Opis:
Chronic exposure to environmental ultraviolet radiation (UVR) plays a key role both in photocarcinogenesis and induction of accelerated skin ageing. Sailors commonly experience a significant number of cutaneous problems, related to the exposure to environmental factors due to their working conditions. Among these factors, sun exposure is able to determine both acute and chronic skin damage, mostly linked to the effects of the ultraviolet (UV) radiation on epidermal and dermal structures. We report a case of sailor with cutis rhomboidalis nuchae, solar elastosis, actinic keratosis (AK), numerous squamous cell carcinoma (SCC) and basal cell carcinoma (BCC) of the head and trunk.

Merkel cell carcinoma (MCC) is a rare cutaneous malignancy, with aggressive behaviour. The discovery of MCC polyomavirus (MCPyV) provided a major insight into pathogenesis of MCC, as MCPyV is present in most MCC tumours. The primary treatment of locoregional MCC is surgery, and sentinel lymph node biopsy should be performed in all cases at clinical stage I-II. Adjuvant radiotherapy decreases disease recurrences in stage I-III of the disease. Chemotherapy is primarily used for palliation and provides overall response rates of approximately 70%; however, the disease often recurs within a few months. All cases of MCC should be managed in an experienced multidisciplinary setting.

Opis:
Clear cell sarcoma (CCS) is a rare soft-tissue sarcoma and is characterized by a translocation t(12;22) (q13;q12). Because of some similarities to maligned melanoma it is called melanoma of soft parts. CCS has characteristic clinical features. It is commonly reported in young adults as slowly growing tumor followed by aggressive course. Its features are: regional lymph node spread and tendency for local recurrence and a propensity for pulmonary metastasis. Metastatic CCS is a very aggressive disease with pure prognosis. Conventional chemotherapy has little role to play in the management of CCS. The article presents the case of a patient with metastatic CCS with massive lung dissemination, metastatic in liver, left adrenal, pelvis and superficial soft tissue. In patient pazopanib therapy resulted in nine-month control of the disease.

Cabozantinib, the latest available in Poland medication for the treatment of renal cell carcinoma, registered in this indication by the European Medicines Agency (EMA) in September 2016, has been available in several cancer centers in Poland since November 2016 as part of the expanded access program. Primary hypothyroidism is a common complication during thyrosine kinase inhibitors (TKI) treatment, although there are few reports of its occurrence during treatment with cabozantinib, which belongs to this medication group. We present a case of rapid development of clinically apparent hypothyroidism after cabozantinib treatment and report data on this complication in the group of our patients.

We presented a 39-year-old male who developed progressive cancer disease 4 years after nephrectomy due to clear cell carcinoma. He was diagnosed with locally reccurence and metastases to the liver, spleen and abdominal muscles. The patient was treated with sunitinib and then after disease progression – with sorafenib. We observed 18 months of cancer control (TKI-TKI). After second progression everolimus was administered. Third line everolimus therapy helped to achieve durable stable disease with PFS 46 months till now (May 2015). The patient remains in very good performance status with minimal toxicity from the regimen. This case illustrates a long term survival for patients with metastatic renal cell carcinoma, a malignancy with historically poor prognosis. The use of three sequential targeted therapies (TKI – TKI – mTORi) helped to achieve over 5 years or disease control, with rarely seen long-term response to third line treatment (mTORi) – where stabilization is good enough. We discussed therapeutic strategies in metastatic renal cell carcinoma according to the literature and therapeutic possibilities in Poland.

Objective: Results of second-line everolimus treatment in clear cell renal carcinoma. Material and methods: Clinical data of 32 patients of the Clinic of Systemic and Metastatic Malignancies, Centre of Oncology, Maria Sklodowska-Curie Memorial Institute (Krakow Branch) treated with second-line everolimus between 2011 and 2016 were analysed. Results: Median progression-free survival was 7 months. Stable disease was observed in 24 cases. Adverse events occurred in 59% of all patients, and the majority were mild and moderate (G1 and G2) according to CTCAE. The most frequent adverse events included anaemia, stomatitis, rash and fatigue. Conclusion: Molecularly targeted drugs offer the only effective and available therapy in Poland, which can be administered in patients with advanced renal cell carcinoma. Second-line everolimus is a valuable therapeutic option in the above mentioned group (following progression on first-line TKI therapy), especially when it is necessary to avoid the accumulation of similar adverse events after TKI treatment (different toxicity profiles).

Basal cell carcinoma is the most common malignancy of man. The use of the topical imiquimod 5% cream offers a noninvasive, nonsurgical, and an effective option for the treatment of primary types superficial basal cell carcinoma (sBCC). We present a case series of patients who used imiquimod 5% cream with good therapeutic effect. The medication had a good tolerability profile and there were no ocular adverse events. Imiquimod 5% cream could be considered the first choice treatment for BCCs in older patients who are poor candidates to surgery.

Around 25,000 people annually worldwide are subject to the procedure of allogeneic hematopoietic stem cell transplantation (HSCT). As the number of procedures has been on the rise, post-transplant care is becoming an essential part of work at transplant centres and general practitioners’ offices. Advances in technology and supportive therapies extend long-term survival of patients after HSCT, which increases the risk of complications resulting from exposure before, during and after transplant procedures. The complications may lead to significant morbidity, deteriorated quality of life, and late mortality in patients who have received hematopoietic stem cells. Statistical analyses have shown that the survival rate of post-HSCT patients is ca. 30% lower than expected, and that infections, organ failure and secondary cancers lead to mortality in this population of patients.

We present a case of a patient with metastatic renal cell carcinoma who was treated with pazopanib in the first-line treatment. Although nephrectomy was not performed, there was a positive reaction to the therapy with multi-targeted tyrosine kinase inhibitor. After 18 months of the palliative treatment, the cancerous kidney was surgically removed and pazopanib was restarted with the effect of further disease remission. The development of hypertension, complete hair discoloration and isolated hyperbilirubinemia occurred, and there was occasionally hypokalaemia in laboratory findings within 48 months of therapy. No serious adverse events were reported.

Pneumonia is one of the most frequent cause of death after hematopoietic stem cell transplantation (HSCT). The objective of this review is to present various aspects of pneumonia in this group of patients, with focus on invasive pulmonary aspergillosis and cytomegalovirus disease, being the most frequent etiological causes of pneumonia after HSCT. The review is aimed at practical approach to diagnostic and therapeutic management of pneumonia after HSCT with special attention to: definitions of infections and level of diagnosis of upper and lower respiratory tract infections, including issues specific for invasive fungal disease, pneumocystosis, cytomegalovirus disease, community acquired respiratory viral infections and bacterial pneumonia. Another topics analyzed in the review are: epidemiology and risk factors for development of infection and risk of death due to pneumonia; invasive and non-invasive diagnostics, including imaging and laboratory biomarkers; methods of pharmacological and environmental prophylaxis and specific targeted therapy of pneumonia after HSCT.

Invasive fungal infections (IFI) are devastating and life-threatening infections affecting especially immunocompromised patients. We report on a case of a 14-year-old boy with myelodysplastic syndrome (MDS), after haploidentical hematopoietic stem cell transplantation, in whom invasive pulmonary aspergillosis (IA) was diagnosed and successfully treated with subsequent (despite the secondary antifungal prophylaxis) IA or breakthrough infection development. Thanks to intensive and broad spectrum antifungal treatment (voriconazole upfront therapy, followed by a combination of voriconazole and micafungin, and eventually by the amphotericin B lipid complex), significant improvement was accomplished.

The case of complete response of squamous cell oesophageal cancer to radiochemotherapy in patient previously operated due to hereditary retinoblastoma is presented. Second cancers in retinoblastoma survivors are the main cause of death in that group of patients in developed countries. There is little data on the outcome of treatment in those patients but the few papers available suggest rather poor prognosis. However, it is not clear whether the biology of cancer determined by RB gene mutation is responsible for that. The data suggesting decreased radio- and chemoresistance in cancers with decreased RB gene expression is discussed in the present paper. The role of that gene in the response to cancer treatment was showed in lung, breast and bladder cancer. Moreover, laboratory studies seem to confirm clinical observations and show that RB gene inhibition leads to increased cytotoxic effect of cisplatin, etoposide and 5-fluorouracil. It seems that without the incorporation of gene expression profiling into clinical practice further improvement in cancer treatment will be difficult to achieve.

Introduction: Primary cutaneous B-cell lymphomas represent primary cutaneous lymphomas. The World Health Organization-European Organization for Research and Treatment of Cancer defines primary cutaneous B-cell limphomas as malignant lymphomas that are confined to the skin at presentation after complete staging procedures. Case report: We report a case of a woman with a cutaneous nodular B-cell lymphoma of the face, with very slow evolution that could easily be misdiagnosed. The patient had two non-ulcerated nodules on the left part of the face. A histological examination on both pieces following surgical excision showed a diffuse infiltrate of atypical B cells. The patient was subsequently directed to the Oncology Department for further investigation, but she died 24 hours afterwards due to one of the acute possible complications of the disease: pulmonary embolism. Conclusion: Cutaneous B-cell lymphoma is an unusual and rare skin neoplasm with a great range of clinical presentations and this is an alarm sign for apparent common skin lesion.
