Objectives: Very little is known regarding the toxicokinetics of inhaled zinc, in particular in the case of female workers and for modern, low exposure settings. Our aim is to evaluate the relationship of external zinc levels to those of serum and urine for female workers. Material and Methods: Eleven female workers (age: 41.7±8 years old, body mass index (BMI): 23.5±4.2 kg/m2) in a galvanizing plant were investigated. Exposure assessment consisted of personal/environmental air samples, and measurement of zinc in serum (collected at the end of first shift of the working week (T1)) and urine, collected before the first shift of the working week (T0), T1 and at the end of the last shift of the working week (T2). Results: Both environmental and personal air samplings for zinc and zinc compounds were below the recommended by the German Research Foundation (Deutsche Forschungsgemeinschaft – DFG) limit values of 2 mg/m3 (7.34±2.8 μg/m3 and 8.31±2.4 μg/ m3, respectively). Serum (118.6±20.9 μg /dl) and urine zinc levels were within reference values for female Italian subjects: the latter increased from 56.4±33.5 μg/dl at T0, to 59.8±37.0 μg/dl at T1, and ultimately 65.4±34.4 μg/dl at T2, but no significant trend was found. End of shift (Spearman’s correlation coefficient p value = 0.027) and differential excretion of urinary zinc (both: T0 vs. T1 and T0 vs. T2) were correlated with airborne zinc concentration (p = 0.002 and 0.006, respectively). Conclusions: In general, our data suggests that urine may be a useful medium also for female in order to assess zinc exposure. Further studies are required in order to evaluate whether differential excretion may be useful for the biomonitoring of zinc exposure in the workplaces also for male workers. Objectives The aim of this study was to evaluate the relationship between high air temperatures and occupational injuries (OIs) occurred during the summer seasons 2000–2013 in agricultural workers from the Autonomous Province of Trento (APT), North-Eastern Italy. Material and Methods Data about OIs for the APT from 2000 to 2013 occurring during the warm season (N = 7325) was provided by the National Institute of Insurance for Occupational Illness and Injury. Daily average and daily maximum temperatures values for the specific geographical site of events were retrieved. Daily temperatures were then assessed in 3 time lags: for the day of the event (lag 0), and for the previous 24 h (lag 1) and 48 h (lag 2). Daily temperatures were then categorized in 3 exposure groups (< 75th, 75–95th and > 95th percentiles). The risk of OIs was assessed as odds ratio (OR) calculated through a Poisson regression model controlled for age, sex, ethnicity and time period, and assuming OI rates for days on which temperature was comprised in < 75th percentile exposure groups as the referent ones. Results Estimated incidence of OIs during the study period was 3.4±2.3 events/day. The peak of work-related accidents occurred on days characterized by severe thermal conditions, and in particular during heat waves (incidence rate ratio = 1.09, 95% confidence interval (CI): 1.02–1.17, p = 0.0165). Days having temperatures higher than 95th percentile, assessed as daily average, both on current days (OR = 1.119, 95% CI: 1.008–1.242) and in lag 1 (OR = 1.125, 95% CI: 1.013–1.249), as well as daily maximum temperatures, were at the highest risk of work-related injuries (OR = 1.144, 95% CI: 1.029–1.272). Conclusions In conclusion, presented findings recommend policymakers to develop appropriate warning/alert systems for agricultural workers regarding high environmental temperatures. Objectives Malignant mesothelioma is closely associated to asbestos exposure. One such exposure may occur through contact with occupationally exposed household members and their belongings. This study examines the features of pleural mesothelioma attributable only to asbestos brought home by another family member. Material and Methods The data sources were 1063 mesothelioma cases diagnosed between 1995 and 2014, from the Friuli Venezia Giulia Mesothelioma Register. In all cases the diagnosis of mesothelioma was based on the pathology report. Exposure information and demographic data were acquired by an occupational medical standardized questionnaire/interview. Results Household-exposure mesothelioma cases included 33 women and 2 men. Relationships were: wives (N = 22), daughters (N = 9), sons (N = 2), and mothers (N = 2). Asbestos exposure in the workers predominantly occurred in shipyards. Out of the 35 pleural cases, 19 were epithelial, 9 biphasic, 3 sarcomatoid, and 4 not specified. The mean age at diagnosis was 77 years old. The mean latency was 59 years, with wives having a significant shorter latency than offspring. Latency was not significantly related to morphology and asbestosis. The overall mean survival was 16 months (median 11 months) but treatment was beneficial (mean 16 months vs. 7 months). Biphasic/sarcomatoid histology and presence of asbestosis were associated with a decreased survival, although not with statistical significance. Conclusions Our data confirms that household exposure increases the risk for pleural mesothelioma amongst women with no history of occupational asbestos exposure. This is an ongoing problem in many countries, as well as in Italy, where the evaluation of a framework for the compensation of these cases is under debate. Objectives Healthcare systems in European countries, including METEOR partner countries, are faced with the aging population, an increase in costs for innovative technologies and medication, a shortage of health professionals, and inequality in access to healthcare. Presented paper aimed to recognize and compare the functioning of healthcare systems between METEOR partner countries and simultaneously check if the current epidemiological situation of COVID-19 has some relationship with the number of medical staff, yearly gross domestic product, or documented percentage of fully vaccinated people. Material and Methods In the model of descriptive epidemiological study, available demographic, socioeconomic, and healthcare organizational data in the Netherlands, Belgium, Italy, and Poland were compared to the epidemiological situation of the COVID-19 pandemic (percentage of fully vaccinated people, incidence, and mortality) in all mentioned countries. Results Obtained data confirmed that the lowest number of physicians, as well as the life expectancy and gross domestic product per capita, is in Poland. Simultaneously, the lower number of medical staff and lower gross domestic product (GDP) correspond to higher mortality due to COVID-19. The percentage of fully vaccinated with the last dose of the primary series was also the lowest in Poland. Conclusions Obtained results confirmed that higher mortality due to COVID-19 in METEOR participants’ countries is related to a lower number of medical staff and weaker GDP. The worse situation was noted in Poland, a country with problems in the functioning healthcare system, including hospital care and a serious shortage of practicing medical staff.

Objectives Work-related musculoskeletal disorders (MSD) are quite frequent in healthcare workers (HCWs), but data about MSD in home-based healthcare workers (HHWs) are lacking. In this study we describe the prevalence of MSD among Italian HHWs. Material and Methods A case-control study was carried out among 300 random-selected female HCWs, the sample comprising 100 HHWs, 100 HCWs with a low exposure to patient handling (MAPO – Movimentazione e Assistenza Pazienti Ospedalizzati – Movement and Assistance of Hospital Patients index 0–5) and 100 HCWs with high exposure to patient handling (MAPO index ≥ 5.01). As a negative control group, 200 visual display unit workers were also randomly selected. Musculoskeletal disorder cases were collected using a standardized case definition. A multivariate logistic regression analysis was performed comparing the MSD prevalence in the 4 groups. Results The overall prevalence of MSD was 17% in the reference group and 28.3% for HCWs. HHWs and HCWs with MAPO index ≥ 5.01 had similar prevalence of neck pain (9% and 11%, respectively), whereas lumbosacral pain prevalence was higher in the HHWs group (31%), with similar results in residential HCWs groups (21% in MAPO index 0–5 group and 25% in MAPO index ≥ 5.01 group). HCWs of group MAPO index ≥ 5.01 and HHWs showed the higher prevalence of upper limb complaints, with a prevalence of 20% and 10%, respectively. In multivariate regression analysis, prevalence of MSD complaints was quite similar in HHWs (adjusted odds ratio ($\text{OR}_\text{adj}$) = 2.335, 95% confidence interval (CI): 1.318–4.138) and in HCWs of the group MAPO ≥ 5.01 ($\text{OR}_\text{adj}$ = 2.729, 95% CI: 1.552–4.797). Conclusions The prevalence of MSD in the examined HCWs was relatively high, with HHWs appearing as a particularly high-risk group for lumbosacral back pain. In higher exposed HCWs, upper-limb symptoms were particularly frequent, probably reflecting the different tasks required to manage residential and homebased patients. In conclusion, this study reaffirms the high prevalence of MSD in HHWs, seemly similar to high-risk residential groups. Int J Occup Med Environ Health 2017;30(2):291–304

Objectives The aim of this survey was to assess the seroprevalence of antibodies against Coxiella burnetii (C. burnetii) in subjects at risk of exposure in Sicily, Southern Italy. Material and Methods Prevalence of IgG antibodies to C. burnetii phase II antigens was evaluated by ELISA in a group of 140 workers at risk of exposure (38 veterinarians, 38 slaughterhouse workers, 44 livestock handlers, 20 laboratory and technical personnel) included in a medical surveillance program and in 42 control subjects. Positive samples were classified as suggestive of prior exposure to C. burnetii. Results Antibodies against C. burnetii were detected in 88 out of 140 (62.9%) exposed workers and in 6 out of 42 (14.3%) subjects of the control group. The variables evaluated did not seem to have a significant effect on seropositivity to Coxiella with the exception of symptoms in the last 6 months preceding the survey. Conclusions Our study demonstrated a high seroprevalence of C. burnetii in the group of exposed workers in comparison to non-exposed subjects of the control group. Clinical illness appears to be rare; nevertheless, physicians should consider Q fever in patients with compatible symptoms and occupational exposure to animals and their products. As aerosols represent the main route of infection in animals and humans, these workers are strongly advised to wear respiratory masks. In addition, occupational physicians should consider routine serologic evaluation and vaccination of occupationally exposed workers.

Objectives This study aims to characterize personal attitudes and knowledge of a sample of Italian occupational physicians (OPhs) towards immunization practice in the case of healthcare workers (HCWs). Material and Methods A total of 90 OPhs (42.2% of males, 57.8% of females, mean age of 50.1±8.3 years old) compiled a structured questionnaire through a telephonic interview. They were asked about the official Italian recommendations for HCWs, their general knowledge of vaccine practice, their propensity towards vaccines (both in general and about specific immunizations), their risk perception about the vaccine-preventable infectious diseases. Eventually, a regression analysis was performed in order to identify factors predictive for vaccine propensity. Results Only 12 out of 90 subjects correctly identified all the 7 recommended immunizations. The hepatitis B virus (HBV) vaccine was correctly identified by 95.6% of the sample, and was also associated with the more positive attitude and the more accurate risk perception. Influenza vaccine had the lowest acceptance (75.9%). Eventually, pertussis, measles, parotitis and varicella vaccines were insufficiently recognized as recommended ones (all cases < 50% of the sample). General knowledge of vaccine and knowledge of official recommendations were significantly correlated with the attitude towards immunization practice (r = 0.259, p = 0.014 and r = 0.438, p < 0.0001). In the regression analysis general knowledge (unstandardized coefficient (B) = 0.300, 95% confidence interval (CI): 0.090–0.510, p = 0.006) and risk perception (B = 0.579, 95% CI: 0.155–1.003, p = 0.008) were significant predictors of the propensity to vaccinate. Conclusions Vaccinations gaps in HCWs may found their roots in OPhs incomplete knowledge of evidence-based recommendations. Specific training programs and formations courses should then be planned. Objectives Occupational medicine is a discipline continually evolving in response to technological advances, changes in workplaces and production processes, emergence of new occupational risks and diseases and modifications in regulatory framework for occupational health and safety. Therefore, the recurrent revaluation of professional activity, information demands and education and training needs of occupational physicians is essential in order to identify methodologies and tools that may contribute to improvement of their professional knowledge and competency. In this regard, we conducted the first large-scale national survey of Italian occupational medicine physicians to define their demographic and professional activity and to assess their information demands, training and updating needs. Material and Methods A random sample of occupational physicians, listed in the national register of the Italian Ministry of Health, was selected to complete a voluntary survey. Subjects recruited in this study were asked to complete 3 different sections (personal and professional information, training and updating needs, professional activity and practice characteristics) of a questionnaire for a total of 35 questions. Results Most of participants were specialized in occupational medicine, worked for a large number of companies and carried out health surveillance on a total number of workers that exceeds 1500. Occupational physicians would like to have a higher training offer towards practical aspects of health surveillance, risks assessment, manual handling of loads, chemical substances and upper limb biomechanical overload. Interestingly, statistically significant differences were observed subdividing the sample into different groups according to the legal requirements to perform the professional activity of occupational physicians in Italy or according to particular aspects of their professional activity. Conclusions This study has provided interesting findings that may help to guide future discussion on alternative and additional instruments and/or methodologies that may be adopted to implement the quality and effectiveness of occupational medicine practice. Objectives Transient exposure with acute effect has been shown to affect the risk of occupational injuries in various industrial settings and at the healthcare workplace. The objective of this study has been to identify transient exposures related to occupational injury risk in an Italian teaching hospital. Material and Methods A case-crossover study was conducted among the employees of the University Hospital of Udine who reported an occupational injury, commuting accident, or incident involving biological risk in a 15-month period in the years 2013 and 2014. The matched-pair interval approach was used to assess the role of acute sleep deprivation whereas the usual frequency approach was used for other 13 transient exposures. Results Sleep hours were not associated with the risk of injuries whereas a significant risk increase was associated with fatigue, rush, distraction, emergency situations, teaching to or being taught by someone, non-compliant patients, bloody operative/work field, excess noise, complex procedures, and anger. Conclusions We identified transient exposures that increased the risk of occupational injuries in an Italian teaching hospital, providing indications for interventions to increase workers’ safety at the healthcare workplace. ObjectivesThe COVID-19 pandemic is due to SARS-CoV-2 coronavirus infections. It swept across the world in the spring of 2020, and so far it has caused a huge number of hospitalizations and deaths. In the present study, the authors investigated serum anti-SARS-CoV-2 antibody prevalence in the period of June 1–September 25, 2020, in 7561 subjects in Modena, Northern Italy.Material and MethodsThe study population included 5454 workers referred to testing by their companies, and 2107 residents in the Modena area who accessed testing through self-referral.ResultsThe authors found the overall seroprevalence to be 4.7% (95% confidence interval [CI] 4.2–5.2%), which was higher in women (5.4%, 95% CI: 4.5–6.2%) than in men (4.3%, 95% CI: 3.7–4.9%), and in the oldest age groups (7.3%, 95% CI: 5.2–9.3% for persons aged 60–69 years, and 11.8%, 95% CI: 8.6–15.1%, for persons aged ≥70 years). Among the occupational categories, the highest seroprevalence was found in healthcare workers (8.8%, 95% CI: 7.0–10.5%), dealers and vehicle repairers (5.2%, 95% CI: 2.9–7.6%), and workers in the sports sector (4.0%, 95% CI: 1.8–6.1%), while there was little or no such evidence for those employed in sectors such as transport and storage, accommodation and restaurant services, and the school system. Conclusions: These results have allowed, for the first time, to assess population seroprevalence in this area of Italy severely hit by the epidemic, while at the same time identifying the subgroups at a higher risk of exposure to SARS-CoV-2.

Opis:
ObjectivesThe aim of the study was to evaluate the clinical presentation and burden of SARS-CoV-2 infections among medical school physicians and residents, mainly young medical doctors. The awareness of COVID‑19 clinical manifestations can improve the early detection of mild cases, possibly reducing further transmission to colleagues and patients.Material and MethodsThe study was carried out in March–May 2020, involving medical school physicians in a teaching hospital in northern Italy, with a working population of 881 medical doctors. Data collection was performed using a structured form investigating clinical and epidemiological information.ResultsOne hundred sixty-two medical doctors contacted the Occupational Health Service reporting acute respiratory symptoms or close contact exposure to a confirmed COVID‑19 case. Among the confirmed COVID‑19 cases, most were male doctors during residency, and 85% presented a mild clinical picture. Fever (70.3%) and cough (51.4%) represented the most prevalent symptoms of COVID‑19. As revealed by the univariate analysis, the prevalence of real-time reverse transcriptase-polymerase chain reaction (RT-PCR) positivity increased with age (OR = 1.08, 95% CI: 1.02–1.14, p = 0.012), working in a COVID‑19 ward (OR = 3.33, 95% CI: 1.09–10.21, p = 0.031), presenting alteration or loss of smell/taste (OR = 10.00, 95%CI: 2.80–35.69, p < 0.001) and myalgia (OR = 3.20, 95% CI: 1.00–10.26, p = 0.046), while being a resident (OR = 0.20, 95% CI: 0.05–0.80, p = 0.030) was associated with reduced odds of being infected, compared to staff physicians. Age and loss of smell/taste were the only factors independently associated with RT-PCR positivity.ConclusionsThe majority of COVID‑19 cases showed a mild clinical syndrome, ranging from absence or paucity of symptoms to common cold or influenza-like symptoms. The findings of the present study increase the accuracy of the clinical diagnosis for the prompt identification and management of suspected COVID‑19 cases, being particularly useful during resurges of the SARS-CoV-2 pandemic.
