Prostate cancer is one of the most frequently diagnosed cancers in men. Number of newly diagnosed cases is increasing due to several factors and the most important ones seem to be: population ageing and more sensitive diagnostic procedures. Secondary – the higher efficacy of treatment with its influence on improving patients’ overall survival and the specific mechanism of action of drugs used in systemic therapy lead to growing population of men suffering from prostate cancer in general and, specifically – patients with castration resistance. It is hormone therapy to play the key role in systemic treatment of prostate cancer with increasing significance of novel drugs focused on inhibition of molecular signal transduction mediated by androgen receptor. Abiraterone acetate is the representative of this therapeutic class. The paper describes the most clinically relevant data regarding the drug.

Children with acute leukemia are at a high risk of invasive fungal disease, which might manifest itself as clinically-resistant entity. The objective of this paper is to present an unusual clinical case of 17-year-old patient treated for acute lymphoblastic leukemia, with early development of disseminated invasive aspergillosis, involving the abdominal, pulmonary and cardiac structures. The patient was subjected to a combined targeted double, and later triple, antifungal therapy together with several debulking surgical interventions. The clinical course indicated a highly clinically-resistant invasive fungal disease, and the treatment was unsuccessful in this case. Limited current experience in triple antifungal therapy, abdominal aspergillosis, Aspergillus endocarditis, and possible causes of failure of antifungal therapy are discussed in the paper.

The problems of antithrombotic and first of all, anticoagulant therapy in patients with concomitant oncological and cardiovascular pathology, especially in gerontological practice, remain to be unsolved. The need to use anticoagulants is dictated by a high risk of thromboembolism in cancer patients combined with age-associated atrial fibrillation. The risk of hemorrhage increases with the age; moreover, there is a risk of dangerous drug interactions due to the need of polymorbid pathology treatment. Thus, the clinical situation sometimes creates difficulties in choosing a proper tactic in patient management and following the existing treatment standards. A clinical case of a patient of an elder age with the recurrence of lung cancer, which has an established high-risk of thromboembolism, and with paroxysmal form of atrial fibrillation and new oral anticoagulants treatment, is considered.

In order to present a reliable picture of hormone-dependent breast cancer treatment in Poland, an on-line survey has been conducted in 19 oncology centres. As a result, data on the treatment of 486 patients have been obtained (405 of them initially presenting with stage I–III of disease advancement, and 81 representing stage IV). It has been concluded that in the majority of cases the treatment in question involves combined therapy, including chemotherapy, hormonal therapy, targeted molecular therapy, radiotherapy, and surgical methods with reference to the group of patients subjected to radical treatment.

For many years, dacarbazine has been the gold standard in systemic treatment. Recently, a significant progress in melanoma therapy has been observed. Purpose of the review: This comprehensive review aims to provide a summary of current research on the utilization of olaparib, a poly(ADP-ribose) polymerase (PArP) inhibitor, in the treatment of ovarian cancer. The review aims to highlight the key findings from recent clinical trials and assess the potential of olaparib as a targeted therapy for improving the prognosis of ovarian cancer patients. recent findings: Ovarian cancer remains a significant global health concern with high mortality rates. While optimal debulking surgery and platinum-based chemotherapy are the standard treatments, the recurrence rates remain substantial. The emergence of PArP inhibitors, particularly olaparib, has intro duced a novel therapeutic approach that targets the genomic instability and DnA repair mechanisms in cancer cells. notable clinical trials, such as SOl O1, SOl O2, and PAOlA-1, have demonstrated the effectiveness of olaparib in significantly improving progression-free survival, particularly in patients with Br CA mutations or homologous recombination deficiency. Additionally, combination therapies involving olaparib, such as those with bevacizumab or entinostat, have shown promising results. Summary: The utilization of olaparib has brought about a paradigm shift in the treatment of ovarian cancer. notably, it has shown significant improvements in progression-free survival and overall survival, particularly in patients with Br CA mutations or homologous recombination deficiency. The exploration of olaparib through various clinical trials and combination therapies continues to provide valuable insights and offer new prospects for ovarian cancer patients. Moreover, the growing understanding of PArP inhibitors holds the potential for further advancements in the prognosis of patients with this formidable condition.

Breast cancer is the most common malignancy in women in Poland and the entire world, but thanks to early diagnostics and efficacious therapeutic methods, its prognosis is good. In some patients with advanced breast cancer, long-term survival is observed, which is why it is essential to select the most appropriate therapy, and to follow up on the course of disease and possible adverse events related to the treatment applied. The slow development of hormone-receptor-positive cancer, and a multitude of therapeutic options constitute a real challenge in palliative treatment. The use of fulvestrant resulted in a good response in both of the presented cases, and did not in any way comprise the daily lives of the patients over a long period of time.

Breast cancer is the most common malignancy among women in Poland. Endocrine therapy is the first line of treatment in hormone-receptor-positive advanced breast cancer. Progression during endocrine therapy is unavoidable. Administration of mTOR inhibitor gives a chance of reversing the acquired resistance. This paper presents a case report of a patient with metastatic breast cancer successfully treated with everolimus added to endocrine therapy.

Malignant pleural mesothelioma is a rare neoplasm with poor prognosis. Surgical complete resection, which is the only radical therapy available, is only possible in a minority of patients who suffer from a locally advanced disease. Radiotherapy can be considered as an adjuvant therapy after surgery or as supportive treatment in palliative care. Attempts are also made at combining it with chemotherapy. In cases of locally advanced, non-resectable or metastatic disease, chemotherapy remains the treatment of choice. The most effective palliative chemotherapy is the pemetrexed plus cisplatin regimen. We describe 3 cases of patients who underwent standard palliative chemotherapy with pemetrexed and cisplatin, manifesting good radiological and clinical response, followed by maintenance therapy with pemetrexed. The use of pemetrexed maintenance therapy helped achieve many years of disease control with acceptable toxicity. The consecutive stages of therapy were continuously discussed with our patients, and their informed consent was obtained. Pemetrexed maintenance therapy is not a standard procedure, but recent findings suggest it may be an efficacious option to consider in selected groups of patients. Further randomized prospective studies are needed, but a limiting factor is the rarity of the disease.

Objective: Results of second-line everolimus treatment in clear cell renal carcinoma. Material and methods: Clinical data of 32 patients of the Clinic of Systemic and Metastatic Malignancies, Centre of Oncology, Maria Sklodowska-Curie Memorial Institute (Krakow Branch) treated with second-line everolimus between 2011 and 2016 were analysed. Results: Median progression-free survival was 7 months. Stable disease was observed in 24 cases. Adverse events occurred in 59% of all patients, and the majority were mild and moderate (G1 and G2) according to CTCAE. The most frequent adverse events included anaemia, stomatitis, rash and fatigue. Conclusion: Molecularly targeted drugs offer the only effective and available therapy in Poland, which can be administered in patients with advanced renal cell carcinoma. Second-line everolimus is a valuable therapeutic option in the above mentioned group (following progression on first-line TKI therapy), especially when it is necessary to avoid the accumulation of similar adverse events after TKI treatment (different toxicity profiles).

Prostate cancer is the most common cancer in men in Poland. The choice of treatment method depends on many factors, including the stage of the disease, the patient's age, life expectancy, the presence of comorbidities and the general condition of the patient. Hormone therapy is one of the basic methods of prostate cancer treatment, both in adjuvant and palliative treatment. Castration-resistant prostate cancer is an incurable disease. However, the introduction of new hormonal drugs to therapy significantly extended the survival time of patients with relatively low harmfulness of therapy.

Phosphatase and tensin homolog deleted on chromosome ten (PTEN) is a human suppressor gene. Its protein product is a bispecific phosphatase playing the complex role in the cell cycle regulating processes and apoptosis by the mechanism of signal transduction into the cell via tyrosine kinase B signaling pathway (PI3K/Akt/mTOR). Reduction or loss of PTEN function is implicated in the pathogenesis of many malignancies, including colorectal cancer. A gradual decrease in the function of PTEN in the sequence of transformations: normal tissue–polyp–adenocarcinoma – disseminated cancer was indicated. The relation between the PTEN loss and the higher clinical severity of colorectal cancer was observed, i.a. higher TNM status and higher tendency to form metastases, leading in some of the studies to shortened patients survival during the observation period. The potential predictive value of the PTEN function loss for the EGFR-targeted therapy in patients with advanced colorectal cancer is the subject of controversy. The potential application of PTEN assessment in clinical practice as a prognostic and/or predictive factor requires further well-designed prospective studies on larger patient population, using the unified methodology. The aim of the study is to summarize the current knowledge on the role of PTEN gene and PTEN protein in the pathogenesis of colorectal cancer and the role of PTEN in clinical practice.

Integrative photodynamic therapy (IPDT) of tumors consists of combined use of different curable factors that extremely increase the effectiveness of IPDT. The leading curable effect of PDT is determined by developing of aggressive photochemical reactions in the tumor. The main destructive factor of these reactions is the generation of free radicals that kill tumor’s cells. For PDT efficacy increasing it is necessary to solve the following problems and assure: easiness of production or synthesis; accumulation high selectivity regarding tumor tissue; low toxicity in light and darkness; singled oxygen generation high quant output; promptly elimination from the organism after the treatment procedure; good absorption in spectral intervals most transparent for tissues (red and infrared intervals); optimal interconversion between quant output and fluorescence quant output. PDT effectiveness may be significantly increased by means of: increasing of photosentisizer selectivity and accumulation in tumor tissue; increasing of tumor tissue photochemical destruction caused by photosensitizer. Realization of integrative PDT using Cyber Laser may increase tumor therapy effectiveness to 94–96% and at the same time will establish a new direction in PDT.

Triptorelin as the luteinizing hormone releasing hormone (LHRH) analogue has its own place among other available forms of androgen-depravation therapy (ADT) of locally advanced and metastatic prostate cancer (PCa). Nowadays, in times of development of new therapies in castration-resistant PCa, ADT remains the back bone therapy, which may be supplemented with one of novel drugs. The results of basic research indicate, that apart from the main mechanism of action based on lowering a testosterone concentration to the castration level, triptorelin may have a direct inhibitory effect on tumor cells. Formulations of tryptorelin are available to administer as 1-month, 3-months and 6-months sustained-release forms that may be given intramuscularly or subcutaneously. Constant concentration of triptoreline is maintained by using special microspheres in drug production. Pharmacokinetic and pharmacodynamic properties of particular forms were extensively tested, which allows for safe usage and retain of predictable and high efficacy. Indicators of effective ADT are fast reduction and maintenance of the state of castration. This phenomenon translates into a decrease of the prostate-specific antigen and longer survival. Triptorelin successfully meets these objectives based on a number of phase I–III studies. There is a noticeable lack of comparative studies on effectiveness of particular ADT forms. This may stem from an assumption, that all LHRH analogues demonstrate similar effectiveness because of the class effect. However, some evidence highlight significant differences in efficacy among these drugs. Triptorelin compares especially favourably, particularly as a drug reducing the testosterone level to the lower recommended values (< 20 ng/dl). Side effect profile during the therapy with triptorelin is largely the result of inhibition of the hypothalamic pituitary-testicular axis. Hormonal disturbances linked to hipoandrogenism cause changes in lipid metabolism and glucose tolerance, which may influence the cardiovascular risk. This article is a review of key reports regarding triptorelin and a summary of the role that triptorelin plays in contemporary ADT in advanced PCa.
