Opis:
Diagnosis and treatment issues among heart failure (HF) patients are becoming one of the most important points in public health of developed countries, largely due to the aging of population and the fact that HF affects mainly the elderly. In this review we would like to focus on pathophysiology of exercise intolerance in patients with heart failure and potential benefits of cardiac rehabilitation (CR). Analysis of articles in the EBSCO database using keywords: heart failure, cardiac rehabilitation, exercise training, pathophysiology. HF can be described as a composite syndrome which results from structural or functional impairment of ventricular filling or blood ejection. Patients have variety of symptoms which usually are nonspecific. The most frequently occurring symptoms of HF are dyspnea and fatigue, which may restrict exercise capacity, and fluid retention. There are many possible pathophysiological factors involved in the development of exercise intolerance. Based on the available literature pathological changes in central hemodynamic function, pulmonary system, skeletal muscles, endothelial function and neurohumoral system can be distinguished. They play a crucial role in the pathogenesis of HF symptoms and represent a potential curative object. HF patients are characterized by diminished functional performance. Exercise training has many potential profits in patients with heart failure, including an increase in peak oxygen uptake, improvement in central hemodynamics, peripheral vascular and skeletal muscle function and has become part of evidence-based clinical therapy in these patients.

This article is a theoretical study on the role of leisure time animation in the process of rehabilitation of juveniles staying in correctional facilities. The work consists of three parts. The first one presents the structural and functional outline of leisure time institution. The second one deals with the offer of rehabilitation, and the third discusses the role of the pedagogy of fun in the context of rehabilitation and physical education sciences. The study emphasizes the fact that the educational function is implemented mainly in the boarding house of the correctional facility. Classes are held in accordance with a predetermined schedule that is tailored to the individual needs of the pupil. The process of rehabilitation should be assisted by fun (including physical activities). It is therefore necessary to enrich traditional games and fun activities including the newer trends, using, among others, ice-breakers, energizers and team-builders. The implementation of the aforementioned play therefore requires the continuous staff performance improvement in this field. Additionally, games and activities pose the possibility of volunteering, making pupils more sensitive to the needs of others.

The objective of this article was to show the educational values of adventure tourism in the process of rehabilitation of juveniles. Theoretical considerations concerning this topic give rise to the following conclusions: 1. Thanks to qualified tourism, young people shape their character and face weaknesses. It helps to change the behavior and create pro-social attitudes. 2. Qualified tourism teaches basic life skills. All of the above-mentioned points prove that part of correctional facilities activity should be based on qualified tourism.

The paper is aimed at assessing the influence of rehabilitation on health of ballroom dancers after sports injuries. At the turn of 2014 and 2015 ballroom dancers from all around Poland were questioned. The surveyed were professional dancers aged between 13 and 30 years. As many as 63 athletes were injured during their career. The rehabilitation of 47 of them took place in the rehabilitation centre under physiotherapeutic supervision. Only 16 of the injured rehabilitated on their own. The opinion poll based on the survey was used as a method. The questionnaire included closed and open questions which were to show the influence of injury and its treatment on future sports career. Questions were detailed and referred to the kinds of injuries and their causes, the process of treatment, duration of rehabilitation, its process and results. Questionnaire results were analysed statistically with the use of the Pearson’s chi-squared test. It has been proved that rehabilitation under physiotherapeutic supervision has positive influence on health of ballroom dancers after injuries. Specialist rehabilitation of sportsmen after sports injuries contributed to the shortening of time of the recuperation and to the complete recovery. The time of recovery of the injured who did not undergo the professional rehabilitation was longer, and it was impossible to restore lost functions in the group of 5% of the surveyed, which resulted in the end of their career. Dancers after sports injures should always undergo professional rehabilitation under the sports doctor’s and physiotherapist’s supervision.

One of the main priorities of stroke rehabilitation is regaining patients’ independence in basic everyday activities. This paper is aimed at assessing impact of early stroke rehabilitation on hip joint mobility of the affected leg in patients after cerebrovascular accidents. The study included 30 subjects (13 men and 17 women) aged on average ±66.1, with hemiparesis, who were treated at the stroke unit and participated in a 4-week rehabilitation program. Measurements of passive and active motion ranges were conducted with a goniometer in both limbs: healthy and affected ones by stroke. There were statistically significant differences in motion ranges between healthy and affected limbs. Examination I revealed that affected limbs amounted to only 40% of physiological range, while in Examination II, the result reached 73%. The most significant motion limitations were noted in terms of bending, adduction and internal rotation, while the least significant in terms of external rotation and abduction. Exercises used during early stroke rehabilitation of hemiparesis patients considerably increased active motion ranges and maintained passive motion ranges in the hip joint of the affected limb. Patients with right-sided hemiparesis experienced much better improvement in hip joint motion of the affected limb. The 4-week period was insufficient to mobilize patients and help them regain full active mobility in the hip joint of the affected limb. These patients required further physiotherapy, until they fully regained functionality.

Heart failure (HF) due to its universality has become a huge challenge for modern medicine. Second part of the twentieth century brought significant changes in the rehabilitation, diagnostic and pharmacological procedures. There are no definitive guidelines for Cardiac Rehabilitation (CR) in HF. Based on previous studies, the article tried to describe and illustrate the mechanism of effective CR and its intensity in HF patients, which could be helpful in CR protocol development. Cardiac Rehabilitation has confirmed efficacy in increased physical level of participation in inter alia, home/work/recreational activities, improved psychosocial well-being, functional independence, prevention of disability, long-term adherence to maintaining physically active lifestyle, improved cardiopulmonary fitness, strength, muscle endurance, and flexibility, reduced cardiovascular events risk and risk of mortality. Before and after CR conduction, baseline and final aerobic capacity should be examined with an ergospirometry test to evaluate CR protocol intensity and check its effectiveness, respectively. Frequency of training-bouts in CR protocol in HF patients were from 3 to 7 days per week, intensity ranged from 40% to 80% VO2max or 9 to 14 on rating of the perceived exertion (RPE) scale or 6 to 20 on the Borg scale. Duration of single bout-exercise ranged from 20 to 60 minutes.

Introduction: Damage to soft tissue in the shoulder area causes significant impairment in the biomechanics of the joint, causing severe pain, inflammation and consequently leading to restricted mobility and functional capacity. The shoulder impingement syndrome is a multifaceted disease entity of diverse etiology. It is assumed that this syndrome is responsible for 44-60% of all ailments in the area of the pectoral girdle. Pathologies of soft tissues in the area of the glenohumeral joint affect 1/3 of the population. The aim of this study was to evaluate the functional capacity of patients with the shoulder area disorder undergoing sanatorium treatment. Materials and Methods: In the study 30 patients were diagnosed with diseases within the shoulder joint before and after rehabilitation treatment. The clinical examination included kinematic measurement of the range of motion (ROM) and the muscle strength of the shoulder joint. Furthermore, the pain intensity was assessed using the VAS. Results: The examination of the range of motion of the affected upper limb demonstrated considerable limitations and statistically significant differences before and after rehabilitation treatment for all the assessed movements. Moreover, the difference between the level of pain according to the VAS before (mean = 6.2) and after physiotherapeutic treatment (mean = 3.7) was demonstrated. The evaluation of linear relationships showed a significant correlation between the VAS and shoulder abduction, shoulder horizontal abduction and muscle strength for internal and external rotation after sanatorium treatment. Conclusions: Progressive changes in the soft tissues of the shoulder joint cause a significant limitation of functional capacity of patients and severe pain. Rehabilitation treatment significantly increases active participation in daily life and improve patient quality of life.

This study examined the test re-test, intrarater and interrater reliability of joint kinematics from the Coach’s Eye smartphone application. Twenty-two males completed a 1-repetition maximum (1-RM) assessment followed by 2 identical sessions using 5 incremental loads (20%-40%-60%-80%-90% 1-RM). Peak flexion angles at the hip, knee, and ankle joints were assessed using 1 experienced practitioner and 1 inexperienced practitioner. The acceptable reliability thresholds were defined as intraclass correlation coefficient (ICC) (r) > 0.70 and coefficient of variation (CV) ≤ 10%. The test re-test reliability of peak hip and knee flexion were reliable across 20-90% 1-RM (r > 0.64; CV < 4.2%), whereas peak ankle flexion was not reliable at any loaded condition (r > 0.70; CV < 20.4%). No significant differences were detected between trials (p > 0.11). The intrarater reliability was near perfect (r > 0.90) except for peak ankle flexion (r > 0.85). The interrater reliability was nearly perfect (r > 0.91) except for hip flexion at 80% 1-RM and ankle flexion at 20% (r > 0.77). Concludingly, the Coach’s Eye application can produce repeatable assessments of joint kinematics using either a single examiner or 2 examiners, regardless of experience level. The Coach’s Eye can accurately monitor squat depth.

The aim of the study was to evaluate the effect of rehabilitation treatment to improve the treatment results of patients operated for ganglions of the wrist. We studied the hands of 77 patients with ganglions of the wrist. 43 women and 34 men aged between 16 and 98 years participated in the study. The open method was used in the surgical treatment to remove the wrist ganglions in all the patients. In the post-operative treatment 46 patients (group I) underwent hand rehabilitation treatment under the supervision of the authors and 31 patients (group II) did not undergo such a treatment. The examination was carried out on all the patients before surgery and at weeks 1 and 4 after surgery. The patients with ganglions of the wrist and pain, impaired hand mobility and impaired hand efficiency were qualified for the surgical treatment. In the rehabilitation treatment stretching exercises were performed as well as loosening and stabilizing the hand and teaching the patients the correct positioning of the upper limb during work and physical activity. The results were statistically analyzed using non-parametric tests such as the chi-square test, signs test and Mann-Whitney test. It was found that in patients after surgical removal of the wrist ganglion rehabilitation treatment has led to the resolution of pain syndromes, movement disorders and efficiency of hand.

Purpose The most frequent cancer among women in Poland and worldwide is breast cancer. The aim of the study was to assess the quality of women's life after unilateral and bilateral breast amputation. Research process The study included 47 women - 24 women after bilateral amputation and 23 unilateral amputees. Two questionnaires were used: European Organization for Research and Treatment of Cancer - Quality of Life Questionnaire: general and breast cancer module to measure the quality of life of patients treated for cancer. Results From among the assessed areas of quality of life, the sphere of emotional functioning was significantly disturbed. The passage of time improved the quality of life of the surveyed women. Thanks to early detection and treatment survival time is extended, however the disease becomes a chronic disorder affecting the psychosomatic sphere that changes the quality of life of these women.

The aim of the study was to evaluate the results of surgical treatment and rehabilitation of patients with trigger thumb and finger. In 40 patients, comprising 30 women and 10 men aged 26 to 64, a total of 42 cases of trigger thumb and finger. In the preoperative period, the severity of changes were studied according to the classification developed by Newport et al. Five patients were classified in the first stage, 28 in the second, 6 in the third, and 1 in the fourth. The mean duration of symptoms was five months. The indication for surgery was a lack of improvement following conservative treatment. All of the patients were treated surgically using the open method by cutting the flexor tendon sheath in part A1. The rehabilitation treatment included exercises to improve the range of mobility of the thumb and fingers and to stretch, grelax, and strengthen muscles. Neuromobilisation and automobilisation exercises were conducted. After 5 months, swelling, pain and restricted mobility of the thumb and fingers subsided in all patients. There were no ‘jumping’ symptoms. Apart from a slight transitory inflammatory reaction in 2 patients there were no complications. In patients with trigger finger, open surgery and competent rehabilitation therapy enables the achievement of very good results, with a low complication rate.

According to the statistical data, there is being observed a definite increase in percentage of children affected by posture defects, particularly scolioses. The aquatic environment facilitates treatment of scolioses, since it provides relief conditions used in order to perform prophylactic, corrective and therapeutic tasks. Hydrokinetic therapy is one of the many rehabilitation methods. It seems, however, that corrective and therapeutic swimming is far too rarely used as an auxiliary measure for treatment of scolioses. This paper presents the concept of treatment of scolioses employing asymmetrical exercises in aquatic environment, the author of which in Poland is Iwanowski. In the study desk research of literature studies was applied along with the analysis of publications, including available study reports, articles, documents and also own video and photographic materials. The author suggests the concept of conservative treatment of scolioses through the application of asymmetrical swimming exercises in aquatic environment. Such procedure is efficient in preventing significant spinal deformations, which may protect against surgical intervention.
