Opis:
Risk management is an activity which integrates recognition of risk, risk assessment, developing strategies to manage it, and mitigation of risk using managerial resources. Some traditional risk managements are focused on risks stemming from physical or legal causes (e.g. natural disasters or fires, accidents, death). Financial risk management, on the other hand, focuses on risks that can be managed using traded financial instruments. Objective of risk management is to reduce different risks related to a pre-selected domain to an acceptable. It may refer to numerous types of threats caused by environment, technology, humans, organizations and politics. The paper describes the different steps in the risk management process which methods are used in the different steps, and provides some examples for risk and safety management.

Opis:
Process of suppliers selection, as well as cooperation between suppliers and manufacturers, are a significant problem for today’s companies. Producers negotiate and arrange with their subcontractors to ensure a continuity of production. Following this, one of the major problem is supplier evaluation and supply risk assessment in order to avoid delays, which may seriously affect the producers’ ability. In the paper there are presented the main types of risk and the methods of its classification and evaluation used in logistic processes performance. The analysed problem is to be discussed on the example of Whirlpool-Polar company from Poland. For the presented company, there is investigated supplier evaluation process, suppliers classification with the relation of their purchasing and bargaining power to the degree of their obligations fulfilment. Moreover, there is defined the supply risk concept, what gives the possibility to compare the suppliers purchasing power with the defined supply risk. Later, there are presented main possibilities to reduce the supply risk by using a “milk run” method with a number of variants of the simulation and its evaluations. The presented results comply with the profitability of transport in the case of round-trip transportation performance. Transport of goods is cost effective when the usage of the truck transport capacity will be at least 85%. Following this, the presented research analysis result gives the possibility to reduce risk in terms of reducing disruptions occurrence consequences, when the probabilities of their appearance remain at the same level.

A widely accepted definition of resilience is the intrinsic ability of a system to adjust its functioning prior to, during, or following changes and disturbances so that it can sustain required operational safety under both expected and unexpected conditions. These concepts have been applied in the process or manufacturing industry with different attempts in switching from the traditional risk management approach to a resilience one, still finding a lack of clarity in the definitions and in the objectives and, consequently, a lack in the methodologies and tools to support those efforts. The attempts and the need for further research or clarification is discussed in this paper.

The sources of the uncertainty in the maritime transport system are described. Furthermore, the two models for human factor reliability are presented. The possibilities of mixed these two methods as the estimators for the probability of correct performance of the navigator are given. The aim of this contribution is to focus attention on a new emerging risk known in the literature as Na-Tech risk (Natural-Technological risk). Na-Tech are technological accidents triggered by natural events, these are of particular interest for high risk industries. The magnitude of such an event is much broader than that related to a technological accident and its management is much more complex. Adequate preparedness, proper emergency planning and effective response are crucial for the prevention and mitigation of the consequences of Na-Techs This contribution gives a general overview of the methodologies, that are currently being developed, in order to integrate these types of scenarios into the conventional quantitative risk analysis for chemical and process industry.

Opis:
In most NATO countries, pilots are prepared in accordance with a modern system of three-stage training: selection, basic and advanced. For each phase, the purpose and scope of the training shall be defined and the aviation equipment together with training support systems should be appropriate selected. The practical training in the air is performed according to the parallelism of the training, which describes the technological relationships between the individual exercises. The purpose of this contribution is to propose a mathematical model which would enable the selection of exercises for each pilot, the appropriate selection of the aircraft and the moment of beginning of each exercise, so that all required exercises are carried out in the shortest possible time. Additionally, in this paper the concept of pilot’s habits and skills improvements is presented.

In the paper, definitions and theoretical results on system operations process, multi-state system reliability, risk and availability modelling are illustrated by the example of their application to a bulk cargo transportation system operating in Gdynia Port Bulk Cargo Terminal. The bulk cargo transportation system is considered in varying in time operation conditions. The system reliability structure and its components reliability functions are changing in variable operation conditions. The system reliability structures are fixed with a high accuracy. Whereas, the input reliability characteristics of the bulk cargo transportation system components and the system operation process characteristics are not sufficiently exact because of the lack of statistical data. Anyway, the obtained evaluation may be a very useful example in simple and quick systems reliability characteristics evaluation, especially during the design and improving the transportation systems operating in ports.

This article addresses selected methodological aspects of a process based management system based on analysis of hazards and threats and risk evaluation for an oil port infrastructure in context of insurance. The oil port terminal is regarded as important system of the critical infrastructure that require careful system oriented approach to deal with integrated aspects of environmental, safety and security management to reduce risk of potential consequences of abnormalities and accidents, especially major accidents with catastrophic consequences. The risk of potential economic losses should be also minimised applying in practice an effective business continuity management system. Careful evaluations of relevant risks carried out for the oil port infrastructure are crucial also for the insurance company. Some requirements and activities of the risk engineer and the underwriter in the insurance process are outlined including important factors influencing risks. It is emphasised that determining and evaluating a set of key performance indicators based on data from site audits and analyses can be useful for the safety management of the oil port and its insurance.

This paper is both a summarization and extension of [6] and [7], where a stochastic model of interacting operations carried out in a generic Baltic Sea Region port was proposed and analyzed. Each operation involves a number of possible unwanted events (critical incidents) whose instances occur randomly and can cause instances of other events affecting this or other operations. This can lead to a cause-effect chain of events affecting one or multiple operations. The model presented in [6] is somewhat complex, therefore it was downgraded to a simpler, application-oriented version demonstrated in [7], where an algorithm computing the risks of critical incidents is constructed and then applied to a real-life example. The current paper, apart from presenting a method of computing the risks of critical incidents, occurring by themselves or resulting from the cascade effect, also features a method of root-cause analysis of such incidents. First, the formulas for the root-cause probabilities are derived, where such a probability quantifies the likelihood that a critical incident occurring in step h of a cascade was caused by another incident that initiated this cascade. Second, an algorithm computing the root-cause probabilities, based on the derived formulas, is constructed. This algorithm is illustrated by its application to the example given in [7]. The presented results can be used as a tool for fault propagation analysis and fault diagnosis applied not only to a port environment, but to any complex industrial system.
