Sea borders are identified as a major entry point in EU. In order to be adequately prepared to manage illegal immigrant’s inflow and mitigate the significant socioeconomic impact. The present paper introduces a comprehensive risk analysis methodological framework for the analysis of the present situation, risk prioritization and mitigation analysis. The proposed approach will be applied to a specific site in a N. Aegean Island, Greece as the country is the access point of over 80% of all immigration flow to the EU.

One of the aims of this work is to create a method of identification of critical elements (elements or groups of elements) in the energy infrastructure, and this method should allow ranking these critical elements by relevance to consumers (the consumer could be from the systems of different energy sector).The risk estimate of an element is one of the proposed sorting criteria for critical elements or their groups. It allows assessing the importance of the combination of critical elements and takes into account the probabilities of faults of these combinations. The key result of the research is the identification of the weakest links in the system, namely those elements, the failure of which (together with other elements) would lead to the worst consequences for the consumers and response to the availability of the infrastructure element.

A widely accepted definition of resilience is the intrinsic ability of a system to adjust its functioning prior to, during, or following changes and disturbances so that it can sustain required operational safety under both expected and unexpected conditions. These concepts have been applied in the process or manufacturing industry with different attempts in switching from the traditional risk management approach to a resilience one, still finding a lack of clarity in the definitions and in the objectives and, consequently, a lack in the methodologies and tools to support those efforts. The attempts and the need for further research or clarification is discussed in this paper.

Rambøll is on behalf of Femern A/S carrying out navigational studies of the vessel traffic conditions in the Fehmarnbelt in connection with the establishment of a future Fehmarnbelt fixed link, a 19 km long bridge or immersed tunnel connection crossing the Fehmarnbelt. The focus is on documenting safety and efficiency for the vessel traffic when a fixed link is crossing the Fehmarnbelt. Rambøll has developed the ShipRisk software package to perform the quantitative risk assessments in the Fehmarnbelt fixed link project. Focus in this article is on describing the background for estimating frequencies of ship accidents in ShipRisk and describe factors influencing the accident scenarios and present the work performed for testing and verifying the model.

Opis:
Dynamic methodologies in reliability and PRA are those that explicitly account for the time element in probabilistic system evolution. Dynamic methodologies are usually needed when the system has more than one failure mode, control loops, and/or hardware/process/ software/human interaction. An overview of the dynamic methodologies proposed to date is given, including those that use dynamic event tree generation, continuous time-state space representation, the cell-to-cell mapping technique and graphical schemes. The use of dynamic methodologies for state/parameter estimation in on-line applications is also discussed. Potential on-line use of dynamic methodologies as operator assistance tools for risk informed accident management or normal operation is described and illustrated.

The Directive 99/92/EC deals with the safety and health protection of workers potentially exposed at explosive atmospheres. The application of this Directive requires the assessment of specific risks due to the presence of potentially explosive atmospheres. These can generally be originated by releases of flammable substances typical of industries classified at major hazard, but they often could be generated also in all other industries where flammable materials are handling. Risk assessment of explosive atmospheres is required in both the cases, to this purpose, in this article a quantitative approach has been proposed. The paper describes the main aspects of the methodology, based on a probabilistic risk assessment, and finally its application to a case-study.

In most NATO countries, pilots are prepared in accordance with a modern system of three-stage training: selection, basic and advanced. For each phase, the purpose and scope of the training shall be defined and the aviation equipment together with training support systems should be appropriate selected. The practical training in the air is performed according to the parallelism of the training, which describes the technological relationships between the individual exercises. The purpose of this contribution is to propose a mathematical model which would enable the selection of exercises for each pilot, the appropriate selection of the aircraft and the moment of beginning of each exercise, so that all required exercises are carried out in the shortest possible time. Additionally, in this paper the concept of pilot’s habits and skills improvements is presented.

Opis:
Monitoring & Detection (M&D) systems are introduced in almost all daily life aspects. Today, it is unlikely to find out a system that is not equipped with a sort of M&D apparatus, even the most ordinary and uncritical systems. Objective assessments of the performances of M&D systems are increasingly needed. Performance assessment may be motivated by commercial interests or by certification concerns if the application field is ordinary or uncritical. While, if the application filed is critical, safety and security aspects may become the major focus. The paper classifies in three classes the models that are the most-frequently used to describe M&D systems. The author proposes for each class a suitable indicator of performance based on M&Dsystems reliability targets.

The scope of the paper is to introduce the solution for improving the risk management methods for seaports. First, the paper presents general information about ports and their main business scope. Next, we introduce the definition of critical infrastructure (CI) according to the EU directive, and we apply it to seaports. The paper also presents the basic concepts of dependencies and interdependencies and introduces a general classification of natural hazards and threats of influencing ports. Moreover, the multiple criteria decision analysis, particularly the application of the AHP method, is presented as the solution for improving seaports risk assessment based on scenarios. Finally, some examples of different approaches for risk assessment improving are presented.

It is state-of-the-art that a Level 1 PSA also includes a fire PSA for all plant operational states. In performing Fire PSA not only the malfunction of the components has to be assessed but also all supply systems and cables have to be traced for a given component. In the past it was assumed in the case of a fire in a compartment that all components and cables in that compartment are not functional anymore. However, this is in many cases a too conservative approach and could lead to overestimated fire induced core damage frequencies. Therefore, a method is necessary to assess in a more realistic manner the effects of cables failures caused by fire. Such a procedure requires a sound data base on equipment, list of cables and their properties as well as cable routing. Two methods are described which are currently developed. One of them is a cable failure mode and effect analysis which is easier to apply in practice.

This paper deals with the modelling and analysis of the reliability of complex systems that use one-shot items during their operation. It includes an analysis of the impact of the reliability of used one-shot items on the resulting reliability of the system as a whole. Practical application of the theoretical knowledge is demonstrated using a model of reliability of an aircraft gun that was used for optimization of the gun’s design during its development and design. Furthermore this paper demonstrates utilization of a proposed model to determine an optimum number of pyrotechnic cartridges that will achieve a required probability of fulfilment of the mission with the given reliability of individual subsystems of the gun, rounds and pyrotechnic cartridges. The proposed procedures of modelling and analysis of the reliability of complex systems with oneshot items were used in practice in designing and development of the PL-20 aircraft gun. Development of the gun was successfully finished in 2005, and the gun was fielded in the armament of the Czech Republic Air Force.

Operating experience from different types of industrial installations has shown that combinations of different types of different hazards occur during the entire lifetime of the installations. Typically site specific occurring hazards cause or induce other hazards to occur. In particular, natural hazards rarely happen alone. Thus, it is very important to note that almost any event combination of hazards is possible and that it is necessary to identify these interactions and find ways to mitigate the effects of hazard combinations. Therefore, it is a basic task to investigate and assess the relevant combination of hazards not only for a single installation but for the respective site/industrial park. In that context domino effects and cascade effects pose particular challenges for risk management to prevent industrial accidents.

The International Atomic Energy Agency has started a few years ago a project to enhance the existing framework for safety goals for nuclear installations and currently is finalizing a publication aimed to provide guidance for a more holistic approach to establishing and utilizing safety goals for nuclear installations. The intention was to develop a more consistent and holistic framework for safety goals that would be composed of a hierarchical structure of qualitative concepts (e.g. defense in depth, various safety requirements) and quantitative risk metrics. The safety goals should be technology neutral at the higher level and should cover both normal operation and accident conditions as well as the risk to workers, public, and the environment. In the paper the development of a hierarchical framework for safety goals is described including a methodology to derive lower-(more technical) safety goals from the higher goals. Moreover a process for assessing the degree of compliance with the safety goals and their consistent use is provided. As an example the developed framework is applied to the German regulatory framework for nuclear installations but focused on nuclear power plants.

Reliable operation of contemporary complex systems depends on selecting efficient maintenance policy, which often must take into account not only the reliability, but also economic factors. In this work, we present an approach which allows evaluation of various possible maintenance scenarios with respect to these two areas. The method is based on the concept of a life curve and discounted cost used to study the effect of equipment aging under different maintenance strategies. The deterioration process is first described by a Markov model and then its various characteristics are used to develop the equipment life curve and to quantify other reliability parameters. Based on these data, effects of various “what-if” maintenance scenarios can be examined and their efficiency compared. Simple life curves are combined to model equipment deterioration undergoing diverse maintenance actions, while computing other parameters of the model allows evaluation of additional critical factors, such as probability of equipment failure. Additionally, the paper deals with the problem of the model adjustment so that the computed frequencies are close to the historical values, which is very important in practical applications of the method.
