Introduction and objective. The quality of a doctor-patient relationship plays a vital role in all fields of medicine. In the case of psychiatry, this role is special as it provides the foundation for the whole therapeutic process. The aim of this study was to investigate the patient’s perspective on psychiatric visits: patient’s attitudes towards the psychiatrist, patient’s view of the patient-psychiatrist relationship, and the patient’s needs and expectations from this relationship. Materials and method. 615 psychiatric outpatients responded to the anonymous questionnaires connected with their attitudes towards the psychiatrist, evaluation of the doctor, and expectations from psychiatric care. The study was conducted in 10 out of 30 public centres for psychiatric care in north-eastern Poland. Results. Generally, the patients liked and positively evaluated their psychiatrists. Patient’s liking for the doctor was connected with the feeling that the doctor also liked the patient, as well as with perceiving the doctor as competent and willing to meet the patient. The longer the treatment with a particular psychiatrist and the rarer need to consult the doctor, the more positive attitude and evaluation of the doctor patients had. According to the patients, the most significant expectations were associated with both conversation with the doctor and receiving emotional support. Conclusions. The key phase for forming the patient-psychiatrist relationship was the first stage of cooperation in which patients created their attitudes towards the doctor without modifying them at further stages. Thus, further studies on learning and developing the ability to establish the relationship with the patient, inspiring the patient’s trust and making psychiatric appointments comfortable from the first meeting, will be highly valuable.

Background: In the work of a physician, not only knowledge, and professional skills (technical/hard) are important, but also psychosocial skills (relational/soft). Objectives: The primary goal of the presented research was evaluation of the level (study of the state) of communication competences of physicians and determination of the factors on which this level depends. An additional goal was analysis of the needs and educational possibilities within the existing models of education in the area of interpersonal communication provided in Medical Universities in Poland. Design, setting and participants: Information about educational curricula available on the websites of 12 Medical Universities in Poland were compared. The self-designed questionnaire and adjective check list were subject to standardization from the aspect of reliability and validity. The study groups included 1) occupationally-active physicians (185 respondents) employed in outpatient departments and hospitals, who were covered by a pre-graduate standard educational programme and not trained in interpersonal communication skills as part of their continuing education; 2) medical students covered by a standard educational programme (246 respondents). Results: The conducted analysis of the educational curricula showed a very narrow scope of problems concerning professional medical communication. The results indicating the general state of respondents’ communication competences within all aspects (motivation, skills, knowledge) were relatively low. That clearly indicated an inadequate educational model (students), and lack of post-graduate training in the area of professional medical communication (physicians). Conclusions: The education of students of medicine should cover selected classes within the scope of professional communication competences. These classes should be based on the systemically designed training of skills. The patterning by students of the relations attitudes observed in practising physicians is insufficient. It is necessary to apply a methodical evaluation of communication competences, diagnosing educational needs of occupationally active physicians in this respect. This allows the preparation of courses in accordance with the needs in the area of professional communication competences.

Introduction and objectives. The aim of the study was describe the factors determining the evaluation of the hospital environment, especially satisfaction with care and individual needs of cancer patients. Material and methods. The study comprised 80 women with endometrial cancer diagnosed and treated surgery in the Clinic of Gynaecology and Obstetrics in Rzeszow, Poland, between 2011–2012. The study used 3 questionnaires: the Goals Attainment Scaling (GAS) questionnaires, and questionnaires developed by the EORTC Quality of Life group, i.e. the QLQ C-30 (general module) and the In- PATSAT-32. Results. Respondents indicated 36 goals/expectations and the most common (over 50%) concerned the normal course of the post-operative period. The overall index of all goals which were met was 7.0 points. General quality of life reported by respondents before surgery was at a medium level (52.3+16.8%). Emotional functioning received the lowest scores (61.0+18.8%). Most respondents assessed manual skills of hospital doctors and nurses as the best in the In-PATSAT 32 scale i.e. 69.9±14.7% and 67.3±16.1%, respectively. The worst ratings concerned access to hospital from the outside (50.8±16.9%) and easy orientation inside the buildings (55.9±16.0%). Conclusions. Analysis of correlations between GAS and the In-PATSAT32 scales proved that they cannot be used interchangeably since they measure different aspects of a patient’s satisfaction with hospital care. For this reason, the application of idiographic and nomothetic tests among cancer patients is helpful for evaluation of the hospital environment.

Introduction. Correctness of insulin treatment depends on both the experience and skills of the doctor and knowledge and behaviour of the patient. Objective. Evaluation the adequacy of insulin doses administered to diabetes patients in ambulatory conditions. Material and methods. The treatment of 59 patients hospitalized in the Diabetology Ward was evaluated at admission, discharge and 3 months after hospitalization. Results. The mean daily doses of insulin significantly differed at times of evaluation and were: 53.90, 39.31 and 43.34 units, respectively (p≈0.000001). A significant reduction of body weight, 90.86 vs. 88.25 kg (p≈0.000001), was obtained only during hospitalization, and was maintained 3 months after discharge (87.86 kg). Significant differences were also noted in the body mass index (33.44 vs. 32.48 vs. 32.37 kg/m2, p≈0.000001). The change in waist circumference was not statistically significant (107.87 vs. 104.89 cm; p≈0.06). A decrease in the number of hypoglycaemia episodes was observed, but were statistically insignificant (25 vs. 23; p≈0.7). Three months after hospitalization an insignificant decrease of HbA1c level was noted (8.41% vs. 8.03%; p≈0.07). Conclusions. During treatment in the Diabetology Ward the procedure of choice was more frequently a reduction than an increase in insulin doses. This management led to the reduction of the patients’ body weight, improvement of glycaemia, without any significant effect on the diabetes control determined by the HbA1c level.

Background: A dissonance between high ‘technical’ professionalism of nurses and a relatively low level of patient satisfaction with received care is a phenomenon observed in many countries. Method: Theoretical concept and review of current published studies. Discussion: Most reviewed studies show that a low level of patient satisfaction occurs in the case of an inadequate interpersonal communication between nurses and patients. Most studies indicate poor effectiveness of shaping communication competences of nurses based on standard education in the area of general psychology and communication knowledge, because this knowledge does not convert itself ‘spontaneously’ into communication competences during occupational activity. Conclusions: It is necessary to supplement educational programmes for nurses with practical courses in professional interpersonal communication. International experience exchange concerning the shaping of nurses’ communication competences may be limited due to cultural, organizational and systems factors.

Introduction. E-Health tools allow a medical facility to set a given patient’s data in order using ICT techniques, and the patient to use those techniques when contacting a given organisation. Materials and method. Secondary statistical data was used in the research. The study was carried out among primary health care patients. Mining for affinity rules was done in the R programme. The apriori and inspect functions from the arules package were used. Moreover, any redundant rules were removed from thoseobtained using the afero-mentioned method. Applying the general description of the affinity analysis method onto the survey described herein, it should be stressed that the aim of using affinity analysis was to discover the rules which contain the sub-transaction B={V_6=1} as a consequent. This was determined by the intention to discover associations regarding the knowledge about a uniform information system that the patients under study might have. Results. In the discovered rules, the antecedent most often contained an indication of the need for introducing a uniform solution as regards telemedicine. Moreover, according to the opinions of ‚conscious‘patients, a uniform IT system should improve the work at primary health care institutions, introducing an on-line booking system for visits should improve the productivity and comfort of doctors, and an IT system should provide unambiguous identification of a patient. Conclusions. There is potential in using affinity analysis within e-Health. The example of affinity analysis described in his study led to the discovery of interesting and important (from the point of view of a medical facility) regularities regarding the knowledge and expectations of patients as regards e-Health.

Introduction. An adverse event is an incident induced while providing health care services or resulting from it, not related to the natural course of a given disease or health condition, which causes or is likely to cause negative consequences for the patient, including their death, a threat to life, the necessity of hospitalisation or its prolongation, permanent or considerable health detriment; or is a foetal disease, congenital defect or the result of foetal damage. Objective. The aim of this analysis is to explore the problem of the occurrence of adverse events from the perspective of doctors and ward nurses who manage wards. Materials and method. The research on the occurrence of adverse events among doctors and nurses (the management staff) was conducted with the use of a postal survey. Results. It was ascertained that 86.5% of the medical personnel had taken part in an adverse event, of which 20.2% took part in an occurrence associated with pharmacotherapy, 16.2% – in an event related to diagnostics and diagnosis, or an infection – 15.7%. 14.2% of respondents were involved in an occurrence linked to a medical device malfunction, and 14.1% – in an adverse event related to an operation. Conclusions. The adverse events most often identified in the nursing professional group are occurrences associated with pharmacotherapy, and in the doctors’ professional group – occurrences related to diagnostics and diagnosis. The research established that the most frequent reason for not informing patients about the occurrence of an adverse event is fear of their filing a complaint. Medical management staff show high acceptance of an adverse event reporting system as a tool for improving patient safety.

High-dose chemotherapy currently used in the treatment of children with cancer may induce late effects on psychosocial and cognitive functioning in some of them. The aim of the study was to evaluate psychosocial functioning of childhood cancer survivors living in rural regions. In total, 29 children entered the study. The children were diagnosed as having cancer between 1993-1995 in a single centre. Each patient was examined 5 years after the completion of cancer therapy. The children had been diagnosed with leukemia and lymphoma (72.4%), and solid tumours. Self-report questionnaires, as well as standardized psychological test (WISC-R, WAIS-R-PL) were performed for the evaluation of psychosocial and cognitive functioning of the patients. Most of the studied cancer survivors living in rural areas did not reveal diffi culties with cognitive and psychosocial functioning. In some patients, however, we observed more diffi culties in verbal tasks and existing major learning diffi culties. Cancer survivors living in rural areas in our region may be more unprivileged due to poor additional supporting services.

Introduction and objective. Patient satisfaction with nursing care is one of important indicators of health care quality. As for pain, it is an indicator of patient health. The goal of the study is to identify socio-demographic and medical factors that differentiate the assessment of satisfaction with nursing care of hospitalized patients. Materials and method. The study involved performing a diagnostic survey using standardized instruments, i.e. the Newcastle Satisfaction with Nursing Scale (NSNS) and the Visual Analog Scale by Barbara J. Headley for pain scoring. The study included 205 patients hospitalized for lumbar pain syndrome on two neurosurgery wards in Poland. The mean patient age was 51 years. Most respondents declared having high school or vocational education. The mean time of hospitalization before the study was 2 days. Results. The results demonstrated that patient satisfaction with nursing care is independent of patient age, education, duration of hospitalization, or number of hospitalizations. In contrast, the experience of nursing care is dependent on education, and less educated patients (primary or vocational education) had higher scores in this category. With regards to pain, it was perceived as more intense by women and by patients hospitalized for a shorter period of time. Conclusions. Satisfaction with nursing care among patients with lumbar pain syndrome does not depend on socio- demographic or medical variables. Experience of nursing care depends on patient’s education.

Introduction and objective. The family is a reliable and permanent source of support for every human being. It is the key link in the welfare system. The aim of the study is to assess parents’ attitudes towards the occurrence of a difficult situation resulting from a child’s chronic illness. Material and methods. The study involved randomly selected parents of children with chronic diseases (N=107). The study was carried out at the Dental Clinic of the ‘Little Prince’ Children’s Hospice in Lublin. Results. The study revealed that for parents a difficult life situation is mainly associated with the disability of a family member or a friend, whereas the action approach is the most frequently chosen attitude towards a child’s chronic disease. In the examined group of parents, a significant correlation was observed between the age of the parents and the choice of a specific parental attitude towards the child’s chronic disease (in terms of intellectual approach). However, no significant correlations were found between the parents’ place of residence and the choice of a specific parental attitude towards the chronic disease of a child. Conclusions. During the diagnosis of chronically ill children, parents should also be educated in this regard. Therapy and psychological help, as well as directing parents to relevant support groups, organizations or associations, will prove immensely valuable while coping with a difficult life situation.

Objective. The aim of the study was determination of the level in expression genes associated with cell stress response in a patient with PTSD. Case study. A 57-year-old PTSD patient, A.P., was the subject of the study. Blood samples were taken from the ulnar vein to assess the relative expression of the tested genes in leukocytes via a quantitative reverse transcription polymerase chain reaction (qRT-PCR). Results. A considerably decreased level in the expression HSF-1, NF-kB, Hif-1 and IL6 m-RNA was observed in the patient when compared to the control group. A low expression in HSPA1A, HSPB1, IL10 and CRP m-RNA compared with the control group was also noted. Conclusion. It is possible that the lower level of transcriptional factors: HSF-1, NF-kB and Hif-1 as well as IL6 m-RNA may be associated with the effect of the PTSD patient’s treatment.

Introduction and objective. The aim of the study was evaluation of the urban and rural differences in ovarian cancer patients’ characteristics at the moment of diagnosis. Materials and methods. The study comprised women with ovarian cancer diagnosed and treated in the Division of Gynecological Surgery of Poznan University of Medical Sciences between 2004–2011. The patients were divided into 3 groups based on their place of residence: 1) patients residing in large cities (over 50,000 people), 2) inhabitants of small towns (below 50,000 people), 3) women from rural areas. Results. Among the studied groups of patients no differences were found in the FIGO stage at diagnosis (p=0.453), histological grade of the tumour (p=0.916), histopathological types of ovarian neoplasms (p=0.431), median tumour volume (p=0.855), presence of fluid in the pouch of Douglas (p=0.872). Women with ovarian cancer residing in large cities had lower median parity (p=0.0005), higher education level status (p=0.0001), and experienced menarche at an earlier age (p=0.039). There were no differences in the use of oral contraception (p=0.93) and body mass index (p=0.23) between the women included in the study. Conclusions. There were no differences in advancement of ovarian cancer at the moment of diagnosis or in tumour type and size between women residing large cities, small towns and rural areas. Several ovarian cancer risk factors were more common among ovarian cancer patients living in urbanized areas.

Introduction. Rheumatoid arthritis (RA) is a chronic, autoimmune-based disease of the connective tissue with still unknown etiology. Numerous studies have indicated the association between Copper (Cu) and ceruloplasmin (Cp) concentrations and pathogenesis of RA. Objective. To compare the concentrations of Cu and Cp in different biological samples and their correlation with the inflammatory process, between a group of patients with RA and a control group of healthy individuals. Materials and Methods. The study enrolled 74 Caucasian patients (20 men and 54 women), aged 29–50 (mean age 39.8±6.1 years) diagnosed with RA. The control group consisted of 30 healthy Caucasian individuals. Copper levels were assessed by atomic absorption spectroscopy. Results. Among RA patients the mean Cu level was significantly higher in serum and hair compartments and significantly lower in erythrocytes, compared with the control group (p<0.01). The Cp concentration was also higher in serum of RA patients (p<0.001). A statistically significant, positive correlation between the Cp serum concentration and the ESR values (rs = 0.38; p<0.007) was found. No significant influence of pharmaceutical treatment (methotrexate, non-steroidal anti-inflammatory drugs, glucocorticoids, calcium, vitamin D3 and sulphasalazine) on serum Cu was found. Conclusions. It seems that the ‘copper status’ in patients with RA, based on the measurement of Cu and Cp levels in blood serum is correlated with presence of the inflammatory process. The hair could serve as a useful, additional diagnostic material. Some other factors, different from the applied treatment, can probably influence the Cu levels in patients with RA.
