoxicity type II. It is characterized by full reversibility after discontinuation of trastuzumab and cardiological pharmacological treatment. In a group of patients, however, systolic cardiac function does not fully recover. The reasons of this unfavorable prognosis are subject of heated discussion.

    HER2 positive breast cancer presents 25–35% of newly diagnosed cases and is associated with more aggressiveness and poorer prognosis. The anti-HER2 monoclonal antibody trastuzumab improved the outcome of women with HER positive breast cancer. Despite the primary effectiveness resistance to trastuzumab occurs. The second drug which interacts with HER2 receptor is lapatinib. There is no cross resistance between both drugs. Lapatinib is dual, reversible tyrosin kinase inhibitor of epidermal growth factor receptor (EGFR) and human epidermal growth factor receptor type 2 (HER2). Based on the results of the clinical trials the efficacy and safety of lapatinib was confirmed.
    Rak piersi z nadekspresją receptora HER2 lub amplifikacją jego genu stanowi ok. 25–30% zachorowań i wiąże się z bardziej agresywnym przebiegiem i gorszym rokowaniem. Wprowadzenie trastuzumabu – pierwszego leku celowanego ukierunkowanego na zewnątrzkomórkową domenę receptora HER2 – poprawiło skuteczność leczenia. Mimo pierwotnej odpowiedzi na leczenie dochodzi do wytworzenia oporności i progresji choroby. Drugim lekiem wpływającym na receptor HER2 jest lapatynib. Nie stwierdzono krzyżowej oporności między obydwoma lekami. Lapatynib to odwracalny inhibitor kinazy tyrozynowej dwóch receptorów EGFR i HER2. Na podstawie badań klinicznych udowodniono jego skuteczność i bezpieczeństwo w leczeniu HER2-dodatniego raka piersi.

    Despite improving screening methods, diagnosis of early breast cancers, new therapies the prognosis in breast cancer remains poor. Advanced, metastatic breast cancer is an incurable, persistent disease. Hormonotherapy is the main treatment of receptor positive breast cancer patients. Till now in case of hormone resistant disease chemotherapy was the only choice nowadays there is possibility to use combination of endocrine and targeted therapy, the mTOR inhibitor everolimus. In HER2 receptor positive breast cancer therapy with trasuzumab improved treatment results. Despite many new treatment methods there is still need for new therapies which could improve the patients outcome.
    Mimo udoskonalania metod badań przesiewowych, wczesnego wykrywania raka piersi i nowych terapii rokowanie w raku piersi pozostaje złe. Zaawansowany, uogólniony rak piersi jest chorobą przewlekłą, nieuleczalną. Podstawową metodą leczenia systemowego nowotworów dodatnich receptorowo jest hormonoterapia. Do niedawna w przypadku oporności metodą z wyboru pozostawała chemioterapia, obecnie możliwe jest zastosowanie skojarzenia leczenia hormonalnego z leczeniem celowanym ewerolimusem – inhibitorem mTOR. W rakach z nadekspresją receptora HER2 rokowanie pacjentek zmieniło wprowadzenie do leczenia trastuzumabu. Mimo dostępności wielu metod leczenia nadal poszukuje się nowych, które poprawią wyniki leczenia.
