Introduction and objective. Adapting to gynaecological cancer is the problem of accepting and coping with the disease and its consequences. The aim of this study is to analyze the relationship between stress intensity and strategies for coping with stress and psychological adaptation to disease in women diagnosed with gynaecological cancer. Materials and method. The study was performed on a group of 102 women diagnosed with gynaecologic malignancy. The mean age of patients was 56.1 (SD ±10.75) years. Three self- designed questionnaires were used to carry out the study: Scale of Perceived Stress (PSS-10), Multidimensional Inventory for Measuring Coping with Stress (Mini-COPE) and the Scale of Mental Adaptation to Cancer (Mini-MAC). Statistical significance was set at p <0.05. Results. In more than half of the women surveyed, the severity of stress experienced during the last month was high. Women with a malignant illness significantly differed (p<0.001) in the styles of coping with the disease, preferring a constructive style (M=43.5; SD±5.76) rather than a destructive style (M=21.7; SD±5.28). The highest median values were obtained by positive revalidation (M=21.9; SD±3.01) and fighting spirit (M=21.6; SD±3.47) strategies. The most prominent indicator of predicting a constructive style of coping with cancer was the coping strategy described as Positive Revalidation (ßeta=0.38; R²=0.41); whereas an Acceptance strategy was a predictor of a destructive style (ßeta=-0.30; R²=0.31). Conclusions. Most women facing gynaecological cancer experience a high level of stress, and try to use active strategies to deal with the disease. The results indicate that there is a need to offer special psychological care to oncologically diagnosed women.

Introduction and objective. Matrix metalloproteinases (MMPs) are proteolytic enzymes responsible for the decomposition of extracellular matrix elements. They play an important role during embryogenesis, wound healing, endometrial epithelial exfoliation, the formation of new blood vessels, and also during cancer development. Throat and larynx tumours are included in a large group of head and neck cancers. These tumours are characterized by a poor prognosis. Despite advances in medical science, cancer treatment is difficult and often ineffective. The aim of the study was to evaluate the level of MMP-3 and MMP-9 in the serum and saliva of patients with pharynx and larynx tumours. Materials and method. Samples of saliva and serum were collected from 60 patients with larynx or throat cancer. Twenty patients without cancer comprised the control group. MMPs in saliva and serum were determined by the ELISA method. Results. In the study group, concentrations of MMP-3 in saliva were from 0.2 – 77.6 ng/ml. Patients with malignant tumours had higher saliva MMP-3 levels than healthy subjects. The concentration of MMP-3 in the serum of the study group ranged from 10.9 – 200.00 ng/ml, which was also higher than in the control group. There were no statistically significant difference in the MMP-9 level between the study and control groups (both in serum and saliva). Conclusions. This study is another element that shows the phenomena taking place at the cellular level during oncological disease. In serum and saliva samples, higher values of MMP3 were found in patients with cancer. The increase in the concentration of this enzyme in the risk group may be used for early detection of tumour transformation and evaluation of treatment.

Introduction and objective. The integrity, stability and composition of sperm chromatin are of great importance in the fertilizing potential of male gametes and their capacity to support normal embryonic development. In this study, the author presents the current state of knowledge about the sperm epigenetic profile and risk of cancer. Abbreviated description of the state of knowledge. The obtaining of pregnancy and the state of health of the baby depends on the quality of the genetic material of both the female and the male. Health behaviours and environmental factors directly affect the quality of sperm, as well as the human egg cell and, consequently, on the reproductive capabilities, the course of pregnancy and the state of the newborn. There exist two thoroughly investigated epigenetic modifications: DNA methylation and histone modifications. The process of DNA methylation can be also a fundamental factor contributing to the development of cancer, where epigenotype undergoes significant modifications. When considering numerous DNA aberrations in the male gamete, the most commonly encountered is DNA fragmentation, particularly in infertile subjects. Surprisingly, an intracytoplasmatic sperm injection study of mice oocytes, using spermatozoa with a high DNA Fragmentation Index (DFI), revealed that a considerable percentage of adults born as a result of this method, showed a significant increase in the incidence of abnormal behavioural tests, malformations, cancer and signs of premature aging. Summary. The issue of assisted procreation raises the need to look for an appropriate treatment for males with sperm chromatin abnormalities. As a result, the fight against smoking addiction becomes the obvious necessity. Moreover, the reasonable solution nowadays seems to be supplementation with micronutrients and folic acid. It has been proved that the process of DNA fragmentation is a phenomenon that intensifies over time. Therefore, there should be a pursuance for, as close as possible, to the moment of ejaculation, application of semen to reproductive techniques. Finally, epigenetic changes are suspected of being one of the factors responsible for the deterioration of male sperm parameters observed in recent decades.

Introduction. Neoplastic diseases are a common cause of death, especially in developed countries. Various methods are used in cancer therapy including natural substances. Terpenes and terpenoids play a special role, including betulin and betulinic acid with the pentacyclic structure of lupane. They have a wide range of pharmacological activity, among which anti-cancer activity attracts the greatest attention. Abbreviated description of the state of knowledge. The review discusses the pharmacological effects of betulin and its derivatives on tumours, and focuses on the potential of its application in the treatment of melanoma. The review study also presents future prospects for the use of this natural substance. Betulin and its derivatives have the ability to influence the processes of apoptosis, angiogenesis and autophagy. Additionally, they have the ability to sensitize, increasing the sensitivity to commonly used methods of cancer treatment. To-date, numerous studies have been carried out on the synthesis of betulinic acid derivatives, which would show greater polarity and activity compared to the starting compound. Due to their greater bioavailability, the modified compounds showed a stronger anti-tumour effect. Conclusion. The results of the above-mentioned studies confirm the anti-cancer properties of betulin. Regarding melanoma, betulin shows significant therapeutic efficacy. This gives new perspectives for the treatment of the most dangerous skin cancer.

Introduction and objectives. Breast cancer (BC) is the most common malignancy and the leading cause of cancer death among women. About 15–20% of all BCs are HER2-positive. Proper assessment of HER2 status is crucial to choose appropriate treatment. The review summarizes data on anti-HER2 drugs used to treat HER2-positive BC and provides basic information on new agents under investigation. Brief description of the state of knowledge. Specific HER2-targeting drugs are available or are being evaluated in clinical trials. Anti-HER2 agents include: monoclonal antibodies, tyrosine kinase inhibitors, antibody-drug conjugates, bispecific antibodies, PI3K/AKT/mTOR inhibitors and heat shock protein 90 inhibitors, HER2-targeting vaccines and CDK4/6 inhibitors. The advent of anti-HER2 therapies increased the time of progression free survival and overall survival in BC patients. Results. Final analysis of the CLEOPATRA trial shows that the combination of trastuzumab, pertuzumab and taxane significantly improved outcomes in metastatic HER2-positive BC and it is currently preferred first-line treatment. The recommended second-line treatment is based on trastuzumab emtansine or on the combination of lapatinib and capecitabine. Some promising agents such as margetuximab or trastuzumab deruxtecan are still under investigation. Conclusions. Anti-HER2 directed treatment undoubtedly improves outcomes among patients with HER2-positive BC. Access to drugs such as trastuzumab, pertuzumab, lapatinib and T-DM1 improves prognosis even in patients with advanced disease. Further studies and clinical trials on novel anti-HER2 therapies are required. Nevertheless, BC treatment is becoming more effective and, hopefully, one day it may be possible to cure patients even with metastases.

Introduction and Objective. Locally advanced rectal cancers witha high risk of recurrence need multimodal treatment Rusing neoadiuvant preoperative chemoradiotherapy or preoperative radiotherapy. Both rectal cancer surgery and the additional chemoradiotherapy or radiotherapy may cause late sequelae. The aim of the study is to present a clinical situation in which a pelvic recurrence of rectal cancer has to be distinguished from therapy side effects. Insufficiency fracture (IF) is one of the therapy side-effects. It can cause pain and decrease mobility and it is a well-known late complication to pelvic radiotherapy, but can be misinterpreted as a local recurrence. Conclusions. It is extremely important to distinguish IF from metastases, which may require biopsies and initiation of potentially toxic treatments such as chemotherapy. MRI and CT scans are complementary modalities to make an accurate diagnosis of IF.

Introduction and objective. Phytoestrogens are one of nutritional factors exhibiting a chemoprotective action, potentially reducing vasomotor symptoms as well as cancerous lesions. The study was ocused on an association between the consumption of phytoestrogens in the diet, blood serum cholesterol concentration and urinary estrogen metabolites levels. Materials and methods. The study was conducted on 55 women in the perimenopausal period, who were characterized by an increased risk of hormone-dependent cancer. Energy and nutritive value of food rations, and especially the level of phytoestrogen consumption in the diets in the examined population, were analyzed using the Wikt Pro computer programme. Estrogen metabolites (2OHE, 16α-OHE1) were determined from 24‑urine samples by the ELISA method. The EMR index was calculated. Plasma total cholesterol concentrations was measured. Results and conclusions. The population with a higher consumption of phytoestrogens and dietary fibre in the diet, at the simultaneous slight limitation of energy value of the food ration, is characterized by an improved blood serum cholesterol concentration and an appropriate estrogen metabolism towards a significant increase in the estrogen index for the cancer lesions in the mammary glands.

Introduction and objective. Ovarian cancer (OC) is the third most commonly diagnosed gynecological cancer among women worldwide and the second most common in Poland. Early-stage ovarian cancer is still very difficult to diagnose and concerns only about 20–30% of all ovarian cancers. Most cases (approximately 70%) of ovarian cancer are diagnosed at more advanced stages (III and IV). The aim of the review is to bring closer new potential biological markers – Nidogen-1 and Nidogen-2 in the diagnosis of ovarian cancer. Brief description of the state of knowledge. To date, the best serum marker for ovarian cancer is Ca-125, but its use as a screening marker is limited due to high false positive rates. Ca-125 could be elevated in other benign and malignant conditions. Serum concentrations of Nidogen-1 and Nidogen-2 are higher in the advanced stagegroup (Stage III and IV), in comparison to the early stage group (Stage I and II) in serous ovarian cancer, and can reflect the tumour burden. Analysis showed that Nidogens discriminate against patients with serous ovarian carcinomas from healthy controls. The concentrations of both of them correlate with concentration Ca-125, especially Nidogen-2. The above biomarkers were compared with the results of the preoperative detection of ovarian cancer that are often used in clinical practice – IOTA Simple Rules, Risk of Malignancy Index and Risk of Ovarian Malignancy Algorithm. Conclusions. Nidogen-1 and Nidogen-2 are new promising biomarkers for ovarian cancer, especially for the serous type, although there is still a need for prospective studies proving their good diagnostic value.

Introduction. Cervical cancer is the fourth most common cancer among women. It is related to persistent HPV infection. In order to improve diagnostic methods, a lot of research has been focused on detecting HPV DNA. A test known as a high- risk HPV test or HPV primary screening provides very encouraging results. Objective.The aim of this review is to present the actual knowledge about the possibilities of cervical cancer screening methods. Particular attention is paid to the question concerning the effectiveness of detecting viral DNA as a screening programme, compared to pap smear. State of knowledge. The HPV primary screening has higher sensitivity than the cervical smear test and it is able to detect lower-stage lesions, which are considered clinically irrelevant. Nonetheless, many HPV infections recede naturally. Therefore, relying only on the results of this test may expose women to unnecessary colposcopies and stress. Due to this fact, women under 30 years should not be screened with the hrHPV test. In view of its limitations, the HPV primary screening strategy is still tested worldwide as a pilot project. In Poland in 2019, a pilot project with the hrHPV test started at the National Institute of Oncology. Conclusions. Detecting viral DNA has its advantages and disadvantages. Further research is still required, but the hrHPV test has a great opportunity to become one of the main screening programmes worldwide, or at least, a valuable addition to cervical smear test.

Pulmonary lymphangitic carcinomatosis is a special type of diffuse metastasis of carcinoma in the lymphatic vessels of the lungs. Lymphangitic carcinomatosis is commonly observed in malignancies of the breast, lung, pancreas, colon and cervix as a strong marker for poor prognosis. Presenting with common respiratory symptoms, it may be easily misdiagnosed as other pulmonary interstitial diseases. Pulmonary lymphangitic carcinomatosis is a rare manifestation of metastatic gastric cancer. The presented case describes a patient with pulmonary lymphangitic carcinomatosis in the course of stomach cancer. The primary diagnosis was established based on the exclusion of other interstitial lung diseases, with the use of imaging techniques as well as biochemical, microbiological and cytological findings. The patient’s general condition was very severe, rendering him bedridden and therefore not eligible for any treatment, including chemotherapy. He died suddenly before final verification of the diagnosis. Pulmonary lymphangitic carcinomatosis should be suspected in patients with advanced gastric cancer, presenting with symptoms and signs of respiratory disease. Imaging techniques are mostly helpful to establish the diagnosis.

Introduction. Abdominal ultrasound is considered an initial examination of the the gastrointestinal tract. The case is presented in which ultrasound (US) detected a ‘pseudokidney’ sign that enabled the tentative diagnosis of a colonic neoplastic lesion. Case report. A 78-year-old patient was admitted to the Department of Internal Diseases due to weight loss and anaemia. US demonstrated abnormalities in the form of intestinal wall thickening with a hyperechoic centre. This finding led to the tentative diagnosis of a neoplastic lesion. After further diagnostic procedures, i.e. colonoscopy and histopathological examination of biopsy specimens, the patient was diagnosed with colon adenocarcinoma. Conclusions. In many cases, experienced US radiologists can initially assess the intestinal wall and other organs, which effectively directs towards further diagnostic management, hence accelerates the diagnosis and decision to apply a given therapeutic option or otherwise.

Introduction and objective: The risk of development colon cancer (CC) is increased significantly among patients with the type 2 diabetes (T2DM). A mechanism responsible for the higher prevalence of CC among diabetic patients may be associated with the immunity system. The aim of this study is to point out the differences in the immunity state in terms of interleukin 2 level among patients with T2DM suffering from CC, and patients without these diseases Material and Methods: 79 patients were included the tests, divided into 4 groups: Group 1–23 people with T2DM, Group 2–23 people with large intestine CC, Group 3–10 people with large CC and T2DM, and Group 4–23 people without T2DM or CC. Each patient had a colonoscopy and those with cancer were confirmed in a histopathological examination. Laboratory measurements included fasting glucose, insulin, C-peptide. The concentration of interleukin-2 in serum was determined with the immunoenzymatic (ELISA) method. Results: The results obtained showed that in patients with T2DM and CC the concentration of interleukin-2 was statistically higher than in the other groups.(4.21±1.61 pg/ml vs. Group 1 -1.64±0.44 pg/ml, Group 2–1.54±0.21 pg/ml, and Group 4–1.70±0.36 pg/ml; p<0.05). Insulin levels, C-peptide and HOMA-IR did not differ significantly between groups, but a tendency was observed to higher values of HOMA-IR and insulin levels in the groups with T2DM alone and T2DM with concomitant CC. Conclusions: The data show differences in the immunity state of patients with T2DM and CC, compared with people without those two diseases. Elevated level of interleukin 2 found in this group, after confirmation in other studies with more patients, could be used as a marker of an increased risk of CC in people with T2DM

Oral epithelial dysplasia and oral cancer prevalence in routine white lesion biopsies - a 6-year retrospective study

Introduction. Leukoplakia and oral lichen planus (OLP) are common diseases manifesting as white lesions that are considered potentially malignant disorders (PMD). Epithelial dysplasia may be an early sign of potency for the future transformation into oral squamous cell carcinoma (OSCC). A routine biopsy and close observation are recommended for persistent white oral lesions. As frictional keratosis may mimic oral leukoplakia, the question arises:Is there a need for a biopsy of persistent white lesion of traumatic origin? Materials and methods. Data from 643 oral tissue biopsies were retrospectively analyzed. A total of 176 (27.37%) results with provisional diagnosis of leukoplakia (36 cases), OLP (77 cases) and frictional keratosis (63 cases) were selected. Retrospective data collected included age, gender, smoking status, provisional and histopathological diagnosis. The data was analyzed to assess the prevalence of epithelial dysplasia and OSCC in terms of age, gender and smoking status. Results. Five (2.84%) cases of OSSC were reported, all of them were graded as G1; four cases of OSCC were found in clinically defined leukoplakia lesions; one case of OSCC (1.3%) was found in OLP biopsy; epithelial dysplasia was reported in 5 lesions (2.84%) provisionally diagnosed as OLP (3 cases), and leukoplakia in 2 cases. No dysplasia or OSCC were found in the lesions diagnosed as frictional keratosis. Conclusions. Epithelial dysplasia and OSCC may be found in leukoplakia or OLP lesions not initially suspected of any malignancy. In some cases, clinical features are not sufficient to diagnose a lesion without histopathology. Frictional keratosis is easily identified by clinicians, and may not require a biopsy in every case. Clinical and histopathological evaluation of the white lesions still needs improvement.
