Opis:
Assurance of data integrity is one of the prerequisites for each computer system. The paper presents a method enabling on-line maintenance of stream data set integrity. This method is implemented in a data stream management system prototype designed to find application in a biomedical monitoring system. In the case of medical computer systems assurance of data integrity is particularly important for documenting formal results and for the patient's safety.

Opis:
Cardiotocography is a biophysical method of fetal monitoring during pregnancy and labour. It is mainly based on recording and analysis of fetal heart activity. The computerized fetal monitoring systems provide the quantitative description of the recorded signals but the effective methods supporting the conclusion generation are still needed. The evaluation of the signal can be made using criteria recommended by FIGO. Nevertheless, the quantitative description of the traces is inconsistent with qualitative nature of the obstetric knowledge. Therefore, we applied the fuzzy system based on Takagi-Sugeno-Kang model to evaluate and classify signals. FIGO guidelines were used for developing a set of fuzzy conditional rules defining the system performance. The proposed system was evaluated using data collected with computerized fetal surveillance system – MONAKO. The classification results confirm the improvement of the fetal state evaluation quality while using the proposed fuzzy system support.

Opis:
Many digital images, especially in biomedical fields, contain some disturbances. The image analysis depends on quality of the images that is why reduction or elimination (if it is possible) the disturbances is the key issue. There are many methods of improvement in the quality of the images and thus improve the quality of the image analysis, among them one of the simplest method is low-pass filtering such as arithmetic mean or its generalization, weighted mean. The basic problem of the weighted mean is the proper selection of the weights. This can be done using adaptive algorithms. This paper presents several such algorithms which are modifications of the existing weighted averaging methods created originally for noise reduction in electrocardiographic signal. The description of the new filtering methods and a few results of its application are also presented with comparison to existing arithmetic average filtering.

Cardiotocography (CTG) is a routine method of fetal condition assessment used in modern obstetrics. It is a biophysical method based on simultaneous recording and analysis of activity of fetal heart, fetal movements and maternal uterine contractions. The fetal condition is diagnosed on the basis of printed CTG trace evaluation. The correct interpretation of CTG traces from a bedside monitor is very difficult even for experienced clinicians. Therefore, computerized fetal monitoring systems are used to yield the quantitative description of the signal. However, the effective methods, aiming to support the conclusion generation, are still being searched. One of the most important features defining the state of fetal outcome is the weight of the newborn. The presented work describes an application of the Artificial Neural Network Based on Logical Interpretation of fuzzy if-then Rules (ANBLIR) to evaluate the risk of the low birth weight using a set of parameters quantitatively describing the CTG traces. The obtained results confirm that the neuro-fuzzy based CTG classification methods are very efficient for the prediction of the fetal outcome.

Opis:
Cardiotocographic monitoring (CTG) is a primary biophysical monitoring method for assessment of the fetal state and is based on analysis of fetal heart rate, uterine contraction activity and fetal movement signals. Visual analysis of CTG traces is very difficult so computer-aided fetal monitoring systems have become a standard in clinical centres. We proposed the application of neural networks for the prediction of fetal outcome using the parameters of quantitative description of acquired signals as inputs. We focused on the influence of the gestational age (during trace recording) on the fetal outcome classification quality. We designed MLP and RBF neural networks with changing the number of neurons in the hidden layer to find the best structure. Networks were trained and tested fifty times, with random cases assignment to training, validating and testing subset. We obtained the value of sensitivity index above 0.7, what may be regarded as good result. However additional trace grouping within similar gestational age, increased classification quality in the case of MLP networks.

Opis:
The method of objective assessment of the correctness mandible bones fracture slit was proposed in the research. At present, treatment results are evaluated by comparing pantomographic pictures made before and after integration. It is not an objective method; the estimation of the integration correctness coefficient is not possible, either. The use of computer processing enables introducing the relative mandible faction dislocation coefficient. Thanks to this, measurable determination of bone faction reposition correctness on pantomograms made after osteosynthesis is possible.

This study is aimed at evaluation of the capability to indicate the preterm delivery risk analysing the features extracted from signals of electrical uterine activity. Free access database was used with signals acquired in two groups of pregnant women who delivered at term and preterm. Signal features comprised classical time domain and spectral parameters of contractile activity, as well as the sample entropy. Their mean values were calculated over all contraction episodes detected in each record and their statistical significance for separating the two groups of recordings was provided. Influence of electrodes location, band-pass filter settings and gestation week was investigated. The obtained results showed that a spectral parameter – the median frequency was the most promising indicator of the preterm delivery risk.

Opis:
Pulse oximetry is a non-invasive measurement method of an arterial blood oxygen saturation. Pulse oximeter is one of the main bedside monitors used in neonatal units. The dispersed location of medical devices in such unit renders impossible to make a fast and valid decision in the case of simultaneous alarming situation. This paper present hardware and software structure of centralised neonatal monitoring system. Presented system can work as a single central station for monitoring of up to twenty four patients. Application of central monitoring system decreases the risk of undetected patients live and health threats. Thanks to this system the medical stuff is less burdened by routine procedures; therefore the medical unit is perceived as child- and mother-friendly.

This work was aimed at assessment of usability of Doppler ultrasound method for the analysis of fetal heart rate variability at a level of single heart beats. The purpose of this study was to check if today fetal monitors provide the signal of accuracy comparable with an electrocardiography method. The virtual instrumentation software for measurement system and processing of acquired signals was implemented using LABVIEW environment. The results obtained reveal that accuracy of today monitors which use Doppler ultrasound technique is sufficient for visual analysis of the FHR traces. However, for the computerized analysis at a beat-to-beat level, this accuracy is below the acceptable value.

Cardiotocography as a simultaneous recording of fetal heart rate (FHR) and uterine contraction activity is a basic method of evaluation of fetal condition. Correct variability of the fetal heart rate is an indirect sign of adequate oxygenation of a fetus. Unfortunately, the reverse case is not always true, signs suggesting pathological changes can also appear in recording when the fetal is not at risk. The cardiotocography shall then be recognized as a more screening than diagnostic method. It will be interesting to develop a non-invasive method being complementary to routine cardiotocography. This method should allow the adequate prediction of a bad neonatal outcome when the test is abnormal. The paper shows the system that makes possible cardiotocograms analysis in parallel with the assessment of additional parameters determined from comparison of mechanical and electrical fetal heart activity signals. The studies are aimed at development of set of parameters that are high correlated with clinical outcome.

Opis:
At present, in mandibular fractures surgical treatment reposition and titanium miniplates osteosynthesis is the most common method used. The treatment results are assessed on the basis of comparative analysis of two radiograms taken before and after fixations of fractures. In this article the objective method of radiological assessment of osteosynthesis is presented. The crucial part of the method is the computer program which was created at The Institute of Computer Biomedical Systems, Silesian University in Katowice. Comparison of placement of bone fragments on a pair of pantomograms taken before and after a particular surgical operation was done. To make the process of the results evaluation an objective one a new parameter - the relative dislocation coefficient - was implemented. Achieved results enable to assess correctness of bone fracture reposition on the pantomographical pictures taken twice, once before and the next time after osteosynthesis.

The paper presents some aspects of telemedical fetal monitoring where the biomedical signals are acquired remotely at patient’s home and wirelessly transmitted to the central computer through the GSM network and Internet. Signals are acquired during the monitoring sessions, and along with analysis results, they create uniform data structure describing the medical history of patient since the first monitoring to the labour. The proposed structure of database was based on the currently used in centralized monitoring systems, and extended by information essential for remote monitoring purposes. Internet-based telemedical systems allow for remote access to collected data, however, it is necessary to secure the database against unauthorized access to patient’s personal data and medical history. The proposed structure of database records allows for easy searching and viewing of patient data forms and monitoring traces. These features cause that the database constitutes valuable research material, which enables to relate parameters of particular monitoring records, acquired during pregnancy, to the real fetal outcome.

Opis:
The paper presents a personal identification method based on lips photographs. This method uses a new approach to the extraction and classification of characteristic features of the mouth from photographs. It eliminates the drawbacks that occur during the acquisition of lip print images with the use of the forensic method that requires special tools. Geometrical dimensions of the entire mouth as well as of the upper and lower lips were adopted as the features, on the basis of which the verification is performed. An ensemble classifier was used for the classification of the features obtained. The effectiveness of the classifier has been verified experimentally.

The paper presents the algebra and declarative query language developed for fetal monitoring system MONAKO. At present, the system architecture is centralised. Recording, analyzing and visualization of data are carrying out in the central unit. Therefore, the system cannot scale in a simple manner. The most important features indicating appropriate fetal development are the measures of instantaneous variability of a fetal heart rate (FHR), describing fluctuations of the beat-to-beat heart intervals. The most popular method for the FHR acquisition is the Doppler ultrasound technique. However, it is very sensitive to various motion artifacts distorting the signal being acquired. The aim of our work was to evaluate the influence of signal loss episodes on the parameters quantitatively describing the instantaneous variability of the FHR. For this purpose we artificially inserted signal loss episodes to the recordings, in different patterns and percentage, in accordance with the real characteristics of the signal loss segments. We particularly would like to answer the question if the signals with significant amount of signal loss can be reliably evaluated by means of instantaneous variability measures, and which of these measures (numerical indices) are more robust to the missing values.
