Noise exposure during lifespan is one of the main causes of hearing loss. The highest risk of noise-induced hearing loss (NIHL) is related to exposures in the workplace, and affects about 7% of the population. Occupational NIHL is irreversible, thus its prevention must be considered a priority. Although current hearing conservation programs (HCPs) have proved to be very beneficial, the incidence of occupational NIHL is still high, reaching about 18% of overexposed workers. This paper reviews recent research on the effects of noise on hearing in pursuit of more effective methods for the prevention of occupational NIHL. The paper discusses the translational significance of noise-induced cochlear neuropathy, as recently shown in animals, and the concept of hidden hearing loss in relation to current NIHL damage risk criteria. The anticipated advantages of monitoring the incidents of the temporary threshold shift (TTS) in workers exposed to high levels of noise have been analyzed in regard to the preclinical diagnostics of NIHL, i.e., at the stage when hearing loss is still reversible. The challenges, such as introducing speech-in-noise audiometry and TTS computational predictive models into HCPs, have been discussed. Finally, the paper underscores the need to develop personalized medical guidelines for the prevention of NIHL and to account for several NIHL risk factors other than these included in the ISO 1999:2013 model. Implementing the steps mentioned above would presumably further reduce the incidence of occupational NIHL, as well as associated social costs.

Occupational voice disorders make the most frequently certified category of occupational diseases in Poland, making up approximately 20% of all cases. This study presents the current knowledge of the etiopathogenesis of occupational voice disorders. It stresses the importance of the evaluation of vocal loading by means of objective measurements. Furthermore, this study discusses the medico-legal aspects of the procedure of certifying occupational voice disorders in Poland. The paper also describes the preventive programs addressed particularly to teachers, including multidisciplinary and holistic management of occupational dysphonia. Their role in the improvement of occupational safety and health (OSH) arrangement for vocally demanding professions is emphasized.

Objectives The aim of the study was to assess the usefulness of bedside examination for screening of vestibular and balance system for occupational medicine purposes. Study group comprised 165 patients referred to Audiology and Phoniatric Clinic due to vestibular and/or balance problems. Caloric canal paresis of 19% was the cut off value to divide patients into 43 caloric-positive vestibular subjects and 122 caloric-negative patients. The latter group comprised 79 subjects revealing abnormalities of videonystagmographic (VNG) oculomotor tests (central group) and 43 subjects with normal VNG. Material and Methods Vestibular and balance symptoms were collected. Five tests were included to bedside examination: Romberg and Unterberger tests, Head Impulse Test (HIT), Dynamic Visual Acuity (DVA) and gaze nystagmus assessment. Results Vestibular and balance symptoms were reported by 82% of vestibular, 73% of central and 40% of VNG-normal patients. Thirteen out of 18 VNG-normal but symptomatic subjects (73%) had abnormal tests in clinical assessment. The sensitivity of bedside test set for vestibular pathology was 88% as compared to caloric test and 68% for central pathology as compared to VNG oculomotor tests. Conclusions The combination of 5 bedside tests reveal satisfactory sensitivity to detect vestibular abnormalities. Bedside examination abnormalities are highly correlated with vestibular/balance symptoms, regardless the normal results of VNG. Thus, this method should be recommended for occupational medicine purposes.

Tinnitus is defi ned as a perception of sound in the absence of an external acoustic stimulus. Several factors are known to infl uence tinnitus, e.g. hearing loss, noise exposure, age, and hypertension. As only certain individuals develop tinnitus in the presence of the above risks and in approximately 50% of cases tinnitus is not attributed to any particular cause, the question arose whether this inter-individual susceptibility to tinnitus could be explained by the infl uence of genetic factors. Objectives: To test the hypothesis that genetic variability in genes of the potassium recycling pathway is associated with increased susceptibility to tinnitus. Materials and Methods: The study group consisted of 626 subjects exposed to occupational noise (128 with tinnitus and 498 without tinnitus). 99 single nucleotide polymorphisms were investigated in 10 genes involved in the potassium recycling pathway in the inner ear, previously selected as putative noise-induced hearing loss (NIHL) candidate genes. Results: Nominally signifi cant associations were obtained for 2 variants in KCNE1 (potassium voltage-gated channel, Isk-related family, member 1) and SLC12A2 (solute carrier family 12, member 2) genes. The first gene contributed to tinnitus that developed independently of hearing loss, while the second one was associated with increased susceptibility to noise-induced hearing loss. Conclusions: Present fi ndings lend support to the notion of potassium recycling pathway genes as possible risk modifi ers of tinnitus in individuals with and without hearing loss. Due to the lack of replication in other independent populations these results should be seen as suggestive.

Objectives: Occupational voice disorders are accompanied by increased tension of the external laryngeal muscle which changes the position of the larynx and consequently disturbs the conditions of functioning of the vocal tract. The aim of the study is to assess the use of osteopathic procedures in the diagnosis and treatment of occupational dysphonia. Material and Methods: Study subjects included 40 teachers with chronic diseases of the voice organ (38 women and 2 men) aged from 39 to 59 (mean age: 48.25). Before and after the voice therapy the osteopathic examination according to Libermann’s protocol was performed as well as phoniatric examination including laryngovideostroboscopy (LVSS), assessment of the maximum phonation time (MPT) and the Voice Handicap Index (VHI) score. The voice therapy, scheduled and supervised by a laryngologist-phoniatrician and conducted by a speech-language pathologist, was supplemented with osteopathic myofascial rehabilitation of the larynx. The chisquare McNemar test and non-parametric Wilcoxon matched pairs test were applied in the statistical assessment. Results: The applied interdisciplinary treatment including osteopathic and vocal therapy resulted in a statistically significant decrease in tenderness of muscles raising the larynx (cricothyroid ligament, sternocleidomastoid muscles, and pharyngeal constrictor muscles) and in lowering the tonus (geniohyoid muscles, pharyngeal constrictor muscles and sternocleidomastoid muscles). A significant improvement was also observed in the case of dysfunction of the cricothyroid joint examined during glissando and yawning, as well as in asymmetry of the thyrohyoid apparatus. Moreover, the therapy resulted in significantly better normalization of the head position and better control of the centre of gravity of the body. Statistically significant post-therapy improvement was observed in the phoniatric examination, including VHI scores, MPT results and parameters of videostroboscopic examination. Conclusions: The use of osteopathic therapy helps significantly improve the functions of the vocal tract in patients with occupational dysphonia.

Objectives The aim of this study was to investigate whether the restriction in neck rotation and increased neck muscle tension could be causally related to vertigo and dizziness. Material and Methods Seventy-one patients reporting vertigo and/or imbalance were divided into 2 groups: 45 subjects with unilateral restriction (R+) and 26 without restriction (R–) of cervical rotation and muscle tension in the clinical flexion-rotation test. The normal caloric test was the inclusion criterion. The control group comprised 36 healthy volunteers with no history of vertigo. The vestibulo-ocular reflex (VOR) and the cervico-occular reflex (COR) were measured through the videonystagmography (VNG) sinusoidal pendular kinetic test in the conditions of not inactivated head and immobilized head, respectively. The VNG-head torsion test (VNG-HTT) nystagmus was recorded. Results Among the reported complaints, neck stiffness, headaches and blurred vision were more frequent in the R+ group than in both the R– group and the control group. VNG revealed an increased COR gain and the presence of VNG-HTT nystagmus in the R+ group only. Similarly, only in the R+ group a positive relationship between COR and VOR was observed. Conclusions Patients with asymmetric restriction in neck rotation and increased neck muscle tension reveal the tendency to have an increased response of the vestibular system, along with co-existing COR upregulation. Further research is needed to investigate the relationships between the activation of cervical mechanoreceptors and dizziness pathomechanisms. Int J Occup Med Environ Health. 2019;32(5):723–33

Objectives It has been shown that monitoring temporary threshold shift (TTS) after exposure to noise may have a predictive value for susceptibility of developing permanent noise-induced hearing loss. The aim of this study is to present the assumptions of the TTS predictive model after its verification in normal hearing subjects along with demonstrating the usage of this model for the purposes of public health policy. Material and Methods The existing computational predictive TTS models were adapted and validated in a group of 18 bartenders exposed to noise at the workplace. The performance of adapted TTS predictive model was assessed by receiver operating characteristic (ROC) analysis. The demonstration example of the usage of this model for estimating the risk of TTS in general unscreened population after exposure to loud music in discotheque bars or music clubs is provided. Results The adapted TTS predictive model shows a satisfactory agreement in distributions of actual and predicted TTS values and good correlations between these values in examined bartenders measured at 4 kHz, and as a mean at speech frequencies (0.5–4 kHz). An optimal cut-off level for recognizing the TTS events, ca. 75% of young people (aged ca. 35 years) may experience TTS >5 dB, while <10% may exhibit TTS of 15–18 dB. Conclusions The final TTS predictive model proposed in this study needs to be validated in larger groups of subjects exposed to noise. Actual prediction of TTS episodes in general populations may become a helpful tool in creating the hearing protection public health policy. Int J Occup Med Environ Health. 2023;36(1):125–38
