Opis:
Objectives This study has researched the significance of histologically raised findings and lung dust analyses in the context of claiming the recognition of and thus compensation for an asbestos-associated occupational disease. Material and Methods For this approach, all findings from the German Mesothelioma Register in 2015 that included lung dust analyses were evaluated and were compared with information on asbestos fiber exposure at work based on fiber years, and with the results of radiological findings. Results For 68 insured persons, recognition of an asbestos-induced lung disease according to Section 4104 of the German Ordinance on Occupational Diseases (Berufskrankheitenverordnung – BKV) could be recommended solely on the basis of the histological examinations of lung tissues and complementary lung dust analyses. Neither did the calculation of the cumulative asbestos dust exposure at work yield 25 fiber years, nor could bridge findings (e.g., plaques) be identified. In addition, the autopsies of 12 patients revealed plaques that had not been diagnosed during radiological examinations. These results show that – irrespective of the prescribed working techniques and radiological diagnosis – pathological/anatomical and histological diagnostics are often the only way for the insureds to demonstrate the causal connection between asbestos and their disease. Even after long intervals of up to 40 years post last exposure, the asbestos fibers would still be easily detectable in the lung tissues evaluated. Conclusions Whenever suitable tissue is available, it should be examined for mild asbestosis with the aid of a lung dust analysis. Otherwise there is a risk that an occupational disease is wrongfully rejected. In the context of health insurance, the lung dust analysis and the resulting proof of the presence of asbestosis often constitute one option of providing evidence of an occupational disease. ObjectivesThe purpose of this study was to characterize a new respiratory syndrome associated with exposure to a humidifier disinfectant (HD) in South Korea that is distinct from the well-known HD-related lung injury (HDLI). The authors identified this condition in 24 study subjects who were family members of patients with definite or probable HDLI (referred to as index cases), and were exposed to HD in the same room as the index cases.Material and MethodsThe authors reviewed medical records of 236 family members in 110 families who were exposed to HD in the same rooms and residences as the index cases.ResultsThey identified 24 family members who were exposed to HD in the same rooms and residences as the index cases, and who developed respiratory disorders that were distinct from HDLI. The clinical signs and symptoms of these individuals were in the upper respiratory tract, such as allergic rhinitis and croup, or in the lower respiratory tract, such as bronchitis and pneumonia. The diffusing capacity of the lung fordetermicarbon monoxide was reduced in 9 of 12 children (data not available for 1 child), and in 4 of 5 adults (data not available for 6 adults). The percent forced vital capacity and forced expiratory volume in one second/forced vital capacity ratios were within the normal ranges in most patients. The computed tomography findings, which mostly indicated non-specific inflammation, were inconsistent with the radiological diagnostic criteria for HDLI, but were abnormal in 8 of 11 adults, and in 2 of 13 children.ConclusionsThe authors propose a new condition, i.e., HD-related respiratory syndrome (HDRS), which is characterized by mild to moderate or atypical respiratory symptoms and signs, and is related to HD exposure, but is distinct from HDLI. The recognition of HDRS may provide a basis for understanding the natural history of HD-related respiratory problems, and for capturing the whole spectrum of HD-related clinical manifestations in the respiratory tract.

Opis:
Objectives Radon concentrations for 31 Polish underground tourist routes were analyzed. The equivalent dose to the lung, the effective dose and the relative risk were calculated for employees of the analyzed routes on the grounds of information on radon concentrations, work time, etc. Material and Methods The relative risk for lung cancers was calculated using the Biological Effects of Ionizing Radiation (BEIR) VI Committee model. Equivalent doses to the lungs of workers were determined using the coefficients calculated by the Kendall and Smith. The conversion coefficient proposed by the International Atomic Energy Agency (IAEA) in the report No. 33 was used for estimating the effective doses. Results In 13 routes, the effective dose was found to be above 1 mSv/year, and in 3 routes, it exceeded 6 mSv/year. For 5 routes, the equivalent dose to lungs was higher than 100 mSv/year, and in 1 case it was as high as 490 mSv/year. In 22.6% of underground workplaces the risk of developing lung cancer among employees was about 2 times higher than that for the general population, and for 1 tourist route it was about 5 times higher. The geometric mean of the relative risk of lung cancer for all workers of underground tourist routes was 1.73 (95% confidence interval (CI): 1.6–1.87). Routes were divided into: caves, mines, post-military underground constructions and urban underground constructions. Conclusions The difference between levels of the relative risk of developing lung cancer for all types of underground tourist routes was not found to be significant. If we include the professional group of the employees of underground tourist routes into the group of occupational exposure, the number of persons who are included in the Category A due to occupational exposure may increase by about 3/4. The professional group of the employees of underground tourist routes should be monitored for their exposure to radon. Int J Occup Med Environ Health 2017;30(5):687–694

Opis:
Ethylene phosphorodifluoridite (C2H4F4O2P2) (CAS No. 3965-00-2) is a colorless corrosive fuming liquid that is used as a stabilizer in the electrolyte of rechargeable batteries. There are no previous reports of toxic effects following exposure to this compound. A 28-year-old male complained of respiratory distress after accidental inhalation of ethylene phosphorodifluoridite for 30 min. The patient developed acute lung injury with noncardiogenic pulmonary edema and was treated with supportive management. The patient fully recovered and was discharged after 7 days without any significant sequelae. The patient’s symptoms were attributed to non-cardiogenic pulmonary edema caused by ethylene phosphorodifluoridite exposure. The case study showed that ethylene phosphorodifluoridite should be added to the list of chemicals that can cause acute lung injury.

Objectives To find possible relationship between asthma exacerbation and metabolomic profile of airways, assessed by non-invasive method – free volatile organic compounds (VOCs) in exhaled air in children. Material and Methods The study included 80 children aged 4–18 years with asthma: 42 children with a min. 3 asthma exacerbations in the past 12 months, and 38 children without a history of exacerbations in the past year. During the study visit, each patient was examined, medical history (including information regarding atopy and eosinophil blood count) was taken, spirometry and fractional exhaled nitric oxide (FeNO) were tested, an exhaled air sample was taken to test for the presence of VOCs, and the patient also completed standardized form – Asthma Control Questionnaire. Volatile organic compounds were measured by combined gas chromatography coupled to mass spectrometry. Results The obtained results of VOCs were correlated with the history of the disease. The 2 gas profiles were defined and they formed 2 clinically distinct clusters (p = 0.085). Cluster 2 was characterized for children with a higher number of bronchial asthma exacerbations and worse lung function parameters (predicted percentage forced expiratory volume in 1 s [FEV1] [p = 0.023], FEV1/ forced vital capacity ratio [FVC] [p = 0.0219]). The results were independent of the age, sex, BMI, atopy (house dust mite allergy) and eosinophil blood count. Conclusions The study findings suggest that a relative group of gases may be a useful predictor of having asthma exacerbations in children. Additionally, a single FeNO value was unlikely to be clinically useful in predicting asthma exacerbations in children. The VOCs profile reflecting the metabolism of the airway epithelium and local microbiota was associated with the course of asthma, which strongly justifies further prospective validation studies.

Objectives The correlation between quartz dust concentrations in the Swiss enterprises and the incidence of silicosis and other related diseases acknowledged as occupational diseases (OD) was investigated. Material and Methods Quartz dust concentrations were obtained from Suva’s databases of occupational health surveillance measurements between 2005 and 2014. Information on quartz dust-related diseases was from medical dossiers of workers with OD acknowledged by Suva between 2005 and 2014. Results The median quartz dust concentration of the 2579 measurements between 2005 and 2014 was 0.09 mg/m³ (alveolar fraction). Out of all measurements, 28% were above the Swiss occupational exposure limit (OEL) of 0.15 mg/m³ (alveolar fraction). One hundred eighty-one individuals suffered from acknowledged quartz dust-related disease (179 silicosis and 2 chronic obstructive pulmonary disease (COPD)). Additionally, 8 out of these workers were diagnosed with lung cancer and 55 with COPD of a non-specified cause. Out of all workers, 46% were exposed to silica dust for the first time before 1975 when the current Swiss OEL was introduced. Out of the foreign workers, 63% began to work abroad, during which they could have at least partly acquired their silicosis. Out of all workers, 75% were ever-smokers. Conclusions The incidence of silicosis decreased drastically from approximately 300 cases/year in the 1970s to fewer than 20 cases/year 20 years ago. Several findings of this study that could help to interpret the ongoing occurrence of the disease include excessive exposure in or outside of Switzerland in former or current times, vulnerability to the development of silicosis due to cigarette smoke, or poor compliance with wearing breathing masks. Objective: The aim of the study was comparison of cancer incidence risk of lungs, stomach, colon, bladder and kidneys from ex-miners of black coal mines and the general male population of the Czech Republic. Materials and methods: The analysis was conducted in two cohorts of ex-miners according to the presence of coalworkers’ pneumoconiosis (CWP). The first cohort included the miners without CWP (N = 6705), and the second cohort included the miners who were compensated for CWP (N = 2158). Personal and occupational data was merged with the data in the National Population Register and the National Oncological Register for the period from 1992 to 2006. Cancer risk in miners in comparison to the general male population of the Czech Republic was evaluated by SIR (Standardized Incidence Ratio) and 95% confidence interval (CI). Results: About twice as high risk of lung cancer was found in miners with CWP (SIR = 2.21; 95% CI: 1.75–2.76). Lung cancer risk correlated with the severity of CWP (simple CWP SIR=1.96; 95% CI: 1.48–2.56, progressive massive fibrosis SIR = 4.29; 95% CI: 2.09–7.87). No increased risk of lung cancer was found in ex-miners without CWP. The risk of malignant neoplasm at the other selected sites was comparable with the risk in the general male population of the Czech Republic. Conclusions: This study found increased lung cancer risk in coal miners with CWP, but not in those without CWP, comparing with the general population. These results served as the basis for the inclusion of lung cancer in association with CWP into a new Czech list of occupational diseases.

Opis:
Objectives The aim of the paper is to explore the role of lung microbiome disorder in lung tissue injury induced by exposure to particulate matter with a maximum diameter of 2.5 μm (PM2.5) and the alleviation effect of auricularia auricular-judae polysaccharide (AAP). Material and Methods Sprague Dawley rats were given PM2.5 suspension at a dose of 20 mg/l twice a week for 8 weeks. Then, 100 mg/kg or 200 mg/kg of AAP was administered to the rats after PM2.5 exposure. The bronchoalveolar lavage fluid (BALF) and lung tissue samples were collected at the end of the experiment. The BALF was meant to detect changes in lung microbiome by 16S sequences and cluster analysis, with the application of the principal component analysis and the partial least squares discriminant analysis. The levels of interferon-γ (IFN-γ), and interleukin (IL)-4, IL-8, and IL-10 in lung tissue were detected by the enzyme-linked immunosorbent assay method. The pathological changes in lung tissue were observed by hematoxylin and eosin staining. Results After PM2.5 exposure, the alveolar septum was widened, and the structures of alveolar walls were destroyed. There was inflammatory cells infiltration in the alveolar space and the interstitial space. Alpha diversity in BALF showed that the Chao1, ACE, Simpson, and Shannon values were increased, and the lung microbiome analysis revealed that the relative abundance of Firmicutes and Clostridium increased, while the relative abundance of Bacteroidetes and Akkermansia decreased. The contents of IFN-γ and IL-8 in lung tissue increased while the content of IL-10 decreased. After the administration of AAP, the alveolar structure damage was alleviated, and the interstitial hemorrhage, edema, and inflammatory cells infiltration were reduced. The Chao1 and ACE values decreased, and the taxonomic abundance values of Akkermansia were much higher. Simultaneously, the contents of IFN-γ, IL-4, and IL-8 decreased, and the content of IL-10 increased. Conclusions It was found that PM2.5 resulted in lung microbiome disorder, which might lead to the inflammation of lung tissue. It was also revealed that AAP could alleviate the inflammatory damage of lung tissue induced by PM2.5.

Objectives This study explores the association between self-reported exposure to traffic-related air pollution and respiratory health symptoms, as well as lung functions and skin prick tests in adolescents living in the vicinity of main roads. Material and Methods The data in the study were acquired using a cross-sectional study conducted between 2004–2005 in Chorzów (Silesia, Poland) among adolescents (N = 936) aged 13–15 years, attending junior high schools. Adverse respiratory health symptoms and exposure to traffic-related air pollution were determined on the basis of a questionnaire. Moreover, all children underwent spirometry and skin prick tests. Multivariable logistic regression with multiple imputation for missing data was used to assess the prevalence of adverse respiratory symptoms in relation to self-reported exposure to traffic-related air pollution, adjusted for socioeconomic and environmental factors. Results Among respiratory tract diseases, asthma and allergic rhinitis associations were statistically significant (OR = 2.16, 95% CI: 1.12–4.15 and OR = 1.69, 95% CI: 1.08–2.64, respectively). Likewise, among respiratory disorders, statistically significant associations were found in the case of wheezes and dyspnea attack (OR = 1.58, 95% CI: 1.10–2.26 and OR = 2.39, 95% CI: 1.56–3.66, respectively), with respect to the vicinity of the main road. Living in the area with high traffic intensity was statistically significantly associated with a higher prevalence of asthma and wheezes (OR = 2.31, 95% CI: 1.22–4.39 and 1.48, 95% CI: 1.09–2.01, respectively). The results obtained did not confirm the relationship between the adopted way of exposure to traffic-related air pollution and lung function indices or skin prick tests. Conclusions Results of the study suggest that children living in the area with intense traffic are more likely to develop respiratory disorders. Moreover, the vicinity of a main road as well as traffic intensity could be suitable in assessing the relationship between road transport and potential health problems among exposed inhabitants. Opis:
Objectives The inhaled endotoxin is considered as a causative factor in the process of acute bronchial obstruction, which can be measured by a decrease in forced expiratory volume in 1 s (FEV₁). The aim of this study was to assess endotoxin exposure among sewage treatment plant workers (STPW) and its effect on across-shift changes in respiratory airflow. Material and Methods A group of 78 STPW from a large sewage treatment plant was studied. Inhalable dust for endotoxin assessment was collected using personal aerosol samplers. Endotoxin was assayed with the kinetic, chromogenic Limulus amebocyte lysate test. Across-shift spirometric measurements were performed on Mondays, after 2-days absence from work, with the use of portable spirometer. The forced vital capacity (FVC), and FEV₁ parameters were analyzed. Multifactor regression modeling was performed to determine parameters significantly associated with endotoxin exposure. Results The concentration of inhalable dust and endotoxin ranged from 0.01–1.38 mg/m³ and 0.68–214 endotoxin units per cubic meter of air (EU/m³), respectively. Endotoxins were characterized with the skewed distribution (arithmetic mean (AM) = 38.8 EU/m³, geometric mean (GM) = 15.4 EU/m³, geometric standard deviation (GSD) = 4.21). Through the use of multifactor analysis, which excluded the main confounders (inhalable dust and smoking habit) it was found that, despite low levels of endotoxin, it had significant impact on the observed across-shift decline in FEV₁ (p = 0.044). For this parameter, the regression slope was additionally calculated (r = –0.017, p = 0.071). Conclusions Relatively low levels of endotoxin among sewage treatment plant workers may cause small, but significant across-shift declines in FEV₁. The observed relationship was independent of organic dust concentrations and smoking habit. The respiratory protection should be provided for STPW.

Objectives: The aim of this study was to verify and analyze the existence of chronic adaptations of lung function in freediving fishermen whose occupation is artisanal fishing. Material and Methods: This was a cross-sectional study involving 11 breath-hold diving fishermen and 10 non-breath-hold diving fishermen (control) from the village of Bitupitá in the municipality of Barroquinha (Ceará - Brazil). Anthropometric measurements, chest and abdominal circumferences as well as spirometric and respiratory muscle strength tests were conducted according to the specifications of the American Thoracic Society/European Respiratory Society (ATS/ERS). In order to compare the measured values versus the predicted values, Student t test was used in the case of parametric test and Wilcoxon test in the case of nonparametric test. To compare the inter-group means Student t test was used for parametric test and Mann-Whitney test for the nonparametric one. The level of significance was set at α = 5%. Results: The forced vital capacity (FVC) (4.9±0.6 l vs. 4.3±0.4 l) and forced expiratory volume in 1 s (FEV₁) (4.0±0.5 l vs. 3.6±0.3 l) were, respectively, higher in the group of divers compared to the control group (p ≤ 0.05). Furthermore, in the group of free divers, the measured FVC, FEV₁ and FEV₁/FVC ratios were significantly greater than the predicted ones. No differences were found between the measured respiratory pressures. Conclusions: These results indicate that breath-hold diving seems to produce chronic adaptations of the respiratory system, resulting in elevated lung volumes with no airway obstruction.

Objectives The aim of this study has been to investigate the frequency of sensitization to horse allergens and clinical symptoms in horse riders. Material and Methods A total of 42 horse riders and 50 healthy individuals were examined by means of skin prick tests for a panel of horse and common animal allergens, and pulmonary function tests were done by spirometry. Results The rate of sensitization to horse allergens was 31% as proven by the skin prick test in horse riders whereas horse sensitization was not seen in the control group. Occupational allergy symptoms were reported by 19 horse riders. Two horse riders with no history of clinical symptoms showed positive skin reactions to horse allergens. Conclusions To decrease the high risk of occupational sensitization among horse riders, workplace conditions should be improved to reduce the load of airborne horse allergens.
