A 59-year-old farmer was admitted to the Department of Trauma Surgery because of pain associated with the presence of a tumour on the side of the right thigh. The patient stated that his biggest problem was great difficulty in moving and handling agricultural machinery due to the aforementioned lesion. On physical examination, the patient presented with numerous cutaneous and subcutaneous neurofibromas, as well as one large plexiform neurofibroma. Diagnosis of Neurofibromatosis type 1 was established, based on National Institutes of Health diagnostic criteria. Moreover, molecular genetic testing found known pathogenic mutation p.Arg1769* in one of the alleles of NF1 gene (heterozygote), typical for Recklinghausen disease. The big plexiform neurofibroma in the patient was not operated on because of lung cancer.

Introduction. Despite the growing interest in the consequences of caring for patients with Huntington disease (pHD), little is known about the family caregivers of such patients in Poland. Identification of their needs can improve caregivers’ wellbeing, the quality of care and condition of pHD. The aim of this study was to understand the social functioning of family caregivers of pHD and their perception of the caregiving role. Materials and methods. Data was collected from 55 family caregivers of pHD. A structured questionnaire was used consisting of 86 questions subsumed into five domains: ‘Problems’ and ‘Feelings related to caregiving’, ‘Attitude toward caregiving’, ‘Satisfaction with life’ and ‘Perception of healthcare services’. Correlations between the different scales and other characteristics were measured as potential predictors of the burden. Non-parametric statistical methods were used in the analysis. Results. Most respondents experienced a high (50.9%) or moderate (30.95%) feeling of burden. Although 70.9% of caregivers perceived caregiving positively, for many it was a source of negative feelings. Only 10.9% of respondents declared that caregiving decreased their QoL. Carers’ perception of caregiving was mostly influenced by their negative experiences with the healthcare system. Respondents’ domicile, religious practices, age, income, marital status, time of diagnosis and of caregiving, patient’s age and stage of disease also influenced their experiences. Conclusions. Health professionals and policy planners should focus on monitoring caregivers’ health, identifying their needs, sources of distress, and supporting caregivers’ coping strategies. They should also be better educated about the clinical and practical aspects of HD.

Introduction. Nowadays, preventive vaccination is still the most effective method of countering and combating infectious diseases. Objective. The objective of the present study was to take into consideration the education of the respondents while learning about their views and attitudes towards both compulsory and recommended preventive vaccination in Poland. Materials and methods. An interview survey was conducted among 370 respondents – students and young parents residing in the Lublin province of eastern Poland. The research tools were an authors’ questionnaire and a Life Orientation Questionnaire (SOS-29). Results. The majority of respondents with secondary education (77.40%), undergraduate education (63.71%), and graduate education (55.07%) stated that sometimes preventive vaccination may have life-threatening consequences. A very small percentage of both groups of respondents with secondary and undergraduate education expressed the opinion that such situations often happen (1.69% vs. 1.61%), but this opinion was not shared by any of the respondents with graduate education. Analyzing the dependency between the sense of coherence and one’s level of education, a significant difference between the respondents with secondary education and undergraduate education was observed. Although differences between respondents with graduate education and respondents with undergraduate education existed, they were not very significant. Conclusions. The educational background of the respondents has an impact on their opinions on preventive vaccination. The great majority of respondents believed that vaccination is necessary in order to avoid many dangerous diseases. Respondents with graduate education evaluated preventive vaccination in a positive or a very positive way. The subjects differed in their global sense of coherence together with its components. The general sense of coherence stood at an average level, with the mean of 129.02 points. The highest results were obtained by the respondents in the area of resourcefulness, which is a belief that they had at their disposal remedial resources, including preventive vaccination.

Paraneoplastic syndromes consist of disorders that accompany benign and malignant tumours, but are not directly related to mass effects or invasion by the primary tumour or its metastases. The pathophysiology of mostly paraneoplastic syndromes is not well known. The usual mechanism of their development is the aberrant production of substances (protein hormones, hormone precursors or hormone-like substances) by tumour tissue or autoimmune disorder. The neurological paraneoplastic syndromes are autoimmune disorders that can affect almost any part of the nervous system. The endocrine paraneoplastic syndromes are often certain clinical syndromes like paraneoplastic hypercalcaemia or the aberrant production of hormones by cancers. Many haematologic conditions, including anaemia, leucocytosis, thrombocytopenia or thrombocytosis, coagulapathy have been reported in association with cancer. Also paraneoplastic rheumatic syndromes have been reported in association of cancer. Skin can be involved in cancer in different ways: by metastases, as a part of a genetic disorder with a cutaneous component in which there is an inherited predisposition to the later development of malignancy, as a part of an acquired syndrome due to the toxicity of a carcinogen that induces malignant change and accompanying skin changes, as a consequence of immunosuppression, or by the development of specific lesions that occur as a paraneoplastic syndromes The most frequently diagnosed dermatologic conditions include acrokeratosis paraneoplastica, Sweet’s syndrome and paraneoplastic pemphigus. The purpose of this article is to describe the most popular in internal clinical practice neurologic, endocrine, haematologic, rheumatologic and dermatologic paraneoplastic syndromes associated with cancer.

Introduction. Fibromyalgia syndrome (FMS) and myofascial pain syndrome (MFPS) can be ranked among disease entities being difficult to diagnose clinically, manifesting themselves mainly through pain in specific hypersensitivity points. Aim. To present the current state of medical knowledge about pain spots appearing on hypersensitive points of soft tissue in the context of selected disease entities. Summary of the knowledge. MFPS is defined as sensory, motor and autonomic complaints, caused by the occurrence of trigger points (TrP). Yet the FMS is stated during the anamnesis on the basis of generalized pain, and pressure achiness of at least 11 out of 18 tender points (TP) of precisely determined location. Patients with FMS report numerous additional complaints – apart from the above mentioned ones; these are however highly non-specific and are not confirmed during routine medical check-ups. There are also no laboratory tests that can confirm presence of TrP being characteristic to MFPS and differentiating it from other muscles’ disease entities. Such points are identified only with the use of palpation. Unfortunately while examining a patient this way TrP – being symptoms of MFPS – can be quite easily confused with TP – being symptoms of FMS. Patients with MFPS which is developing in consequence of long-lasting global disorder of muscle tension balance and sensitivity of nociceptors as a result of chronically remaining pain, frequently suffer from achiness fulfilling the criteria of generalized pain. Moreover – in effect of static overload of soft tissues (especially of tonic muscles) – there occur hypersensitive palpable areas (points). Stimulating them cause lively reaction of the patient. Described symptoms can suggest a suspicion of FMS – the more so that making a diagnosis of MFPS does not exclude its coexistence. Having this in mind, there is a pretty large group of authors who raise a supposition that the differential diagnosis between TrP and TP should be observed in the quantitative rather than in the qualitative categories, despite the still binding definition and nomenclature. Recapitulation. Looking at the MFPS and at the FMS from the perspective of evolution of knowledge about them and from the point of view of period when scientific researches were conducted and their results published, it must be stated that during last years a considerable progress has been obtained in scope of better understanding of pathogenesis and pathophysiology of pain in specified points of soft tissue hypersensitivity, and the parallel clinical studies – confirming the hypotheses that were made – clearly increased the diagnostic and therapeutic capabilities of clinical practice.

Introduction and objective. Deleted in Liver Cancer (DLC) proteins belong to the family of RhoGAPs and thus are believed to operate as negative regulators of the Rho family of small GTPases. Rho proteins are considered to be significant links between numerous cellular pathways. So far, DLC1 – the first identified member from Deleted in Liver Cancer family – has been established as a tumour suppressor in hepatocellular carcinoma. As shown by many studies, DLC1 expression is reduced by gene loss or epigenetic silencing in this type of cancer. The expression and function of its close family relative DLC3 is less known. The presented study determined the expression and cellular localization of DLC3 protein in hepatocellular carcinoma tissue. Materials and methods. The protein level in two types of hepatocellular carcinoma: typical and fibrolamellar, were assessed by the immunohistochemical approach. Results. DLC3 immunoreactivity was found to be present in the cytoplasm of normal hepatocytes. In hepatocellular carcinoma sections, DLC3 was detected primarily in the cytoplasm of cancer cells, although in a small percentage of cancer cells cell nuclei were also positively stained. Morphometric analysis followed by statistical evaluation showed that the DLC3 immunoreactivity in the tumour sections was comparable to the one observed in non-cancerous liver specimens. Conclusions. The results obtained indicate that DLC3 protein, contrary to DLC1, is commonly expressed in hepatocellular carcinoma. It appears that members of the DLC family, although structurally highly related, may function differently during HCC development.

The rising number of patients with liver dysfunction due to overwhelming usage of drugs and alcohol has paved the path for researchers in an interest in herbal medicine. This is because there are only a few universally effective and available options for the treatment of common liver diseases, such as cirrhosis, fatty liver and chronic hepatitis. Herbal treatment has been used to alleviate disorders related to liver and other internal organs for many centuries in the eastern world, and have currently become a favourable therapy internationally for pathological liver conditions. In recent years, researchers have used scientific methods to evaluate the effects of plants for the treatment of liver ailments, although in many cases the mechanisms and modes of action of these plants, as well as their therapeutic effectiveness, have not been confirmed. Several hundred plants have been examined to-date, but only a handful has been studied thoroughly. Among these, the current study discusses a compilation of herbal medicines used for liver protection, such as Silymarin (milk thistle), Liv-52, Camellia sinensis (green tea), Glycyrrhizaglabra (licorice) and FuzhengHuayu. The increasing use of herbal medicines reflects their perceived effectiveness in the treatment and prevention of disease, and the belief that these treatments are safe because they are ‘natural’. The presented study evaluates the effects of herbal extracts in the treatment of liver diseases, provides a general understanding of the actions of herbal medicines, a background for understanding the hepatoprotectiveness of herbs, and the challenges that are faced by the scientific community in researching thoroughly of each and every compounds of the herbs.

Opis:
Noonan syndrome is a congenital developmental disorder characterized by short stature, facial dysmorphia, congenital heart defects and skeletal anomalies, with as estimated incidence of between one per 1000 and one per 2500 live births. The aim of our study is to present a case of dental surgical treatment of a child with such disorder. It is widely reported that children with mental disabilities receive less dental and medical care than their nondisabled counterparts. Because of this problem, it seems justified to describe the procedures of treating such children with dental issues, basing on a case of a girl with Noonan Syndrome.
